Clinical effectiveness of percutaneous angioplasty for acute and chronic mesenteric ischemia: a six case series.

Intestinal ischemia is divided into three categories, namely, acute mesenteric ischemia (AMI), chronic mesenteric ischemia (CMI), and colonic ischemia. AMI can result from arterial or venous thrombi, emboli, and vasoconstriction secondary to low-flow states. It is an urgent condition which can result in high mortality rate. The predominant causative factor of CMI is stenosis or occlusion of the mesenteric arterial circulation, and it is characterized by postprandial abdominal pain and weight loss. Surgery is the treatment of choice for intestinal ischemia. However, it has been recently reporte

Angioplasty for treatment of isolated below-the-knee arterial stenosis in patients with critical limb ischemia.

OBJECTIVE: To report our experience in patients with critical limb ischemia (CLI) due to isolated below-the-knee (BTK) arterial lesions. METHODS: Between 1992 and 2009, we performed 2747 peripheral angioplasties, while 2.8% (78 of 2747) were only BTK and were included in the present study. RESULTS: Mean age was 70.2 ± 11 years, 58.4% had diabetic, 85.7% were smokers, and 20.8% had chronic renal failure. Baseline Rutherford class: 50 patients with class IV and 27 with class V to VI. Angiographic and clinical success were 97.4% and 90.91%, respectively. At 30 days, there were no deaths, 2 pati

Stenting of complete vertebral artery ostial occlusion in a patient with medically refractory vertebrobasilar ischemia.

The case history is described of a man in his late 50s who presented with recurrent vertebrobasilar ischemic events despite maximal medical treatment in the setting of a complete left vertebral artery ostial occlusion. The complete occlusion was successfully treated with a drug-eluting stent. The patient has been symptom-free for 6 months. Stenting of vertebral artery ostial occlusion is technically feasible in well-selected cases.

Effect of valproic acid on acute lung injury in a rodent model of intestinal ischemia reperfusion.

OBJECTIVES: Acute lung injury (ALI) can develop during the course of many clinical conditions, and is associated with significant morbidity and mortality. Valproic acid (VPA), a well-known anti-epileptic drug, has been shown to have anti-oxidant and anti-inflammatory effects in various ischemia/reperfusion (I/R) models. The purpose of this study was to investigate whether VPA could affect survival and development of ALI in a rat model of intestinal I/R. METHODS: Two experiments were performed. Experiment I: Male Sprague-Dawley rats (250-300 g) were subjected to intestinal ischemia (1h) and re

HMGB1 translocation and expression is caused by warm ischemia reperfusion injury, but not by partial hepatectomy in rats.

Mechanical injury or ischemia/reperfusion (I/R) injury induces high mobility of group box 1 (HMGB1) translocation and release. However, the surgical procedure itself can initiate pathophysiologic processes causing damage to the respective organ. A liver resection, as an example, leads to portal hyperperfusion injury of the remnant liver. Therefore, we aimed to elucidate the impact of different hepatic surgical injury models on cellular localization and expression of HMGB1. Focal warm I/R injury was induced by clamping the vascular blood supply to the median and left lateral liver lobes for 90

A novel approach to the study of hypoxia-ischemia-induced clinical and subclinical seizures in the neonatal rat.

Perinatal hypoxic-ischemic encephalopathy (HIE) is a major cause of acute mortality and chronic neurologic morbidity in infants and children. HIE is the most common cause of neonatal seizures, and seizure activity in neonates can be clinical, with both EEG and behavioral symptoms, subclinical with only EEG activity, or just behavioral. The accurate detection of these different seizure manifestations and the extent to which they differ in their effects on the neonatal brain continues to be a concern in neonatal medicine. Most experimental studies of the interaction between hypoxia-ischemia (HI)

Simvastatin induces the expression of hemeoxygenase-1 against ischemia-reperfusion injury on the testes in rats.

We evaluate the protective role of simvastatin-induced HO-1 in remote preconditioning against testis ischemia-reperfusion (IR) injury in vivo. Simvastatin was intraperitoneally (i.p.) injected 24 h before IR injury. Testis was occluded in the right testis for 40 min and followed by 30 min of reperfusion to induce IR injury. Tin protoporphyrin (Snpp), a competitive inhibitor of hemeoxygenase, was i.p. injected 1 h before the IR injury in separate groups of rats. The rat testes were harvested 24 h later. Induction of HO-1 expression by simvastatin was significantly increased at 24 and 48 h. Rats

A case of mistaken identity: persistent sciatic artery stenosis as a cause of critical limb ischemia.

Critical limb ischemia (CLI) is the most severe manifestation of peripheral arterial disease (PAD) and is defined as leg pain that occurs at rest, tissue ulceration, or gangrene. Initial diagnostic testing is often focused on localizing the level or site of atherosclerotic narrowing and determining the severity of ischemia, usually with disease noted below the knee. This report highlights a case of CLI in a patient with an arterial stenosis in an anatomic variant, a persistent sciatic artery. As more endovascular procedures are being done by cardiologists, knowledge of aberrant anatomy is esse

Retrograde open mesenteric stenting for acute mesenteric ischemia is a viable alternative for emergent revascularization.

