Permanent Ocular Injury Following Paintball Pellet Hit: A Medicolegal Case.

Paintball is a ubiquitous recreation, with severe and occasionally irreversible injuries. In this study, a rare medicolegal case of paintball-related closed globe blunt ocular injury was described. An 18-year-old boy who was hit in his right eye by a paintball pellet presented with severe eye pain and blurred vision. Ophthalmologic examinations showed lid edema, conjunctival hyperemia, conjunctival laceration, subconjunctival hemorrhage, corneal edema, anterior vitreous hemorrhage, congested sclera, commotio retinae, vitreous hemorrhage, retinal hemorrhage, macular edema, and macular hole. Cerebrovascular disease in the era of left ventricular assist devices with continuous flow: risk factors, diagnosis, and treatment.

Cerebral infarction and hemorrhage are among the principal sources of morbidity and mortality associated with mechanical circulatory support. There has been particular concern with the recently reported increased rate of device thrombosis in continuous-flow left ventricular assist devices. The neurologic management of stroke among these patients has limited data to inform current clinical practices. In this investigation we review the available literature on stroke in patients with continuous-flow left ventricular assist devices, with a focus on treatment algorithms in the acute setting and fo

Clinical, epidemiological and prophylactic aspects of dyspepsia.

Dyspepsia or indigestion is a term that refers to a multitude of symptoms and disorders of the digestive system which cause suffering in many patients. Dyspepsia means pain or discomfort that is centrally localized in the upper abdomen. The following categories of symptoms may also be present: pain localized in the right or left hypochondrium, dysphagia, upper gastrointestinal bleeding, acute abdomen, symptoms of reflux, transit disorders. On the other hand, the term "discomfort" was defined in order to designate a condition characterized by or associated with early satiety, upper abdominal bl

Low-voltage pattern and absence of sleep-wake cycles are associated with severe hemorrhage and death in very preterm infants.

UNLABELLED: Amplitude integrated electroencephalogaphy (aEEG) is becoming an important tool for the assessment of cerebral activity in preterm newborns. Describing the relationship between early aEEG patterns and intraventricular hemorrhage (IVH) can improve our knowledge of neurological injury in the preterm newborn. The aim of this prospective study was to identify early changes in the aEEG in premature newborns that could be associated to severe neurological lesion/death. Preterm newborns with a birth weight ≤1,500 g and/or 32 weeks of gestation were included. aEEG monitoring was performed

Bleeding gastroduodenal ulcers in patients without Helicobacter pylori infection and without exposure to non-steroidal anti-inflammatory drugs.

BACKGROUND/AIM: A high risk of bleeding in Helicobacter pylori (H. pylori)-negative, non-steroidal anti-inflammatory drugs (NSAID)-negative ulcers highlights the clinical importance of analysis of the changing trends of peptic ulcer disease. The aim of the study was to investigate the risk factors for ulcer bleeding in patients with non-H. pylori infection, and with no NSAIDs use. METHODS: A prospective study included patients with endoscopically diagnosed ulcer disease. The patients were without H. pylori infection (verified by pathohistology and serology) and without exposure to NSAIDs and

Cerebral microbleeds and cognition in patients with symptomatic small vessel disease.

BACKGROUND AND PURPOSE: Cerebral microbleeds (CMBs) are common in cerebral small vessel disease. They may cause cognitive impairment, possibly via white matter tract disruption but previous studies have produced inconsistent results. We determined whether CMB number and location are associated with impaired cognition in symptomatic small vessel disease and whether any association was independent of other magnetic resonance imaging markers of small vessel disease. METHODS: One hundred sixteen patients with lacunar stroke and radiological leukoaraiosis were studied. Neuropsychological assessmen

Clinical characterization of canine platelet procoagulant deficiency (Scott syndrome).

BACKGROUND: Platelet function defects are rare causes of bleeding diatheses; however, disease prevalence might be underestimated because diagnosis requires assessment of specific parameters of platelet activation. OBJECTIVES: The goal of this study was to characterize the clinical presentation of canine Scott syndrome (CSS), an intrinsic platelet function defect first identified in a closed colony of German Shepherds (GSD). ANIMALS: Eleven (n = 6 female) client-owned GSD affected with CSS that sought veterinary care for one or more episodes of abnormal bleeding. METHODS: Retrospective revie

Management of late postoperative complications of bariatric surgery.

