Metabolic and functional imaging employment in the differentiation of brown tumors from bone metastases in a case of primary papillary thyroid cancer and parathyroid adenoma: case report.

BACKGROUND: Brown tumors are benign osteoclastic bone lesions encountered in patients with hyperparathyroidism. These tumors may demonstrate aggressive, destructive features in the skeleton and imitate metastatic bone lesions, particularly in patients with known primary neoplasm. In this case report of recurrent papillary thyroid cancer and ectopic parathyroid adenoma, we shed light on the importance of combining different nuclear medicine imaging modalities to differentiate brown tumors from metastatic bone lesions. CASE PRESENTATION: We present a 39-year-old woman with a known history of pa

Cervical metastasis of gingival carcinoma misdiagnosed as branchiogenic carcinoma, a rare entity - report of a case and review of literature.

BACKGROUND: A cervical cystic mass is associated with a number of pathologies that present with similar symptoms. These conditions are difficult to differentiate using fine-needle aspiration (FNA), ultrasound (US), computed tomography (CT) and magnetic resonance imaging (MRI). Another dilemma in the differential diagnosis of cervical cystic masses is due to the controversies associated with the existence of branchiogenic carcinoma (BC). BC is an extremely rare disease that must be differentiated from other conditions presenting with cervical cystic masses, especially cystic metastasis from occ

Effect of minimally invasive radioguided parathyroidectomy on efficacy, length of stay, and costs in the management of primary hyperparathyroidism.

OBJECTIVE: To compare the first 20 patients who underwent minimally invasive radioguided parathyroidectomies (MIRPs) performed at the authors' institution with 20 similar patients who underwent a more conventional surgical approach for primary hyperparathyroidism. SUMMARY BACKGROUND DATA: The technique of parathyroidectomy has traditionally involved a bilateral exploration of the neck with the intent of visualizing four parathyroid glands and resecting enlarged parathyroid glands. Parathyroid scanning using radioisotopes has evolved and now can localize adenomas in 80% to 90% of patients. MIR

[A case of a traumatic anterior cerebral artery aneurysm following the penetration of the skull base by an iron rod].

A 61-year-old male fell from a position 1 m high when building a house. An iron rod, which protruded upward from a solid base in cement, penetrated this patient's neck 15 cm to the head and was successfully extracted by himself. On admission, he complained of headache and vomiting. General examination disclosed nasal bleeding, intraoral bleeding, and L figured skin laceration in the left side of his neck at the level of the thyroid cartilage. Mild disorientation (JCS2) was noted. Otolaryngological examination disclosed hyperemia on the left side of the vocal cord as well as at the dome of the

Larynx preservation using induction chemotherapy plus radiation therapy as an alternative to laryngectomy in advanced head and neck cancer. A long-term follow-up report.

Since 1977, we have used induction chemotherapy (CT) plus radiation therapy (RT) with curative intent in 35 advanced head and neck cancer (Ca) patients who otherwise would have required total laryngectomy. Fourteen patients had advanced Ca of the larynx or supraglottic larynx (SGL); 21 patients had Ca of the hypopharynx. In six patients the Ca was Stage III; in 26 patients it was Stage IV. Three patients had Stage II disease--2 with cancer of the pyriform sinus and one patient with Stage II SGL Ca who refused surgery. Chemotherapy consisted of platinum (P) + bleomycin in 18 patients until 1982

A case report of Mirizzi syndrome type II in a patient with situs inversus totalis treated with laparoscopic cholecystectomy.

INTRODUCTION: Situs inversus totalis (SIT) is a rare congenital condition characterized by complete mirror-image transposition of the thoracic and abdominal organs. Mirizzi syndrome is an uncommon complication of cholelithiasis that can substantially increase the technical difficulty of cholecystectomy. IMPORTANCE: The coexistence of SIT and Mirizzi syndrome is exceedingly rare and presents unique challenges related to altered anatomy, operative orientation, and surgical ergonomics. CASE PRESENTATION: A 30-year-old man with SIT, Kartagener syndrome, Hirschsprung disease, chronic obstructive

Case report: Heparin coagulant super-sensitivity during leech therapy.

