Neuroinflammatory Mechanisms of Adult Sepsis-Associated Encephalopathy: Implications for Blood-Brain Barrier Disruption and Oxidative Stress.

Sepsis is a syndrome of life-threatening acute organ dysfunction caused by a dysregulated host response to infection. Sepsis-associated encephalopathy (SAE) refers to the diffuse brain dysfunction observed in sepsis cases, clinically characterized by a spectrum of neuropsychiatric manifestations ranging from delirium to coma. SAE is independently associated with increased short-term mortality and long-term neurological abnormalities, with currently no effective preventive or treatment strategies. The pathogenesis is intricate, involving disruptions in neurotransmitters, blood-brain barrier (BB

Neuroimaging Findings of CAR T-Cell-Associated Neurotoxicity: A Review.

Chimeric antigen receptor T-cell (CAR-T) therapy has remarkable efficacy in treating refractory hematologic malignancies. However, CAR-T therapy may induce neurotoxic effects in some patients. Common symptoms of neurotoxicity range from early signs such as headache, confusion, delirium, and aphasia to severe manifestations such as seizures, motor weakness, increased intracranial pressure, cerebral edema, and coma. Magnetic resonance imaging (MRI) can offer invaluable insight into resulting abnormalities in the structure, physiology, and function of the central nervous system. This review aims

Mental Health in Women.

Mental health illness has been increasing worldwide. The prevalence of mental illness and is higher among females than among males. It is estimated that one in 5 women experience a common mental health disorder. This article highlights gender disparities in the risk, prevalence, and presentation of different mental health disorders. Nearly all survivors of critical illness experience 1 or more domains of the post-intensive care syndrome. We review different mental health disorders including anxiety disorders, mood disorders, psychotic disorders, and post-intensive care syndrome, and medication

De Novo Psychiatric Disorders in a Woman With Giant Prolactinoma Treated With Cabergoline.

Dopamine agonists are the first-line treatment of prolactinomas. The risk of developing de novo psychiatric symptoms during dopamine agonist therapy is low. Herein, we report the case of a 42-year-old woman with a giant prolactinoma who developed a psychiatric disorder after 1 day of cabergoline therapy initiation. She presented with amenorrhea, galactorrhea, headaches, and disturbed vision. Biological investigations revealed hyperprolactinemia (2975 ng/ml) with gonadotropin deficiency. Pituitary MRI showed a giant pituitary adenoma. The patient was treated with cabergoline at the dose of 1 mg

The complexity of neuropsychiatric manifestations of COVID-19 in South Africa.

SARS-CoV-2 was first identified in Wuhan City, China, in 2019. Initially it was associated with the development of pulmonary disease, but research over the past 2 years has identified effects on multiple systems. Neuropsychiatric manifestations of COVID-19 have been reported in countries around the world, including new-onset psychosis in patients with no personal or family psychiatric history. We present the first case series describing neuropsychiatric manifestations of patients in Johannesburg, South Africa (SA). All four patients presented with their index-episode psychosis, and evidence of

Sedation depth during spinal anesthesia and survival in elderly patients undergoing hip fracture repair.

Low intraoperative Bispectral Index (BIS) values may be associated with increased mortality. In a previously reported trial to prevent delirium, we randomized patients undergoing hip fracture repair under spinal anesthesia to light (BIS >80) or deep (BIS approximately 50) sedation. We analyzed survival of patients in the original trial. Among all patients, mortality was equivalent across sedation groups. However, among patients with serious comorbidities (Charlson score >4), 1-year mortality was reduced in the light (22.2%) vs deep (43.6%) sedation group (hazard ratio [HR], 0.43; 95% confidenc

Psychiatric disorders associated with burns.

A study was made of burns patients who were referred for psychiatric problems. There were a total of 69 subjects, divided into three groups. The first group comprised thirty-four cases who attempted suicide by burning themselves--there were more women than men, the majority were less than forty years, and Indians were overrepresented. The majority of these were suffering from schizophrenia or a major depressive disorder. The second group of twenty-three patients were those who had a non-psychotic psychological reaction to their burns. The majority were also less than 40 years of age, and the m

Practical management of urinary incontinence.

A comprehensive evaluation of incontinence using office-based techniques may discover reversible problems such as delirium, urinary infections, vaginitis, depression, drug effects, endocrinologic causes, immobility, or impaction. Treatment of these causes may alleviate incontinence. Bedside urodynamics are a sensitive, specific, and useful office-based technique that can identify detrusor instability, overflow incontinence, or stress incontinence. Low urinary flow rate, difficulty with catheterization, slow bladder filling or severe urgency, bladder capacity over 600 ml, and post-void residual

Non-pharmacological interventions for sleep promotion in hospitalized children.

BACKGROUND: Healthy sleep is an important component of childhood development. Changes in sleep architecture, including sleep stage composition, quantity, and quality from infancy to adolescence are a reflection of neurologic maturation. Hospital admission for acute illness introduces modifiable risk factors for sleep disruption that may negatively affect active brain development during a period of illness and recovery. Thus, it is important to examine non-pharmacologic interventions for sleep promotion in the pediatric inpatient setting. OBJECTIVES: To evaluate the effect of non-pharmacologic

Efgartigimod as a Steroid-sparing Therapy for Evans Syndrome: A Case Report.

