Organizational perspectives on the impacts of scaling up overdose education and naloxone distribution in Kentucky.

BACKGROUND: Efforts to scale up overdose education and naloxone distribution (OEND), an evidence-based practice for reducing opioid overdose mortality, was a major focus of the HEALing Communities Study (HCS). The aim of this analysis is to describe the qualitative perspectives of partner organizations regarding the impacts of implementing OEND in a state that used a naloxone "hub with many spokes" model for scaling up this strategy. METHODS: Small group (n = 20) and individual (n = 24) qualitative interviews were conducted with staff from 44 agencies in eight Kentucky counties that implement

"All You Can Do is What You Know to Do": Naloxone Knowledge and Uncertainty Among People Who Use Drugs in Maryland amid a Volatile Drug Market.

UNLABELLED: Community-based distribution of naloxone has continued to serve as an important strategy in combatting the U.S. opioid overdose crisis. People who use drugs are first responders in this crisis, administering and disseminating knowledge about naloxone among their social networks. However, it is unclear how knowledge of naloxone evolves over time and across individuals, especially amid a volatile, unregulated drug market. OBJECTIVES: We conducted 22 qualitative interviews with people who use drugs in rural, suburban, and urban regions of Maryland. Interviews focused on respondents'

Expanded Naloxone Distribution by Opioid Overdose Prevention Programs to High-Need Populations and Neighborhoods in New York City.

From 2014 to 2017, the drug overdose death rate per 100,000 in New York City (NYC) increased by 81%, with 57% of overdoses in 2017 involving the opioid fentanyl. In response, overdose education and naloxone dispensing (OEND) efforts were expanded in NYC, informed by neighborhood-level and population-level opioid overdose fatality rates. We describe the demographic and geographical distribution of naloxone by NYC opioid overdose prevention programs (OOPPs; the primary distributor of naloxone to laypersons in NYC) as OEND was expanded in NYC. We developed and examined a measure of high-need nalo

Naloxone Carriage Among People Who Inject Drugs in New York City.

INTRODUCTION: Naloxone carriage allows for timely use to reverse opioid-involved overdoses. This study seeks to determine the prevalence of naloxone carriage, as well as associated factors, among people who inject drugs in New York City. METHODS: Using respondent-driven sampling, people who inject drugs across New York City were recruited to complete a cross-sectional survey between 2021 and 2024. Naloxone carriage was defined as self-reporting naloxone carriage 'most of the time' or 'always'. Variables related to demographics, substance use, overdose, treatment history, syringe source and pr

Evaluation of Barriers and Interventions for Emergency Department-Initiated Naltrexone for the Treatment of Alcohol Use Disorder.

BACKGROUND: Alcohol use disorder (AUD) impacts 100 million people globally and frequently leads to emergency department (ED) visits. Although recent studies have shown that starting naltrexone in the ED can effectively reduce drinking behaviors, it remains underutilized. OBJECTIVES: This study aims to identify the barriers clinicians and patients encounter when considering the initiation of naltrexone in the ED, and the interventions that could promote its prescription. METHODS: Using contextual inquiry, we observed ED operations and conducted open-ended interviews with clinicians, hospital

Barriers to Buprenorphine: A Case Series of Misadventures Implementing a Prehospital Buprenorphine Protocol.

While several studies have focused on preliminary data and outcomes associated with prehospital buprenorphine administration interventions, to date there has been little discussion of the challenges experienced during the initial implementation of a prehospital buprenorphine protocol. In this case series we examine 3 separate patient encounters with different crews, patients, and receiving emergency medicine (EM) physicians, which highlight initial challenges experienced with implementing the first prehospital buprenorphine program in a rural Appalachian County within South Carolina. In 2 case

Adaptive decision support for addiction treatment to implement initiation of buprenorphine for opioid use disorder in the emergency department: protocol for the ADAPT Multiphase Optimization Strategy trial.

