Prevalence and risk factors for obesity after liver transplantation: a single-center experience.

BACKGROUND: The study of weight gain after transplantation and its associated factors is necessary to propose strategies to prevent and treat this problem. OBJECTIVES: This study aims to investigate factors affecting the development of obesity after liver transplantation (LTx). PATIENTS AND METHODS: Medical records of 343 liver transplantation cases, which were followed between January 2001 and January 2010 at Dokuz Eylul University, were retrospectively analyzed. Patient pre-liver transplantation height, body weight, body mass index (BMI) measurements, as well as changes in body weight at t

The breakthrough phenomenon during alpha-interferon therapy of chronic hepatitis C: incidence, management, and outcome.

OBJECTIVES: Some patients treated with alpha-interferon (alpha-IFN) for chronic hepatitis C (CHC) initially respond with normalization of ALT only to encounter a rise in ALT while still on the drug. This phenomenon is called breakthrough (BT). We reviewed our experience with BT to clarify its incidence, pathogenesis, management, and outcome. METHODS: Charts from 71 consecutive patients with CHC treated with alpha-IFN were reviewed. Forty of these patients were part of a study of 1-yr escalating dose alpha-IFN, initiated at 2 million units (MU) 3 times per week. Endpoints that were evaluated w

Frequency and associated factors of hepatic hemangioma in systemic lupus erythematosus patients undergoing abdominal imaging: a retrospective cohort study.

Hepatic hemangioma may be detected in patients with systemic lupus erythematosus (SLE). This study aimed to determine the frequency, radiological characteristics, and potential associated factors of hepatic hemangioma in SLE patients undergoing abdominal imaging. We retrospectively evaluated 221 of 402 SLE patients followed at a tertiary university hospital who underwent abdominal imaging for clinical indications. Demographic, clinical, laboratory, and radiological data were collected from medical records. Factors associated with hepatic hemangioma were analyzed. A total of 221 SLE patients wh

[Diabetes mellitus and chronic liver diseases. Part 1: general mechanisms of etiology and pathogenesis].

In recent years there has been an active discussion about the relationship between diabetes mellitus (DM) and chronic liver diseases (CLD). On the one hand, patients with diabetes have an increased risk of developing CLD. On the other hand, patients with CLD very often identify abnormal glucose metabolism which ultimately leads to impaired glucose tolerance and the development of diabetes. This review outlines potential causal relationships between some CLD and DM. Common mechanisms that provoke metabolic and autoimmune disorders in the development of various nosologies of the CKD group, leadi

[Epithelial-mesenchymal transition and liver fibrosis : guilty or not guilty?].

Liver fibrosis is due to excessive deposition of extracellular matrix by fibroblasts that are actived in response to chronic liver injury; including viral, autoimmune, toxic and mechanical stresses These cells produce many of the constituents of the matrix, including type I, type II and type V collagen and fibronectin. However, the precise origin of these fibroblasts is controversial. Quiescent hepatic stellate cells are generally believed to be the main source of fibrogenic cells. but accumulating evidence suggests that they are not the sole culprits. It was recently proposed that hepatocytes

Leflunomide for the treatment of rheumatoid arthritis and autoimmunity.

Leflunomide, a new oral immunomodulatory agent, is effective for the treatment of rheumatoid arthritis. Its mechanism of action in suppressing inflammation is based in its inhibition of dihydroorotate dehydrogenase, an enzyme responsible for de novo synthesis of pyrimidine containing ribonucleotides. It is the first disease-modifying antirheumatic drug approved for treatment of rheumatoid arthritis with an indication for retardation of joint damage by radiography. Side effects are generally mild and include diarrhea, rashes, reversible alopecia, and elevation of hepatic transaminases. Despite

[The microsomal antibody (LKM) in liver pathology: its association with other autoantibodies].

