[Clinical value of hyperbaric oxygen therapy for the treatment of chronic wounds].

The physical and biological effects of hyperbaric oxygen therapy (HBOT) are used to treat various medical conditions, such as decompression sickness after diving accidents or in cases of gas embolism or CO-poisoning. Another, generally less well-known indication for HBOT is the use of HBOT in supporting the healing of chronic wounds. HBOT corrects tissue hypoxia, stimulates angiogenesis and collagen synthesis, reduces environmental oedema and has anti-inflammatory and anti-microbial effects. Various clinical studies have shown a positive effect of HBOT on wound healing, particularly in diabeti

Umbilical abscess caused by Helcococcus kunzii: a case report and literature review.

INTRODUCTION: Helcococcus kunzii is a commensal bacterium found in the human skin that seldom causes infection. While there is documentation of abscesses; such as foot, breast, and brain abscesses; caused by H. kunzii, there are no reports of its causing umbilical abscesses. CASE PRESENTATION: A case of scar tissue infection caused by H. kunzii in a diabetic patient is described. A single pathogen was isolated from a culture of the abscess extract and was identified as H. kunzii using matrix-assisted laser desorption/ionization-time of flight (MALDI-TOF) and 16S rRNA gene analysis. The patien

Wireless matrix metalloproteinase-9 sensing by smart wound dressing with controlled antibacterial nanoparticles release toward chronic wound management.

Chronic wounds cause serious health and economic burdens on patients and society. Herein, a wireless and flexible smart wound dressing was developed for matrix metalloproteinase-9 (MMP-9) monitoring and antimicrobial treatment toward chronic wound management. The highly sensitive radio frequency MMP-9 sensor was realized based on a bioresponsive hydrogel with the bioactive peptide sequences. Taking advantage of the flexible inductive-capacitive (LC) circuit and bioresponsive hydrogel, the wireless and wearable smart wound dressing offered an efficient strategy for in-situ wound analysis. A Leg to Stand On: Working With Marginal Decision-making Capacity in a Patient With a Severe Leg Infection and Schizophrenia.

Assessment of a patient's capacity to make treatment decisions and working with the wishes of a patient with mental illness against the best medically indicated plan is a complex and dynamic task. It is particularly challenging when the course of deterioration of the illness is meandering and slow, and the time horizon for recovery is uncertain, providing no clear point of entry for definitive crisis intervention. High-impact decisions concerning body integrity, such as the amputation of a leg, further complicate the task. To highlight these challenges and complexities, we present the case of

Clinical evidence supporting ceftaroline fosamil and ceftobiprole for complicated skin and soft tissue infections.

PURPOSE OF REVIEW: Ceftaroline and ceftobiprole are advanced generation cephalosporins with activity against methicillin-resistant Staphylococcus aureus (MRSA). This review summarizes their clinical efficacy for complicated skin and soft tissue infections (cSSTIs). RECENT FINDINGS: Both these agents retain excellent in vitro activity against both MRSA and Gram-negative isolates from patients with CSSTIs. Both these agents are registered for the management of cSSTIs based on the results of large scale phase III noninferiority trials. Ceftaroline and ceftobiprole are noninferior to the combinat

The role of genetic factors and monocyte-to-osteoclast differentiation in the pathogenesis of Charcot neuroarthropathy.

Charcot neuroarthropathy is a chronic, progressive condition of the skeletal system that affects some patients with diabetic neuropathy. It results in progressive destruction of bones of the foot and disorganisation of pedal joints and ligaments. Effective prevention and treatment for Charcot neuroarthropathy remain a challenge. Currently, there are no reliable repeatable markers to identify patients with diabetes who are at higher risk of developing Charcot neuroarthropathy. The pathogenesis underlying the development of Charcot neuroarthropathy also remains unclear. In this review, we provid

The FDA and designing clinical trials for chronic cutaneous ulcers.

