Fixation of Fresh Femoral Neck Fractures Using Fibular Strut Graft Along with Cannulated Screws.

BACKGROUND: Intracapsular femoral neck fractures can result from trivial trauma in the elderly or high-energy trauma in younger age-groups. In younger patients aged <60 years, the femoral head should be conserved to avoid the long-term complications of replacement arthroplasty. The options of osteosynthesis include closed/ open reduction and internal fixation with/without bone grafting. Internal fixation alone does not provide rigid fixation, owing to cavities in the posterior part of the femoral head and neck. Fibular grafts augment union and provide strength to the posterior cortex during

[Combined Posteromedial and Anterolateral Approach in the "floating position" - Two Column Fracture of the Tibia Head].

Background The posterior tibial plateau is often affected in intra-articular tibial fractures. Moreover, treating these posterior fractures is recognised as an important prognostic factor. Open reduction and internal fixation of lateral and posterior two column tibial plateau fractures can be achieved via a combined reversed L-shape approach and an anterolateral approach in the floating position without intraoperative repositioning of the patient. Material and Methods The operative procedure starts in the posterior position with visualisation of the posterior column after lateral retraction of

Multiple Unilateral Upper Limb Fractures in the Pediatric Setting - A Case Report.

INTRODUCTION: Lateral condyle fractures and both bone forearm fractures account for 10-15% and 13-40% of all elbow fractures in children, respectively. About 5% of forearm fractures in children have associated supracondylar fractures; hence, any forearm fractures warrant a careful examination of the elbow, and any radiographs taken should visualize the elbow joint above and wrist joint below for other injuries. We report a case of multiple upper limb fracture in a child, comprising lateral condyle and both bone forearm fractures. CASE REPORT: A 5-year-old boy was admitted through the emergenc

Management of a rare complication after screw fixation of a pediatric tibial spine avulsion fracture: a case report with follow-up to skeletal maturity.

Avulsion of the tibial spine is functionally equivalent to rupture of the anterior cruciate ligament in an adolescent athlete. It therefore presents to general orthopaedists as well as a wide variety of orthopaedic subspecialty surgeons, including traumatology sports medicine, and pediatrics. Restoration of normal knee kinematics is dependent on anatomic reduction and fixation of the avulsed fragment. Because this injury is typically sustained by the skeletally immature patient, epiphyseal fixation is ideal to avoid physeal injury, which can lead to angular limb deformity. We present a case, t

Orthogonal plating of Vancouver B1 and C-type periprosthetic femur fracture nonunions.

BACKGROUND: Periprosthetic femoral shaft fractures are a significant complication after total hip arthroplasty (THA). Plate osteosynthesis has been the mainstay of treatment around well-fixed stems. Nonunions are a rare and challenging complication of this fixation method. We report the outcomes of a novel orthogonal plating surgical technique for Vancouver B1 and C-type periprosthetic fractures that previously failed open reduction internal fixation (ORIF). METHODS: A retrospective review identified all patients with Vancouver B1/C THA periprosthetic femoral nonunions from 2010 to 2015. Excl

Fixation of the neglected subcutaneous osteochondral fragment of large medial femoral condyle defect in adult male: A case report.

Knee osteochondral fractures are among the most common orthopaedic injuries, and their diagnosis and treatment is always open to debate among orthopaedic surgeons. In this case report, we described the surgical treatment and 2 years of follow-up of an adult male patient who underwent delayed osteochondral fixation with an iliac graft. A medial parapatellar incision was made to perform open reduction and internal fixation. The fracture surface at the medial condyle of the femur was debrided with a curette, followed by placement of the tricortical graft harvested from the iliac crest into the de

Long oblique/spiral mid-shaft metacarpal fractures of the fingers: treatment with cerclage wire fixation and immediate post-operative finger mobilisation in a wrist splint.

The cerclage wire technique of internal fixation for displaced long oblique/spiral metacarpal shaft fractures has not gained popularity for two reasons: many believe that wire migration is a real possibility and that the fixation technique is not rigid enough to allow immediate postoperative finger mobilisation. In this report, the authors review the results of 19 cases of long oblique/spiral mid-shaft metacarpal shaft fractures of the fingers treated by cerclage wires and immediate postoperative mobilisation of all finger joints. The study included 17 men and two women with a mean age of 35 (

Posterior approach in treating sacral fracture combined with lumbopelvic dissociation.

Type III Denis fracture of the sacrum is rare clinically, constituting approximately 16% of all sacral fractures. Because it is often complicated with neurologic injuries, treatment is crucial and difficult. Several surgical options are available for the treatment of type III Denis sacral fracture with lumbopelvic dissociation. The authors report 21 patients admitted to the hospital from February 2002 to May 2012 who had type III Denis sacral fracture combined with lumbopelvic dissociation. All of the patients were treated with posterior sacral lamina decompression, sacral nerve root decompres

[Fractures of the shaft of the humerus: systematic plate fixation. Anatomic and functional results in 156 cases and a review of the literature].

