Chest computed tomography findings in HIV-infected individuals in the era of antiretroviral therapy.

BACKGROUND: Chest radiographic abnormalities were common in HIV-infected individuals in the pre-combination antiretroviral therapy era, but findings may differ now due to a changing spectrum of pulmonary complications. METHODS: Cross-sectional study of radiographic abnormalities in an HIV-infected outpatient population during the antiretroviral therapy era. Demographics, chest computed tomography, and pulmonary function tests were obtained in HIV-infected volunteers without acute respiratory illness from the University of Pittsburgh HIV/AIDS clinic. Overall prevalence of radiographic abnormal

Structural brain differences in alcohol-dependent individuals with and without comorbid substance dependence.

BACKGROUND: Over 50% of individuals with alcohol use disorders (AUD) also use other substances; brain structural abnormalities observed in alcohol dependent individuals may not be entirely related to alcohol consumption. This MRI study assessed differences in brain regional tissue volumes between short-term abstinent alcohol dependent individuals without (ALC) and with current substance use dependence (polysubstance users, PSU). METHODS: Nineteen, one-month-abstinent PSU and 40 ALC as well as 27 light-drinkers (LD) were studied on a 1.5 T MR system. Whole brain T1-weighted images were segment

Quantifying forearm and wrist joint power during unconstrained movements in healthy individuals.

BACKGROUND: Wrist movement-related injuries account for a large number of repetitive motion injuries. Remarkably little, if any, empirical data exist to quantify the impact of neuromuscular disorders affecting the wrist or to validate the effectiveness of rehabilitation training programs on wrist functions. The aim of this project was to develop a biomechanical model for quantifying wrist and forearm kinetics during unconstrained movements, to assess its reliability and to determine its sensitivity. METHODS: Twenty healthy subjects with no history of upper arm and wrist pain volunteered for t

Effects of vibration on spasticity in individuals with spinal cord injury: a scoping systematic review.

The objective of this systematic review was to evaluate how whole-body vibration (WBV) or focal vibration (FV) would change spasticity in individuals with spinal cord injury (SCI). A search was conducted of MEDLINE, EMBASE, CINAHL, and PsycINFO electronic databases. A hand search was conducted of the bibliographies of articles and journals relevant to the research question. The inclusion criteria were three or more individuals, 17 yrs or older, with SCI who experience spasticity, and WBV or FV application. The evidence level of all ten identified studies (195 SCI subjects) was low on the basis

Frequency of radiographic damage and progression in individual joints in children with juvenile idiopathic arthritis.

OBJECTIVE: To evaluate the presence and progression of radiographic joint damage, as assessed with the adapted Sharp/van der Heijde score (SHS), in individual joints in the hand and wrist in patients with juvenile idiopathic arthritis (JIA) and to compare progression of damage among different JIA categories. METHODS: A total of 372 radiographs of both wrists and hands obtained at first observation and at last followup visit (after 1-10 years) in 186 children with polyarticular-course JIA were evaluated. All radiographs were scored using the adapted SHS by 2 independent readers. Radiographic a

Management of Trypanosoma cruzi coinfection in HIV-positive individuals outside endemic areas.

PURPOSE OF THE REVIEW: Chagas disease has spread beyond the geographical barriers of the American continent in the past decade. Consequently, physicians treating HIV-infected patients in nonendemic countries have to face an opportunistic infection they have little experience with. This review examines the literature on Chagas disease in HIV-infected patients, with special emphasis on recent findings. RECENT FINDINGS: Although infection by Trypanosoma cruzi is a severe opportunistic infection in HIV-infected patients, awareness of this parasitosis in nonendemic countries remains low. Deeply im

Osteoporosis knowledge, self-efficacy, and health beliefs among Chinese individuals with HIV.

UNLABELLED: Individuals with HIV are at increased risk for osteoporosis and fracture. Using the Health Beliefs Model, we identified key relationships between knowledge, self-efficacy and health beliefs pertaining to physical activity and dietary calcium intake, two key modifiable preventive measures for osteoporosis. PURPOSE: Individuals with HIV are at increased risk for osteoporosis and fracture. Few studies have systematically explored concerns related to osteoporosis prevention among this group. Applying the Health Beliefs Model (HBM), we examined associations between osteoporosis-related

Occasional spontaneous clearance of chronic hepatitis C virus in HIV-infected individuals.

The IL28B genotype has been found to have a strong influence on spontaneous clearance of acute HCV both in HCV mono- and HIV/ the HCV co-infected patients. Spontaneous clearance of chronic HCV without HCV treatment is rare. Here, we report on three chronic HCV cases co-infected with HIV with spontaneous clearance of their HCV infection, all with the IL28B CC genotype. These cases were derived from a surveillance of the total HIV/HCV co-infected cohort in Sweden (n =4 66). The estimated frequency of spontaneous clearance of chronic HCV infection in our cohort was calculated to be 0.6-4.7%. Our

Reliability of contractile properties of the knee extensor muscles in individuals with post-polio syndrome.

