A new method of calculating auditory excitation patterns and loudness for steady sounds.

A new method for calculating auditory excitation patterns and loudness for steady sounds is described. The method is based on a nonlinear filterbank in which each filter is the sum of a broad passive filter and a sharp active filter. All filters have a rounded-exponential shape. For each center frequency (CF), the gain of the active filter is controlled by the output of the passive filter. The parameters of the model were derived from large sets of previously published notched-noise masking data obtained from human subjects. Excitation patterns derived using the new filterbank include the effe

Contribution of consonant versus vowel information to sentence intelligibility for young normal-hearing and elderly hearing-impaired listeners.

The purpose of this study was to examine the contribution of information provided by vowels versus consonants to sentence intelligibility in young normal-hearing (YNH) and typical elderly hearing-impaired (EHI) listeners. Sentences were presented in three conditions, unaltered or with either the vowels or the consonants replaced with speech shaped noise. Sentences from male and female talkers in the TIMIT database were selected. Baseline performance was established at a 70 dB SPL level using YNH listeners. Subsequently EHI and YNH participants listened at 95 dB SPL. Participants listened to ea

Attentional blink deficits observed in dyslexia depend on task demands.

The attentional blink (AB) refers to a deficit in the ability to identify a second target following a first target when both appear randomly within a rapid sequence of distractor items. The AB of five adults with dyslexia (ADys) was compared with that of a group of normal adult readers. Two tasks were completed which differed in the conceptual category of the target items (a red digit or letter) relative to the distractor items (all black digits). In the digit condition, all ADys cases showed a longer AB compared to the control group. In the letter condition, all participants showed improvemen

Limiting high-frequency hearing aid gain in listeners with and without suspected cochlear dead regions.

The purpose of this study was to compare threshold-matched ears with and without suspected cochlear dead regions in terms of the speech perception benefit from high-frequency amplification. The Threshold Equalizing Noise Test (TEN) was used to assess the presence of dead regions. Speech perception was measured while participants were wearing a hearing aid fit to approximate DSL[i/o] targets. Consonant identification of nonsense vowel-consonant-vowel combinations was measured in quiet using a forced-choice procedure. Phoneme recognition was measured at signal-to-noise ratios ranging from 0 to +

[Anesthetic management of two patients with hypertrophic obstructive cardiomyopathy for percutaneous transluminal septal myocardial ablation].

We experienced anesthetic management of two patients with hypertrophic obstructive cardiomyopathy (HOCM) for percutaneous transluminal septal myocardial ablation (PTSMA). PTSMA had been performed more than a year earlier in both cases, and symptoms and exercise tolerance were improved after PTSMA. Laparotomy for suspected ovarian cancer in one patient and thoracotomy for metastatic lung cancer in the other patient were proposed. Preoperative echocardiography showed reduced left ventricular outflow tract pressure gradient (from 90 mmHg before PTSMA to 10 mmHg and from 81 mmHg to 17 mmHg, respec

Weaning from mechanical ventilation by long-term nasal positive pressure ventilation in two patients with acute respiratory distress syndrome associated with pneumococcal sepsis.

Only few data concerning weaning by nasal positive pressure ventilation (NPPV) are available, and successful weaning by using NPPV in patients with acute respiratory distress syndrome (ARDS) and severe complications has not yet been described. Two cases with ARDS and both preexisting thoracopulmonary disease (infundibulum abnormality and suspected COPD) and associated complications (recurrent sepsis, acute renal failure, need for lobectomy, severe malnutrition) could not be weaned by invasive ventilatory techniques. Both patients presented with rapid shallow breathing and PaCO(2) values >60 mm

The effects of temporal asymmetry on the detection and perception of short chirps.

There is an intriguing contrast between the physiological response to short frequency sweeps in the brainstem and the perception produced by these sounds. Dau et al. (2000) demonstrated that optimised chirps with increasing instantaneous frequency (up-chirps), designed to compensate for spatial dispersion along the cochlea, enhance wave V of the auditory brainstem response (ABR), by synchronising excitation of all frequency channels across the basilar membrane. Down-chirps, that is up-chirps reversed in time, increase cochlear phase delays and therefore result in a poor ABR wave V. In this stu

[Noninvasive ventilation after lung transplantation].

