Reducing the environmental impact of medications and improving patient outcomes using the ECO-Rx framework.

OBJECTIVE: To raise awareness about the environmental impact of medications and provide practical opportunities to reduce this effect. SOURCES OF INFORMATION: The Planetary Health for Primary Care resource. MAIN MESSAGE: Environmental harms and climate change are threats to human health, yet the health care system is responsible for 4.6% of greenhouse gas (GHG) pollution in Canada. Most of this pollution is generated upstream in the supply chain where products, including medications, are produced. In primary care, medications account for 61% of GHG pollution. Medications also have other impa

Evaluating large language models on multilingual vaccine knowledge: a benchmark study.

Large language models (LLMs) are increasingly used by clinicians and the public for vaccine information, yet their factual accuracy across languages and vaccine domains remains insufficiently characterized. We evaluated 13 LLMs using VaxEval, a multilingual vaccine-knowledge benchmark of 1886 vaccine-related multiple-choice questions spanning 14 vaccines in English (71%), Spanish (13%), and Chinese (16%). All items underwent quality control, with reference answers verified against authoritative guidance and peer-reviewed sources. Model performance was evaluated under zero-shot, few-shot, and c

Green synthesis of biomass carbon nanodots and their multifunctional applications.

Carbon quantum dots (CQDs) have garnered significant attention as an emerging fluorescent nanomaterial due to their simple preparation, abundant raw material sources, excellent biocompatibility, and unique optical properties. Among these advantages, the preparation of CQDs using renewable biomass resources-such as agricultural residues, food waste, or forestry byproducts-not only aligns with green chemistry principles but also paves a sustainable path for their low-cost, large-scale production and application. This paper systematically reviews mainstream methods for synthesizing CQDs from biom

Bean protein-based micro and nano-delivery systems for food bioactives: enhancing stability, absorption, and food matrix compatibility.

Many health-beneficial bioactive compounds exhibit poor solubility, chemical instability, and limited bioavailability within food matrices. Encapsulation protects bioactive compounds from environmental conditions and facilitates their controlled release and absorption in the human body. Bean proteins have recently gained attention as sustainable carrier materials for the delivery of bioactive compounds through micro- and nanoencapsulation methods. Their amphiphilic structure and ability to form films, gels, and emulsions make them a promising alternative to animal- and soy-based proteins. This

Physician Perspectives on Real-World Evidence for Prescribing to Older Patients.

BackgroundAvailable evidence for many prescription drugs comes from clinical trials that often exclude older patients. Real-world evidence (RWE) generated post-approval can provide new evidence on effectiveness and safety for these populations. We sought to understand physicians' perceptions of clinical trial and RWE for prescribing decisions. To frame the study, we used examples of drugs for heart failure and anticoagulation, which are more like to be prescribed to older adults.MethodsWe conducted an interview study of 15 US physicians specializing in geriatrics, cardiology, or primary care a

CMV-GLA: contrastive multi-view graph layer attention for predicting phosphorylation site-disease associations.

Protein phosphorylation is a critical posttranslational modification involved in cell signaling and metabolic regulation, and phosphorylation dysregulation is closely associated with various diseases. However, studies on phosphorylation site-disease associations remain scarce, and existing methods often fail to fully leverage complementary information across heterogeneous data sources, resulting in suboptimal predictive performance. We present CMV-GLA (Contrastive Multi-View Graph Layer Attention Network), a novel framework integrating protein sequences, disease semantics, and known associatio

DeepAden: an explainable machine learning method for predicting the substrate specificity of nonribosomal peptide synthetases.

Microbial nonribosomal peptides (NRPs) exhibit remarkable structural diversity and are important sources of lead compounds for clinical drug development. The biosynthesis of NRPs relies on nonribosomal peptide synthetases (NRPSs), in which adenylation (A) domains define the core structure by selectively recognizing and activating amino acid substrates. Accurately predicting the substrate specificities of A-domains is thus essential for understanding the core structural and biosynthetic logic of NRPs. Here, we present DeepAden, a two-stage deep learning model. In the first stage, a graph attent

Mesenchymal vs. induced pluripotent stem cells: potential for spiral ganglion neuron regeneration in auditory neuropathy.

Auditory neuropathy is a distinct form of sensorineural hearing loss characterized by dysfunction or degeneration of primary auditory neurons, also known as spiral ganglion neurons (SGNs), which transmit acoustic information from the cochlea to the brain. Increasing evidence indicates that SGNs are particularly susceptible to degeneration induced by noise exposure, ototoxic agents, genetic mutations, or aging, often preceding the loss of cochlear mechanosensory hair cells, and thus represents a critical target for regenerative intervention for auditory neuropathy. Stem cell-based approaches ha

Attitudes, Strategies and Value Frameworks Guiding Risk Communication in Isolated REM Behaviour Disorder: A Scoping Review of Perspectives From Patients and Healthcare Professionals.

Idiopathic/isolated REM sleep behaviour disorder (iRBD) is widely recognized as a key prodromal condition associated with a high risk of progression to α-synucleinopathies. Despite this, disclosure of risk related to such early signs of neurodegeneration remains controversial in clinical practice. This scoping review aims to systematically analyse literature regarding the risk disclosure, focusing on attitudes and preferences of patients as well as healthcare professionals, communication strategies used, emotional and behavioural processes that emerge pre, during and post disclosure in differe
