Hippocampal dysmetabolism contributes to cognitive loss in autoimmune encephalitis and focal temporal epilepsy.

INTRODUCTION: Autoimmune encephalitis (AE) is associated with severe cognitive disability. Brain metabolic dysfunction has been linked to encephalopathy in neurodegenerative disorders; however, its role in the development of cognitive loss in AE has not been studied. We hypothesized that cognitively impaired patients with AE will demonstrate altered brain metabolism and immune activation, and these measures will correlate with cognitive scores. METHODS: The hippocampal and cortical metabolites related to neuronal integrity, oxidative metabolism, and glial activation were assessed using single

Genome-wide consensus transcriptional signatures identify synaptic pruning linking Alzheimer's disease and epilepsy.

Alzheimer's disease (AD) and epilepsy (EP) share a complex bidirectional relationship, yet the molecular mechanisms underlying their comorbidity remain insufficiently explored. To identify potential transcriptional programs across animal models and human patients with AD and EP, we conducted a comprehensive genome-wide transcriptomic analysis. Our investigation included mouse models of temporal lobe epilepsy (pilocarpine- and kainic acid-induced; n = 280), AD transgenic models (7 transgenic models expressing human tau or amyloid pathology; n = 257), and performed cross-species validation in hu

Blood-brain barrier-associated serum proteins as potential diagnostic biomarkers of epilepsy aetiology.

AIM OF STUDY: We aimed to assess serum levels of proteins associated with the blood-brain barrier (BBB) in two aetiological populations of patients with epilepsy in the interictal period: genetic and structural-metabolic epilepsy. CLINICAL RATIONALE FOR STUDY: Proteins associated with the BBB are emerging as potential indicators and/or inducers of pro-epileptogenic changes within the central nervous system (CNS). Dysfunction of BBB is known to contribute to the development of epilepsy, while on the other hand dysregulated BBB might be the result of seizures. MATERIAL AND METHODS: Serum level

Risk of Poststroke Epilepsy Among Young Adults With Ischemic Stroke or Intracerebral Hemorrhage.

IMPORTANCE: Poststroke epilepsy (PSE) is a major complication among young adults and is associated with problems with functional recovery and daily life. Although scores have been developed to predict risk of PSE, they have not been validated among patients with stroke at a young age. OBJECTIVES: To investigate both the risk of and risk factors for PSE at a young age and validate current PSE risk scores among a cohort of young adults. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used data from ODYSSEY (Observational Dutch Young Symptomatic Stroke Study), a prospective cohort study co

Structural insights into heterohexameric assembly of epilepsy-related ligand-receptor complex LGI1-ADAM22.

Leucine-rich glioma-inactivated 1 protein (LGI1) is a secreted neuronal protein consisting of the N-terminal leucine-rich repeat (LRR) and C-terminal epitempin-repeat (EPTP) domains. LGI1 is linked to epilepsy, a neurological disorder that can be caused by genetic mutations of genes regulating neuronal excitability (e.g. voltage- or ligand-gated ion channels). ADAM22 is a membrane receptor that binds to LGI1 extracellularly and interacts with AMPA-type glutamate receptors via PSD-95 intracellularly to maintain normal synaptic signal transmission. Structural analysis of the LGI1-ADAM22 complex

Molecular mechanisms of neurosteroids in temporal lobe epilepsy: current insights and therapeutic perspectives.

Neurosteroids (NSs) are endogenous progesterone derivatives that act as natural allosteric modulators of GABAA receptors, exerting distinct effects from conventional receptor agonists. They play critical roles in modulating neuronal excitability, cognitive function, and stress responses, and have shown potential in mitigating seizure severity and frequency, particularly in temporal lobe epilepsy (TLE). TLE is a common drug-resistant focal epilepsy characterized by complex pathophysiology involving hippocampal sclerosis, neuroinflammation, synaptic dysfunction, and alterations in GABAA recept

Do Larger Resections Cut It? Relating Temporal Lobe Epilepsy Surgery and Seizure Outcome.

Anterior temporal lobe resection (ATLR) results in seizure freedom in half of individuals with drug-resistant temporal lobe epilepsy (TLE). Some investigators have suggested that larger resections lead to greater chance of seizure freedom, while others report no relationship. In this study, we examine the relationship between resection size and seizure freedom through (i) total volume analysis and (ii) a mass univariate regional approach.Patient demographics and resection volumes were collected for 283 patients who underwent subsequent ATLR, and seizure freedom was measured after 12 months. Disorders of arousal and sleep-related hypermotor epilepsy are interrelated. Some new viewpoints.