BACKGROUND: The prevalence of obesity is increasing worldwide and the past decade has witnessed an exponential rise in the number of bariatric operations performed. As a consequence, an increasing number of patients are presenting to non-specialist units with complications following bariatric procedures. This article outlines the management of the most common late postoperative complications that are likely to present to the general surgeon. METHODS: A search was conducted for late postoperative complications after bariatric surgery using PubMed, Embase, OVID and Google search engines, and co

Spleen deflation and beyond: the pros and cons of Janus kinase 2 inhibitor therapy for patients with myeloproliferative neoplasms.

The myeloproliferative neoplasms (MPNs) essential thrombocythemia (ET), polycythemia vera (PV), and primary myelofibrosis (MF) are malignancies that frequently harbor the recurrent somatic point mutation JAK2(V617F). The discovery of this mutation has fueled the development of Janus kinase 2 (JAK2) inhibitors. Available results have indicated that JAK2 inhibitors are particularly effective at reducing spleen size. However, the activity of these agents is multifaceted and also involves a marked improvement of systemic symptoms and, for those agents with dual JAK1 and JAK2 inhibitory activity, a

Anosognosia for hemiplegia with preserved awareness of complete cortical blindness following intracranial hemorrhage.

A 51-year-old woman presented with anosognosia for hemiplegia (AHP), neglect, and a complete loss of vision, for which she was almost immediately aware. Neuroimaging studies revealed intracranial hemorrhages in the medial temporal lobes bilaterally, extending back to the occipital cortex, but sparing the calcarine cortex. A large right frontal-parietal hemorrhage which extended to the posterior body of the corpus callosum was also observed. The patient's vision slowly improved, and by 11 months post onset, formal visual fields revealed improvement primarily in the left upper quadrants only. In

Bilateral trochlear nerve palsies following dorsal midbrain haemorrhage.

Bilateral trochlear nerve palsies without other signs of dorsal midbrain syndrome following spontaneous midbrain haemorrhage is extremely rare. We report the case of a 37-year-old man with bilateral trochlear nerve palsies causing superior oblique palsies (SOP) from dorsal midbrain haemorrhage which recovered with conservative management. The report highlights the need for imaging in patients with spontaneous bilateral superior oblique (BSO) motility deficits.

Detection of silent cerebrovascular disease refines risk stratification of hypertensive patients.

OBJECTIVE: Detection of preclinical hypertension-related cardiorenal damage has been recommended in the identification of patients most at risk of cardiovascular complications. The inclusion of silent cerebrovascular disease (SCD) as an additional marker of hypertensive organ involvement might improve risk stratification. METHODS: In 192 hypertensive patients (98 men) without a history of cardiovascular and cerebrovascular disease, a mean age of 51.6 +/- 12.3 years and untreated office blood pressure levels of 170 +/- 23/104 +/- 12 mmHg, we obtained detailed information on preclinical cardiac

[Diffuse alveolar hemorrhage in the immunocompetent host: diagnostic and therapeutic management].

Diffuse alveolar hemorrhage is a bleeding originating from the pulmonary acinus. Number of causes are possible, that can be divided in immune and non immune causes. Immune mediated diffuse alveolar hemorrhages are mainly due to small vessels vasculitis (Wegener granulomatosis, microscopic polyangiitis), systemic lupus erythematosus and antiglomerular basement membrane antibody disease. Early immunosuppressive treatment is required, mostly with pulse methylprednisolone and cyclophosphamide. Plasmapheresis are added in antiglomerular basement membrane antibody disease and refractory systemic lup

Complications in septoplasty.

The most frequent complications of septoplasty are deformities, infections, and perforations. The effects of each of these complications, however, can be very different. Dislocations and deformities of the septum may result not only in an impaired airway but also in visible deformities of the entire nasal base and dorsum. A patient who underwent septoplasty can be "stigmatized." Infections may lead not only to septal abscess but also to endocranial complications such as meningitis or septicemia with endocarditis. Permanent perforations of the nasal septum can result in significant symptoms if

[Dissociation of conceptual and linguistic information based on a case study].

INTRODUCTION: Grouping the stimuli that we perceive in abstract categories is basic to understand our surroundings. Research gathered in the last few years with neuroimage techniques and brain damaged patients shows the role of the left temporal area in the organization of the semantic categories. CASE REPORT: A young aphasic patient study is reported who, after a cerebral haemorrhagia in the left temporo-parieto-occipital area, shows significant semantic difficulties. The results of this woman in several linguistic and semantic tasks indicate that her difficulties are conceptual rather than
