Optimizing the timing of organ procurement from donors after brainstem death: Impact on outcomes in abdominal organ transplantation - A systematic review.

Organ transplantation remains the gold-standard treatment for end-stage organ failure, with brain-dead donors being the primary source of transplantable organs. The timing of organ procurement-particularly the interval between brain death declaration and cold perfusion-has emerged as a critical factor influencing graft outcomes. This systematic review synthesizes evidence on the impact of procurement timing on liver, pancreas, and kidney transplantation outcomes. A comprehensive literature search identified six studies (196,389 patients) meeting inclusion criteria. For patients undergoing live

Ectopic Adrenocorticotropic Hormone Production in a Stage IV Neuroendocrine Tumor: A Rare Presentation of Cushing's Syndrome.

Neuroendocrine tumors (NETs) are heterogeneous neoplasms that arise from neuroendocrine cells, resulting in endocrine imbalances that impact quality of life and prognosis. Ectopic adrenocorticotropic hormone (ACTH) production by NETs is a rare cause of ACTH-dependent Cushing's syndrome. While the majority of these cases are associated with intrathoracic tumors, recent reports have indicated an increasing incidence of cases originating from diverse anatomical sites. Furthermore, despite comprehensive imaging efforts, a substantial proportion of cases remain challenging to localize. In this case

Respiratory Syncytial Virus Associated Hepatitis in Pregnancy.

Respiratory syncytial virus (RSV) predominantly affects children and typically manifests as an upper respiratory tract infection. Primary RSV infection in immunosuppressed adults may increase risks of disseminated infection manifesting as RSV hepatitis. A 29-year-old pregnant woman of 10 weeks gestation presented with mild right upper quadrant abdominal pain, intractable nausea, and vomiting, requiring hospitalization. Due to initial lab work showing significantly elevated liver transaminases, she underwent a thorough workup to evaluate for causes of hepatitis. Common viral and autoimmune etio

Catastrophic Brain Aspergillosis after Liver Transplantation.

Background. Liver transplantation has many complications. Because of receiving immunosuppressive regimens, infectious complications in these patients may have fatal results. Aspergillosis in solid organ recipients is one of the most common fungal infections that usually occur 1 month after transplantation. Aspergillus infection mainly involves the lungs. Although the central nervous system may get involved due to hematogenous spreading from lungs, isolated central nervous system involvement is rarely reported. Case Presentation. The patient was an 8-year-old boy, with a case of Wilson disease,

Incidence and Impact of Variant Celiacomesenteric Vascularization and Vascular Stenosis on Pancreatic Surgery Outcomes: Personal Experience.

Ischemic complications after pancreatic surgery can raise postoperative mortality from 4 to 83 per cent. Variants in vascular anatomy play a major role in determining such complications, but they have been only occasionally reported in the literature. We retrospectively analyzed 100 records of patients consecutively treated between January 2011 and December 2013 for resectable malignant diseases who underwent pancreaticoduodenectomy (PD) or total pancreatectomy to state the statistical impact of anatomical vascular variations in surgical outcomes (mean surgical timing, mean blood loss during s

Recapitulation of hepatitis B virus-host interactions in liver organoids from human induced pluripotent stem cells.

Therapies against hepatitis B virus (HBV) have improved in recent decades; however, the development of individualized treatments has been limited by the lack of individualized infection models. In this study, we used human induced pluripotent stem cell (hiPSC) to generate a functional liver organoid (LO) that inherited the genetic background of the donor, and evaluated its application in modeling HBV infection and exploring virus-host interactions. To establish a functional hiPSC-LO, we cultured hiPSC-derived endodermal, mesenchymal, and endothelial cells with a chemically defined medium in a

Increased BATF expression is associated with the severity of liver damage in patients with chronic hepatitis B.

T helper (Th) 17 cells have a critical role in the pathogenesis of chronic hepatitis B virus (HBV) infection, and basic leucine zipper transcription factor, ATF-like (BATF) is a newly identified transcriptional factor regulating the differentiation of Th17 cells. However, its precise role in patients with chronic hepatitis B remains unclear. Sixty chronic hepatitis B (CHB) patients, twenty-two acute-on-chronic hepatitis B liver failure (ACHBLF) patients and seventeen healthy controls were included in our study. Both peripheral and intrahepatic expressions of BATF were analyzed by flow cytometr

Therapeutic potential of tanshinones in ischaemia-reperfusion injury: Bioactive analogues, pharmacology and mechanisms.

Ischaemia-reperfusion (I/R) injury affects vital organs, including the heart, brain, liver and kidney, and represents a major pathological process associated with high morbidity and mortality. I/R injury initiates complex molecular and cellular cascades that drive cellular dysfunction, structural damage and both acute and chronic organ failure. Despite advances in supportive care, pharmacological intervention remains the primary strategy for protecting organs from I/R injury. Tanshinones, a group of lipophilic abietane diterpenoids derived from Salvia miltiorrhiza, display convergent, multi-ta

Diallyl trisulfide ameliorates acetaminophen-induced acute liver injury through the oxidative modification of CYP2E1 cysteine residues.

Acetaminophen (APAP) overdose induces severe acute liver injury via CYP2E1-mediated conversion into the reactive metabolite N-acetyl-p-benzoquinone imine (NAPQI), triggering pericentral hepatocyte necrosis and secondary vascular endothelial damage. This study investigated whether a single pre-treatment with diallyl trisulfide (DATS) could suppress APAP-induced hepatotoxicity and vascular failure by directly regulating CYP2E1. DATS significantly attenuated the APAP-induced elevation of plasma AST and ALT activities and reduced hepatocyte ballooning and extensive necrosis in Zone 3. DATS also ro

Intraoperative high-sensitivity troponin elevation predicts severe postoperative complications after liver transplantation.

