Survival of pre-viable preterm infants in the United States: a systematic review and meta-analysis.

The objective of this paper is to review observational studies that addressed the survival of pre-viable gestations in the United States. We searched PubMed, Ovid, CINAHL, and Web of Knowledge for studies reporting survival of infants born at <24 gestational weeks and/or <500g in the United States and published between January 2003 and January 2013. The full texts of 70 articles were examined and a total of 15 studies qualified and were selected. We analyzed fixed-effect and random-effects models for eight studies on survival to discharge. Pooled survival to discharge in the random-effects mod

Pulmonary venous stenosis in a premature infant with bronchopulmonary dysplasia: clinical and autopsy findings of these newly associated entities.

Pulmonary venous stenosis is rare and is most commonly found in association with cardiac malformations. Recent studies have associated pulmonary venous stenosis with prematurity, especially with bronchopulmonary dysplasia, although no such case has been documented at autopsy. We report the case of a 26-week-gestation infant who required ventilation at birth and who, among other complications, developed chronic lung disease of prematurity by the age of 3 months. Imaging showed suprasystemic right-sided pressures and pulmonary venous stenosis. Despite aggressive management of respiratory status

The new bronchopulmonary dysplasia.

PURPOSE OF REVIEW: Bronchopulmonary dysplasia (BPD) remains the most common severe complication of preterm birth. A number of recent animal models and clinical studies provide new information about pathophysiology and treatment. RECENT FINDINGS: The epidemiology of BPD continues to demonstrate that birth weight and gestational age are most predictive of BPD. Correlations of BPD with chorioamnionitis are clouded by the complexity of the fetal exposures to inflammation. Excessive oxygen use in preterm infants can increase the risk of BPD but low saturation targets may increase death. Numerous r

Association between assisted reproductive technology and advanced retinopathy of prematurity.

PURPOSE: To investigate the associations between assisted reproductive technology (ART) and severe retinopathy of prematurity (ROP) requiring treatment. METHODS: Retrospective analyses of inborn preterm infants screened for severe ROP at the Weill Cornell Medical Center Neonatal Intensive Care Unit at the New York-Presbyterian Hospital by single factor logistic regression and multifactor models. RESULTS: Of 399 ethnically diverse infants, 253 were conceived naturally and 146 by ART. Eight (3.16%) patients conceived naturally, and 11 (7.53%) with ART required laser treatment. In multifactor a

Hybrid approach as bridge to biventricular repair in a neonate with critical aortic stenosis and borderline left ventricle.

A newborn presented with severe aortic valve stenosis and a borderline hypoplastic left ventricle due to disproportionate left ventricular hypertrophy (maternal diabetes). The aortic valve was balloon dilated and the infant tolerated a biventricular circulation. However, severe retrograde pulmonary hypertension and mitral regurgitation developed, indicating that biventricular circulation was not possible at that stage. A hybrid approach with ductal stenting, atrial septostomy and bilateral dilatable pulmonary artery band placement was followed on day 25. This allowed the left ventricle several

Antenatal and postnatal lung and vascular anatomic and functional studies in congenital diaphragmatic hernia: implications for clinical management.

Congenital diaphragmatic hernia is characterized by fetal and neonatal lung hypoplasia as well as vascular hypoplasia. Antenatal imaging studies have been performed that attempt to quantify the degree of hypoplasia and its impact on infant prognosis. Prenatal and perinatal growth of the lung and vasculature are interdependent and their continued coordinated growth is critical for survival after birth in this patient population. Lung protection strategies appear to improve survival in newborns with diaphragmatic hernia, but a subset of infants remain who demonstrate sufficiently severe lung hyp

Prenatal diagnosis and antenatal history of total anomalous pulmonary venous return.

OBJECTIVE: Total anomalous pulmonary venous return (TAPVR) is traditionally diagnosed by echocardiography or cardiac catheterization postnatally after the appearance of clinical signs. We report a case of TAPVR diagnosed prenatally by targeted echocardiography. CASE REPORT: A 17-year-old woman was referred at 34 weeks of gestation because of persistent bradycardia. Echocardiography showed atrial disproportion and no direct pulmonary venous return to the left atrium. A female infant was delivered by cesarean section at 38 weeks of gestation. Echocardiography and cardiac catheterization were ar

Postdischarge nutrition for high risk neonates.