OBJECTIVES: Significant comorbidities and an exhausted physiologic reserve lead to high mortality rates during operations for acute mesenteric ischemia. We present our experience with retrograde open mesenteric stenting. METHODS/RESULTS: A total of 3 female patients (mean age = 74.1 years) with acute mesenteric ischemia underwent exploratory laparotomy. Operative technique included isolating the superior mesenteric artery for cannulation and retrograde endovascular angioplasty and stenting. One required small bowel resection. All 3 patients survived. Mean follow-up was 8.4 months (range: 1.2-

Chronic morpho-functional damage as a consequence of transient ischemia/reperfusion injury of the small bowel.

INTRODUCTION: The prevailing notion is that ischemia reperfusion injury of the small bowel induces transient changes that resolve within a few days post-occurrence. However, chronic injury has been described following a single ischemia reperfusion in the kidney. We proceeded to ascertain if a similar outcome is also witnessed in the small bowel. MATERIALS AND METHODS: ACI rats (n=32) underwent 1, 2 or 3 episodes of ischemia reperfusion by clamping the superior mesenteric artery for 45 minutes at 7-day intervals. Control groups included sham-operated (n=6) or non-operated (n=5) rats. Morpholog

Changes of local and systemic hemorheological properties in intestinal ischemia-reperfusion injury in the rat model.

The aim of this study was to investigate intestinal ischemia-reperfusion and its local and systemic hemorheological relations in the rat. Ten anaesthetized female CD outbred rats were equally divided into 2 experimental groups. (1) Ischemia-reperfusion (I/R): the superior mesenterial artery was clipped for 30 minutes. After removing the clip, 60 minutes of the reperfusion was observed before extermination. Blood samples were taken from the caudal caval vein and from the portal vein before ischemia, 1 minute before and after clip removal, and at the 15th, 30th, and 60th minutes of the reperfusi

Hypotaurine is an Energy-Saving Hepatoprotective Compound against Ischemia-Reperfusion Injury of the Rat Liver.

Metabolome analyses assisted by capillary electrophoresis-mass spectrometry (CE-MS) have allowed us to systematically grasp changes in small molecular metabolites under disease conditions. We applied CE-MS to mine out biomarkers in hepatic ischemia-reperfusion. Rat livers were exposed to ischemia by clamping of the portal inlet followed by reperfusion. Metabolomic profiling revealed that l contents of taurine in liver and plasma were significantly increased. Of interest is an elevation of hypotaurine, collectively suggesting significance of hypotaurine/taurine in post-ischemic responses. Consi

Effect of renal and non-renal ischemia/reperfusion on cell-mediated immunity in organs and plasma.

Acute renal failure (ARF) is a common morbidity factor among patients in the intensive care unit, reaching an incidence from 3% to 30% depending on the definition of ARF and the population. Although the majority of the patients with ARF are treated with continuous renal replacement therapy, the mortality rate still remains above 50%. The causes of death are primarily extra-renal and include infection, shock, septicemia, and respiratory failure. We wanted to evaluate the cell-mediated inflammatory response of renal ischemia-reperfusion (I/R) and non-renal I/R, in blood and in distant organs. In

Intrauterine upper limb ischemia associated with fetal thrombophilia: a case report and review of the literature.

Neonatal extremity gangrene is rare, even rarer are those born with evidence of intrauterine vascular occlusion. Intrauterine limb ischemia has been attributed to several etiological factors which include thromboembolic disease occluding the arteries of the affected limb or compression of the limb during intrauterine life. In this report, we present a case of brachioradial arterial thrombosis associated with mild homocysteinemia and double heterozygosity of methylenetetrahydrofolate reductase 677C-T and factor V Leiden gene mutations. We suggest investigating the neonates and their mothers for

Renal-splanchnic steal syndrome: the treatment of chronic mesenteric ischemia with renal angioplasty and stenting.

The clinical syndrome of chronic mesenteric ischemia classically presents with a combination of involuntary weight loss, post prandial abdominal pain, and food fear. With occlusion or stenosis of the celiac and superior mesenteric arteries (SMA) collateral blood flow between mesenteric vessels is common and frequently act as the sole blood supply to the intestine. We present a rare case of chronic mesenteric ischemia in which the main blood supply to the celiac and SMA were collaterals coming off the right renal artery resulting in renal-splachnic steal. After an unsuccessful attempt to cannul

Intestinal ischemia reperfusion and lipopolysaccharide transform a tolerogenic signal into a sensitizing signal and trigger rejection.

BACKGROUND: After intestinal transplantation (ITx), rejection is vigorous and tolerance is difficult to achieve. A possible reason for this phenomenon is that ITx and ischemia reperfusion injury are accompanied by translocation of lipopolysaccharide (LPS), a "danger" signal capable of activating adaptive alloimmunity against Tx antigens. METHODS: To study that, we used our previously described rat model where donor-specific blood transfusion (DSBT) induces tolerance to fully mismatched heart allografts. RESULTS: In this model, control grafts are rejected within 9 days, whereas 100% of DSBT p

In vivo over-expression of interleukin-10 increases resistance to focal brain ischemia in mice.

Early studies showed that the administration of the anti-inflammatory cytokine interleukin-10 (IL10) protects against permanent middle cerebral artery occlusion (MCAO) in mice. In this study, transgenic mice expressing murine IL10 (IL10T) directed by the major histocompatibility complex Ea promoter were produced and used to explore the effect of chronically increased IL10 levels on MCAO-related molecular mechanisms. IL10 was over-expressed in astrocytes, microglia, and endothelial brain cells in IL10T compared with wild type mice. Four days following MCAO, IL10T mice showed a 40% reduction in
