The development and flux of the University of Minnesota Survivorship Program: progress, challenges, and opportunities.

For the past 30 years, the University of Minnesota's Cancer Survivorship Program has been dedicated to providing exceptional care to patients who have lived the cancer experience. Our model is consultative, risk-stratified, and oncologist-led but executed predominately by advanced practice providers. Care is personalized and serves three survivor populations: children, adults, and patients who received BMT with over 500 new patients evaluated annually. As guidelines and survivorship standards have changed, our clinical programs have evolved from a focus on survivorship care plans to supportive

[Cognitive function groups in older people and risk factors].

BACKGROUND: Risk stratification for cognitive impairment in communities originated from autopsies in cognitively healthy individuals. The Diagnostic and Statistical Manual of Mental Disorders, 5th edition (DSM-5) incorporates subjective and objective cognitive function (CF) measures to explore neurocognitive disorders (NCD). It can be applied in communities to explore the factors that may influence positively or negatively. OBJECTIVE: To obtain the relationship between groups of CF classified using the DSM-5 categories, and sociodemographic factors, habits, comorbidities and motor function in

Effects of speech tracking method using hearing aids for hearing disorder in older people.

OBJECTIVES: Speech tracking is defined as a complex act or task of listening in which the learner tracks the speech heard and repeats it as accurately as possible while paying attention to incoming contextual information. This ability involves auditory input, speech output, repetition, and divided attention. In this small-scale study, we aimed to use a speech-tracking method to train older adults with hearing loss and examine its effects on hearing loss from multiple perspectives, including hearing handicap, frailty, and neuropsychological testing. METHODS: Auditory rehabilitation was provide

Association of the triglyceride glucose index with all-cause and cardiovascular mortality in a general population of Iranian adults.

BACKGROUND: The triglyceride glucose (TyG) index is a new and low-cost marker to determine insulin resistant which may be a predictor of cardiovascular disease (CVD). Although available evidence showed that its association with CVD mortality (CVM) and all-cause mortality (ACM) may differ in different populations, scarce data are available in this regard specially in low and middle-income countries. PURPOSE: To examine the association between TyG index and risk of CVM and ACM in Iranians. METHODS: This prospective cohort study included 5432 adults (age ≥ 35 years) with no history of CVD event

The effectiveness of a nurse-led home-based cardiac rehabilitation based on instant information intervention in the prognosis of patients with TAVR: A randomized controlled trial protocol.

BACKGROUND: Patients with poor prognosis after TAVR surgery require high-quality cardiac rehabilitation. Digital home-based rehabilitation based on real-time information intervention can improve outcomes. However, the parameters of the text message intervention, such as text message content, intervention duration, text message frequency, and other parameters, may vary. METHODS: This is an evaluator-blinded randomized controlled trial. TAVR patients will be recruited from the cardiology intensive care unit of a hospital and randomly assigned to either the control group or the experimental grou

Evaluation and management of urinary incontinence in nursing home residents: unique considerations for an at-risk population.

Overactive bladder (OAB) is a common diagnosis in the nursing home (NH) population, with up to 77% of NH residents having OAB or associated incontinence. NH residents have high rates of medical comorbidities that can impact OAB, such as frailty, decreased cognitive function, and lower functional status. Aside from understanding medical comorbidities, the clinician must also be aware of the nuanced social and structural factors at play due to the NH environment and interactions with nursing staff and caretakers. Investigational Use of Real-World Data as a Hybrid Control in Pancreatic Ductal Adenocarcinoma From the Randomized Phase Ib/II MORPHEUS Trial.

Enrolling adequate numbers of patients into the control arm of randomized controlled trials (RCTs) often presents barriers. There is interest in leveraging real-world data (RWD) from electronic health records (EHRs) to construct external control (EC) arms to supplement RCT control arms and form hybrid control (HC) arms. This investigation showed the use of an HC arm in second-line metastatic pancreatic ductal adenocarcinoma (PDAC). The RCT experimental arm (atezolizumab + PEGylated recombinant human hyaluronidase (Atezo + PEGPH20)) was compared with an HC arm consisting of patients treated wit

Prevalence and relevance of cachexia as diagnosed by two different definitions in patients undergoing hemodialysis: A retrospective and exploratory study.

OBJECTIVE: Cachexia is present in various chronic diseases and is associated with decreased quality of life and increased risk of morbidity and mortality. However, evidence regarding the association of cachexia with prognosis in patients undergoing hemodialysis is limited. We assessed cachexia using two definitions and compared prevalence, functional impairment, and prognostic impact in patients undergoing hemodialysis. METHODS: We enrolled outpatients undergoing hemodialysis at two centers retrospectively. We assessed cachexia using the conventional cachexia (Evans' criteria) and the Asian W

[Risk and complication profiles of orthogeriatric patients in elective hip and knee joint replacement].