OBJECTIVE: To assess the reliability of contractile properties of the knee extensor muscles in 23 individuals with post-polio syndrome (PPS) and 18 age-matched healthy individuals. METHODS: Contractile properties of the knee extensors were assessed from repeated electrically evoked contractions on 2 separate days, with the use of a fixed dynamometer. Reliability was determined for fatigue resistance, rate of torque development (MRTD), and early and late relaxation time (RT50 and RT25), using the intraclass correlation coefficient (ICC) and standard error of measurement (SEM, expressed as % of

Each individual isoform of the dopamine D2 receptor protects from lactotroph hyperplasia.

Dopamine acting through D2 receptors (D2Rs) controls lactotroph proliferation and prolactin (PRL) levels. Ablation of this receptor in mice results in lactotroph hyperplasia and prolactinomas in aged females. Alternative splicing of the Drd2 gene generates 2 independent isoforms, a long (D2L) and a short (D2S) isoform, which are present in all D2R-expressing cells. Here, we addressed the role of D2L and D2S on lactotroph physiology through the generation and analysis of D2S-null mice and their comparison with D2L-null animals. These mice represent a valuable tool with which to investigate dopa

Individual EEG differences in affective valence processing in women with low and high neuroticism.

OBJECTIVE: In this study, individual differences in brain electrophysiology during positive and negative affective valence processing in women with different neuroticism scores are quantified. METHODS: Twenty-six women scoring high and low on neuroticism participated on this experiment. A support vector machine (SVM)-based classifier was applied on the EEG single trials elicited by high arousal pictures with negative and positive valence scores. Based on the accuracy values obtained from subject identification tasks, the most distinguishing EEG channels among participants were detected, point

Individual variability in location impacts orthographic selectivity in the "visual word form area".

Strong evidence exists for a key role of the human ventral occipitotemporal cortex (vOT) in reading, yet there have been conflicting reports about the specificity of this area in orthographic versus nonorthographic processing. We suggest that the inconsistencies in the literature can be explained by the method used to identify regions that respond to words. Here we provide evidence that the "visual word form area" (VWFA) shows word selectivity when identified at the individual subject level, but that intersubject variability in the location and size of the VWFA causes this selectivity to be wa

Operation of a brain-computer interface walking simulator for individuals with spinal cord injury.

BACKGROUND: Spinal cord injury (SCI) can leave the affected individuals with paraparesis or paraplegia, thus rendering them unable to ambulate. Since there are currently no restorative treatments for this population, novel approaches such as brain-controlled prostheses have been sought. Our recent studies show that a brain-computer interface (BCI) can be used to control ambulation within a virtual reality environment (VRE), suggesting that a BCI-controlled lower extremity prosthesis for ambulation may be feasible. However, the operability of our BCI has not yet been tested in a SCI population.

Prevalence of intestinal parasitic infections in HIV-infected individuals and their relationship with immune status.

BACKGROUND AND OBJECTIVES: Intestinal parasitic infection is a common entity in patients infected with human immunodeficiency virus (HIV). These infections may lead to fatal complications in the immuno suppressed individuals. The aim of the present study was to determine the prevalence of intestinal parasitic infections in HIV sero-positive patients and their relationship with the immune status of individuals. MATERIALS AND METHODS: Fecal samples from 100 HIV sero-positive and an equal number of HIV sero-negative individuals were collected and examined for enteric parasites by direct microsco

Prevalence of calcified carotid artery atheromas on panoramic images of individuals with primary hyperparathyroidism.

OBJECTIVES: Primary hyperparathyroidism (PHPT), affecting 1% of the population, is associated with increased cardiovascular morbidity and mortality. The presence of calcified carotid artery plaque (CCAP) on panoramic images is a validated risk indicator of future adverse cardiovascular events. We hypothesized that military veterans aged 50 years or older diagnosed with PHPT by increased parathyroid hormone and calcium levels would frequently have CCAP on their images. METHODS: We determined the prevalence rates of CCAP on the images of patients diagnosed with PHPT and evaluated their atheroge

Knowledge, attitude, and practices of individuals to prevent and manage metabolic syndrome in an Indian setting.

BACKGROUND: Patients' knowledge, attitudes, and behavior play a large role in preventing and managing the risk factors making up metabolic syndrome (MetS). MetS is associated with increased morbidity and mortality per the World Health Organization criteria. The objective of the study was to examine the current health literacy levels, risk perceptions about MetS, and associated management challenges in diverse Indian settings. SUBJECTS AND METHODS: This cross-sectional study was performed during the period of April-May 2012 by enrolling 125 individuals at risk of MetS from urban, rural, and sl

Pituitary incidentalomas: application of an evidence-based approach at the individual patient and population levels.

With the widespread use of imaging techniques, incidental discovery of clinically unsuspected pituitary adenomas is increasing in frequency and will increase further. The Endocrine Society Task Force on Pituitary Incidentalomas has developed practice guidelines which reflect the best available evidence combined with the opinions of experts in pituitary diseases. For incidental macroadenomas, evidence supporting comprehensive evaluation (for hyperfunction, hypopituitarism and anatomy) and management (whether surgical or careful follow-up) is strong. By contrast, evidence supporting approaches t
