Identification of conductive hearing loss in young infants using tympanometry and wideband reflectance.

OBJECTIVE: The goal of the study was to evaluate the effectiveness of tympanometry and wideband reflectance (WBR) in detecting conductive hearing loss (CHL) in young infants. METHODS: Type of hearing loss was determined using auditory brainstem response using air- and bone-conducted tone bursts in 84 ears from 70 infants (median age = 10 weeks). Of these 84 ears, 60 are included in the current analysis: 43 with normal hearing (NH) and 17 with CHL. Tympanometry was measured using probe tone frequencies of 226, 678, and 1000 Hz. Tympanograms were evaluated in two ways: (1) Acoustic middle ear a

The role of transient-evoked otoacoustic emission testing in the evaluation of elderly persons.

OBJECTIVE: The purposes of this study were: To determine the quantitative and qualitative changes that occur in transient-evoked otoacoustic emissions (TEOAEs) in older individuals without addressing the effect of aging alone and without correction for hearing loss of the subject selection. To investigate the clinical value of measuring TEOAEs in the routine audiological evaluation of older people reasoning that a finding of hearing loss in the presence of TEOAEs could indicate a form of presbycusis with a primary central component. DESIGN: Click-evoked otoacoustic emissions (CEOAEs) were mea

Trajectory of hearing loss in children with unilateral hearing loss.

INTRODUCTION: The aim of this study was to quantify the amount of deterioration in hearing and to document the trajectory of hearing loss in early identified children with unilateral hearing loss (UHL). We also examined whether clinical characteristics were associated with the likelihood of having progressive hearing loss. METHODS: As part of the Mild and Unilateral Hearing Loss Study, we followed a population-based cohort of 177 children diagnosed with UHL from 2003 to 2018. We applied linear mixed models to examine hearing trends over time including the average amount of change in hearing.

Blast injury of the ear: an overview and guide to management.

Blast injury is uncommon in many parts of the world but sporadic cases occur. The ear is particularly susceptible to damage and easily forgotten in patients with multiple injuries. The aim of this review article is to draw together the more important aspects of blast ear injury for those who are unfamiliar with it and to serve as a reminder of the problems to others. It covers the interactions of blast waves with the ear giving a summary of the mechanisms and types of injury. The management of blast-related injuries is discussed.

Unusual Presentation of a Rare Disease: A Case Report of Fungal Necrotizing Otitis Media and Mastoiditis.

Acute necrotizing otitis media is a severe middle ear infection which causes necrosis of the tympanic cavity. A 54-year-old female was presented who suffered from diabetes mellitus and end-stage renal disease presenting with severe otalgia, initially thought to be necrotizing otitis externa. She rapidly progressed to total necrosis of the tympanic membrane. Further assessments confirmed a fungal disease caused by Candida tropicalis and Aspergillus flavus. Immediate antifungal therapy with amphotericin B and subsequent radical mastoidectomy managed to control the disease with no sign of necrosi

Bilateral Otic Manifestation of Pseudogout: A Unique Case Report of Conductive Hearing Loss.

Pseudogout is characterized by the deposition of calcium pyrophosphate dihydrate crystals (CPPD), primarily affecting large joints. Extra-articular manifestations, particularly in the head and neck region, are exceedingly rare. We report a unique case of bilateral isolated pseudogout of the middle ear manifesting with progressive conductive hearing loss as the first and only symptom of pseudogout. Otoscopy and CT scan often yield a differential diagnosis that includes tumors or cholesteatoma, necessitating surgery with histopathological examination. The definitive diagnosis is confirmed upon i

[Traumatology of the lateral skull base].

The most common cause of lateral skull base fractures are still road traffic accidents, followed by falls. The radiologic classification into otic capsule-sparing or otic capsule-violating fractures correlates well with an increased risk of injury to the sensitive structures of the middle ear with otic capsule-violating fractures. In case of immediate onset complete facial nerve paralysis, decompression surgery is generally recommended if bony impingement can be demonstrated on high-resolution CT of the temporal bone. For many other facials nerve complications, recent publications have achieve

Unlearning the ABCs of Tympanometry.

Interpretation of tympanometry commonly relies on the historical convention of classifying findings according to large and arbitrary threshold shifts of tympanometric peak pressure (TPP). This convention had value for prior generations of otolaryngologists in diagnosing severe, chronic middle ear disease requiring surgical intervention but may not be well suited for the present-day evaluation of less severe disease. The existing definition of a type C curve (less than -100 daPa) is likely insensitive to detect subtle abnormalities, including some presentations of obstructive eustachian tube dy

Cleidocranial Dysplasia Causing Respiratory Distress in Neonates: A Case Report and Literature Review.

Cleidocranial dysplasia (CCD; OMIM 119600) is a rare autosomal dominant skeletal dysplasia, which is mainly characterized by persistently open or delayed closure of fontanelle, patent skull sutures, abnormal clavicles, pectus excavatum, short stature, supernumerary teeth, and sinus and middle ear infections. It is caused by Runt-related transcription factor 2 (RUNX2; OMIM 600211) mutations. Herein, we present a rare case of CCD with neonatal respiratory distress, who had abnormal midfacial features and wide fontanelle. Also, pectus excavatum was noted. He was transferred to our department, adm

Endoscopic Management of Pediatric Cholesteatoma.

