[Neurophysiology in Creutzfeldt-Jakob disease].

INTRODUCTION: Neurophysiological studies conducted in subjects who it is suspected are suffering from Creutzfeldt-Jakob disease (CJD) are usually aimed at searching for what is called a typical EEG. However, the EEG is a dynamic test and therefore subject to variations in time. Furthermore, there are other kinds of neurophysiological tests that may also be of interest, and the lack of typical traces in the new variant of the disease forces us to explore other diagnostic approaches. METHOD: We performed a clinical EEG correlation in the course of the evolution of the disease, in which we obser

Infantile epileptic syndromes and metabolic etiologies.

Inherited metabolic disorders can cause onset of epilepsy in the first year of life. Epilepsy rarely dominates the clinical presentation, which is more frequently associated with other neurologic symptoms, such as mental retardation, hypotonia and/or dystonia, or vigilance disturbances. The pathogenesis of seizures is multifaceted; inherited metabolic disorder can affect the balance between excitatory and inhibitory chemical mediators, eliminate an energetic substrate at the cerebral level, cause in utero brain malformation, or provoke acute brain lesions. Some clinical disorders that strongly

[Continuous nodal reciprocal rhythm and Steinert's disease: treatment by double chamber stimulation. A case report].

We report a case of continuous supraventricular tachycardia in a patient affected by Steinert's myotonic dystrophy. The investigation of this tachycardia showed that there was a "slow-fast" common nodal re-entry, rendered continuous by the existence of significant conduction defects in the fast pathway and the slow anterograde pathway. Implantation of a double chamber cardiac stimulator, necessary for conduction defects present in the basal state in this patient allowed, with the evolution of the conduction defects, the complete eradication of reciprocal rhythm entry, without resorting to abla

Pitfalls in diagnosis of epilepsy of Janz and its implications.

84 patients of juvenile myoclonic epilepsy (JME) of Janz were studied. Diagnosis was confirmed using clinical and electro-encephalographic (EEG) criterias. 58 (78%) patients of JME were referred as 'refractory or uncontrolled seizures'. Ignoring myoclonic episodes and non-use of activation procedures in EEG were important reasons for diagnostic delay. Sodium valproate (VPA) or clonazepam are the drugs of choice while phenobarbitone (PB), carbamazepine (CZ), and phenytoin (PHT) are ineffective. Clinical spectrum of JME is slightly different in India. Family history of epilepsy or JME is not for

The effects of caloric vestibular stimulation on EEGs in patients with central vertigo.

The EEGs of 30 patients with central vertigo were examined by caloric vestibular stimulation (CVS). The control group consisted of 30 persons without vertigo disturbances. In the standard procedure (and after hyperventilation), there was no statistically significant difference between the two groups of patients. In comparison to the EEGs of healthy persons, CVS produced statistically significant differences ( P<0.001) in the EEGs of patients with vertigo.

Antisense studies of brain GABAA receptors.

gamma-Aminobutyric acid (GABA) is the principal inhibitory neurotransmitter in the brain. The GABAA receptor complex, which is assumed to have a pentamer structure assembled from different polypeptide subunits, contains the binding sites for several clinically important compounds, e.g., the benzodiazepines and the barbiturates. A dysfunction of GABAergic inhibitory neurotransmission mediated via the GABAA receptor has been hypothesised to be a central factor in the pathogenesis of epilepsy. Antisense technology is based on the possibility of selectively inhibiting gene expression at the level

A study on the prevalence of accessory auricle anomaly in Turkey.

OBJECTIVE: Accessory auricular anomaly is a small elevation of skin containing a bar of elastic cartilage localized most commonly just anterior to the tragus or ascending crus of the helix. The anomaly may exist isolated or may be associated with other congenital anomalies of the first arch. The purpose in this study is to detect prevalence of accessory auricle in Turkey and find out whether it is associated with other craniofacial anomalies or hearing loss. METHODS: The study was performed on 850 children from the age of 7 to 9 during a screening program in primary schools. Complete otolaryn

Photic driving in the electroencephalogram of children and adolescents: harmonic structure and relation to the resting state.

In order to identify latent bioelectrical oscillators, 15 normal subjects (aged 9-17 years, 8 males, 7 females) were subjected to intermittent photic stimulation. The EEG amplitude spectra corresponding to the 11 fixed frequencies of stimulation presented (3-24 Hz) were combined to form "profiles" of the driving reaction in the right occipital area. The driving response varied with frequency, and was demonstrable in 70-100% of cases (using as criterion peak amplitudes 20% larger than those of the neighbors). The strongest responses were observed at the frequency closest to the alpha peak of th

Diazepam-induced changes in sleep: role of the alpha 1 GABA(A) receptor subtype.

