Structural Determinants of Telehealth Use Among Patients Presenting to an Urban Safety-Net Emergency Department.

BACKGROUND: Telehealth expanded rapidly during and after the COVID-19 era. However, adoption remains uneven in safety-net populations, where structural barriers may limit use and worsen disparities. OBJECTIVE: To identify structural and healthcare access factors associated with telehealth use among adults presenting to an urban safety-net emergency department (ED) and evaluate whether telehealth use was associated with ED utilization. METHODS: We conducted a cross-sectional survey of adults presenting to an urban safety-net ED in the Bronx, New York, between July and November 2025. The surve

Understanding drivers of emergency department visits among patients from populations disproportionately affected by acute care use during cancer treatment: insights from qualitative interviews.

PURPOSE: Emergency department (ED) visits are frequent during cancer treatment, burdensome for patients and caregivers, and disproportionately affect those from marginalized groups. While many such events may be preventable, rates in practice remain stubbornly high, and patient perspectives on the drivers of ED use remain underexplored, particularly among marginalized patients. METHODS: Within a quality improvement initiative aimed at reducing sociodemographic disparities in ED use, we conducted semi-structured interviews with adults receiving systemic anticancer therapy at a large academic m

Health Care Use by Immigrant Generation in Low-Income Children With Special Health Care Needs.

OBJECTIVE: We aimed to describe social stressors and health care access in children with special health care needs (CSHCN) from low-income households by immigrant generation and determine the association between immigrant generation and health care utilization. METHODS: This was a retrospective cross-sectional study of CSHCN living at or below 200% of the federal poverty level using the National Survey of Children's Health from 2018 to 2022. Exposures were demographics, immigrant generation (first-generation, second-generation, or nonimmigrant), social stressors, and health care factors. Outc

The effect of job aids on knowledge retention among Patent and Proprietary Medicine Vendors trained to administer injectable contraceptives: longitudinal results from implementation science in Nigeria.

BACKGROUND: To increase access to voluntary family planning (FP) services, Nigerian policymakers are debating how to task share injectable contraceptive services to drug shop owners known as Patent and Proprietary Medicine Vendors (PPMVs). Task sharing FP services to drug shops is a promising practice, but information is needed on how to ensure high quality FP services. This analysis assesses the effects of job aids on PPMVs' knowledge of injectable contraceptives 9 months after receiving a standardized training. METHODS: One hundred ninety-four PPMVs were trained on FP counseling and adminis

Clinical Transformation in Care for Patients With Sickle Cell Disease at an Urban Academic Medical Center.

This article demonstrates effects on utilization of a clinical transformation: changing locus of care from a dedicated sickle cell day unit to an approach that "fast-tracks" patients through the emergency department (ED) into an observation unit with 24/7 access. Retrospective quantitative analyses of claims and Epic electronic medical record data for patients with sickle cell disease treated at Thomas Jefferson University (inpatient and ED) assessed effects of the clinical transformation. Additionally, case studies were conducted to confirm and deepen the quantitative analyses. This study was

[Healthcare situation for children, adolescent and adult victims of sexual violence in Germany : A critical overview].

In Germany, since 2010 the debate regarding sexual assault victims, additionally due to extensive activities of the independent commissioner for questions regarding sexual abuse of children (Unabhängiger Beauftragter für Fragen des sexuellen Kindesmissbrauchs, UBSKM), has received increased attention from the public and the healthcare system. Sexual violence is a burdening experience regardless of age and can have a lasting impact on the physical, psychological and/or social life of victims. Empirically, it has been found that more than half of the adult population suffering from mental disord

Multilevel predictors of concurrent opioid use during methadone maintenance treatment among drug users with HIV/AIDS.

BACKGROUND: Ongoing drug use during methadone maintenance treatment (MMT) negatively affects outcomes of HIV/AIDS care and treatment for drug users. This study assessed changes in opioid use, and longitudinal predictors of continued opioid use during MMT among HIV-positive drug users in Vietnam, with the aim of identifying changes that might enhance program efficacy. METHODS: We analyze data of 370 HIV-positive drug users (mean age 29.5; 95.7% male) taking MMT at multi-sites. Opioid use was assessed at baseline, 3, 6, and 9 months using interviews and heroin confirmatory urine tests. A social

What do we know about 7vPCV coverage in Aboriginal and Torres Strait Islander children? A 2007 update.

In 2001, a publicly funded 7 valent pneumococcal conjugate vaccine (7vPCV) program commenced for Aboriginal and Torres Strait Islander children aged less than 2 years. This study updates early estimates of 7vPCV coverage in Aboriginal and Torres Strait Islander children using Australian Childhood Immunisation Register data between 31 December 2004 and 30 September 2007. We chose four 3-month birth cohorts of children and assessed their immunisation status at 12 months of age for pneumococcal conjugate vaccine and for 'fully immunised'. After the introduction of universal childhood conjugate pn

Unofficial policy: access to housing, housing information and social services among homeless drug users in Hartford, Connecticut.

BACKGROUND: Much research has shown that the homeless have higher rates of substance abuse problems than housed populations and that substance abuse increases individuals' vulnerability to homelessness. However, the effects of housing policies on drug users' access to housing have been understudied to date. This paper will look at the "unofficial" housing policies that affect drug users' access to housing. METHODS: Qualitative interviews were conducted with 65 active users of heroin and cocaine at baseline, 3 and 6 months. Participants were purposively sampled to reflect a variety of housing

Effect of a targeted obstructive sleep apnea consult to improve access and quality at a sleep disorders center.

STUDY OBJECTIVES: We sought to evaluate whether a targeted obstructive sleep apnea (OSA) consult (TOSAC) protocol that reduced sleep-specialist time spent with patients suspected of having OSA would improve quality of care compared with the previous clinical method for evaluation of patients internally referred for suspected OSA. METHODS: Prospective cohort design. TOSAC patients received oximetry and criteria screening prior to a split-night polysomnography and a 45-minute sleep-specialist consult, while control patients received consult, testing, and a follow-up visit (90 minutes of sleep-s

Inequalities in rural health care: differences in surgical intervention between metropolitan and rural Western Australia.

BACKGROUND: To compare surgical procedure rates between metropolitan and rural/remote residents in Western Australia (WA). METHODS: The WA Data Linkage System was used to identify all patients who underwent a procedure for cataract, ureteric calculi or urinary outflow obstruction symptoms for the time periods 1981-2000, 1981-1997 and 1981-1995, respectively. Age-standardized procedure rates were calculated and Poisson regression modelling was used to estimate effects of locality of residence and demographic covariates. RESULTS: Overall, rural/remote patients underwent first-time procedures f

Latinx Youth's Mental Health Needs and Socioeconomic Factors Associated with Service Utilization.

This study examined mental health needs and risk factors associated with service use among Latinx high school students in two cities in the United States. We explored how socioeconomic characteristics, school location, youth and parental nativity, and self-perceived clinical needs were associated with the odds of youths seeing a mental health provider. Data were collected from 306 Latinx youths during the 2018-19 school year. Most youths (78%) self-reported symptoms of anxiety, trauma, or depression above the clinical range. None of these clinical needs predicted service utilization. Youth exp