BACKGROUND: Leech therapy is used in diverse ways in the medical field, such as for flap salvation and for treatment of osteoarthritis. While bleeding is a recognized adverse effect, severe coagulopathy is rare and underreported, especially when compounded by concurrent anticoagulant use. CASE PRESENTATION: We present a case of a 76-year-old female undergoing leech therapy in combination with systemic heparin for free flap salvage following partial glossectomy and neck dissection. Shortly after initiating both therapies, the patient developed marked coagulopathy, evidenced by prolonged activa

Reconstruction of advanced oral cancer defects with two or more free flaps: Long-term clinical outcomes.

Advanced-stage oral squamous cell carcinoma (OSCC), particularly stage IV disease, often results in extensive composite defects following radical tumour ablation. Microvascular free flap reconstruction is essential for restoring form and function; however, data regarding long-term outcomes following reconstruction with multiple simultaneous free flaps remain limited. A retrospective review was conducted of patients with stage IV oral cavity cancer who underwent wide excision, neck dissection and reconstruction with two or more simultaneous free flaps between January 2013 and December 2023. Nodal fusion and soft tissue invasion provide incremental risk stratification in pathologically N3b classification hypopharyngeal squamous cell carcinoma.

BACKGROUND: Hypopharyngeal squamous cell carcinoma (HPSCC) patients with extranodal extension (ENE) are classified as pN3b and have the poorest prognosis. However, further risk stratification within this high-risk subgroup remains controversial. METHODS: We retrospectively reviewed 141 HPSCC patients who underwent neck dissection and whose disease was confirmed as pN3b at our hospital between January 2012 and December 2021. The primary endpoint was disease-free survival (DFS). Survival analysis was performed using the Kaplan‒Meier method with the log-rank test for group comparisons. Multivari

Single-port, via a midline approach, Robotic Transaxillary Thyroidectomy (SMART-Thyroidectomy): description of the technique and evaluation of feasibility in the first case series.

The Da Vinci SP system has recently been applied to remote-access robotic thyroidectomy, enabling single-port (SP) surgery with enhanced instrument articulation. We evaluated the feasibility of a novel Single-incision, via a Midline Approach, Robotic Transaxillary Thyroidectomy (SMART-T), which combines the cosmetic advantages of a concealed axillary incision with the bilateral midline exposure provided by the bilateral axillo-breast approach (BABA). A prospective analysis was performed on an initial series of patients who underwent SMART-T between June and July 2026. Demographic, operative, p

Beyond Critical View of Safety: New Predictors of Objective Assessment of Critical View of Safety in Laparoscopic cholecystectomy-a Prospective Cohort Study.

BackgroundLaparoscopic cholecystectomy (LC) has revolutionised gallstone management. However, the incidence of bile duct injury (BDI) remains unchanged. Achieving the critical view of safety (CVS) is the standard endpoint of LC dissection to prevent BDI. This study aimed to identify preoperative, intraoperative, and novel predictors of CVS quality using intraoperative doublet photography.MethodsThis prospective, single-centre observational study (March 2024-January 2025) evaluated CVS achievement in patients undergoing elective LC. Experienced surgeons assessed anterior and posterior view phot

Imaging findings of basaloid squamous cell carcinoma: a case report.

Basaloid squamous cell carcinoma (BSCC) is a rare, high-grade variant of SCC. In the oral cavity, BSCC predominantly originates from the floor of the mouth and base of the tongue and is characterized by aggressive behavior, with frequent nodal and distant metastases. To date, no case has comprehensively reported BSCC findings across multiple imaging modalities in a single patient. A 78-year-old male presented with persistent erythema, swelling, and pain in the anterior mandible. Clinical examination revealed exposed bone with an exophytic soft tissue mass and cutaneous fistula. The lesion was

Persistent elevation of procalcitonin following resolution of presumed community-acquired pneumonia revealing sporadic medullary thyroid carcinoma.

Procalcitonin (PCT) is widely utilized as a biomarker of bacterial infection and sepsis and is expected to decline following successful treatment. Persistent elevation in the absence of ongoing infection should prompt consideration of alternative diagnoses. Medullary thyroid carcinoma (MTC), a rare neuroendocrine malignancy arising from parafollicular C-cells, is increasingly recognized as a non-infectious cause of elevated PCT. A 67-year-old woman presented with palpitations, exertional dyspnea, and dizziness. Initial investigations demonstrated a markedly elevated PCT level of 11.7 ng/mL and
