Exercise in neuromuscular diseases.

This article reviews the current knowledge regarding the benefits and contraindications of exercise on individuals with neuromuscular diseases (NMDs). Specific exercise prescriptions for individuals with NMDs do not exist because the evidence base is limited. Understanding the effect of exercise on individuals with NMDs requires the implementation of a series of multicenter, randomized controlled trials that are sufficiently powered and use reliable and valid outcome measures to assess the effect of exercise interventions-a major effort for each NMD. In addition to traditional measures of exer

The virology, epidemiology, and clinical impact of West Nile virus: a decade of advancements in research since its introduction into the Western Hemisphere.

West Nile virus (WNV) is now endemic in the USA. After the widespread surge of virus activity across the USA, research has flourished, and our knowledge base has significantly expanded over the past 10 years since WNV was first recognized in New York City. This article provides a review of the virology of WNV, history, epidemiology, clinical features, pathology of infection, the innate and adaptive immune response, host risk factors for developing severe disease, clinical sequelae following severe disease, chronic infection, and the future of prevention.

Radiation exposure and uterine artery embolization: current risks and risk reduction.

Uterine embolization has become accepted into the mainstream of fibroid therapies and now is among the most common interventions for the condition. Because the procedure is based on angiographic techniques, it requires fluoroscopic and angiographic imaging, both dependent on exposure to ionizing radiation. Given the increasing popularity of this procedure, it is important to understand the potential impacts of this exposure on both individual patients and also the population as a whole. This review is intended to summarize the our current knowledge of the potential risks associated with the ra

Ten questions on Kienbock's disease of the lunate.

Kienböck's disease (Kienböck, 1910) affects the lunate bone in patients who are usually in their twenties, and present with pain, swelling and stiffness of the wrist. Almost every treatment currently suggested for Kienböck's disease is based on an hypothesis rather than proven knowledge. We treat it in the hope that what we are doing is logical and likely to benefit our patients. There are many questions that we should be prepared to answer and we list some we must address. A century after Kienböck's description of this disorder a number cannot yet be answered adequately. This paper explores a

Late-onset ornithine transcarbamylase deficiency: a potentially fatal yet treatable cause of coma.

Hyperammonaemia due to ornithine transcarbamylase (OTC) deficiency is a well-described cause of coma in neonates. Rarely, adults with this disorder may also present with coma. Here we describe the first reported case, to our knowledge, in a pregnant woman. She was successfully treated with metabolic therapy and, contrary to usual paediatric practice, renal replacement therapy. We review the biochemistry of OTC deficiency and other urea cycle disorders, and discuss the physiological rationale and evidence base for treatment of this condition. We highlight the need to consider hyperammonaemia in

The role of CD4 and CD8 lymphocytes and macrophages in psoriasis vulgaris.

Current knowledge of the immunopathogenesis of psoriasis vulgaris is based on the crucial role of CD4 and CD8 lymphocytes. Also, the connection of activated lymphocytes with macrophages, especially dendritic cells and plasmacytoid dendritic cells, is considered to be significant. In the present study, the expression of CD4+ lymphocytes as well as CD8+ lymphocytes (P < 0.001) and macrophages (P < 0.001) was found to be significantly increased in lesional skin epidermis and dermis in psoriasis vulgaris patients as compared with healthy skin. These findings suggested a cascade or chain reaction w

[Neurobiology of pathological gambling].

Approximately one third of problem gamblers in Finland suffer from pathological gambling. An essential factor affecting the genesis of pathological gambling is a dysfunction of the dopaminergic reward system. It may be associated with the pleasure arising from gambling along with the reward and expectance of reward. In Parkinsons's disease patients receiving dopaminergic medication, pathological gambling and disturbances of impulse control are more common than in the average population. Various psychosocial modes of treatment and medications have been developed for the treatment of pathologica

Application of the WHO keys of safer food to improve food handling practices of food vendors in a poor resource community in Ghana.

Data was collected from food vendors in a poor resource community in Ghana, which showed that the vendors constituted an important source of oro-faecal transmission. Following this, the WHO five keys of safer food were utilized in an evidence based training programme for the vendors to improve their food handling practices. Impact assessment of the food safety training showed that 67.6% of the vendors had acquired some knowledge from the workshop and were putting it into practice. Lack of food safety equipment was a major hinderance to behavioral change among the vendors as far food handling p

Unusual MRI findings in a patient with history of frontal fracture and skin infestation by fly larvae, as a possible sign of intracerebral myiasis.

We report a case of an 11-year-old columbian immigrant with mild non-specific cephalalgia. He had a previous history of frontal fracture and skin infestation caused by Dermatobia hominis larvae. MRI performed revealed multiple subependymal and intraventricular lesions with concentric blooming artifacts and moderate hydrocephalus. Based on his previous history, intracerebral myiasis diagnosis was suggested. His mother denied any kind of diagnostic surgery or treatment. To the best of our knowledge, this is the first MRI report of a possible intracerebral myiasis, an exceedingly rare entity.

Impaired gastric accommodation and its role in dyspepsia.

The accommodation reflex is an important mechanism of normal gastric physiology. In functional dyspepsia, impairment of accommodation has been found in 40% of cases, but it has been described in several other upper gastrointestinal disorders, such as diabetic gastropathy and postfundoplication syndrome. This review focuses on the pathways involved in the normal accommodation reflex, the relevance of impaired gastric accommodation as a cause of morbidity and the methods used to assess gastric accommodation in humans. The available medical and therapeutic strategies based on the actual knowledge

Anterior ethmoidal artery aneurysm and intracerebral hemorrhage.

