Identifying Factors Influencing Decision Making in Patients Diagnosed with Carotid Body Tumors: An Exploratory Study.

BACKGROUND: Carotid body tumors (CBTs) are rare highly vascularized and slow enlarging tumors arising from the paraganglionic tissue at the carotid bifurcation. Main treatment options for CBTs are surgical resection or "wait and scan" strategy. The choice for either strategy may be equally good medically in many patients. A structured "shared decision making" (SDM) might be helpful for guiding patients. OBJECTIVES: To develop an SDM strategy for the surgical treatment, we aim to (1) identify considerations and factors involved in the decision making of patients with CBTs and (2) evaluate the

Characteristics and unmet care needs of unbefriended residents in long-term care: a qualitative interview study.

Objectives: This study explored the impact of being 'unbefriended' for residents in Canadian long-term care (LTC) homes. Residents are 'unbefriended' if they lack decision-making capacity and family or friends to act as their legal representative. Research suggests that unbefriended individuals may have unmet needs and experience poor quality of care due to their limited social support. Our specific objectives were to identify resident characteristics, their unmet care needs, and implications for quality of care and quality of life. Methods: We conducted semi-structured interviews with 39 LTC s

Opioid Use Disorder: Medical Treatment Options.

Opioid use disorder is highly prevalent and can be fatal. At least 2.1 million Americans 12 years and older had opioid use disorder in 2016, and approximately 47,000 Americans died from opioid overdoses in 2017. Opioid use disorder is a chronic relapsing condition, the treatment of which falls within the scope of practice of family physicians. With appropriate medication-assisted treatment, patients are more likely to enter full recovery. Methadone and buprenorphine are opioid agonists that reduce mortality, opioid use, and HIV and hepatitis C virus transmission while increasing treatment rete

Screening for familial cancer risk: Focus on breast cancer.

A breast or an ovarian cancer occurring at a young age and/or in a family where other cases preexist suggests that those patients should be candidates for screening for mutations. Despite decades of medical research, less than 30% of cases with a suggestive personal and/or family history of hereditary breast cancer have an identified causative gene mutation. The vast majority of these cases are due to a mutation in one of the highly penetrant breast cancer genes (BRCA1, BRCA2, PTEN, TP53, CDH1, and STK11) and various guidelines direct the management of these patients. A minority of cases are d

Incorporating truncating variants in PALB2, CHEK2, and ATM into the BOADICEA breast cancer risk model.

PURPOSE: The proliferation of gene panel testing precipitates the need for a breast cancer (BC) risk model that incorporates the effects of mutations in several genes and family history (FH). We extended the BOADICEA model to incorporate the effects of truncating variants in PALB2, CHEK2, and ATM. METHODS: The BC incidence was modeled via the explicit effects of truncating variants in BRCA1/2, PALB2, CHEK2, and ATM and other unobserved genetic effects using segregation analysis methods. RESULTS: The predicted average BC risk by age 80 for an ATM mutation carrier is 28%, 30% for CHEK2, 50% fo

Diversity of the clinical presentation of the MMR gene biallelic mutations.

Constitutional mismatch repair-deficiency, due to biallelic mutations of MMR genes, results in a tumour spectrum characterized by leukaemias, lymphomas, brain tumours and adenocarcinomas of the gastro-intestinal tract, occurring mostly in childhood. We report here two families illustrating the phenotypic diversity associated with biallelic MMR mutations. In the first family, two siblings developed six malignancies including glioblastoma, lymphoblastic T cell lymphoma, rectal and small bowel adenocarcinoma with onset as early as 6 years of age. We showed that this dramatic clinical presentation

Fibromyalgia and disability adjudication: no simple solutions to a complex problem.