Twenty patients found LKM antibody positive during the routine screening for autoantibodies, were studied for the association with other autoantibodies. Sixteen sera (80%) were found positive either for ANA (50%) or SMA (50%) or both (20%). Eleven patients (55%) were over 40 years old, 3 patients were HBsAg+ve, 4 have toxic and alcohol-related liver disease. These results provide evidence that the LKM antibody is not confined to young people chronic active hepatitis, but is present also in CALD of adult with various etiology. Its association with other autoantibodies (especially ANA) suggest t

[Horton's disease. Current problems].

Much that has been written in the last few years concerning temporal arteritis has allowed us to better clarify the clinical picture, and for example frequency of hepatic involvement. The atypical forms of the disease such as those that involves large arteries, or those in which sedimentation rate is normal, are better known. It is in such atypical cases, particularly in a young patient, that an external carotid arteriography is most useful. Though the pathophysiology of the disease is not well understood, serologic and histologic arguments exist in favor of an autoimmune etiology, in which th

Macrophage-CD8⁺ T Cell Spatial Coupling Defines an Innate-Adaptive Injury Niche in Human Checkpoint Inhibitor Hepatotoxicity.

BACKGROUND & AIMS: Immune-mediated liver injury from immune checkpoint inhibitors (ILICI) is a major immune-related adverse event that limits cancer immunotherapy, yet its tissue-level immunobiology is poorly defined and its management is largely extrapolated from autoimmune hepatitis (AIH). We previously identified a tri-cellular CD8+ T cell-macrophage-hepatocyte injury niche in a murine model of ILICI; here, we tested whether this niche is recapitulated in human disease. METHODS: We applied imaging mass cytometry with a 32-marker panel to liver biopsies from patients with ILICI (n = 12), AI

Features and outcome of AIH patients without elevation of IgG.

BACKGROUND & AIMS: High IgG levels are considered a hallmark of autoimmune hepatitis (AIH). A subgroup of patients with AIH has IgG within the normal range despite evidence of clinical disease activity. The clinical significance of this biomarker has not been explored. METHODS: In a European multicentre study we compared biochemical, clinical and histological features from patients with AIH and normal IgG-values at diagnosis to an age- and sex-matched control group of patients with typical AIH presenting with elevated IgG. Data were assessed at diagnosis, after 12 months of therapy and at las

[Lessons from macrophagic myofasciitis: towards definition of a vaccine adjuvant-related syndrome].

Macrophagic myofasciitis is a condition first reported in 1998, which cause remained obscure until 2001. Over 200 definite cases have been identified in France, and isolated cases have been recorded in other countries. The condition manifests by diffuse myalgias and chronic fatigue, forming a syndrome that meets both Center for Disease Control and Oxford criteria for the so-called chronic fatigue syndrome in about half of patients. One third of patients develop an autoimmune disease, such as multiple sclerosis. Even in the absence of overt autoimmune disease they commonly show subtle signs of

Remission of Nephrotic Proteinuria after Liver Transplantation in a Case of Abernethy Malformation Type Ib and Membranoproliferative Glomerulonephritis.

Abernethy malformation is a rare congenital vascular anomaly defined by the absence or severe hypoplasia of the portal vein, resulting in portosystemic shunting. The metabolic and immunologic consequences such as hepatic encephalopathy and pulmonary complications are well recognized. However, renal involvement of Abernethy malformation has been reported rarely . We report a 13-year-old boy who presented with severe anemia, pneumonia and nephrotic-range proteinuria (14.18 g/24 h). Imaging demonstrated type Ib Abernethy malformation, with complete agenesis of the portal vein and direct drainage

Rare Case Report of Ticagrelor-Induced Hepatotoxicity.

Ticagrelor, a potent P2Y12 receptor inhibitor, is used widely to manage acute coronary syndrome owing to its superior efficacy in reducing cardiovascular events compared to clopidogrel. Despite its favorable safety profile, this paper reports a case of ticagrelor-induced hepatotoxicity. A 66-year-old male with a history of pituitary macroadenoma presented with elevated liver enzymes during a preoperative assessment for tumor removal. The patient was asymptomatic. Ultrasonography revealed diffusely decreased parenchymal echogenicity with prominent portal venular wall echogenicity, suggesting ac

Behçet's disease and factor V Leiden: A thrombogenic synergy causing budd-chiari syndrome.