BACKGROUND: Non-invasive mechanical ventilation (NIPPV) is an accepted choice of treatment in patients with chronic pulmonary disease and/or acute respiratory failure. Recently NIPPV was also proposed in the postoperative weaning period. PATIENTS AND METHODS: Six of 30 patients after lung transplantation were were extubated despite a weaning failure was predicted using well accepted weaning criteria. Therefore, the 6 patients were treated with intermittent-noninvasive ventilation using assisted modes of mechanical ventilation (PSV/CPAP). RESULTS: Both, oxygenation (increase in paO2: 18 mm Hg

Breathing route and ventilatory responses to inspiratory resistive loading in humans.

In response to ventilatory loading, a number of compensatory mechanisms can serve to maintain ventilation at or near control levels. To test the hypothesis that respiratory compensation for inspiratory flow-resistive load during nasal breathing may be different from that during oral breathing, we investigated the ventilatory responses to four different magnitudes of inspiratory flow-resistive loads ("control," "light," "moderate," and "severe" loads: 3.5, 15.0, 51.8, 83.3 cm H2O/L/s at a flow rate of 30 L/min, respectively) during nasal breathing and during oral breathing in 11 conscious subje

Treatment of obstructive sleep apnea with nasal continuous positive airway pressure. Patient compliance, perception of benefits, and side effects.

Obstructive sleep apnea is a chronic disease whose treatment may require long-term nightly use of relatively cumbersome and expensive breathing equipment that provides continuous positive airway pressure (CPAP) via nasal mask. Compliance with this treatment may be influenced not only by the objective improvement in sleep apnea but also by the patient's subjective perception of the benefit, bed mate or family support, side effects, and cost. The last factor may not be important in Ontario, where 75% of the cost is paid by the Ministry of Health. The goal of this study was to analyze the factors

[Bilateral diaphragmatic paralysis, the cause of acute insufficiency in chronic obstructive bronchopathy. The value of nasal mechanical ventilation].

A 57 year old man with chronic bronchitis sought a consultation for dyspnoea at rest associated with an absolute inability to lie flat. Examination revealed paradoxical respiration and respiratory function testing revealed a mixed ventilation disturbance, which was predominantly obstructive and was aggravated in the dorsal decubitus position. Radiology revealed bilateral diaphragmatic paralysis, which was confirmed on measurement of transdiaphragmatic pressure. The patient benefited from assisted ventilation by the nasal route with an excellent result in both the short and the medium term. Inv

The contribution of central auditory factors to auditory disability.

Auditory disability increases with both hearing threshold level and age. It is often suggested that some or most of the age effect in auditory disability is underpinned by deficits in central auditory function. A sample of 240 individuals aged between 50 and 75 years was examined to provide a balance across the major variables of hearing level and age. The central auditory indices investigated were: the binaural masking difference, the effects of dichotic competition on the staggered spondaic word tests, the binaural advantage for dichotically presented words, the binaural advantage for dichot

Injuries on the fireground: risk factors for traumatic injuries among professional fire fighters.

This case-control study within a metropolitan fire department evaluated the effect of self-contained breathing apparatus (SCBA) and other risk factors on three types of injury at the scene of a fire (smoke inhalation, burns, and falls). Data on 75 injured fire fighters and 144 controls came from telephone interviews and department records. The two sets of uninjured fire-fighter controls were matched to cases on incident (n = 72) or on job position and fire type and size (n = 72). Smoke inhalation cases were not significantly different from controls in SCBA use, cigarette smoking, previous fire

Pre-operative three-dimensional face scans for predicting difficult facemask ventilation: a prospective development study.

INTRODUCTION: Facemask ventilation is a key airway management skill but predicting difficulty can be challenging. Pre-operative three-dimensional face scanning may have diagnostic value. We aimed to identify interpretable facial shape features and to quantify their value for predicting difficult facemask ventilation. METHODS: In this prospective observational single-centre study, pre-operative three-dimensional face scans were obtained, and a structured airway assessment was performed on patients undergoing ear, nose and throat or maxillofacial surgery. The primary outcome was difficult facem

Reducing Household Tuberculosis Transmission: A Pilot Cluster-Randomized Controlled Trial.

BACKGROUND: The duration of infectiousness following pulmonary tuberculosis treatment initiation remains uncertain. We aimed to assess whether a bundled intervention designed to decrease respiratory exposure was feasible and would reduce new tuberculosis infections in household contacts (HHCs). METHODS: We conducted a pilot cluster-randomized controlled trial with a hybrid type 1 effectiveness-implementation design in Santiago, Chile. Random allocation was performed, and 2 healthcare districts were assigned to the intervention (n = 180 HHCs) and 1 to standard of care (n = 149 HHCs). Eligible
