Leptospira interrogans Aer2: an Unusual Membrane-Bound PAS-Heme Oxygen Sensor.

In this study, we provide the first characterization of a chemoreceptor from Leptospira interrogans, the cause of leptospirosis. This receptor is related to the Aer2 receptors that have been studied in other bacteria. In those organisms, Aer2 is a soluble receptor with one or two PAS-heme domains and signals in response to O2 binding. In contrast, L. interrogans Aer2 (LiAer2) is an unusual membrane-bound Aer2 with a periplasmic domain and three cytoplasmic PAS-heme domains. Each of the three PAS domains bound b-type heme via conserved Eη-His residues. They also bound O2 and CO with similar aff

Exploring Participants' Experiences of a Web-Based Program for Bulimia and Binge Eating Disorder: Qualitative Study.

BACKGROUND: Guided cognitive behavioral self-help is a recommended first-line treatment for eating disorders (EDs) such as bulimia nervosa (BN) or binge eating disorder (BED). Online versions of such self-help programs are increasingly being studied in randomized controlled trials (RCTs), with some evidence that they can reduce ED symptoms, although intervention dropout is variable across interventions. However, in-depth research into participants' experiences and views on the acceptability of web-based interventions is limited. OBJECTIVE: This is a qualitative process study of participants'

Rates of switching to second-line antiretroviral therapy and impact of delayed switching on immunologic, virologic, and mortality outcomes among HIV-infected adults with virologic failure in Rakai, Uganda.

BACKGROUND: Switch from first to second-line ART is recommended by WHO for patients with virologic failure. Delays in switching may contribute to accumulated drug resistance, advanced immunosuppression, increased morbidity and mortality. The 3rd 90' of UNAIDS 90:90:90 targets 90% viral suppression for persons on ART. We evaluated the rate of switching to second-line antiretroviral therapy (ART), and the impact of delayed switching on immunologic, virologic, and mortality outcomes in the Rakai Health Sciences Program (RHSP) Clinical Cohort Study which started providing ART in 2004 and implement

[Modern methods of early screening for preeclampsia and pregnancy-induced hypertension--a review].

Preeclampsia remains to be a serious perinatal complication and early screening for this disease to identify the high risk population before the first symptoms develop constitutes a considerable clinical challenge. Modern methods of screening for preeclampsia and pregnancy-induced hypertension include patients history biochemical serum markers and foetal DNA and RNA in maternal serum. They aid the process of developing an optimal protocol to initiate treatment in early pregnancy and to reduce the rate of complications. Our review presents an overview of the novel methods and techniques used fo

Discrimination of multichannel-compressed speech in noise: long-term learning in hearing-impaired subjects.

OBJECTIVE: It takes time for an individual to obtain optimal benefit from a new hearing aid. This research examines the possibility that similar long-term learning can be seen in consecutive laboratory studies of multichannel compression (MCC). DESIGN: Three studies of different parameters of MCC processing, carried out over the period of 1 yr, included the same 15 hearing-impaired subjects and one identical MCC-processing condition. The full-range MCC had 8, 12, or 16 independent frequency channels, using a Robinson-Huntington compression algorithm. The City University of New York nonsense s

Identification and characterization of two functional domains in cytochrome P-450BM-3, a catalytically self-sufficient monooxygenase induced by barbiturates in Bacillus megaterium.

In a previous publication (Narhi, L. O. and Fulco, A. J. (1986) J. Biol. Chem. 261, 7160-7169) we described the characterization of a soluble 119,000-dalton P-450 cytochrome (P-450BM-3) that was induced by barbiturates in Bacillus megaterium. This single polypeptide contained 1 mol each of FAD and FMN/mol of heme and, in the presence of NADPH and O2, catalyzed the oxygenation of long-chain fatty acids without the aid of any other protein. We have now utilized limited trypsin proteolysis in the presence of substrate to cleave P-450BM-3 into two polypeptides (domains) of about 66,000 and 55,000

Retinal biomarkers in schizophrenia spectrum disorders: evidence and implications for the neurodevelopmental and neurodegenerative models.

Retinal morphological and functional alterations, such as changes in the thickness and volume of the retinal neural layers, architecture of the microvasculature, and functioning of neurons, have been observed in schizophrenia and have been interpreted in terms of neurodegenerative aspects of the disorder. However, little consideration has been given to the issue of whether, and the extent to which, these retinal differences may reflect neurodevelopmental features of schizophrenia. There are also no current conceptualizations that integrate retinal alteration findings in schizophrenia across di

Factors influencing the accuracy of clinical decision-making in dental trauma: knowledge, experience, and educational attitudes among medical students.

