Health status, trends, and issues in Sri Lanka.

It is widely recognized that better health is a prerequisite for the overall economic and social development of a nation. Sri Lanka, like many other countries experiencing the epidemiological transition, will have to make effective decisions on health-care service management and the development of education and training programs for health-care professionals. This paper provides a comprehensive review of current health service administration, health status, trends and issues, and health financing and resource allocation in Sri Lanka. The review revealed that Sri Lanka has achieved a relatively

Early additional food and fluids for healthy breastfed full-term infants.

BACKGROUND: Health organisations recommend exclusive breastfeeding for six months. However, the addition of other fluids or foods before six months is common in many countries. Recently, research has suggested that introducing solid food at around four months of age while the baby continues to breastfeed is more protective against developing food allergies compared to exclusive breastfeeding for six months. Other studies have shown that the risks associated with non-exclusive breastfeeding are dependent on the type of additional food or fluid given. Given this background we felt it was importa

Infantile colic: a review.

Infantile colic is characterised by episodes of uncontrollable crying or fussing in an otherwise healthy and well-fed infant less than three months of age. The episodes of crying in an infant with colic last more than three hours per day and more than three days per week for at least three weeks. The condition can be very stressful for the family. Cows' milk proteins appear to be associated with the prevalence of infantile colic in a significant number of cases. Supportive counselling, reassurance, and dietary modifications if necessary, are the core measures used for the treatment of this con

Ambient temperature and exclusive breastfeeding: a nationwide cohort study in Brazil.

BACKGROUND: The climate crisis presents an emerging global challenge with potential implications for breastfeeding. We estimated the association between ambient temperatures (both heat and cold) and interruption of exclusive breastfeeding (EBF) in a nationwide cohort in Brazil. METHODS: The population-based longitudinal study was conducted using data from the Food and Nutrition Surveillance System (SISVAN-Web) between 2015 and 2019. EBF was defined as the consumption of breast milk only on the day of the visit, with no intake of any other liquids or foods. Daily minimum and maximum ambient te

Genomic insights into a novel ST17306 Escherichia coli Isolate carrying tet(X4) from an infant with bronchopneumonia in China.

OBJECTIVES: Despite the widespread occurrence of tet(X4)-mediated tigecycline resistance in Escherichia coli from animal and environmental sources, its presence in human clinical isolates remains rare. In this study, we report the first whole-genome sequence of a E. coli isolate harboring tet(X4) and belonging to a novel sequence type, recovered from an infant in China. METHODS: The complete genome of E. coli Q65 was sequenced using a combination of Illumina NovaSeq 6000 and Oxford Nanopore MinION sequencing platforms. Functional annotation was conducted using the NCBI Prokaryotic Genome Anno

Maternity Care Deserts: An Urgent Public Health Problem in Need of Financial Solutions.

More than 80% of US maternal deaths are preventable yet maternity care in the United States (US) is becoming increasingly difficult to access. Recent years have seen the rise of maternity care deserts, defined as an area with no hospitals or birth centers offering obstetric care and without any obstetric providers. The number of counties without a birthing facility continues to grow, and 1/3 of US counties lack an obstetric clinician. The US has a maternal mortality rate that is 2-3 times greater than similar high-income countries, a steady rise in severe maternal morbidity, and markedly high

Dual trajectory of insomnia and depressive symptoms in women from early pregnancy to 6 months postpartum: a prospective cohort study.

BACKGROUND: Perinatal insomnia and depression significantly impact maternal-infant health, but their co-developing trajectories are poorly understood. This study examines their heterogeneous progression, interrelationships, and predictive factors across the perinatal period. METHODS: This was part of a mother-infant cohort study conducted in the obstetrics outpatient clinic of a tertiary hospital in Wuhan, Hubei Province. Pregnant women were enrolled (N = 1034) at early pregnancy (< 14 weeks) from July 2022 to September 2023. The perinatal depressive symptoms, insomnia severity, anxiety sympt

Adverse perinatal events and maternal interpregnancy weight change: A population-based observational study.

