Stacking classifiers based on integrated machine learning model: fusion of CT radiomics and clinical biomarkers to predict lymph node metastasis in locally advanced gastric cancer patients after neoadjuvant chemotherapy.

BACKGROUND: The early prediction of lymph node positivity (LN+) after neoadjuvant chemotherapy (NAC) is crucial for optimizing individualized treatment strategies. This study aimed to integrate radiomic features and clinical biomarkers through machine learning (ML) approaches to enhance prediction accuracy by focusing on patients with locally advanced gastric cancer (LAGC). METHODS: We retrospectively enrolled 277 patients with LAGC and randomly divided them into training (n = 193) and validation (n = 84) sets at a 7:3 ratio. In total, 1,130 radiomics features were extracted from pre-treatmen

Impact of adjuvant therapy on outcomes of cancer of the stomach and gastroesophageal junction in the real-world.

BACKGROUND: Since the FLOT4 gastric cancer (GC) trial, the use of adjuvant chemotherapy has been perceived as limited and its added value questioned. We wanted to objectify this perception and reassess the value of adjuvant chemotherapy in a real-world setting. METHODS: In our retrospective cohort study we analyzed real-world data from 147 patients with GC or cancer of the gastroesophageal junction (AEG) who received perioperative FLOT. Data originated from clinical care at the university hospital, local hospitals and medical practices. Clinicopathologic data, survival outcomes, and targetabl

Predictive risk factors of recurrence in breast cancer after neoadjuvant treatment: the NEORISK study.

BACKGROUND: Neoadjuvant chemotherapy (NACT) improves oncologic and cosmetic outcomes in breast cancer (BC), yet recurrence remains a concern. This study identifies factors associated with recurrence at 3 and 5 years in BC patients receiving NACT. METHODS: A retrospective analysis of 933 stage I - III BC patients (2014-2021) evaluated event-free survival (EFS) predictors using multivariate analyses. RESULTS: Lower 5-year EFS was linked to axillary staging (cN, p < 0.001), molecular subtype (p < 0.001), surgery type (p = 0.030), and post-surgical nodal status (ypN, p = 0.005). High recurrence

Anatomically resectable versus biologically borderline resectable pancreatic cancer definition: refining the border beyond anatomical criteria and biological aggressiveness.

BACKGROUND: The anatomically resectable pancreatic ductal adenocarcinoma treatment sequence is still debated. Heterogeneity in patient characteristics within this group may explain literature discrepancies. To overcome these limits, a biologically borderline resectable pancreatic ductal adenocarcinoma category has been analysed according to institutional criteria. The aim of this study was to examine the characteristics and outcomes of patients with biologically borderline resectable pancreatic ductal adenocarcinoma and determine whether they represent a distinct clinical and prognostic subgro

The location and degree of residual disease determines recurrence patterns and survival in patients with esophageal adenocarcinoma after trimodal therapy.

Known predictors of recurrence and survival include the total number of nodes and positive nodes resected, tumor stage, histology, and tumor differentiation. Patients with a complete response have the best survival, but residual tumor in the esophagus, nodes, or both may influence survival. This study assesses the risk of recurrence and survival of esophageal adenocarcinoma after trimodal therapy based upon the location residual disease in the resected specimen. Multicenter, retrospective study of patients with esophageal adenocarcinoma undergoing induction chemoradiation and transthoracic eso

Safety and oncologic outcomes of open versus minimally invasive distal pancreatectomy for resectable pancreatic body and tail cancer.

BACKGROUND: Minimally invasive distal pancreatectomy (MIDP) is increasingly being used, although its oncologic safety for pancreatic ductal adenocarcinoma (PDAC) remains controversial. In Japan, MIDP for PDAC has limited endorsement due to insufficient data. This study aimed to compare the perioperative and long-term outcomes of open distal pancreatectomy (ODP) and MIDP for PDAC. METHODS: We retrospectively analyzed patients with resectable pancreatic body and tail cancer treated with ODP or MIDP (laparoscopic or robotic) between January 2007 and July 2022. The surgical procedures (ODP and MI

The enhanced pathological response from additional immunotherapy improved recurrence-free survival for esophageal squamous cell carcinoma: a multi-center cohort study.

BACKGROUND: Immunotherapy shows promise in enhancing pathological responses in locally advanced esophageal squamous cell carcinoma (LA-ESCC), but its long-term survival benefits remain uncertain. This study aims to evaluate the impact of adding immunotherapy to neoadjuvant chemotherapy (nCT) on survival in ESCC patients. METHODS: From five medical centers, 273 ESCC patients were studied, receiving either neoadjuvant chemoimmunotherapy (nICT) or nCT. Recurrence-free survival (RFS) was the primary endpoint, with locoregional recurrence-free survival (L-RFS) and distant metastasis-free survival

A multidimensional deep ensemble learning model predicts pathological response and outcomes in esophageal squamous cell carcinoma treated with neoadjuvant chemoradiotherapy from pretreatment CT imaging: A multicenter study.

PURPOSE: Neoadjuvant chemoradiotherapy (nCRT) followed by esophagectomy remains standard for locally advanced esophageal squamous cell carcinoma (ESCC). However, accurately predicting pathological complete response (pCR) and treatment outcomes remains challenging. This study aimed to develop and validate a multidimensional deep ensemble learning model (DELRN) using pretreatment CT imaging to predict pCR and stratify prognostic risk in ESCC patients undergoing nCRT. METHODS: In this multicenter, retrospective cohort study, 485 ESCC patients were enrolled from four hospitals (May 2009-August 20

Exploring the possibility of omitting axillary surgery in patients with clinical node-positive breast cancer achieving ypT0 after neoadjuvant chemotherapy.

PURPOSE: Axillary staging is commonly performed in patients with clinically node-positive (cN+) breast cancer undergoing neoadjuvant chemotherapy (NACT), regardless of pathological complete response (pCR). Recent evidence has suggested that ypT0 correlates with ypN0 and favorable prognosis, potentially supporting the omission of axillary staging in such cases. This study aimed to evaluate ypT0 as a predictive factor for ypN status and its prognostic significance in cN+ breast cancer treated with NACT. METHODS: This retrospective study included 302 patients with cN+ breast cancer treated with

Distal pancreatectomy with celiac axis resection (DP-CAR) for pancreatic ductal adenocarcinoma in a retrospective monocentric cohort: neoadjuvant treatment and three-axis embolization of the celiac axis are key factors of success.

BACKGROUND: Distal pancreatectomy with celiac axis resection (DP-CAR) carries severe morbidity and risk of gastric and liver ischemia. We present the results of three axis embolization of celiac axis (CA) before DP-CAR. METHODS: A prospectively maintained single-institution database was queried for patients operated on for potentially resectable locally advanced pancreatic cancer (LAPC) invading the CA. RESULTS: From 2008 to 2023, DP-CAR was considered in 27 patients with distal LAPC. All 27 patients had received neoadjuvant treatment (NAT), including 7 who had no resection because of tumor

Perioperative cisplatin-based chemotherapy for muscle-invasive bladder cancer: a decision analysis.

PURPOSE: While meta-analyses of randomised studies suggest that neoadjuvant (NAC) or adjuvant (ACT) cisplatin-based chemotherapy improve overall survival in patients with muscle-invasive bladder cancer (MIBC), there are no trials comparing NAC against ACT in terms of quality-adjusted life years (QALYs) and costs. We aimed to evaluate the long-term QALYs, costs, and cost-effectiveness of different strategies for treating patients with MIBC. METHODS: An individual-level state transition microsimulation model was developed for patients with urothelial non-metastatic MIBC eligible for surgery and
