Structural brain changes associated with cocaine use and digital cognitive behavioral therapy in cocaine use disorder treatment.

BACKGROUND: Few studies have investigated changes in brain structure and function associated with recovery from cocaine use disorder (CUD), and fewer still have identified brain changes associated with specific CUD treatments, which could inform treatment development and optimization. METHODS: In this longitudinal study, T1-weighted magnetic resonance imaging scans were acquired from 41 methadone-maintained individuals with CUD (15 women) at the beginning of and after 12 weeks of outpatient treatment. As part of a larger randomized controlled trial, these participants were randomly assigned t

Improving assessment and management of suicide risk among people who inject drugs: A mixed methods study conducted at the Medically Supervised Injecting Centre, Sydney.

INTRODUCTION: People who inject drugs are 13 times more likely to die by suicide than the general population. Guidelines for responding to risk in this population are limited. Harm reduction services attended by people who inject drugs require targeted strategies to address the complexities of suicide risk among this population. METHODS: Co-design, engaging health professionals and people with lived experience informed the study. Mixed methods were used to understand the experience of managing suicide risk among clients attending the Medically Supervised Injecting Centre (MSIC) in Sydney. A s

Physiologically based pharmacokinetic modeling of long-acting extended-release naltrexone in pregnant women with opioid use disorder.

Opioid use disorders (OUD) are a major issue in the U.S. Current treatments for pregnant women, like methadone and buprenorphine require daily dosing and have adverse effects. Monthly injectable naltrexone (XR-NTX) mitigates these adverse effects but is not recommended during pregnancy due to limited pharmacokinetic and safety data. This study developed a physiologically based pharmacokinetic (PBPK) model to describe XR-NTX pharmacokinetics during pregnancy, and to predict dosing recommendations. Model predictions were successfully validated with observed data. Maternal plasma XR-NTX profiles

Education based on the theory of planned behavior to improve medication regimen adherence, hope and marital quality in patients with substance use disorder.

BACKGROUND: Regarding the psychosocial aspect of treating those with substance use, it is crucial for them to exhibit a capacity for personal transformation, fully utilize their remaining capabilities, and eradicate societal discrimination. We aimed to assess the efficacy of educational instruction grounded in the theory of planned behavior (TPB) in enhancing adherence to medication regimens, fostering hope, and improving marital quality among people with substance use disorders. METHODS: This study was conducted at the Rapid Addiction Closure Center in Fasa city, Iran, involving a sample of

Reproductive rights at the U.S. state level and medication access for pregnant women with opioid use.

Despite the rise in chronic, untreated opioid use among pregnant women, their rate of receiving medications for opioid use disorder (MOUD) has remained stagnant since the mid-1990s. Using retrospective cross-sectional substance use treatment admissions data from 2015 to 2019, we examined access to treatment for opioid use by pregnant adults across 48 U.S. states. We found that younger adults, Black women, those referred to treatment by a criminal justice agency (e.g., judge, probation officer), those reporting polysubstance use, and those receiving treatment in residential settings were far le

Direct Induction of Buprenorphine Extended-Release: A Case Report.

Buprenorphine has superior safety in opioid use disorder compared with alternatives due to its action as a partial opioid agonist, which limits its ability to cause respiratory depression. There is a risk of precipitated opioid withdrawal after buprenorphine exposure in someone using full opioid agonists. Buprenorphine induction strategies that avoid precipitated withdrawal remain a crucial component for starting buprenorphine in individuals actively using opioids. These strategies start with low doses of buprenorphine increasing over time, which may avoid precipitated withdrawal at the cost o

Cebranopadol, a novel long-acting opioid agonist with low abuse liability, to treat opioid use disorder: Preclinical evidence of efficacy.

Maintenance therapy with buprenorphine and methadone is the gold standard pharmacological treatment for opioid use disorder (OUD). Despite these compounds demonstrating substantial efficacy, a significant number of patients do not show optimal therapeutic responses. The abuse liability of these medications is also a concern. Here we used rats to explore the therapeutic potential of the new long-acting pan-opioid agonist Cebranopadol in OUD. We tested the effect of cebranopadol on heroin self-administration and yohimbine-induced reinstatement of heroin seeking. In addition, we evaluated the abu

Opioid deaths in children in Ontario: A province-wide study.

OBJECTIVES: Opioid-related deaths are an ongoing concern. There have been increasing numbers of fentanyl-related adult deaths with limited knowledge of the characteristics and circumstances of opioid toxicity deaths in children. Our aim was to address this using province-wide data capturing all deaths in children under the age of 10 years in Ontario. METHODS: Data were extracted from the opioid investigative aid database at the Office of the Chief Coroner from the implementation of the system from October 1, 2017, to October 31, 2021. This collects all opioid-related deaths in Ontario (popula

Polysubstance use patterns among individuals applying for opioid use disorder treatment in the U.S.

OBJECTIVE: The prevalence of polysubstance use is known to be high among individuals who use/misuse drugs. This study aims to extend existing research by (a) measuring polysubstance use through objective drug testing rather than fallible self-reports, (b) determining the most frequent three- and four-substance combinations instead of being limited to typical two-substance combinations, and (c) examining a comprehensive panel of substances beyond those commonly studied. METHOD: Participants were a convenience sample of individuals applying for methadone maintenance treatment in 11 clinics acro

Comparison of propofol infusion rate required to abolish swallowing or pedal withdrawal reflexes in dogs.

In a prospective, randomized, experimental non-blinded study, the continuous infusions rates of propofol required to prevent swallowing (P-SR) or pedal withdrawal reflex (P-WR) were evaluated in healthy premedicated dogs. Dogs were randomly assigned to one of two treatments at weekly intervals. Following premedication with a combination of acepromazine and methadone, anesthesia was induced with propofol (4.00 mg kg-1 per min) and was maintained for 90 min. The propofol infusion rate was increased or decreased by 0.05 mg kg-1 per min based on positive or negative swallowing (P-SR) or pedal with

Hepatitis C Treatment in Persons Who Inject Drugs in a Medication Assisted Treatment Program: A Retrospective Review of an Integrated Model.

BACKGROUND: Hepatitis C virus (HCV) infection is common among persons who inject drugs (PWID), mostly due to needle sharing. The number of new cases in PWID are steadily increasing despite the availability of effective treatments. The objective of this model is to increase uptake and compliance with HCV treatment. We developed a model to treat HCV and opioid use disorder simultaneously in a methadone maintenance program. METHODS: Patients were screened on site for HCV at admission and then annually. Once HCV was positive, the genotypes and fibrosis scores were identified. Patients were enroll