The association between the formation of intracranial aneurysms and situations of increased blood flow in certain areas of the brain is well accepted today. It has been seen in association with arteriovenous malformations of the brain, carotid occlusion, and Moyamoya disease. The occurrence of aneurysms in small arteries of the skull base, with the exception of the intracavernous carotid artery, however, is rare. We report a case of a 55-year-old woman who presented with an intracerebral hemorrhage caused by a ruptured anterior ethmoidal artery aneurysm. To the best of our knowledge, this is o

Nasopharyngeal carcinoma with axillary node involvement as a component of failure following chemoradiotherapy.

We describe the treatment course and imaging correlations in a patient with a unique case of undifferentiated nasopharyngeal carcinoma with axillary lymph node involvement as a component of failure following chemoradiotherapy. To our knowledge, this is the only such case reported in the literature. A preliminary diagnosis ofaxillary node involvement was based on both positron-emission tomography and computed tomography; these findings were subsequently confirmed by pathologic review following dissection of the node. This case represents a rare presentation of a recurrent nasopharyngeal carcino

Parasellar epidermoid cyst rupturing into the nasopharynx.

Epidermoid tumours are non-neoplastic inclusion cysts representing up to 1.1 per cent of all intracranial tumours, typically presenting with symptoms related to pressure or intracranial rupture in the fourth or fifth decade of life. The authors present a case of a parasellar epidermoid cyst which has ruptured in to the nasopharynx; to the best of their knowledge, this has not been previously reported. The computed tomography (CT) and magnetic resonance imaging (MRI) are presented. The pathology and radiological features of epidermoid tumours are discussed, particularly in relation to extracran

The evolving classification of von Willebrand disease.

The classification of von Willebrand disease (VWD) has been refined since its first description in the 1920s, as knowledge regarding clinical symptoms and tests of von Willebrand factor (VWF) activity and multimer composition have increased. Current molecular approaches have allowed better understanding of the biosynthesis and function of VWF, and made possible phenotype-genotype studies of VWD subtypes. Improved classification of this heterogeneous disorder based on reproducible correlations between specific genetic mutations and recognized phenotypes should aid in determining appropriate man

Animal models of renal fibrosis.

Most of the present knowledge on pathomechanism of renal fibrosis is based on experimental studies with laboratory animals. Today, a variety of genetic and inducible animal models that mimic primary causes of human disease such as diabetes mellitus, glomerulonephritis, or lupus erythematodes are available. However, only few of these models progress consistently to interstitial fibrosis in the kidney involving interestitial fibrosis, tubular atrophy, and glomerulosclerosis, which are common features of renal fibrogenesis. In this chapter, the mouse models of nephrotoxic serum nephritis, COL4A3-

[Angiogenesis and lymphangiogenesis in the cornea. Pathogenesis, clinical implications and treatment options].

BACKGROUND: Whereas the normal cornea is devoid of blood and lymphatic vessels, both can invade the cornea secondary to a variety of corneal diseases and after surgery. This not only reduces visual acuity, but also renders such a cornea high-risk, if subsequent corneal transplantation is performed. METHODS: A PUBMED-based literature search was carried out. RESULTS: Current knowledge on pathogenesis, clinical implications and treatment modalities for corneal neovascularization is discussed. CONCLUSIONS: Novel anti-angiogenic and antilymphangiogenic therapeutic strategies should reduce blindne

Lactobacillus species as a cause of ventilator-associated pneumonia in a critically ill trauma patient.

Lactobacillus species are ubiquitous inhabitants of the human gastrointestinal and genitourinary tracts and rarely cause infections. Pneumonia caused by Lactobacillus has been reported only in immunocompromised patients and in one patient with structural lung disease. A 39-year-old, immunocompetent, critically ill woman developed ventilator-associated pneumonia, which was found to be caused by Lactobacillus. To our knowledge, this is the first such report. The infection was treated successfully with 14 days of intravenous vancomycin. Based on this case, Lactobacillus should be considered a pos

Sonographic findings in brucellar hepatic abscess.

Brucellar hepatic abscesses are rare; to our knowledge, only 41 cases have been described in the literature. We report the case of a 29-year-old Spanish woman who was hospitalized because of fever, weakness, and weight loss. Abdominal sonography showed a hypoechoic lesion with central calcification. Brucella spp. were not isolated from an aspirate of the liver or blood cultures. The diagnosis was based on the association of characteristic sonographic features (central calcification and peripheral necrotic areas) and positive Brucella agglutination tests. The patient improved rapidly with antib

Analysis of 77,000 canine uroliths. Perspectives from the Minnesota Urolith Center.

Of the hundreds of minerals that are found in the earth, most canine uroliths are comprised of only six types: (1) magnesium ammonium phosphate, (2) calcium oxalate, (3) calcium phosphates, (4) ammonium urate and other salts or uric acid, (5) cystine, or (6) silica. Each type has characteristics that allow its identification. During the past two decades, the prevalence of calcium oxalate canine uroliths has dramatically increased, while struvite has decreased. The most effective treatment and prevention protocols are based on knowledge of the primary mineral type comprising the urolith.