BACKGROUND: Adjudication of disability claims related to fibromyalgia (FM) syndrome can be a challenging and complex process. A commentary published in the current issue of Pain Research & Management makes suggestions for improvement. The authors of the commentary contend that: previously and currently used criteria for the diagnosis of FM are irrelevant to clinical practice; the opinions of family physicians should supersede those of experts; there is little evidence that trauma can cause FM; no formal instruments are necessary to assess disability; and many FM patients on or applying for dis

How siblings of pediatric cancer patients experience the first time after diagnosis: a qualitative study.

BACKGROUND: Siblings of pediatric cancer patients have a higher risk of developing emotional, behavioral, and social problems. However, little is known about specific experiences of this population in the first time after diagnosis. OBJECTIVE: The purpose of this qualitative study was to describe the experiences of siblings of pediatric cancer patients in different areas of life in the first half-year after the cancer diagnosis. METHODS: Semistructured interviews were conducted with 7 siblings of pediatric cancer patients (ages 11-18 years). Siblings were asked about their experiences in the

Auditory vocal analysis and factors associated with voice disorders among teachers.

UNLABELLED: Teachers are professionals who demand much of their voices and, consequently, present a high risk of developing vocal disorders during the course of employment. OBJECTIVE: To identify factors associated with vocal disorders among teachers. METHOD: An exploratory cross-sectional study, which investigated 476 teachers in primary and secondary schools in the city of Salvador, Bahia. Teachers answered a questionnaire and were submitted to auditory vocal analysis. The GRBAS was used for the diagnosis of vocal disorders. RESULTS: The study population comprised 82.8% women, teachers wi

Constructing access in predictive medicine. Comparing classification for hereditary breast cancer risks in England, Germany and the Netherlands.

In the first decade of the twenty-first century, predictive forms of medicine, largely associated with genetics, have become increasingly prominent. This has given rise to questions about the social consequences of this development, for example with regard to the distribution of health care access. Drawing on qualitative interviews with clinic staff and public officials and on document analyses, we analyse how access to risk assessment and monitoring for hereditary breast cancer predispositions in Germany, the Netherlands and England is produced through the interaction of risk classification a

Approach to the diagnosis and management of lipoprotein disorders.

PURPOSE OF REVIEW: Disorders of lipoprotein metabolism are frequently encountered in clinical practice. Although the severe genetic hyperlipidemias are relatively infrequent, prompt recognition and treatment can prevent complications, such as atherosclerosis and pancreatitis. The secondary dyslipidemias, due to medication or other metabolic disorders (hypothyroidism, renal or hepatic diseases), must be identified and treated. With the growing epidemic of obesity, dyslipidemias are a component of the metabolic syndrome. RECENT FINDINGS: The stratification of cardiovascular risk now includes fa

Ehrlichia under our noses and no one notices.

Ehrlichia chaffeensis, an obligately intracellular bacterium, resides within a cytoplasmic vacuole in macrophages, establishes persistent infection in natural hosts such as white-tailed deer and canids, and is transmitted transstadially and during feeding by ticks, particularly Amblyomma americanum. Ehrlichial cell walls contain glycoproteins and a family of divergent 28 kDa proteins, but no peptidoglycan or lipopolysaccharide. The dense-cored ultrastructural form preferentially expresses certain glycoproteins, including a multiple repeat unit-containing adhesin. Ehrlichiae attach to L-selecti

Bayesian analysis for cystic fibrosis risks in prenatal and carrier screening.

PURPOSE: Risk assessment is an essential component of genetic counseling and testing, and Bayesian analysis plays a central role in complex risk calculations. We previously developed generalizable Bayesian methods to calculate the autosomal recessive disease risk of a fetus when one or no mutation is detected, and another, independent risk factor is present. Our methods are particularly useful for calculating the CF disease risk for a fetus with echogenic bowel. In genetics practice, however, there are other scenarios for which our previous methods are inadequate. METHODS AND RESULTS: We prov

Colposcopic accuracy in a residency training program: defining competency and proficiency.