BACKGROUND: Traumatic dental injuries (TDI) require prompt and appropriate first aid; however, non-dental healthcare professionals often lack sufficient knowledge to manage such cases effectively. This study aimed to evaluate the accuracy of clinical decision-making among medical students in dental trauma scenarios and to identify factors influencing their performance, including knowledge, experience, and educational attitudes. MATERIALS AND METHODS: A cross-sectional survey of 291 medical students assessed knowledge, decision-making accuracy, and educational attitudes toward dental trauma ma

An assessment of autistic and parkinsonian movement profiles to inform selective classification algorithms.

BACKGROUND: Movement differences in autism have attracted growing attention in recent years. Anecdotally, autistic movement has been likened to that of Parkinson's Disease (PD). Given that PD assessments are primarily movement-based, it is important to ensure that autistic individuals are not scoring highly on PD diagnostic criteria due to autism-related movement differences. Quantifying overlap in movement profiles and identifying distinguishing features is essential, particularly given increased PD diagnosis rates in the autistic population. METHODS: We conducted the first direct comparison

Rationalisation of a thrombophilia panel using laboratory medicine Delphi-like consensus evaluation and secondary artificial intelligence-based simulation assessment.

OBJECTIVES: It is important to review laboratory test panels regularly and omit unnecessary tests. This avoids overdiagnosis and makes laboratory work more targeted. Artificial intelligence is increasingly being discussed as a possible aid in such decisions. The aim of the study was to revise an existing thrombophilia panel with the help of a modified Delphi consensus of laboratory physicians and to examine whether large language models (LLMs) can mimic such decision-making processes and serve as a support tool. METHODS: The study was conducted in two steps. First, six experts evaluated vario

Breast Health Training Program for Traditional Healers: A Pathway to Early Breast Cancer Detection in Tanzania.

BACKGROUND: In sub-Saharan Africa, traditional healers often serve as primary healthcare providers and are the first point of contact for patients. Given this, they are uniquely positioned to aid in early breast cancer detection. To evaluate this, we implemented a breast cancer training program to equip traditional healers in Tanzania with foundational knowledge and CBE skills, aiming to improve early detection and timely treatment. AIMS: To implement a breast cancer training program to equip traditional healers in Tanzania with foundational knowledge and CBE skills, aiming to improve early d

PREGO Protocol: Point-of-Care Ultrasound for Evaluating Third-Trimester Pregnancy in the Emergency Department.

BACKGROUND: Knowledge about a third-trimester pregnancy is essential for guiding management of a pregnant patient presenting to the emergency department (ED) in labor or with an acute obstetrical complaint. Unfortunately, patients in their third trimester may present without prenatal obstetric care or with a condition (e.g., ill, altered, or trauma) in which information about the pregnancy is not immediately available. Moreover, many rural and community hospitals do not have an obstetrician available during all hours of the day. Our proposed point-of-care ultrasound (POCUS) PREGO protocol prov

Daily-level associations between sleep hygiene, cannabis use, and sleep health outcomes.

OBJECTIVE: Many use cannabis to promote sleep despite insufficient evidence to recommend cannabis as a sleep-aid. Sleep health may instead be promoted through sleep hygiene, though research among those using cannabis is nascent. This study evaluated daily within-person associations among aggregate and individual sleep hygiene behaviors, cannabis use, and sleep outcomes. METHOD: Adults with regular cannabis use (N = 85) completed baseline measures and up to 14 daily diaries (n = 1,021) on cannabis use and sleep. RESULTS: Multilevel models indicated that aggregate sleep hygiene engagement was

Validating the Factor Structure of the Fujisawa Social Capital Scale (FSCS) Among Community-Dwelling Older Adults Participating in "Kayoi-no-ba": The Gunma Aging Cohort Study.

Social capital is a fundamental social determinant of health for older adults in rapidly aging societies; however, existing comprehensive instruments often impose high cognitive friction through complex recall tasks, potentially leading to systematically excluding vulnerable populations. This study validated the factor structure of the Fujisawa Social Capital Scale (FSCS) - an independent, brief tool designed for inclusive community-based practice - among community-dwelling Japanese older adults participating in "Kayoi-no-ba" (neighborhood social participation hubs). Data were analyzed from 58

Survival predictors in Vietnamese elderly AML patients treated with decitabine: Real-world evidence from a low- and middle-income country.

BACKGROUND: Hypomethylating agents such as decitabine represent a key treatment option for elderly acute myeloid leukemia (AML) patients who are unfit for intensive chemotherapy. However, real-world evidence from low- and middle-income countries (LMICs), including Vietnam, remains scarce. This study aimed to identify baseline clinical and laboratory predictors of 1-year overall survival (OS) in elderly Vietnamese AML patients treated with decitabine.