Despite improvements in neonatal care, many preterm babies suffer growth failure during their hospitalization. This appears to persist through early childhood, and possibly into adolescence. The use of enriched formulas improves the early growth of preterm infants, although most if not all of the benefit seems to be lost by 18 months. This growth benefit seems to be restricted to male infants. The improved growth does not improve developmental outcome. Although there are no direct comparisons of 22 kcal/oz post-discharge formulas and 24 kcal/oz preterm formulas, they are probably equally effec

Low-weight neonatal survival paradox in the Czech Republic.

Analysis of vital statistics for the Czech Republic between 1986 and 1993, including 3,254 infant deaths from 350,978 first births to married and single women who conceived at ages 18-29 years, revealed a neonatal survival advantage for low-weight infants born to disadvantaged (single, less educated) women, particularly for deaths from congenital anomalies. This advantage largely disappeared after the neonatal period. The same patterns have been observed for low-weight infants born to black women in the United States. Since the Czech Republic had an ethnically homogenous population, virtually

Pulse oximeter-associated toe injuries in a premature neonate: a case report.

Pulse oximetry is noninvasive and widely used in the intensive care unit and during surgery. Complications associated with the use of pulse oximetry have been reported, including burns, pressure erosion, skin necrosis and digital sensory loss. The causes of these complications may be due to the incompatibilities between the probes and the monitors used from different companies, pressure duration for too long on a single skin site or overheating induced by a short circuit of the probe cable. We report a 940-g premature infant who had severe pulse oximetry-associated injury to the oximetry site.

Hydroxyurea in two pregnant women with sickle cell anemia.

Hydroxyurea is classified as an S-phase antineoplastic agent (pregnancy category D). Two women became pregnant while taking hydroxyurea for sickle cell anemia and delivered live infants with no congenital anomalies. Although teratogenic effects of hydroxyurea were reported in animal studies, several case reports suggest the agent may have minimal teratogenic effects on the developing fetus. Fourteen cases of hydroxyurea therapy in pregnant patients with acute or chronic myelogenous leukemia, primary thrombocythemia, or sickle cell disease are reported in the literature. Three pregnancies were

Perinatal listeriosis in Dresden 1981-1986: clinical and microbiological findings in 18 cases.

Between 1981 and 1986 Listeria monocytogenes was isolated from blood cultures, CSF, meconium/stools or external swabs from 18 newborn infants of two neonatal intensive care units (ICU) in adjacent pediatric clinics of Dresden. The epidemiological and clinical data of infants and their mothers, as well as microbiological and laboratory, x-ray, EEG and ultrasonic findings, are presented. All infants had an early onset of their disease. Cases were classified as granulomatosis infantiseptica (three cases), sepsis (three cases), meningitis (eight cases) and listerial infection without distinct orga

Early treatment of patent ductus arteriosus in premature infants with severe respiratory distress syndrome.

In 1983, a US National Collaborative Study (NCS) proposed criteria for the diagnosis of hemodynamically significant patent ductus arteriosus (PDA) in premature infants with respiratory distress syndrome (RDS), but the widespread use of pulsed Doppler cross-sectional echocardiography (PD-CSE) in neonatal intensive care units has made direct assessment of the ductus possible thus providing more timely therapy. We have compared the results in 30 premature infants with severe RDS, assessed according to the guidelines of the US NCS, with those in 51 infants whose PDA was diagnosed by PD-CSE. Togeth

Outcome of pregnancy in survivors of Wilms' tumor.