BACKGROUND: The determination of proficiency in procedural skills has evoked keen interest, but meaningful guidelines are limited by the absence of pertinent clinical data. Colposcopic accuracy is defined as the clinical correlation between a colposcopic impression and a histologic report. The colposcopic accuracy of physicians in a university-based family practice residency colposcopy program was evaluated. METHODS: Demographic information, clinical findings, and laboratory results of patients evaluated by colposcopy were prospectively recorded. Colposcopic accuracy was calculated based on t

From Calling to Calculated Choice: Generational Shifts in Career Perceptions of Rural Practice Among Indian Doctors.

ObjectiveTo explore how three generational cohorts of Indian medical practitioners perceive rural practice and identify the feedback mechanisms driving these shifts across career stages. We frame these shifts as a question of everyday health system resilience: the system's capacity to sustain rural service delivery under chronic, accumulating stress rather than discrete shocks.MethodsExploratory qualitative study using semi-structured in-depth interviews, by telephone or video across nine Indian states. Forty-four participants formed three groups: senior rural health leaders with 20+ years' ex

Integrating Oral Health Into Occupational Therapy: Knowledge, Practices, Barriers and Facilitators-A Systematic Review.

BACKGROUND: Oral health is essential for overall wellbeing and daily functioning, yet its integration into occupational therapy remains limited. Despite the profession's potential to promote oral health through person-centred, occupation-based approaches, evidence of this integration is scarce. This systematic review explores occupational therapists' (OTs) knowledge and practices in oral health care and examines the barriers and facilitators to incorporating oral health care into their clinical practice. METHODS: A comprehensive search was conducted across four databases: MEDLINE (Ovid), CINA

Service-level and practitioner-level recommendations for better care of people with dementia who live alone and have no informal caregivers.

CONTEXT: A substantial proportion of people with dementia live alone without informal caregivers, yet services often assume family involvement, increasing risks of unmet need and adverse outcomes. OBJECTIVE: This study aimed to generate service-level and practitioner-level recommendations to best support people with dementia who live alone and lack informal caregivers. METHODS: A qualitative phenomenological co-design workshop was conducted with seven practitioners from statutory, voluntary, and private-sector roles across England. Prompts informed by earlier qualitative work were used as a

Knowledge and Practice Regarding Stroke Prevention and Associated Factors Among Hypertensive Patients at Selected Public Hospitals in Addis Ababa, Ethiopia: A Cross-Sectional Study.

BACKGROUND AND AIMS: Stroke is a leading cause of death and disability worldwide, and hypertension is its strongest modifiable risk factor. The protection that treatment offers, however, depends on whether patients recognize their own risk and sustain preventive behavior. This has not been examined in Addis Ababa, where patients have comparatively good access to specialist follow-up. We assessed knowledge and practice regarding stroke prevention at two public hospitals in the city, and the characteristics associated with each. METHODS: An institution-based cross-sectional study was conducted

Palliative sedation as a cultural solution in Israel: a philosophical-ethical inquiry into end-of-life practice in a multicultural society.

Palliative sedation (PS) is a morally complex end-of-life practice that sits at the intersection of medicine, law, religion, and culture. While often framed as a purely clinical intervention aimed at alleviating intractable suffering, this article argues that PS functions—particularly in pluralistic societies such as Israel—as a form of ethical mediation and cultural accommodation. Through a philosophical-theoretical inquiry, this paper explores PS not as a universally justified act, but as a “cultural solution”: a pragmatic ethical practice that enables care to proceed amid deep moral disagre

The fine line between deliberate play and deliberate practice within talent development in grassroots football: evidence from a one-year longitudinal case study.

INTRODUCTION: While former studies into talent development in football have considered elite clubs and retrospective studies, this single-case study provides detailed data on the training activities of a 13-year-old male football player in a grassroots club who later became a professional at the highest level in Norway. METHODS: An intrinsic longitudinal single-case study design was adopted to develop an in-depth, contextually grounded understanding of the load and forms of physical activity within and beyond organized football as performed and experienced by one specific player over the cour
