Understanding and discriminating the normal and abnormal elements of the intracranial electroencephalogram (iEEG) is essential in decision-making for epilepsy surgery. The hippocampus is widely acknowledged as a key structure in decision-making processes for surgical treatment in temporal lobe epilepsy and epilepsies that involve the mesial temporal structures. This review will provide a summary of the current state of our knowledge and understanding regarding normal and abnormal features of the iEEG of the human hippocampus.

Informed nondissent is a form of shared decision-making in which clinicians provide a medically appropriate recommendation while observing for explicit objection, rather than requiring affirmative agreement. This approach emphasizes transparency, clinician responsibility, and the opportunity for dissent. Clinicians and parents in the neonatal intensive care unit (NICU) are frequently tasked with values-based, emotionally charged goals-of-care decisions for critically ill newborns with poor prognoses. When NICU parents indirectly express readiness to change goals of care without explicitly verb

BACKGROUND: Care for athletes with congenital heart disease (CHD) is challenging owing to limited evidence to guide participation decisions. We present a case regarding return-to-play recommendations in a triathlete with CHD.
CASE SUMMARY: A 22-year-old triathlete with repaired D-transposition of the great arteries and supravalvular pulmonary stenosis presented with dyspnea during competition. Imaging revealed moderate right ventricular enlargement, moderate pulmonic regurgitation, a dilated neo-aortic root, and a reimplanted left anterior descending coronary artery with high-risk anatomic fe

BACKGROUND: This study aimed to characterize practices and decision-making for extracorporeal membrane oxygenation (ECMO) for congenital anomalies of the kidney and urinary tract (CAKUT).
METHODS: General practices (GP) section inquired about institutional practices and barriers, ECMO criteria, and dialysis. The hypothetical cases (HC) illustrated four clinical scenarios with varying degrees of renal severity for ECMO candidacy.
RESULTS: Then, 99 (42 centers) and 91 (38 centers) physicians completed the GP and HC components, respectively. The majority considered ECMO on a case-by-case basis

Hunger leads to less healthy food choices, but its effect on decision-making in non-food domains is less clear. To close this gap, we investigated to what extent hunger state affected choice across domains by examining attentional patterns and cognitive mechanisms underlying decision-making across different domains. We implemented a within-subject design in which participants completed a food choice, an intertemporal discounting, and a social preferences task in hungry and sated states while their eye-movements were recorded. Hungry participants were more likely to look at and subsequently sel

Modern risk-adapted management of thyroid cancer involves risk stratification as an active, dynamic process that begins with the detection of a thyroid nodule and continues throughout the clinical course of diagnosis, active surveillance or treatment, and follow-up. Rooted in clinicopathological staging, which can be further refined with the molecular risk characterization of the tumor, initial management plans are developed and modified over time on the basis of the natural history of the disease and the response to therapy. We present a clinical framework for therapeutic decision making that

Ambulatory care facilities play a critical role in many healthcare systems worldwide. To ensure efficient care provision, we must match care demand with care supply. To support provider decision-making, this article reviews Operations Research planning problems, the corresponding planning and control decisions that must be made when opening up or running an ambulatory care facility, and their data requirements. We give an overview of demand and supply-related data that an ambulatory care facility can collect and comment on the consequences for decision-making if some of that data is missing. W

Prison inmate samples present a high prevalence of impulsivity- and compulsivity-related behavioral problems. The Probabilistic Reversal Learning Task (PRLT) is a useful tool to assess decision-making, and we explore its associations with inmates' personality disorder (antisocial personality disorder, APD; obsessive-compulsive personality disorder, OCPD; or both) and history of drug abuse. Mixed-effects methods were used to model acquisition and reacquisition curves across PRLT, in a sample of 275 prison inmates diagnosed with OCPD, APD, or both. Two aspects were assessed: general discriminati

The management of patients with atrial fibrillation (AF) requires intricate clinical decision-making to optimize outcomes. In everyday clinical practice, physicians undergo difficult choices to better manage patients with AF. They need to balance thromboembolic and bleeding risk to focus on patients' symptoms and to manage a variety of multiple comorbidities. In this review, we aimed to explore the multifaceted dimensions of clinical decision-making in AF patients, encompassing the definition and diagnosis of clinical AF, stroke risk stratification, oral anticoagulant therapy selection, consid

The COVID-19 pandemic accelerated a widespread shift to telehealth among mental health professionals to prioritize both providers' and clients' safety. Telehealth is likely here to stay; however, there is limited practical guidance for clinicians about how to make decisions regarding who should proceed with care via telehealth versus in-person. There also is virtually no data on the effectiveness of hybrid approaches to care; yet this can be an attractive option with potential clinical benefit. This paper provides practice-informed guidance to support shared clinical decision-making between cl