Ligands acting at the benzodiazepine (BZ) site of gamma-aminobutyric acid type A (GABA(A)) receptors currently are the most widely used hypnotics. BZs such as diazepam (Dz) potentiate GABA(A) receptor activation. To determine the GABA(A) receptor subtypes that mediate the hypnotic action of Dz wild-type mice and mice that harbor Dz-insensitive alpha1 GABA(A) receptors [alpha1 (H101R) mice] were compared. Sleep latency and the amount of sleep after Dz treatment were not affected by the point mutation. An initial reduction of rapid eye movement (REM) sleep also occurred equally in both genotypes

Charles Bonnet syndrome with auditory hallucinations: a diagnostic dilemma.

Charles Bonnet syndrome (CBS) appears to be commoner in elderly people than was previously thought. Nonetheless, as yet, no firm diagnostic criteria have been established for CBS, leaving the need for well-defined diagnostic criteria describing this syndrome unfulfilled. Two recently proposed diagnostic criteria for CBS insist on an absence of hallucinations in modalities other than visual. However, we experienced 2 cases suffering from probable CBS who experienced auditory hallucinations. In this report, we recommend that even if CBS coexists with other hallucinations, we should not exclude C

[Electro-clinical concordance for childhood absence epilepsy in monozygotic twins].

INTRODUCTION AND OBJECTIVE: The objective of the study was to analyze the EEG tracings during the seizures of two monozygotic twins, shown on molecular analysis, diagnosed as having infantile epilepsy with absences. The recordings are particularly interesting since the genotypes of the patients are identical. This allowed comparison between the similarities and differences in discharges so that they could be better defined. RESULTS: The 3 Hz spike and wave complexes were the same as the classical descriptions in typical absences. No 'W' spike and wave complexes morphology, discharge fragmenta

A laboratory study of sleep in Asperger's syndrome.

Asperger's syndrome (AS) is a pervasive developmental disorder that may fall along the autistic spectrum. We compared the sleep of eight patients with AS with that of participants matched for age and gender. Patients with AS showed decreased sleep time in the first two-thirds of the night, increased number of shifts into REM sleep from a waking epoch, and all but one patient showed signs of REM sleep disruption. EEG sleep spindles were significantly decreased while K complexes and REM sleep rapid eye movements were normal. Three patients with AS, but none of the comparison participants, showed

Absolute and relative predictor values of some non-invasive and invasive studies for the outcome of anterior temporal lobectomy.

BACKGROUND: There is controversy in the literature regarding the strategy used to obtain better outcomes after performing an anterior temporal lobectomy (ATL). Some investigators prefer to reduce the risks and costs of the predictor studies despite the fact that the number of patients cured after ATL (no seizures with no medication) is relatively small. Other investigators prefer to attempt a total cure in all patients by using all available predictor studies regardless of risks and costs. The latter strategy was the aim of the present work. METHODS: The absolute (%) and relative (ratio diffe

Correlation between clinical stages and EEG findings of subacute sclerosing panencephalitis.

In this retrospective study 67 patients with SSPE seen between the years 1980 and 1998 were reviewed. Using the criteria of SSPE diagnosis (clinical signs, characteristic EEG patterns, high titres of measles antibodies in the serum and CSF), the patients were divided into two groups. Group A fulfilled all criteria, however, due to the inability of measuring measles antibody before 1987, it was not possible to observe the third criterion in Group B. Among 67 patients, groups A and B consisted of 51 boys and 16 girls ranging in age between 1 to 23 years, mean age 13.1. The male/female ratio was

Coherence of gamma-band EEG activity as a basis for associative learning.

Different regions of the brain must communicate with each other to provide the basis for the integration of sensory information, sensory-motor coordination and many other functions that are critical for learning, memory, information processing, perception and the behaviour of organisms. Hebb suggested that this is accomplished by the formation of assemblies of cells whose synaptic linkages are strengthened whenever the cells are activated or 'ignited' synchronously. Hebb's seminal concept has intrigued investigators since its formulation, but the technology to demonstrate its existence had bee

[Autosomal dominant nocturnal frontal lobe epilepsy: the syndrome].

The identification of the autosomal dominant nocturnal frontal lobe epilepsy (ADNFLE) in 1994 was rapidly followed by that of other familial forms of non lesional partial epilepsies (familial temporal lobe epilepsy, autosomal dominant partial epilepsy with variable foci). Since then around forty families with ADNFLE have been described, most of them having only 3 or 4 affected individuals. The epilepsy usually begins during childhood (mean age at onset: 11 years). The seizures mainly consist of motor elements which can be dystonic, tonic or hyperkinetic (bipedal automatisms, pelvic thrashing m