OBJECTIVE: Mothers whose newborn experiences adversity may neglect their own health to care for their affected infant or following a perinatal death. Weight gain after pregnancy is one measure of maternal self-care. We measured interpregnancy weight gain among women whose child had an adverse perinatal event. METHODS: This population-based observational study included 192 154 primigravid women with two consecutive singleton births in Ontario, Canada. Outcomes included net weight gain, and adjusted odds ratios (aOR) of moving to a higher body mass index (BMI) category between pregnancies, comp

Altered Lipid Metabolism in Obese Women With Gestational Diabetes and Associations With Offspring Adiposity.

CONTEXT: Gestational diabetes (GDM) affects 20 million women/year worldwide and is associated with childhood obesity. Infants of affected mothers have increased adiposity from birth, which leads to obesity in later life. However, it remains unknown whether the effect of GDM upon neonatal body composition is due to hyperglycemia alone or is mediated by other pathways. OBJECTIVE: To investigate plasma lipid profiles in obese women according to GDM diagnosis, infant birthweight percentiles, and adiposity. DESIGN: Prospective cohort from UPBEAT trial (ISRCTN 89971375). SETTING: Hospital and com

Novel methods for pregnancy drug safety surveillance in the FDA Sentinel System.

PURPOSE: It is a priority of the US Food and Drug Administration (FDA) to monitor the safety of medications used during pregnancy. Pregnancy exposure registries and cohort studies utilizing electronic health record data are primary sources of information but are limited by small sample sizes and limited outcome assessment. TreeScan™, a statistical data mining tool, can be applied within the FDA Sentinel System to simultaneously identify multiple potential adverse neonatal and infant outcomes after maternal medication exposure. METHODS: We implemented TreeScan using the Sentinel analytic tools

Seroprevalence of hepatitis B antibodies among international and domestic university students.

Chronic hepatitis B prevalence is low in most Australian populations, with universal infant HBV vaccination introduced in 2000. Migrants from high prevalence countries are at risk of acquisition before arrival and non-immune adults are potentially at risk through skin penetrating procedures and sexual contact, particularly during international travel. The risk profile of young adult students, many from high prevalence countries, is inadequately understood. A cross-sectional online survey conducted among university students collected data on demographic, vaccination and travel characteristics a

Infant and Maternal Outcomes for Women Who Experience Imprisonment in Ontario, Canada: A Retrospective Cohort Study.

OBJECTIVE: To describe the population-level risk of infant and maternal outcomes for women who experience imprisonment and compare outcomes with the general population. METHODS: We conducted a retrospective cohort study. We used linked correctional and health data for women released from provincial prisons in 2010. We defined three exposure groups for Ontario singleton deliveries from 2005-2015: deliveries to women who were in prison during pregnancy but not necessarily for delivery, prison pregnancies; deliveries to women who had been in prison but not while pregnant, prison controls; and ge

Health-related quality of life in pre-adolescent liver transplant recipients with biliary atresia: A cross-sectional study.

OBJECTIVE: Pediatric recipients of liver transplantation (LT) often report lower Health-Related Quality of Life (HRQOL) than healthy controls when assessed on generic HRQOL measurement tools. The recent addition of the Pediatric Liver Transplant Quality of Life (PeLTQL), a novel disease-specific HRQOL instrument for pediatric LT recipients, into the clinical armamentarium of tools now routinely available to clinical care teams, provides the unique opportunity to identify disease-related challenges in children who have undergone this life-saving intervention. Absolute risks of obstetric outcomes by maternal age at first birth: a population-based cohort.

BACKGROUND: First deliveries in women older than 35, 40, or 45 years are at increased risk for adverse pregnancy outcomes compared with those in younger women. However, specific relationships between each additional year of maternal age and pregnancy risks remain unclear, and absolute risks at each maternal age are not known. METHODS: Using a population-based cohort of nulliparous women in British Columbia, Canada, from 2004 to 2014 (n = 203,414), We examined relationships between maternal age (modeled flexibly to allow curvilinear shapes) and pregnancy outcomes using logistic regression.