Evans syndrome is characterized by the coexistence of immune thrombocytopenia (ITP) and autoimmune hemolytic anemia (AIHA). Corticosteroids are the first-line therapy. However, many patients become steroid-dependent or experience significant adverse effects. An 80-year-old man with a 33-year history of ITP was admitted with laboratory findings suggestive of hemolytic anemia and he was diagnosed with Evans syndrome. High-dose corticosteroids achieved hematologic control, but they also caused delirium, necessitating dose reduction. Rituximab produced a limited response. Efgartigimod was initiate

Protracted encephalopathy and subacute combined degeneration associated with chronic nitrous oxide use: a case report.

Nitrous oxide is a dissociative hallucinogen that is increasingly used recreationally, in part due to its widespread availability. Its use is known to cause subacute combined degeneration via inactivation of vitamin B12; it may also result in acute delirium and chronic progressive encephalopathy. Though current practice guidelines call for treatment of neurological sequelae of nitrous oxide use with vitamin B12 supplementation, a paucity of long-term outcome data limits our ability to guide extended courses of treatment. In this report, we discuss a case of protracted encephalopathy associated

Depth of anesthesia monitoring: Current evidence, clinical impact, and future directions.

Depth of anesthesia monitoring has become an important component of modern anesthetic practice. Electroencephalography-based technologies, such as the bispectral index, entropy, and Narcotrend, allow clinicians to assess the level of consciousness more objectively and guide anesthetic titration more precisely. This narrative review summarizes current evidence on the relationship between depth of anesthesia monitoring and major perioperative complications. Available evidence suggests that maintaining an appropriate depth of anesthesia reduces the risk of intraoperative awareness, hypotension as

La Famiglia è Tutto: Why Families Must Be at the Bedside.

Open Intensive Care Units (ICUs) represent one of the most significant transformations in the humanization of health care by removing temporal, physical, and relational barriers. While limitations on visiting hours are often implemented to alleviate concerns about infection spread or issues with interference with care, numerous studies have demonstrated overwhelming benefits of family members at patients' bedside, including reduced incidence of delirium and shorter hospital stays. Family members are vital teammates in the patient recovery process and, with clear engagement methods, can help pr

Thiamine deficiency in oncology: Clinical insights from two cases of Wernicke's encephalopathy.

Wernicke's encephalopathy (WE) is a neurological disorder primarily caused by thiamine deficiency, typically presenting with malnutrition, confusion or delirium, oculomotor abnormalities (such as nystagmus or ophthalmoplegia), and ataxia. Although commonly associated with alcoholism, WE is frequently observed in oncology patients but is often under-recognised, leading to inadequate diagnosis and treatment. Malnutrition and rapid weight loss in cancer patients can increase the risk of this condition. We report two patients with cancer who presented with significant malnutrition and weight loss,

Management of intractable emergence agitation with emotional and behavioural disinhibition in a child with anxiety requiring multiple anaesthetics.

Paediatric oncology patients require multiple anaesthetics for diagnostic and therapeutic procedures. Anxiety due to major life stressors, repeated procedures and other cancer therapy is common among these patients and may contribute to the incidence of emergence agitation. Here, we present the case of a 9-year-old child with procedural anxiety and relapsed acute B-cell lymphoblastic leukaemia requiring multiple anaesthetics for various procedures. The patient experienced severe disinhibition and agitation without delirium upon anaesthesia emergence. After conventional pharmacological and non-

Delivering safe, person-centred care for acutely unwell older people on virtual wards.

A virtual ward can provide hospital-level care for older people in their usual place of residence during an episode of acute illness. Care on a virtual ward may be delivered through a mix of in-person home visits, telephone or video calls and remote monitoring. This model of care can prevent unnecessary inpatient admissions, which in turn can prevent the development of associated complications in this patient population, such as deconditioning, delirium and hospital-acquired infections. However, there are barriers to the use of virtual wards in the care of older people. This article provides a

Postoperative sepsis-associated neurocognitive disorder: mechanisms, predictive strategies, and treatment approaches.

Sepsis is a critical condition characterized by an abnormal immune response to infection, resulting in systemic inflammation, organ failure, and high mortality rate. Postoperative sepsis, accounting for nearly one-third of all sepsis cases, predominantly affects the elderly and individuals with pre-existing conditions, with fatality rates between 30 and 50%. Surgical stress induces immune, hormonal, and metabolic disturbances, heightening susceptibility to immune dysregulation and sepsis. Neurocognitive disorders related to postoperative sepsis, which share pathophysiological similarities with

Awake Venovenous Extracorporeal Membrane Oxygenation in the Intensive Care Unit: Challenges and Emerging Concepts.

Extracorporeal membrane oxygenation (ECMO) is an advanced treatment for severe respiratory failure. Implantation of ECMO before invasive ventilation or extubation during ECMO has been reported and is becoming increasingly popular. Avoidance of sedation and invasive ventilation during ECMO (commonly referred to as "awake ECMO") may have potential advantages, including a lower rate of delirium, shorter mechanical ventilation time, and the possibility of undergoing early rehabilitation and/or physiotherapy. However, awake ECMO is also associated with several risks, such as self-inflicted lung inj

Safety of Remimazolam in Vulnerable Populations.

Vulnerable populations require careful consideration in drug selection due to their unique physiological conditions. Remimazolam, an innovative ultra-short-acting benzodiazepine, presents several advantages, including rapid onset, brief duration of action, and high predictability. Despite the extensive literature on remimazolam, few studies have specifically evaluated its safety in vulnerable patient populations. This review offers a comprehensive analysis of the safety profile of remimazolam in vulnerable populations, including cardiovascular diseases, neurological disorders, hepatic and rena