Behçet's Disease (BD) is a multisystem inflammatory disorder that can lead to severe vascular complications, including Budd-Chiari Syndrome (BCS), a rare but life-threatening condition characterized by hepatic vein obstruction. The co-occurrence of BD and inherited thrombophilia, such as Factor V Leiden mutation, significantly increases the risk of thrombosis, complicating the clinical management of affected individuals. In this case, a 16-year-old female initially presented with nonspecific symptoms of generalized fatigue and bone pain, which later progressed to abdominal distension and signi

Splenectomy Promotes Macrophage Polarization in a Mouse Model of Concanavalin A- (ConA-) Induced Liver Fibrosis.

BACKGROUND: Splenectomy can improve liver function and survival in patients with autoimmune hepatitis (AIH) and liver cirrhosis. We investigated the underlying mechanism in a mouse model of concanavalin A- (ConA-) induced liver fibrosis. METHODS: We used ConA to induce immune liver fibrosis in BALB/c mice. Splenectomy was performed alone or with the administration of dexamethasone (DEX). Changes in blood and liver tissues were evaluated. RESULTS: Mice treated with ConA for 7 weeks developed advanced liver fibrosis, while splenectomy suppressed liver fibrosis. Although the populations of macr

Hepatic Sarcodosis presenting as portal hypertension in a young boy.

A 13-year-old boy, known case renal stone disease came with the complaints of abdominal pain along with low grade fever. On examination, hepatosplenomegaly was noted while his lab reports showed a low hemoglobulin with a raised ESR. His blood and urine cultures showed no growth. Viral markers, autoimmune profile, C and p ANCA were all negative apart from a raised serum IgG level. Ultrasound abdomen showed a hyperechoic liver with an enlarged spleen along with splenic varices and minimum ascites. Ultrasound hepatic doppler was normal. Serum AFP levels were normal while workup for Wilson's disea

An unusual association between hemophagocytic lymphohistiocytosis, mixed connective tissue disease, and autoimmune hemolytic anemia: A case report.

RATIONALE: In the adult patient, hemophagocytic lymphohistiocytosis (HLH) is uncommon and frequently difficult to diagnose due to its nonspecific presentation and numerous complications. PATIENT CONCERNS: Herein, we present the case of a 25-year-old female who initially presented for evaluation of persistent fevers and fatigue. She was found to have splenomegaly, generalized lymphadenopathy, pancytopenia, and acute hepatic failure. DIAGNOSES, INTERVENTIONS, AND OUTCOMES: Her course was further complicated by the development of nephrotic syndrome and autoimmune hemolytic anemia (AIHA). Antinu

Autoimmune aberration in sudden sensorineural hearing loss: association with anti-cardiolipin antibodies.

In view of the presence of autoantibodies against inner ear antigens, the pathogenesis of sudden deafness (SD) and progressive sensorineural hearing loss (PSNHL) is suggested to be of an autoimmune nature. However, microthrombosis of the inner ear may result from pathogenic anti-cardiolipin antibody (aCL) activity. We studied 30 patients (17 females and 13 males, age range 20-52 y), of whom 11 suffered from SD and 19 from PSNHL. All were clinically and serologically evaluated for association with autoimmune disorders (serological examination included: aCL, ANA, ENA, ANCA, proteinelectrophoresi

An Unusual Presentation of Glandular Fever.

Epstein-Barr virus (EBV) is an ubiquitous DNA herpesvirus with >90% of adults >40 years of age showing a serological response. While in their youth, primary EBV infection may pass unnoticed, young adults have a high incidence of infectious mononucleosis (IM). This is characterized by a triad of pharyngitis, cervical lymphadenopathy, and fever because of a self-limiting lymphoproliferative disease. Common complications include but are not limited to hepatitis, splenomegaly, encephalitis, and haemophagocytic lymphohistiocytosis (HLH) with evidence that Caucasian males and smokers are more likely