BACKGROUND: The care of geriatric patients undergoing elective orthopedic surgery is becoming increasingly more important due to demographic trends. Compared to geriatric traumatology, however, there are still no established orthogeriatric care models in Germany and therefore hardly any scientific data. The aim of this study was to describe the risk and complication profiles in older patients with elective hip and knee replacements. METHODS: In a prospective study data were collected from orthogeriatric patients with indications for elective hip and knee replacement surgery who fulfilled defi

Rapidly Progressive High-Grade Leiomyosarcoma in an Elderly Patient.

An 84-year-old female with a history of hypertension, diabetes, and hypothyroidism initially presented in November 2023, with a rapidly enlarging (19.5 cm) left proximal thigh mass. Biopsy diagnosed high-grade leiomyosarcoma, which doubled in size within two weeks, confirming aggressive biology. In January 2024, the patient, who had been ambulating independently one year prior to her diagnosis, underwent radical resection and femoral neurolysis, and initiated radiotherapy, without receiving neoadjuvant chemotherapy due to cachexia. Three months postoperatively, in April 2024, the patient prese

Sepsis in elderly patients: the role of neutrophils in pathophysiology and therapy.

Sepsis is among the most important causes of mortality, particularly within the elderly population. Sepsis prevalence is on the rise due to different factors, including increasing average population age and the concomitant rise in the prevalence of frailty and chronic morbidities. Recent investigations have unveiled a "trimodal" trajectory for sepsis-related mortality, with the ultimate zenith occurring from 60 to 90 days until several years after the original insult. This prolonged temporal course ostensibly emanates from the sustained perturbation of immune responses, persevering beyond the

Hand grip strength as a proposed new vital sign of health: a narrative review of evidences.

Hand grip strength (HGS) serves as a fundamental metric in assessing muscle function and overall physical capability and is particularly relevant to the ageing population. HGS holds an important connection to the concept of sarcopenia, which encompasses the age-related decline in muscle mass, strength, and function. It has also been reported to indicate the health of an individual. We reviewed the interplay between HGS and various health parameters, including morbidity and mortality, by carrying out a literature search on PubMed, Scopus and Google Scholar between 10 and 30 August 2023, to iden

Hand Grip Force-Time Curve Indicators Evaluated by Dynamometer: A Systematic Review.

BACKGROUND: Handgrip strength (HGS) is an indicator of muscular strength, used in the diagnosis of sarcopenia, undernutrition, and physical frailty as well as recovery. Typically, the maximum HGS value is used; however, recent evidence suggests the exploration of new indicators provided based on the force-time curve to achieve a more comprehensive assessment of muscle function. Therefore, the objective was to identify indicators of the HGS profile beyond maximum HGS, based on force-time curves, and to systematize knowledge about their applications to various types of samples, health issues, an

Low psoas muscle index has a negative impact on survival after minimal invasive esophagectomy for cancer in European men.

INTRODUCTION: Sarcopenia, a key component of frailty in cancer patients, is associated with complicated procedures and worse survival after esophageal resection. The psoas muscle index (PMI) has been implicated as a possible sarcopenia imaging marker. This retrospective study aims to elucidate the effect of PMI and BMI in a cohort in Europe after totally minimally invasive esophagectomy for cancer. METHODS: The study included 318 consecutive adult patients (261 men and 57 women) who underwent minimally invasive esophagectomy for cancer between January 2016 and April 2021 in a German Universit

Evaluation of waist-to-calf ratio as a diagnostic tool for sarcopenic obesity: a cross-sectional study from a geriatric outpatient clinic.

BACKGROUND: Proponents argue that a high waist-to-calf ratio (WCR) may indicate an imbalance between muscle and fat in the body, making it a potential predictor for sarcopenic obesity (SO). The WCR is a new index incorporating both measurements, providing a reliable approach for assessing the imbalance between abdominal fat and leg muscle mass. The present study aimed to examine the association of WCR with SO and reveal the predictive effect of SO in community-dwelling older adults. METHODS: The study population was composed of 234 geriatric outpatients with obesity. WCR was calculated by div

Nutrition in the prevention and treatment of skeletal muscle ageing and sarcopenia: a single nutrient, a whole food and a whole diet approach.

Loss of skeletal muscle strength and mass (sarcopenia) is common in older adults and associated with an increased risk of disability, frailty and premature death. Finding cost-effective prevention and treatment strategies for sarcopenia for the growing ageing population is therefore of great public health interest. Although nutrition is considered an important factor in the aetiology of sarcopenia, its potential for sarcopenia prevention and/or treatment is still being evaluated. Nutrition research for sarcopenia utilises three main approaches to understand muscle-nutrition relationships, eval

Clinical validation of C12FDG as a marker associated with senescence and osteoarthritic phenotypes.

Chronic conditions associated with aging have proven difficult to prevent or treat. Senescence is a cell fate defined by loss of proliferative capacity and the development of a pro-inflammatory senescence-associated secretory phenotype comprised of cytokines/chemokines, proteases, and other factors that promotes age-related diseases. Specifically, an increase in senescent peripheral blood mononuclear cells (PBMCs), including T cells, is associated with conditions like frailty, rheumatoid arthritis, and bone loss. However, it is unknown if the percentage of senescent PBMCs associated with age-a