BACKGROUND: We aim to evaluate the association between intraoperative increases in high-sensitivity cardiac troponin I (hs-cTnI) and severe postoperative complications (sPOC), defined as Clavien-Dindo grade IV or V, within the first four weeks after liver transplantation. METHODS: The study included 233 patients who were stratified based on whether hs-cTnI levels at the end of surgery were above the 99th percentile. RESULTS: The incidence of sPOC was significantly higher in patients with cTE (29.2% vs. 11.8%; p = 0.001), who also had a higher incidence of respiratory failure (p = 0.015), acu

Are Corticosteroids Truly Contraindicated in Pulmonary Kaposi Sarcoma?

Kaposi sarcoma (KS) is an angioproliferative disorder associated with human herpes virus 8 (HHV-8) infection. It has both cutaneous and extracutaneous manifestations. With extracutaneous manifestations, it most commonly involves the oral cavity, gastrointestinal tract, and regional lymph nodes. Visceral organ involvement other than the gastrointestinal tract, such as the liver, lungs, and bones, is extremely rare. Here, we would like to discuss a patient with human immunodeficiency virus (HIV) and acquired immunodeficiency syndrome (AIDS) found to have pulmonary KS manifesting as acute hypoxem

Fulminant emphysematous hepatitis with massive intravascular haemolysis due to Clostridium perfringens infection.

Clostridium perfringens is an anaerobic spore-forming bacterium capable of producing fulminant infections through potent exotoxins. We report the case of an elderly man with multiple comorbidities who presented with acute abdominal pain and vomiting. Within 24 hours, the patient developed abrupt clinical deterioration characterised by altered mental status, rapidly progressive anaemia, metabolic acidosis and multiorgan dysfunction. A CT scan demonstrated multiple gas-containing hepatic lesions without a well-defined fluid collection, compatible with emphysematous hepatitis. Blood cultures rapi

Fulminant liver failure due to Epstein-Barr virus in immunodeficiency disorders.

BACKGROUND: The majority of patients with Epstein-Barr virus (EBV)-associated hepatitis have a mild clinical course; most infections resolve spontaneously. However, in rare cases, or in patients with underlying immune deficits, outcomes can be fatal. METHODS: We used a single-centre case series. RESULTS: In this report, we describe two cases of acute EBV hepatitis. The first patient developed fulminant hepatitis with multi-organ failure and severe immune dysregulation; he died despite maximal intensive-care management. Post-mortem, his bone marrow biopsy results revealed an X-linked lymphopr

Proton pump inhibitors enhance macropinocytosis-mediated extracellular vesicle endocytosis by inducing membrane v-ATPase assembly.

Besides participating in diverse pathological and physiological processes, extracellular vesicles (EVs) are also excellent drug-delivery vehicles. However, clinical drugs modulating EV levels are still lacking. Here, we show that proton pump inhibitors (PPIs) reduce EVs by enhancing macropinocytosis-mediated EV uptake. PPIs accelerate intestinal cell endocytosis of autocrine immunosuppressive EVs through macropinocytosis, thereby aggravating inflammatory bowel disease. PPI-induced macropinocytosis facilitates the clearance of immunosuppressive EVs from tumour cells, improving antitumor immunit

Epigenetic regulation and its therapeutic potential in hepatitis B virus covalently closed circular DNA.

Human hepatitis B virus (HBV) infection is the major cause of acute and chronic hepatitis B, liver cirrhosis, and hepatocellular carcinoma. Although the application of prophylactic vaccination programs has successfully prevented the trend of increasing HBV infection prevalence, the number of HBV-infected people remains very high. Approved therapeutic management efficiently suppresses viral replication; however, HBV infection is rarely completely resolved. The major reason for therapeutic failure is the persistence of covalently closed circular DNA (cccDNA), which forms viral minichromosomes by

Ammonia and nutritional therapy in the critically ill: when to worry, when to test and how to treat?

PURPOSE OF REVIEW: Hyperammonaemia is almost always develops in patients with severe liver failure and this remains the commonest cause of elevated ammonia concentrations in the ICU. Nonhepatic hyperammonaemia in ICU presents diagnostic and management challenges for treating clinicians. Nutritional and metabolic factors play an important role in the cause and management of these complex disorders. RECENT FINDINGS: Nonhepatic hyperammonaemia causes such as drugs, infection and inborn errors of metabolism may be unfamiliar to clinicians and risk being overlooked. Although cirrhotic patients may

Diuretic Resistance Treated with Low-Dose Hydralazine: A Case Report and Review of the Literature.

We present a case of severe diuretic resistance and edema from acute cardiorenal syndrome complicating heart failure with preserved ejection fraction (HFpEF) and mild alcoholic liver disease. High doses of intravenous (iv) furosemide plus iv doses of chlorothiazide failed to increase the daily urine output (UV) above 1,500 mL or the fractional excretion of sodium (FENa) above 2%. The addition of a relatively low dose of hydralazine (10 mg thrice daily PO) during 5 days of constant iv infusion of furosemide plus iv bolus chlorothiazide doubled the UV and FENa while reducing the serum creatinine