Pediatric cholesteatoma occurs in one of two forms: congenital cholesteatoma, developing from embryonic epidermal cell rests or acquired cholesteatoma, associated with a focal defect in the tympanic membrane. This disease has been traditionally managed with the operating microscope, often requiring mastoidectomy for adequate visualization of and access to the middle ear and mastoid cavities. Recently, advances in endoscopic equipment have enabled otologists to manage most cases of pediatric cholesteatoma via a minimally-invasive, transcanal endoscopic approach. This review discusses the curren

Postoperative Hearing Loss in a Patient With Crouzon Syndrome: A Case Report.

Sudden postoperative hearing loss is rare, and thought to be caused by a variety of mechanisms. Here we report on a patient with Crouzon syndrome who experienced multiple episodes of postoperative hearing loss, with persistent hearing loss occurring after she received nitrous oxide as part of a general anesthetic. Nitrous oxide is known to cause pressure changes in closed air spaces. Patients with craniofacial syndromes may have acoustic nerve compression from skull base and cartilage anomalies that cause narrowing of the internal acoustic meatus. These anatomic variations may make patients mo

Deconstructing Pneumococcal Progression from Colonization to Disease.

Despite advances in treatment and prevention, the pneumococcus continues to be a dominant cause of severe pneumonia and sepsis and of otitis media, sinusitis, and nonbacteremic pneumonia. Lewnard and colleagues (Infect Immun 86:e00727-17, 2018, https://doi.org/10.1128/IAI.00727-17) used a unique data set of nasopharyngeal and middle ear fluid samples to provide further insight into the progression of nasopharyngeal pneumococcal colonization to disease. They report the comparative rate of progression from colonization to otitis media by serotype, providing insight into how conjugate vaccines th

Extramedullary Hematopoiesis Presenting as Bilateral Glomus Tympanicum-First Case Report in a Pediatric Patient and With Bilateral Presentation.

To the best of our knowledge, this is the first case report of middle ear extramedullary hematopoiesis (EMH) in a pediatric patient as well as the first bilateral presentation reported in both children and adults. We report a 13-year-old African-American female with sickle cell disease who developed bilateral hearing loss, with magnetic resonance imaging findings consistent with bilateral glomus tympanicum (GT). Upon excisional biopsy, however, EMH was diagnosed histologically. Besides its novelty, this case highlights the importance of considering EMH in the differential diagnosis of GT inclu

Diffusion weighted imaging for the detection and evaluation of cholesteatoma.

Cholesteatoma is a collection of keratinous debris and stratified squamous epithelium. It is trapped in the middle ear and can lead to bony erosion. The disease is treated surgically often followed by a second-look procedure to check for residual tissue or recurrence. Cholesteatoma has specific signal-intensity characteristics on magnetic resonance imaging with very high signal intensity on diffusion weighted imaging (DWI). Various DWI techniques exist: Echo-planar imaging (EPI)-based and non-EPI-based techniques as well as new approaches like multi-shot EPI DWI. This article summarizes all te

Demodulation of quasi-quadrature interferometric signals for use in the totally implantable hearing aids.

We propose and experimentally prove an algorithm for demodulation of interferometric signals, modified for use in a totally implantable hearing aid device. A fiber optic configuration, which generates two quasi-quadrature signals by a passive 3x3 coupler, for a non-contact detection of the middle ear ossicle vibration is employed. We simulated the ossicle vibration and large movements and demonstrated the effectiveness of the algorithm to compensate changes of the signal DC values and the phase shift introduced by the coupler. Applying the proposed algorithm we obtained the output signal stabi

Sudden Sensorineural Hearing Loss: A Hearing Emergency.

Sudden sensorineural hearing loss (SSNHL) is a rare but emergent condition that afflicts approximately 4,000 Americans annually (Ciorba, Faita, Bianchini, Aimoni, & Scanelli, 2013; Scapa & Friedland, 2009; Stachler et al., 2012). It causes unilateral nerve loss of hearing that may not recover left untreated. Because SSNHL is uncommon, practitioners may be unaware of the urgency or the protocol for immediate testing and treatment. Aural fullness, a common presenting symptom of SSNHL, overlaps with more common outer and middle ear problems resulting in misdiagnosis and/or delayed diagnosis and t

Primary tuberculosis of the eustachian tube causing otitis media with effusion.

Eustachian tube (ET) dysfunction may cause pathological changes in the middle ear, including recurrent acute otitis media and otitis media with effusion (OME). Mechanical obstruction of the ET may be caused by primary tumor-like lesions arising from ET or secondary ET infiltration due to nasopharyngeal and parapharyngeal space tumor. Tuberculosis is known to affect almost every organ in the body, and it should be a concern of each and every medical practitioner. However, tuberculosis of the ET has not been reported in the literature previously. This article reports primary tuberculosis arising

Audiological investigation of otitis media in children with atopy.

Otitis media (OM) describes an inflammatory process within the middle ear space that is generally associated with accumulation of fluid and that may lead to hearing loss, learning difficulties, and delays in language development. The pathogenesis of OM is multifactorial, involving the adaptive and native immune system, eustachian tube dysfunction, viral and bacterial load, and genetic and environmental factors. The involvement of IgE-mediated allergic reactions in the pathogenesis of OM has been suggested by clinical observations of a high prevalence of OM among patients with allergies. Eviden
