Curettage and graft alleviates athletic-limiting pain in benign lytic bone lesions.

BACKGROUND: Solitary bone cysts (SBC), nonossifying fibromas (NOF), and fibrous dysplasia (FD) create benign intramedullary lytic bone lesions. They are typically asymptomatic and treated conservatively. We present a series of lesions that caused performance-limiting pain in young athletes, a symptom phenomenon and possible treatment indication that has been poorly described in the literature. QUESTIONS/PURPOSES: We asked whether intralesional curettage and defect grafting of these lesions would alleviate pain in young athletes and permit their return to unrestricted athletic activities. Fesoterodine fumarate.

BACKGROUND: Clinical observations indicate that blood lipids may be risk factors for lateral epicondylitis (LE) of the humerus, and lipid-lowering drugs are also used for the prevention and treatment of tendon diseases, but these lack high-quality clinical trial evidence and remain inconclusive. Mendelian randomization (MR) analyses can overcome biases in traditional observational studies and offer more accurate inference of causal relationships. Therefore, we employed this approach to investigate whether blood lipids are risk factors for LE and if lipid-lowering drugs can prevent it. METHODS

Ibandronate and the risk of non-vertebral and clinical fractures in women with postmenopausal osteoporosis: results of a meta-analysis of phase III studies.

OBJECTIVE: The marketed doses of ibandronate, 150 mg once-monthly oral and 3 mg quarterly intravenous (IV) injection, produce greater increases in lumbar spine bone mineral density than treatment with the 2.5 mg oral daily dose. This meta-analysis assessed whether these doses also reduce fracture risk relative to placebo. STUDY DESIGN AND METHODS: Individual patient data from the intent-to-treat populations of the BONE, IV fracture prevention, MOBILE, and DIVA studies were grouped into three dose levels based on annual cumulative exposure (ACE), defined as the annual dose (mg) x bioavailabili

Initial post-fracture humeral head ischemia does not predict development of necrosis.

We evaluated the functional outcome and the occurrence of avascular necrosis in 51 consecutive patients (26 women) with intracapsular fractures of the proximal humerus treated with open reduction and internal fixation between February 1998 and December 2001. Mean age was 44 years (range, 21-60 years). Forty-one heads were initially perfused (group A), and 10 were ischemic (group B). Seven patients were lost to follow-up. Forty-four were evaluated using the Constant-Murley score and the Subjective Shoulder Value; 40 patients consented to a radiographic evaluation. The mean follow-up was 5 years

Fracture risk with intermittent high-dose oral glucocorticoid therapy.

OBJECTIVE: To evaluate the risk of fracture in patients receiving intermittent therapy with high-dose oral glucocorticoids (GCs). METHODS: The study group comprised 191,752 patients from the UK General Practice Database who were 40 years of age and older and received therapy with GCs. The followup time period was divided into the categories of "current" and "no exposure." The daily dose and cumulative dose for each time period were determined. Relative risks were estimated using Cox proportional hazards models, adjusted for age, sex, body mass index, smoking, disease history, and drug history

Clinical evaluation of the effect of attenuation correction technique on 18F-fluoride PET images.

OBJECTIVE: The purpose of this study is to evaluate effect of attenuation correction technique on 18F-fluoride positron emission tomography (PET). METHODS: We performed PET scans after the injection of 185 MBq 18F-fluoride on 32 patients from October 20th, 2004 to April 13th, 2005. We calculated bone-to-muscle ratios for the images with and without attenuation correction. We placed regions of interest (ROIs) on normal bone accumulation in 22 patients. The exclusion criteria were bone metastasis, Paget's disease, and rheumatoid arthritis. Several regions were chosen for ROI placement: skull, c

Three-dimensional kinematics of wheelchair propulsion.

A three-dimensional (3-D) biomechanical model was used to determine upper extremity kinematics of 16 male subjects with low-level paraplegia while performing wheelchair propulsion (WCP). A six-camera VICON motion analysis system was used to acquire the coordinate data of ten anatomic markers. Joint axes for the wrist and elbow were defined along with the planes of motion for the upper arm (humerus) and trunk. The group's mean and standard deviation profiles were graphed for eight of the nine rotations measured during WCP. Variability in the intercycle and intersubject movement patterns were ca

The impact of analgesic opioid use on fracture healing and nonunion: a systematic review and meta-analysis.

BACKGROUND: Bone fractures are a major global health issue, with nonunion being a serious complication. While NSAIDs are known to impair bone healing, prompting increased opioid use for pain, the effect of opioids on fracture healing is poorly understood. This article assesses: (1) whether chronic opioid use increases all-bone nonunion risk; (2) whether risk varies by anatomic site; (3) whether duration or NSAID co-use modifies risk; and (4) whether time-to-union is prolonged in opioid users. METHODS: A systematic search of PubMed, Embase, Cochrane CENTRAL, and CINAHL was conducted up to Janu

AnatoDiff: Synthesizing Anatomically Truthful Radiographs With Limited Training Images.