Substance use disorder (SUD) is characterized by continued drug use despite adverse consequences. Methcathinone is a new type of psychoactive substance that is associated with high excitement and impulsive behaviors. However, it is unclear if individuals with methcathinone use disorders (MCUD) are with impaired decision-making ability. We analyzed the task performance in 45 male MCUD subjects and 35 male matched healthy controls (HC) with intertemporal decision-making task. Constant sensitivity discounting model was used to estimate potential changes in both discounting rate and time sensitivi

A large amount of evidence shows that after a cancer diagnosis, patients significantly reduce their level of physical activity. Usually, this reduction is attributed to cancer-related fatigue. However, to our knowledge, no study has clearly demonstrated that fatigue alters effort-based decision-making in cancer. This mini-review aimed to provide evidence that chronic fatigue in cancer patients causes changes in brain connectivity that impact effort-based decision-making. Indeed, three patterns of activation to compensate for dysfunctional networks have been reported: greater variability in the

How does the brain internally represent a sequence of sensory information that jointly drives a decision-making behavior? Studies of perceptual decision-making have often assumed that sensory cortices provide noisy but otherwise veridical sensory inputs to downstream processes that accumulate and drive decisions. However, sensory processing in even the earliest sensory cortices can be systematically modified by various external and internal contexts. We recorded from neuronal populations across posterior cortex as mice performed a navigational decision-making task based on accumulating randoml

Cognitive training and brain stimulation show promise for ameliorating age-related neurocognitive decline. However, evidence for this is controversial. In a Registered Report, we investigated the effects of these interventions, where 133 older adults were allocated to four groups (left prefrontal cortex anodal transcranial direct current stimulation (tDCS) with decision-making training, and three control groups) and trained over 5 days. They completed a task/questionnaire battery pre- and post-training, and at 1- and 3-month follow-ups. COMT and BDNF Val/Met polymorphisms were also assessed. C

PURPOSE: Hip arthroplasty is rarely indicated in the treatment of per- and intertrochanteric femur fractures. Although the majority of fractures are amenable to closed- or open reduction and internal fixation (CRIF/ORIF), in some patients the complexity of the fracture or other patient-related factors may cause the orthopaedic surgeon to consider arthroplasty as the treatment of choice. Decision-making is challenging, and a reliable score has not yet been established.
RESULTS: Reviewing literature, several predictors of inferior outcome after CRIF/ORIF in per- and intertrochanteric fractures

This work investigates whether inhibition impairments influence the decision making process in pathological gamblers (PGs). The PG (N=51) subjects performed the Iowa Gambling Task (IGT as the measure of the decision making process) and two tests of inhibition: the Stroop (interference inhibition), and the Go/NoGo (response inhibition), and were compared with demographically matched healthy subjects (N=57). Performance in the IGT block 1 and block 2 did not differ between the groups, but the differences between the PGs and healthy controls began to be significant in block 3, block 4 and block 5

The frontal variant of frontotemporal dementia is a significant neurological condition worldwide. There exist few treatments available for the cognitive and behavioural sequelae of fvFTD. Previous research has shown that these patients display risky decision-making, and numerous studies have now demonstrated pathology affecting the orbitofrontal cortex. The present study uses a within-subjects, double-blind, placebo-controlled procedure to investigate the effects of a single dose of methylphenidate (40 mg) upon a range of different cognitive processes including those assessing prefrontal corte

The prefrontal region of the brain, including the ventromedial sector which supports reasoning and decision-making, may undergo disproportionate aging in some older persons, but the empirical evidence is decidedly mixed. To help resolve this, we tested 80 neurologically and psychiatrically healthy Younger (aged 26-55) and Older (aged 56-85) adults on a "Gambling Task", which provides a close analog to real-world decision-making by factoring in reward, punishment, and unpredictability, yielding a sensitive index of ventromedial prefrontal function. A subset of the Older group manifested a decis

Clinical guidelines often make recommendations on the use of diagnostic tests. Compared with sensitivity and specificity, the use of pre- and post-test probabilities allows a more explicit and rational selection and interpretation of diagnostic tests. Ideally, clinical guidelines relating to diagnosis should routinely incorporate this information to enhance individualised decision making. We report our experience of incorporating pre- and post-test probabilities into a guideline on the investigation of women with postmenopausal bleeding developed by the Scottish Intercollegiate Guidelines Netw

INTRODUCTION: Decision-making is one of the most critical components of people's lives. It involves cognitive factors, which are responsible for processing the stimuli present in the task, remembering past experiences, and assessing the possible consequences that emotional decisions can have. So far, there are no studies of this nature in Ecuador, so this research seeks to establish the characteristics of the Ecuadorian population. With the results obtained, we expect to gain insight into the way this population handles decisions in daily life.
OBJECTIVE: To relate decision-making to behavior