Treatment of chronic wounds can present a challenge, with many patients remaining refractory to available advanced therapies. As such, there is a strong need for the development of new products. Unfortunately, despite this demand, few new wound-related drugs have been approved over the past decade. This is in part due to unsuccessful clinical trials and subsequent lack of Food and Drug Administration (FDA) approval. In this article, we discuss the FDA approval process, how it relates to chronic wound trials, common issues that arise, and how best to manage them. Additionally, problems encounte

[Radiology of the foot in chronic alcoholism].

The authors have reviewed the radiographic patterns of the foot in 28 patients affected by neuropathic joint disease complicating alcoholism, out of a series of 82 chronic alcoholic patients. Sixteen of them were also affected by diabetes mellitus. On the basis of X-ray findings, lesions were divided into three groups, reflecting the evolution of the disease: 1) early changes, especially affecting the soft tissues and joints; 2) definite lesions, consisting of fractures, osteolysis, bone destruction and amputation, periarticular debris; 3) "healing" signs, simulating degenerative joint disease

[Hypervascularisation in diabetic patients shown by angiography (author's transl)].

Two diabetic patients are described who showed widened and tortuous arteries, increased blood flow and increased filling of small arteries, with simultaneous filling of arteries and veins (increased flow through arterio-venous anastomoses). In one patient, during an observation period of four years, occlusion of both common iliacs developed, as well as a block of one femoral artery and mural changes in the other. Inthe second patient, areas of occlusion and hypervascularisation co-existed. The first patient showed osteolytic changes, also involving the joints, whereas the second patient develo

Radiographical outcomes of a cellular based allograft following foot/ankle arthrodesis in patients with risk for non-union.

Morbidity associated with autograft harvest has led to the need for alternative bone grafts during fusion surgical procedures. The purpose of this study is to evaluate the efficacy of a cellular bone allograft (CBA) in patients who underwent foot/ankle fusion surgery. Retrospective data of patients who underwent foot/ankle arthrodesis using a CBA between XXXX and XXXX were collected from a single site. Patients were at least 18 years of age at the time of surgery and had ankle/foot surgery with Trinity ELITE CBA as the primary or only bone graft. Patients' radiographic union was assessed at th

Calcaneotalotibial arthrodesis by retrograde intramedullary nailing using expert tibia nail for charcot osteoneuropathy of the foot: A case series.

INTRODUCTION: Charcot osteoneuroarthopathy (CN) is a progressive degenerative arthropathy determined by the interaction of neuropathy, osteopaenia and proinflammatory cytokines. The aim of treatment is to maintain the foot on plantigrade position, recover foot deformity, osseous stability, and prevent ulceration. Intramedullary fixation in calcaneotalotibial arthrodesis has been described in promotion of rigid internal fixation with minimal soft tissue violation producing deformity correction, minimal periosteal aggression, vascular damaged and good functional outcome, with less postoperative

The use of a reverse flow sural fasciocutaneous flap in a patient with multiple trauma: a case report.

Provision of soft-tissue coverage for defects in the distal leg and foot is a challenge, especially in patients with multiple injuries and major soft-tissue defects. Major flap reconstruction requires expertise and the results are variable, with high morbidity. We report a case in which a reverse flow sural fasciocutaneous flap was used for treatment of an open fracture-dislocation of the right ankle after repeated debridement in a 64-year-old man with a history of chronic smoking, diabetes mellitus, and hypertension.

S1 Guideline on Infected Interdigital Intertrigo (also called Gram-Negative Toe Web Infection).

Infected interdigital intertrigo is an exudative, macerating, mixed infection of the toe webs in which gram-negative bacteria (Pseudomonas aeruginosa and Enterobacterales) seem prevalent, but in which gram-positive pathogens (Staphylococcus [S.] aureus, streptococci, enterococci) and fungi (dermatophytes, yeasts) also occur. The former German term "gram-negative foot infection" misleadingly implied a soft-tissue infection of the foot caused by gram-negative bacteria. The updated term "infected interdigital intertrigo" is an internationally used designation, as is the term "gram-negative toe we

Complications of Circular Ring External Fixation of the Foot and Ankle.