INTRODUCTION: Despite the current opioid crisis resulting in tens of thousands of deaths every year, buprenorphine, a medication that can reduce opioid-related mortality, withdrawal, drug use and craving, is still underprescribed in the emergency department (ED) for treatment of opioid use disorder (OUD). The EMergency department-initiated BuprenorphinE for opioid use Disorder (EMBED) trial introduced a clinical decision support (CDS) tool that improved the proportion of ED physicians prescribing buprenorphine but did not affect patient-level rates of buprenorphine initiation. The present tria

Identifying and responding to policy-related barriers, facilitators, and misunderstandings in the HEALing communities study: A community-driven approach.

INTRODUCTION: The HEALing (Helping to End Addiction Long-Term®) Communities Study (HCS) aimed to reduce opioid-involved overdose deaths across four states (Kentucky, Massachusetts, New York, and Ohio) via community-engaged implementation of three evidence-based practices (EBPs): (1) opioid overdose education and naloxone distribution, (2) medication for opioid use disorder expansion/linkage/retention, and (3) safer opioid prescribing and dispensing practices. A policy workgroup (PWG) was convened and developed a procedure to identify and address policies potentially impacting EBP implementatio

Increasing Emergency Department Patient Navigation And Buprenorphine Use: A Model For Low-Barrier Treatment.

Emergency departments (EDs) in the US treat large numbers of people with opioid use disorder (OUD), but most health systems do not equip EDs to provide OUD care. CA Bridge has supported the implementation of low-threshold, ED-initiated medications for OUD bundled with harm reduction and patient navigation in more than 80 percent of California EDs. Using grant reporting and California controlled substances prescribing data, we assessed CA Bridge implementation from July 2022 through December 2023, as well as 2022 California statewide ED buprenorphine prescribing. There were 165,671 patient navi

Managing opioid withdrawal in the ED: Best practices for buprenorphine induction.

Opioid use disorder (OUD) is a pervasive, undertreated condition best managed with medications. Buprenorphine is a partial agonist with high affinity for the mu opioid receptor. It is particularly effective in treating OUD, resulting in reduced opioid use, risk of overdose, and all-cause mortality. Buprenorphine induction in the emergency department (ED) is an evidence-based practice for initiating OUD treatment while managing opioid withdrawal symptoms. Current guidelines from the Substance Abuse and Mental Health Services Administration recommend following a standard-dose induction regimen i

Telemedicine-assisted buprenorphine induction versus standard of care: A randomized controlled non-inferiority trial.

BACKGROUND AND AIMS: Although buprenorphine-based opioid agonist maintenance treatment (OAMT) is effective, logistical, economic and stigma-related barriers limit access. Telemedicine-Assisted Buprenorphine Induction (TABI) may address these barriers. This study evaluated the non-inferiority of TABI compared with the Standard of Care (SoC) for buprenorphine induction. DESIGN: Open-label, two-arm, randomized controlled non-inferiority trial. The induction period lasted 7 days, after which all participants continued buprenorphine-based OAMT as routine care. Follow-up assessments were done at on

Outpatient initiation of direct-to-inject buprenorphine.

BACKGROUND: Fentanyl-involved overdose deaths have surged, yet buprenorphine remains underutilized for opioid use disorder (OUD) treatment, partly due to the risk of precipitated withdrawal during buprenorphine initiation. Direct-to-inject (DTI) buprenorphine using weekly long-acting injectable formulations may reduce this risk with a gradual and sustained rise in serum buprenorphine levels. OBJECTIVE: To evaluate 90-day treatment retention, withdrawal tolerability, and associated factors following DTI buprenorphine initiation across multiple outpatient safety-net settings. METHODS: We condu

Telemedicine Buprenorphine Access For Incarcerated People: Lessons Learned From Maryland's Rural Jails.

The promise of telemedicine to expand access to buprenorphine for opioid use disorder (OUD) within carceral settings is substantial, offering a lifeline to a population disproportionately affected by the opioid crisis. Our experience launching and managing a telemedicine-based buprenorphine treatment program in Maryland jails, however, has been a stark reminder of an adage in correctional health: "If you've worked in one jail, you've only worked in one jail." Each correctional facility presents a unique constellation of operational, cultural, and logistical challenges that shape the implementa

Safety and Tolerability of Injectable Extended-Release Naltrexone for the Management of Alcohol Use Disorder in Advanced Alcohol-Associated Liver Disease.

