Systemic Platelet Activation and Respiratory Exacerbations, Pulmonary Symptoms, and Mortality among Current and Former Smokers in SPIROMICS.

RATIONALE: Platelet activation is elevated in chronic obstructive pulmonary disease (COPD) and associated with self-reported respiratory symptoms. Observational studies link the antiplatelet drug aspirin to lower exacerbation rates and fewer symptoms. However, it is unknown if platelet activation predicts incident respiratory exacerbations or mortality. OBJECTIVE: Is systemic platelet activation prospectively associated with respiratory exacerbations and mortality among ever-smokers with or at risk for COPD? METHODS: We measured two systemic platelet activation biomarkers, urinary 11-dehydro

Serum brain-derived neurotrophic factor remains elevated after long term follow-up of combat veterans with chronic post-traumatic stress disorder.

Attempts to correlate blood levels of brain-derived neurotrophic factor (BDNF) with post-traumatic stress disorder (PTSD) have provided conflicting results. Some studies found a positive association between BDNF and PTSD diagnosis and symptom severity, while others found the association to be negative. The present study investigated whether serum levels of BDNF are different cross-sectionally between combat trauma-exposed veterans with and without PTSD, as well as whether longitudinal changes in serum BDNF differ as a function of PTSD diagnosis over time. We analyzed data of 270 combat trauma-

Spinal cord stimulation (SCS) improves decreased physical activity induced by nerve injury.

Spinal cord stimulation (SCS) is used to manage treatment of neuropathic pain to reduce pain and hyperalgesia and to improve activity. Prior studies using animal models of neuropathic pain have shown that SCS reduces hyperalgesia; however, it is unclear whether SCS affects physical activity. Therefore, we tested whether nerve injury (spared nerve injury [SNI] model) reduced physical activity levels, and whether SCS could restore these decreased activity levels. We tested whether SCS given over a long duration (6 hr daily for 3 months) remained effective. We compared SNI with uninjured controls

Relative importance of fall risk factors in identifying falls among community-dwelling older adults: A cross-sectional retrospective study.

This study determined the relative strength of the association between fall risk factors and falls among community-dwelling older adults. Ninety-four older adults participated in this cross-sectional study. Their fall history in the past year and four categories of fall risk factors, including sensory (foot tactile sensation), motor (muscle strength and power), physical (balance, mobility, ankle range of motion, and physical activity level), and mental (fear of falling and cognitive functions), were collected. Logistic regression indicated that sensation, leg muscle power, and knee extensor st

Association of stress hyperglycemia ratio with malnutrition, sarcopenia, and frailty in older adults: a cross-sectional study.

BACKGROUND: Stress hyperglycemia ratio (SHR), calculated using admission glucose and glycated hemoglobin (HbA1c), has emerged as a marker of acute metabolic stress and adverse outcomes. However, its relationship with major geriatric syndromes remains unclear. This study investigated the association between SHR and malnutrition, sarcopenia, and frailty in older adults attending a geriatric outpatient clinic. METHODS: This retrospective cross-sectional study included patients aged ≥ 65 years who underwent comprehensive geriatric assessment between January 2022 and January 2026. SHR was calculat

Childhood hunger experiences were associated with adverse health outcomes in older Brazilian adults: Evidence from the Brazilian Longitudinal Study of Aging (ELSI-Brazil).

We aimed to assess whether childhood hunger experiences are associated with adverse health outcomes in old age. A cross-sectional study was conducted using data from the second wave of the ELSI-Brazil (2019-2021). Childhood hunger was assessed by the following question: "From birth until you were 15 years old, have you ever had a lack of food in your home and ended up going to bed feeling hungry?". Outcomes were obesity, multimorbidity, limitation in basic activities of daily living (BADLs), frailty, cognitive impairment, and depressive symptoms. Logistic regression was used in data analysis.

Impact Assessment of the Allergy Fact Checker, a Clinical Decision Support Tool for Noninvasive Beta-Lactam Antibiotic Allergy Label Delabeling: Protocol for a Multicenter Crossover Cluster-Controlled Study.

BACKGROUND: Beta-lactam allergy labels (BLALs), especially penicillin allergy labels, are frequently recorded in hospitalized patients and are associated with increased use of broad-spectrum and second-line antibiotics. Most BLALs are incorrect, but current allergy workups require invasive testing and specialized resources. We recently developed a strictly noninvasive, electronic patient record-embedded clinical decision support tool, the Allergy Fact Checker (AFC), which proactively identifies potentially incorrect BLALs by detecting uneventful re-exposures to the culprit or other beta-lactam

Changes in the indications for and outcomes of gastrostomy tube placement: an analysis of serial, prospective multi-site audits in Northern England.

OBJECTIVE: Gastrostomy tubes placed either endoscopically (percutaneous endoscopic gastrostomy (PEG)) or radiologically (radiologically inserted gastrostomy (RIG)) are established as standard methods for medium to long-term enteral feeding. There is a paucity of large-scale audit data to identify the main indications, complication rates and outcomes of patients having a PEG or RIG in the UK. The aim of our study was to determine changes in the use of and outcomes from gastrostomy tube placement in a large cohort of patients across the North East of England and North Cumbria. METHODS: The Nort