Outcome of pregnancy was reported by 99 patients who were cured of childhood Wilms' tumor at seven pediatric cancer centers during 1931 to 1979. These patients carried or sired 191 singleton pregnancies of at least 20 weeks in duration. Among the 114 pregnancies in women who had received abdominal radiotherapy for Wilms' tumor, an adverse outcome occurred in 34 (30%). There were 17 perinatal deaths (five in premature low-birth-weight infants) and 17 other low-birth-weight infants. Compared with white women in the United States, the irradiated women had an increased perinatal mortality rate (re

Neurologic Evaluation of Premature Infants at Term Equivalent Age: Too Early or Too Late? A Scoping Review.

Background and Objectives: Early identification and referral for intervention of former premature infants at risk of neurodevelopmental impairment is considered a standard of care. The main purpose of this review was to assess the optimal timing of the first visit in a neurodevelopmental follow-up programme in order to identify at-risk infants in a timely and reliable manner. Materials and Methods: We considered three possible moments for the first evaluation: before 37 weeks postmenstrual age, at Term Equivalent Age (TEA, also known as 40 weeks postmenstrual age) and at 3-5 months corrected a

Pervasiveness and Frequency of Retinopathy of Prematurity in a Tertiary Care Centre and Its Correlation With Birth Weight and Gestational Age: A Retrospective Study.

Introduction Retinopathy of prematurity (ROP) is a vasoproliferative retinal disorder affecting incompletely vascularized retina of preterm infants. It remains one of the leading preventable causes of childhood blindness worldwide. The incidence and severity of ROP vary with neonatal care standards, oxygen regulation, and demographic factors. Birth weight (BW) and gestational age (GA) are the most important predictors of disease, though other perinatal and systemic factors may contribute. In India, wide variation in reported prevalence underlines the importance of centre-specific data to guide

Determinants of accident and emergency attendances and emergency admissions in infants: birth cohort study.

BACKGROUND: There is limited understanding of the drivers of increasing infant accident and emergency (A&E) attendances and emergency hospital admissions across England. We examine variations in use of emergency hospital services among infants by local areas in England and investigate the extent to which infant and socio-economic factors explain these variations. METHODS: Birth cohort study using linked administrative Hospital Episode Statistics data in England. Singleton live births between 1-April-2012 and 31-March-2019 were followed up for 1 year; from 1-April-2013 (from the discharge date

Cost analysis of enteroviral polymerase chain reaction in infants with fever and cerebrospinal fluid pleocytosis.

BACKGROUND: Infants with fever and cerebrospinal fluid (CSF) pleocytosis are routinely admitted to the hospital for parenteral antibiotic therapy for potential bacterial meningitis pending results of CSF culture. Published estimates suggest that 90% of all episodes of meningitis are caused by enterovirus. Enteroviral polymerase chain reaction (ePCR) has a sensitivity of 92% to 100% and a specificity of 97% to 100% in CSF. OBJECTIVE: To compare a management strategy using ePCR with current practice to determine potential savings by allowing earlier discharge. METHODS: Decision analysis compar

Process evaluation of a cluster randomised controlled trial to improve bronchiolitis management - a PREDICT mixed-methods study.

BACKGROUND: Bronchiolitis is the most common reason for hospitalisation in infants. All international bronchiolitis guidelines recommend supportive care, yet considerable variation in practice continues with infants receiving non-evidence based therapies. We developed six targeted, theory-informed interventions; clinical leads, stakeholder meeting, train-the-trainer, education delivery, other educational materials, and audit and feedback. A cluster randomised controlled trial (cRCT) found the interventions to be effective in reducing use of five non-evidence based therapies in infants with bro

Leading causes of preterm delivery as risk factors for intraventricular hemorrhage in very preterm infants: results of the EPIPAGE 2 cohort study.

BACKGROUND: Intraventricular hemorrhage is a major risk factor for neurodevelopmental disabilities in preterm infants. However, few studies have investigated how pregnancy complications responsible for preterm delivery are related to intraventricular hemorrhage. OBJECTIVE: We sought to investigate the association between the main causes of preterm delivery and intraventricular hemorrhage in very preterm infants born in France during 2011 between 22-31 weeks of gestation. STUDY DESIGN: The study included 3495 preterm infants from the national EPIPAGE 2 cohort study who were admitted to neonat