NREM parasomnias overlap with sleep-related hypermotor epilepsies in multiple ways. Both conditions are characterized by fragmented sleep; share symptomatic features; have family-, and population-genetic relationships; and NREM parasomnia-episodes are underlain by sleep-wake dissociation likely featuring SHE seizures as well. Most importantly, the two conditions have very similar links with NREM sleep's high homeostatic pressure periods closely related to sleep microstructure including sleep-like micro-arousals, the dynamics of cyclic alternating pattern A1 and slow wave bistability. While the

Microscale maps of bursting dynamics across human hippocampal slices from patients with epilepsy.

Neuronal firing patterning in the dentate gyrus of patients with epilepsy remains unknown at the microcircuit level. Advancements in high-density CMOS-based microelectrode arrays can be harnessed to study network activity with unprecedented spatial and temporal resolution. We use novel computational methods with high-density electrophysiology recordings to spatially map network activity of human hippocampal brain slices from six patients with mesial temporal lobe epilepsy. Two slices from the dentate gyrus exhibited synchronous bursting activity in the presence of low magnesium media with kain

Repeat Expansions with Small TTTCA Insertions in MARCHF6 Cause Familial Myoclonus without Epilepsy.

BACKGROUND: Familial adult myoclonus epilepsy (FAME) is a rare autosomal dominant disorder caused by the same intronic TTTTA/TTTCA repeat expansion in seven distinct genes. TTTTA-only expansions are benign, whereas those containing TTTCA insertions are pathogenic. OBJECTIVE: We investigated the genetic basis of dominant cortical myoclonus without seizures in two unrelated families. METHODS: Repeat-primed polymerase chain reaction (PCR), long-range PCR, and nanopore sequencing were used to detect and characterize expansions at known FAME loci. RESULTS: We identified a novel repeat expansion

Progression of Amyloid Accumulation in Late Adulthood Among People With Childhood-Onset Epilepsy.

BACKGROUND AND OBJECTIVES: Previous research has demonstrated increased brain amyloid plaque load in individuals with childhood-onset epilepsy in late middle age. However, the trajectory of this process is not yet known. The aim of this study was to determine whether individuals with a history of childhood-onset epilepsy show progressive brain aging in amyloid accumulation in late adulthood (Turku Adult Childhood-Onset Epilepsy study, TACOE). METHODS: Adults from a prospective population-based cohort of individuals with childhood-onset epilepsy, originally recruited 1961-1964, together with m

Methionine Sulfoximine as a Tool for Studying Temporal Lobe Epilepsy: Initiator, Developer, Attenuator.

Methionine sulfoximine (MSO) is a compound originally discovered as a byproduct of agene-based milled flour maturation. MSO irreversibly inhibits the astrocytic enzyme glutamine synthase (GS) but also interferes with the transport of glutamine (Gln) and of glutamate (Glu), and γ-aminobutyric acid (GABA) synthesized within the Glu/Gln-GABA cycle, in this way dysregulating neurotransmission balance in favor of excitation. No wonder that intraperitoneal administration of MSO has long been known to induce behavioral and/or electrographic seizures. Recently, a temporal lobe epilepsy (TLE) model bas

Juvenile Myoclonic Epilepsy Imaging Endophenotypes and Relationship With Cognition and Resting-State EEG.

Structural neuroimaging studies of patients with Juvenile Myoclonic Epilepsy (JME) typically present two findings: 1-volume reduction of subcortical gray matter structures, and 2-abnormalities of cortical thickness. The general trend has been to observe increased cortical thickness primarily in medial frontal regions, but heterogeneity across studies is common, including reports of decreased cortical thickness. These differences have not been explained. The cohort of patients investigated here originates from the Juvenile Myoclonic Epilepsy Connectome Project, which included comprehensive neur

Ictal scalp EEG patterns are shaped by seizure etiology in temporal lobe epilepsy.

OBJECTIVE: To investigate how etiology and seizure localization influence ictal scalp electroencephalographic (EEG) patterns in temporal lobe epilepsy (TLE). METHODS: We retrospectively analyzed ictal EEG features from 504 focal seizures recorded in 189 TLE patients with various etiologies who underwent resective surgery. RESULTS: For seizure onset patterns (SOPs), α/β onset was more common in the low-grade tumor group (38.4%) than in the hippocampal sclerosis (HS) group (14.1%, p < 0.001). The ictal EEG duration was shorter in the tumor group compared to the focal cortical dysplasia (FCD),

Twins with temporal lobe epilepsy: genetic contributions to hippocampal sclerosis and other subtypes.

Temporal lobe epilepsy is the most common focal epilepsy in adults. While temporal lobe epilepsy was historically perceived to have a largely acquired aetiology, growing evidence points to important genetic contributions. There are several temporal lobe epilepsy subtypes, including mesial temporal lobe epilepsy with or without hippocampal sclerosis, but the relative genetic contributions to each of these subtypes have not been directly studied. In this study, we use the classical twin model in 80 twin pairs where at least one twin had temporal lobe epilepsy. We assessed the genetic contributio