PURPOSE OF THE STUDY: Based on our experience with plate fixation of humeral shaft fractures and an analysis of the international literature, we attempted to answer the following questions. What functional outcome and what complications can be expected after surgery? Are the classical complications of open fracture surgery (screw fixation, wiring, plate fixation without compression.) as frequent after plate fixation using the Müller technique? MATERIAL AND METHODS: We reviewed our series of 156 humeral shaft fractures (61 p. 100 men, mean age 45 years) including 21 cases of multiple trauma an

Adjunctive IlluminOss Fixation for a Vancouver B1 Periprosthetic Femoral Fracture: A Case Report.

Open reduction and internal fixation with plates and intramedullary nails is often the standard approach for periprosthetic femoral shaft fractures. However, in patients with recent hip arthroplasty, these traditional methods may be invasive, difficult to position, or carry a higher risk of periprosthetic complications. The IlluminOss system (IlluminOss Medical, East Providence, RI, USA) offers a minimally invasive alternative that delivers a light-curable polymer through a balloon catheter that takes the shape of the intramedullary canal without the need for rigid instrumentation that may ind

Arthroscopic treatment of avulsed tibial spine fractures using a transosseous sutures technique.

Severely displaced tibial spine fractures should be treated surgically to restore joint congruity and cruciate integrity with reduction and fixation through an arthrotomy or arthroscopic techniques. Arthroscopy is preferred as it allows for accurate diagnosis and treatment of associated injuries and reduction and fixation of all types of tibial spine fractures while reducing the morbidity associated with open techniques. We report the clinical and radiographical results of 11 cases treated with a technique of arthroscopic internal fixation with non-absorbable sutures, after an average follow-u

Low-velocity gunshot wounds of the metacarpal: treatment by early stable fixation and bone grafting.

The results of early stable fixation of 64 metacarpal fractures due to low-velocity gunshot wounds in 49 patients were reviewed retrospectively. These reconstructive procedures were performed between 1 and 7 days after injury and involved stable internal fixation supplemented with primary iliac crest bone graft in 40 fractures with bone loss or comminution. Postoperatively, the hands were splinted with 90 degrees of metacorpophalangeal flexion while early interphalangeal motion was emphasized. Follow-up averaged 12 months, with a range of 4 months to 7 years. Primary bone union was achieved in

Minimally invasive posterior plate osteosynthesis of a periprosthetic distal tibia shaft fracture: case report and review of literature.

INTRODUCTION AND IMPORTANCE: Periprosthetic fractures of the femur and tibia after total knee arthroplasty (TKA) can be difficult to treat due to pre-existing conditions and implant-related factors. The reported rate of periprosthetic fractures of tibia and femur after TKA is between 0.3 to 2.5%, in which the tibia is less frequently affected. CASE PRESENTATION: This case report presents a patient with a distal tibia periprosthetic fracture in the presence of a long-stemmed revision TKA. Surrounding soft tissues were critical due to multiple previous surgeries with existing hardware due to a

Comparison of 39 post-traumatic tibia bone transports performed with and without the use of an intramedullary rod: the long-term outcomes.

Bone transport can be performed with an external fixator alone or with the monorail technique which entails the combination of a fixator and an intramedullary nail. The purpose of this study was to compare the complication rates and long-term outcomes of these methods. Two groups of patients, the external fixator (n = 21) and the monorail group (n = 18), were compared. The average follow-up period was 7.9 ± 5.6 years and the mean defect length 8.3 ± 3.1 cm. Healing was achieved in 19 (90%) and 13 (72%) of the fixator and monorail patients, respectively. Six patients underwent amputations becau

Role of Hybrid Fixator in Proximal Tibial Fractures: A Prospective Evaluation of Clinical and Radiological Outcomes.

Background Management of complex proximal tibial fractures is challenging due to periarticular comminution and associated soft-tissue compromise. Hybrid external fixation has been proposed as an alternative to internal fixation in such cases. This study aimed to evaluate the functional and radiological outcomes of hybrid external fixation in proximal tibial fractures. Methods This was a prospective observational study conducted at a tertiary care teaching hospital over one year. Thirty adult patients with periarticular or intra-articular proximal tibial fractures were treated using hybrid exte

Temporary Internal Stabilisation of Critical Long-bone Defects: A Strategy for Staged Limb Reconstruction.

BACKGROUND AND AIMS: Critical long-bone defects remain among the most demanding problems encountered in orthopaedic trauma and reconstructive surgery. These defects arise from a variety of causes-high-energy trauma, blast injuries, bone infection, or oncological resection-and their management almost always demands a staged approach involving infection control, soft-tissue handling, and eventual restoration of skeletal continuity. A fundamental challenge throughout this process is keeping the affected limb mechanically stable between the initial operation and the definitive reconstructive proce

Metatarsal fractures.

Metatarsal fractures can present with a variety of situations. Ranging from the relatively benign, isolated central metatarsal fracture to the crush injury leading to extensive damage of the soft tissue and osseous components, these fractures can cause a significant inconvenience to the patient. With the exception of fifth metatarsal base fractures, little standardization is available for the treatment of metatarsal fractures. Controversy still exists regarding the proper treatment of various patient populations for junctional fifth metatarsal fractures. This article discusses the fractures of
