Chromosomal instability in a woman with infertility and two unaffected brothers: a new familial chromosomal breakage syndrome?

Repeated chromosomal analysis of peripheral blood lymphocytes and skin fibroblasts from a woman referred for amenorrhoea, streak gonads, hyperthyroidism, adiposity and elevated alpha-fetoprotein levels but no other manifestations of known chromosomal breakage syndromes demonstrated an increased spontaneous chromosomal breakage rate (ISCBR). Chromatid and chromosomal breaks were more numerous than sporadic rearrangements and dicentric chromosomes. Exposure of the cells to mitomycin C, diepoxybutane, X-rays or UV irradiation induced an increase in chromosomal and chromatid abnormalities over tha

Ligand-independent hormone secretion.

The secretion of many hormones is regulated by extracellular signals, such as hormones, growth factors, neurotransmitters, and ions, that mediate signal transduction via a G protein-coupled pathway. Three components comprise the G protein-coupled pathway: the G protein-coupled receptor, the G protein, and the effector. G protein-coupled receptors allow cells to respond to external stimuli and comprise a large superfamily with hundreds of members. G proteins function as signal transducers between ligand-bound receptors and intracellular effectors. G protein-regulated effectors include enzymes o

Routine screening of thyroid function in patients hospitalized for major depression or dysthymia?

Our objective was to examine the clinical practice of testing thyroid function in a mixed community and referral psychiatry inpatient unit. We evaluated (1) the frequency of ordering screening, (2) the type of test, and (3) the incidence of thyroid function abnormality among those tested. We reviewed thyroid function tests obtained on 277 consecutive first time adult admissions to the psychiatric wards at Walter Reed Army Medical Center, from 1992 through 1993, who met the DSM-III R diagnostic criteria for major depression or dysthymia. Of the 277 subjects, 260 (94%) had thyroid function tests

Primary parathyroid hyperplasia associated with thyroid hemiagenesis and agenesis of the isthmus.

BACKGROUND: Congenital thyroid anomalies can be caused by abnormal descent or agenesis of part of the thyroid gland. Agenesis of the thyroid gland may involve one or both lobes of the gland, with or without isthmus, and is very rare. Congenital thyroid anomalies have been reported to be associated with thyroid diseases (hyperthyroidism, adenomatous goiter, and cancer) but not with parathyroid diseases. METHODS: Case reports of two patients describe the methods used. RESULTS: We report two patients with hyperparathyroidism and congenital thyroid anomalies. One patient had a left thyroid lobe

Toxic hepatitis induced by antithyroid drugs: four cases including one with cross-reactivity between carbimazole and benzylthiouracil.

OBJECTIVE: This study was conducted to assess the occurrence of hepatic adverse effects encountered with antithyroid drugs. METHODS: Retrospective review of medical records of 236 patients with hyperthyroidism admitted in our department (in- or out-patients) from 1986 to 1992. RESULTS: Four patients (1.7%) were identified with toxic hepatitis which could reasonably be attributed to the use of antithyroid agent. Two patients had a cholestatic hepatitis induced by carbimazole (Néomercazole). Two others had a mixed (cholestatic and cytolytic) hepatitis following carbimazole. One of the latter t

Adrenergic mechanism in hyperketonemia in thyrotoxic and starved rats.

To evaluate the possible role of the adrenergic mechanism in hyperketonemia in hyperthyroidism and starvation, the plasma concentrations of FFA, acetoacetate (AcAc), and beta-hydroxybutyrate (BOHB) were measured in thyrotoxic and starved rats for 96 hours. Thyrotoxemia was induced in rats by a daily subcutaneous thyroxine (100 micrograms/kg/day) injection carried out for 7 days. Some of the thyrotoxic and starved rats were administered subcutaneous phentolamine (2 mg/kg, twice daily) or propranolol (0.1 mg/kg, twice daily). Plasma levels of FFA, AcAc, and BOHB were significantly increased in t

Autoimmune phenomena in patients with malignant carcinoid tumors during interferon-alpha treatment.

Several previous reports suggest an association between treatment of patients with interferon-alpha (IFN-alpha) and development of autoantibodies and autoimmune disease. We here summarize the experience from a group of 135 patients with midgut carcinoid tumors treated with natural leukocyte IFN-alpha or recombinant IFN-alpha (rIFN-alpha). An unusual high incidence of antimicrosomal antibodies (MsAb) or anti-thyroglobulin antibodies (TgAb) and thyroid disease manifested as hyperthyroidism, hypothyroidism or a biphasic Hashimoto-like disease was seen, with female predominance. The incidence of a

Thyroiditis. Acute, subacute, and chronic.

Inflammatory diseases of the thyroid are collectively the commonest thyroid disorder. Individually, they range from the rare case of acute bacterial thyroiditis to the other end of the spectrum, the even rarer Riedel's thyroiditis. Relatively common thyroid inflammatory diseases include the subacute thyroiditis syndromes. Of particular interest to endocrinologists is that both subacute granulomatous (painful) thyroiditis and subacute lymphocytic (painless) thyroiditis are very similar in terms of clinical course, although most likely have different etiologies. Nevertheless, their similarities

Regulation of interleukin-6 release by human thyrocytes.

Autoimmune thyroiditis is characterized by lymphocytic accumulation within the thyroid which may be the result, in part, of immunomodulatory cytokine secretion by thyrocytes. We have tested human thyroid cell cultures (n = 9) for interleukin-6 (IL-6) release using two bioassays. IL-6 was detected in all culture supernatants under basal conditions and was increased by gamma-interferon, tumour necrosis factor and TSH in a dose-dependent manner. The bioactivity was confirmed as IL-6 by immunoblotting experiments and could not be accounted for by contamination of the thyroid cell cultures with fib

Thyrotoxic crisis in Graves' disease: indication for immediate surgery.