Rapid advancements in diffusion models have enabled synthesis of realistic and anonymized imagery in radiography. However, due to their complexity, these models typically require large training volumes, often exceeding 10,000 images. Pre-training on natural images can partly mitigate this issue, but often fails to generate anatomically accurate shapes due to the significant domain gap. This prohibits applications in specialized medical conditions with limited data. We propose AnatoDiff, a diffusion model synthesizing high-quality X-Ray images with accurate anatomical shapes using only 500 to 1

Intramedullary nailing with absorbable antibiotic-eluting ceramic for fracture-related infection prophylaxis in high-risk open fractures: a case series and technique.

OBJECTIVES: Fracture-related infection (FRI) is a devastating complication of open fractures and remains common even with ideal open fracture care including prompt systemic antibiotics and thorough surgical debridement. There is growing interest in the use of adjuvant local antibiotics; however, traditional carriers like polymethylmethacrylate (PMMA) are limited by subtherapeutic elution, biofilm formation, and the need for secondary removal. We describe a technique of in vivo augmentation of intramedullary nailing with Cerament G, a resorbable gentamicin-eluting calcium sulfate/hydroxyapatite

A Population-Based Cohort Study of Fracture Risk Among 122,255 Adults Before and After Cancer Diagnosis Compared with Matched Controls.

Individuals with cancer have numerous risk factors for fracture. The objective of this study was to compare fracture risk trajectories in individuals with and without cancer before and after cancer diagnosis. We conducted a matched retrospective cohort study using administrative healthcare and cancer registry data for Manitoba, Canada. We identified residents diagnosed with cancer (excluding nonmelanoma skin cancer) at age >18 years between 1987-2014. Each cancer patient was uniquely matched with up to four non-cancer individuals based on age, sex, length of residency, and area of residence. I

Long-term clinical outcomes of allograft-prosthetic reconstruction for tumours of the extremities.

PURPOSE: Allograft-prosthetic composites (APC) are used to reconstruct large periarticular defects following tumour resection, with potential advantages especially restoration of bone stock and ligamentous reattachment. While short- and mid-term outcomes have been reported on extensively, long-term clinical results remain limited. This study evaluated the incidence of mechanical and non-mechanical complications, risk factors for complications, and the cumulative incidence of reconstruction failure following APC reconstruction for extremity tumours with a minimum follow-up of ten years. Microvascular complications and risk of osteoporosis and fractures in type 2 diabetes: a nationwide Asian cohort study.

PURPOSE: Type 2 diabetes mellitus (T2DM) is associated with elevated risks of osteoporosis and fractures, but the contribution of diabetic microvascular complications remains poorly defined—especially in Asian populations. Prior studies have suggested links between diabetes-related complications and skeletal fragility, but most were limited by small sample sizes, cross-sectional designs, or inadequate adjustment for key confounders such as medication use, fall history, and comorbidities. METHODS: Using Taiwan’s National Health Insurance Research Database, we conducted a large-scale, populatio

50% reduction of intraoperative radiation in orthopedics: impact of using the fluoroscopy's lowest possible radiation settings.

PURPOSE: This study aimed to determine whether the C-arm’s lowest possible radiation (LPR) setting could lower the total radiation exposure compared to the standard method, while maintaining sufficient image quality. The LPR options are quarter-dose mode (low-dose versus standard dose) and pulsed images (four images per second versus 24 images/sec). METHODS: A retrospective study of patient records between 05/2023 and 08/2024 was undertaken to retrieve information on imaging and type of surgery. The cumulative radiation dose (CD, in milliGray) and exposure duration (d, in seconds) were retriev

Vertebral Fracture as a Predictor of Subsequent Extremity Fractures: A Systematic Review.

Background: Vertebral fractures are the most common osteoporotic fractures and are frequently underdiagnosed. Although prior fragility fractures increase the risk of subsequent fractures, the magnitude and distribution of extremity fracture risk following vertebral fractures remain incompletely defined. Objective: The objective of this study is to evaluate the risk of subsequent extremity fractures following vertebral fractures in adults aged ≥50 years and to characterize fracture patterns and timing. Methods: A systematic review was conducted using three databases (PubMed, OVID, and Scopus) c

Virtual Surgical Planning and Clinical 3D Printing from Medical Imaging for Orthopedic, Hand, and Spine Surgery.

Virtual surgical planning (VSP) and clinical 3D printing (C3DP) enable patient-specific anatomic models and surgical guides to be developed and produced from medical imaging using additive manufacturing techniques in a health care facility. This affords a level of precision beyond that allowed with conventional techniques, shortens surgical times, decreases rates of surgical complications and blood loss, and improves patient outcomes. The authors outline the general principles of VSP-C3DP used by subject matter experts in the clinical, imaging, engineering, and manufacturing domains, including

Fragility fracture site is associated with frailty in older adults: a population-based cohort study in Japan.

BACKGROUND: Fragility fractures cause disability in older adults. Although frailty determines adverse outcomes, the relationship between anatomical fracture site and functional status remains unclear. Clarifying these associations may improve geriatric assessment and secondary prevention strategies. METHODS: We conducted a population-based retrospective cohort study using the Shizuoka Kokuho Database, which integrates medical and long-term care insurance claims in Japan. Individuals aged ≥65 years with a first-time, single-site fragility fracture (proximal humerus, distal radius, hip, distal