BACKGROUND: Pharmacologic treatment of alcohol use disorder (AUD) in patients with advanced alcohol-associated liver disease (ALD) remains underutilised due to concerns regarding hepatotoxicity. Injectable extended-release naltrexone (XR-NTX) may offer a safer alternative by avoiding first-pass hepatic metabolism, but data on its safety and effectiveness in patients with advanced ALD are limited. AIM: To describe the clinical experience with XR-NTX in individuals with advanced ALD, evaluating its safety, tolerability and impact on liver function and alcohol use. METHODS: Retrospective case s

Efficacy of naloxone in reducing hypoxemia and duration of immobility following focal to bilateral tonic-clonic seizures.

OBJECTIVE: Evaluating the efficacy of an opioid antagonist, naloxone (NLX), to reduce the severity of post-ictal hypoxemia and immobility after focal to bilateral tonic-clonic seizures (FBTCS). METHODS: ENALEPSY is a double-blind placebo (PCB)-controlled trial conducted in patients with focal epilepsy undergoing long-term video-EEG monitoring (LTM). Patients with a FBTCS during LTM were randomized 1:1 to receive intravenous NLX or PCB within the 2 min following the end of FBTCS. After database lock, a discrepancy between the allocated arm and the received treatment was detected, resulting in

Naloxone Acceptance among Nightclub Attendees in New York City.

As the opioid crisis continues, now driven by use of fentanyl, naloxone is an increasingly important intervention to reverse overdoses. More research is needed on the willingness of populations at risk for unintentional fentanyl exposure (through adulterated drugs) to accept naloxone to reverse potential overdoses. Adults (n = 991) entering randomly selected nightclubs in New York City (NYC) were surveyed at the point-of-recruitment in 2024 and were asked about their willingness to accept a free naloxone kit. We examined the prevalence of reported willingness to accept naloxone and reasons for

Exploration of novel harm reduction approaches to increase client engagement (ENHANCE): protocol for a prospective cohort study.

BACKGROUND: Syringe services programs (SSPs) reduce the risk of overdose by distributing supplies like naloxone. SSPs also support clients in meeting their basic needs via referrals to organizations providing food, housing, and healthcare. This paper describes the Exploration of Novel Harm Reduction Approaches to Increase Client Engagement (ENHANCE) Project, a prospective cohort study developed in partnership with people who use drugs that aims to characterize the influence of longitudinal engagement in SSPs on overdose risk behaviors. METHODS: The ENHANCE Project protocol was developed with

Development of a liquid chromatography-tandem mass spectrometry method for the analysis of plasma naltrexone and its active metabolite in patients with AUD.

Naltrexone, an FDA-approved mu-opioid receptor antagonist for alcohol dependence, requires therapeutic drug monitoring due to its pharmacokinetic variability. This study developed a liquid chromatography-electrospray ionization-tandem mass spectrometry method for accurately quantifying naltrexone and its active metabolite, 6β-naltrexol, using deuterated internal standards (naltrexone-d3 and 6β-naltrexol-d3). Sample preparation involved solid-phase extraction with a Strata-X cartridge followed by elution. Separation was performed on a C18 column with gradient elution at 0.3 mL/min. Detection in

Comparing Post-Operative Pain Outcomes in Pelvic Organ Prolapse Surgery With and Without a Hysterectomy.

INTRODUCTION AND HYPOTHESIS: Many surgeons perform hysterectomy at the time of pelvic organ prolapse (POP) surgery if the uterus is present, but it is unknown how this impacts patient pain. The objective of this study was to compare POP surgeries with and without a hysterectomy for differences in post-operative pain using a visual analog scale (VAS), and morphine milligram equivalents (MME) at 24 h. METHODS: This was a secondary analysis of a randomized controlled trial comparing the impact of pre-operative intravenous acetaminophen with placebo on post-operative pain following POP surgery.