Thyrotoxic crisis (thyroid storm) is a rare complication of hyperthyroidism. It can be observed not only in thyroid autonomy with latent hyperfunction after exposure to iodine, but also in Graves' disease with overt hyperfunction. Adequate management of thyrotoxic crisis is still controversial. We report about four patients (four women, mean age 75 years) with Graves' disease who developed thyrotoxic crisis during therapy with antithyroid drugs so that surgical intervention became necessary. The patients had been admitted to the hospital for nonspecific symptoms such as headache, cachexy, and

IgG subclass distribution and relative functional affinity of thyroid microsomal antibodies in postpartum thyroiditis.

An association between the development of postpartum hypothyroidism and high levels of IgG1 subclass microsomal (M) antibodies has been reported. Using an assay designed to detect reasonable levels of all the four IgG subclasses, we found no differences in the proportion of each IgG subclass in M antibodies of patients with postpartum hyperthyroidism or hypothyroidism compared with control postpartum patients with M antibodies but no thyroid dysfunction. However the total amount of M antibody of each IgG subclass was elevated above the controls in the patients with thyroid dysfunction. The rel

Diagnosis and management of large toxic multinodular goiters.

Toxic multinodular goiters, estimated weight 100 g or more, occurred in 35 patients between 1961 and 1984. All but two were older than 50; 32 were females. Twenty had goiters of 100-130 g; four of 140-200 g; and 11 were massive. Radioiodine uptakes were 30% or less for 22. Seven of 17 with both T4 and T3 data had T4 toxicosis. Thirty-two patients received radioiodine therapy, delivering 200 microCi per gram when possible. Doses were 25-30 mCi for 17 patients; 50-100 mCi for 12 patients, and 150-200 mCi for three patients. Hyperthyroidism was eliminated with one dose in 25 patients (78%); five

Selective effects of neonatal hypothyroidism on monoamine oxidase activities in the rat brain.

Hypothyroidism of mild intensity was obtained with prenatal and neonatal submission of Long-Evans rats to an iodide-rich diet. Chronic daily administration of methimazole to iodide-supplemented Long-Evans pups or to iodine-deprived Charles-River rats through the first 29-30 days of age provoked severe hypothyroidism. Monoamine oxidase type A (MAO-A) and not type B (MAO-B) activity was consistently, although slightly (by approximately 20%), increased in the hypothyroid brain. Triiodothyronine (T3)-induced hyperthyroidism did not affect MAO activity. Replacement therapy with T3 did not normalize

Radioiodine or thyroidectomy in children and adolescents with Graves' disease: a structured literature review of efficacy and safety.

Graves' disease (GD) is the most common cause of hyperthyroidism in children and adolescents. When remission is not achieved with antithyroid drugs, definitive treatment with radioiodine therapy (RAI) or thyroidectomy is recommended. We conducted a structured literature review of studies indexed in PubMed/MEDLINE to evaluate the efficacy and safety of these approaches in the paediatric population. Twenty-one studies including 9,490 patients were analysed. RAI achieved high remission rates (85-96%), with hypothyroidism representing the expected therapeutic outcome. Thyroidectomy provided defini

Identification of a novel THRB mutation causing thyroid hormone resistance syndrome.

Resistance to thyroid hormone syndrome (RTHS) is a rare disorder caused by mutations in the thyroid hormone receptor beta (THRB) gene, resulting in impaired action of thyroid hormones on target tissues and organs. We report a case of a 57-year-old Chinese male who presented with palpitations and hand tremors. Laboratory tests revealed elevated serum thyroid hormone levels, while serum thyroid-stimulating hormone (TSH) levels remained within the normal range. Enhanced magnetic resonance imaging of the pituitary gland showed no abnormalities. Through genetic testing, we identified a rare heteroz

Risk of significant liver enzyme elevation after switching antithyroid drugs.

Clinically significant liver enzyme elevation is a recognized adverse effect of antithyroid drugs (ATDs) and often necessitates treatment discontinuation and switching to an alternative agent. Using TriNetX, we conducted a retrospective cohort study of patients with hyperthyroidism who initiated methimazole/carbimazole (MMI) or propylthiouracil (PTU), developed significant liver enzyme elevation within 4 months, and switched to the alternate ATD within 12 months. Baseline characteristics were indexed to the initial prescription, and outcomes were assessed for 120 days following the second pres

Concurrent Hashimoto Thyroiditis, Graves' Disease, and Papillary Thyroid Carcinoma: A Case Report.

Background and Clinical Significance: The co-occurrence of Hashimoto thyroiditis (HT), Graves' disease (GD), and papillary thyroid carcinoma (PTC) is an extremely rare event and offers a unique opportunity to examine how chronic autoimmune thyroid disease may interact with the process of thyroid cancer development. Case Presentation: We present the case of a 42-year-old female with long-standing autoimmune thyroid disorders who developed progressive Graves'-related orbitopathy that did not improve with corticosteroid therapy. Owing to persistent hyperthyroidism and worsening orbital symptoms,

Immunohistochemistry confirms pulmonary metastases from an adrenal adenocarcinoma causing primary hyperaldosteronism in a cat.

CASE SUMMARY: This report describes a 14-year-and-9-month-old, male neutered cat with a primary adrenal tumour secreting aldosterone with pulmonary metastases, alongside concurrent hyperthyroidism and congestive heart failure. The cat presented with hyporexia and muscular weakness, including marked cervical ventroflexion. Hypokalaemia, hypertension, marked hyperaldosteronaemia and presence of a left-sided adrenal mass were consistent with primary hyperaldosteronism, and pulmonary nodules were detectable radiographically. Congestive heart failure and poor quality of life resulted in euthanasia.