BACKGROUND: Circular ring external fixation has been found to be successful in the treatment of a wide range of foot and ankle pathologies, particularly in patients with multiple comorbidities and soft-tissue compromise, but the procedures are technically demanding, and complications are common. The primary goal of this study was to identify complications associated with circular ring external fixation, and the secondary aim was to identify any independent risk factors associated with them. METHODS: Institutional review board approval was obtained, and a retrospective review was performed of

Serous Atrophy of Bone Marrow in the Feet Diagnosed via Magnetic Resonance Imaging: A Case Report.

Serous atrophy of bone marrow (SABM) is characterized by focal replacement of bone marrow elements with extracellular gelatinous substances. It has been associated with a wide range of chronic conditions, including anorexia nervosa, malignancy, chronic kidney disease, and certain chronic infections. Previous literature has reported the disorder as primarily diagnosed via bone marrow biopsy and occurring outside of the distal extremities. Herein we describe a case of SABM occurring in the feet diagnosed via magnetic resonance imaging (MRI), a phenomenon that is rarely reported. The patient is a

Percutaneous deep venous arterialization with balloon angioplasty salvaged a life-threatening critical limb in a hemodialysis patient after repeated pedal angioplasty failed: A case report.

Chronic limb-threatening ischemia (CLTI) with severe below-the-ankle (BTA) lesions is often difficult to revascularize with endovascular treatment (EVT) and surgical treatment. We present a case of successful limb salvage using percutaneous deep venous arterialization (pDVA) in a patient with CLTI whose BTA lesion reconstruction failed. A 57-year-old man with diabetes mellitus and end-stage renal failure on maintenance hemodialysis was referred to our hospital because of gangrene in the second and third toes of his left foot. EVT was repeated for the anterior tibial artery, posterior tibial ar

Fracture Patterns in Type 1 and Type 2 Diabetes Mellitus: A Narrative Review of Recent Literature.

PURPOSE OF REVIEW: In this narrative review, we have summarized the literature on fracture risk in T1DM and T2DM with a special focus on fracture site, time patterns, glucose-lowering drugs, and micro- and macrovascular complications. RECENT FINDINGS: T1DM and T2DM were associated with an overall increased fracture risk, with preferent locations at the hip, vertebrae, humerus, and ankle in T1DM and at the hip, vertebrae, and likely humerus, distal forearm, and foot in T2DM. Fracture risk was higher with longer diabetes duration and the presence of micro- and macrovascular complications. In T2

[Type 2 diabetes in the elderly, which specific features?].

Type 2 diabetes, whose prevalence has increased among elderly aged over 75 years, has a number of specific features which differ from that in young people: heterogeneous population, association with other cardiovascular risk factors and several comorbidities, different therapeutic constraints and risks, and lower life expectancy. By using a standardized geriatric assessment it is possible to determine therapeutic and glycemic goals for each patient. In the elderly, main complications of diabetes are hypoglycemia and foot lesions. In order to avoid malnutrition, lifestyle and dietary rules shou

Amputations in general practice.

A retrospective review was carried out, of cases of amputation, which were performed between 1985 and 1999 at the Baptist Medical Centre, Ogbomoso with the aims of documenting our experience and complications. The majority of the operations were performed by trainee family practitioners. There was a preponderance of males (M/F ratio of 1.89:1) and over half of the patients were between 21 and 50 years of age. Most of the patients were admitted for trauma and most of them underwent a two stage operation. Slightly less than half of the amputations performed were below knee amputation (BKA). Elev

Minor amputations on the feet after revascularization for gangrene. A consecutive series of 95 limbs.

A consecutive series of 89 patients (95 limbs) with gangrene were operated on with amputation of toes or some distal part of the foot after arterial reconstruction. 43 patients had diabetes. Healing was achieved in 81/82 feet when the reconstruction was open. Amputation below or above the knee was required in 4/5 limbs when the reconstruction failed. 8 patients died before healing. The median time to healing was 30 (17-452) days, after a single amputation, and 115 (36-466) days, when more than one procedure had been necessary. We concluded that amputations on the feet for gangrene usually heal
