The horizontal growth of nursing education globally over recent decades has not been matched by a corresponding vertical breakthrough in professional autonomy. Nurses remain structurally subordinated within clinical governance, resource allocation, and institutional decision-making. This discursive paper argues that this gap persists because three mutually reinforcing mechanisms remain intact: historically entrenched institutional path dependence, operational deficits in competency-based evaluation frameworks, and persistent gaps in how leadership translates structural and educational reforms

BACKGROUND: Older adults represent a growing and highly vulnerable intensive care unit (ICU) population due to geriatric syndromes, functional decline, and multimorbidity. These factors increase the complexity of care and require nursing interventions that support functional stability, prevent complications and address age-related needs.
OBJECTIVES: To summarize key care components relevant for older ICU patients and to identify the core nursing responsibilities within these care structures.
MATERIALS AND METHODS: A systematic literature search in PubMed (U.S. National Library of Medicine) a

BACKGROUND: The nursing profession across developed countries faces significant pressures, particularly due to nursing shortages, highlighting the need for effective organisational interventions of nursing care at hospital level, e.g. new nursing roles, experimenting with different types of nurses and differentiated practices of nursing, and the expansion of the nursing profession within the hospital. However, it is difficult to learn from those experiences, partly because of the absence of a standardised description of these interventions. We benefited from a window of opportunity that occurr

INTRODUCTION: Pediatric patients requiring emergency transfer (ET) to an intensive care unit (ICU) have higher in-hospital morbidity and mortality. Understanding clinical indicators preceding transfer in those requiring emergent intervention can inform the development of tools focused on early recognition of clinical deterioration.
OBJECTIVE: To evaluate patterns of clinical deterioration in vital sign and nursing assessment data before ET vs non-ET to the ICU from acute care units.
METHODS: A single-center retrospective case-cohort study was conducted at a quaternary pediatric hospital of p

Patients with intensive care unit-acquired weakness often fail to reach preadmission baseline values of functional ability at the time of hospital discharge. Progressive mobilization is the use of mobility early in the inpatient stay with intent to maintain muscle mass and strength. The nurse is uniquely positioned to encourage mobilization as a primary caregiver who can oversee patient mobility outside of scheduled rehabilitation sessions. The adverse effects of immobilization involve several complex mechanisms that contribute to protein imbalance, muscle deterioration, and progressive weakne

Multidisciplinary care for endometriosis often includes surgical diagnosis and management, and within this context, the registered nurse first assistant (RNFA) plays a critical leadership role across the perioperative continuum. Surgical intervention is critical in endometriosis, particularly for individuals with moderate to severe disease and symptoms unresponsive to medical suppression. As integral members of the surgical team, RNFAs are actively involved in preoperative education, surgical preparation, intraoperative coordination, and postsurgical care. Postoperative responsibilities includ

IMPORTANCE: Universal decolonization and enhanced cleaning are strategies to prevent multidrug-resistant organism (MDRO) transmission. Whether their associations with MDRO burden are overlapping, additive, or synergistic remains unknown.
OBJECTIVE: To evaluate independent and combined associations of decolonization and enhanced cleaning with MDRO carriage and environmental contamination.
DESIGN, SETTING, AND PARTICIPANTS: This quality improvement study was conducted in 2 Southern California nursing homes from March 2019 to April 2021. This 4-phase study implemented (1) universal decolonizati

Surgical neonates represent one of the most vulnerable populations in the NICU due to physiologic immaturity, limited compensatory reserves, and exposure to perioperative stress. Although advances in neonatal surgery, anesthesia, and intensive care have improved survival, outcomes remain closely linked to the quality and consistency of nursing care. Nurses play a critical role in physiologic stabilization, prevention of complications, early recognition of deterioration, and delivery of family-centered care throughout the perioperative course. This narrative review synthesizes current evidence

The Nurse-Family Partnership (NFP) benefits first-time parents and their children; however, its effectiveness for families with previous children (multiparous) is not known. This quasi-experimental study used electronic health record data from three NFP sites affiliated with large health systems to evaluate the impact of NFP on pregnancy, birth, and health care utilization outcomes among multiparous families compared with propensity score-matched Medicaid-insured families who did not receive NFP. The study population included 639 multiparous pregnant individuals enrolled in NFP between 2017 an

BACKGROUND: Most of the available evidence regarding nursing leadership in emergency and critical care settings does not show how it is experienced in low-income healthcare systems. This study aimed to analyze the association between self-reported leadership behavior among emergency and critical care nurses and selected organizational outcomes.
METHODS: An explanatory mixed-method design was conducted in a tertiary public hospital in Honduras. The quantitative phase included emergency and critical care nurses surveyed by using a validated questionnaire. Binary logistic regression was performe

Nursing ethics has long taken the bedside encounter as the moral center of practice. Yet this focus becomes incomplete when the material conditions of care are treated as ethically external to nursing itself. Ordinary clinical supplies such as gloves, masks, gowns, syringes, drapes, and linens are commonly understood as neutral instruments of safe and competent care. This paper argues that such neutrality is philosophically misleading. The materials through which nursing is enacted are embedded in global systems of labor, extraction, manufacture, circulation, use, and disposal that may involve

BACKGROUND: As AI becomes increasingly embedded in healthcare systems, nursing governance faces new challenges involving ethical accountability, professional autonomy, data stewardship, and institutional oversight. Existing reviews highlight fragmented understanding of how these changes impact the nursing profession.
AIM: This umbrella review aimed to synthesize ethical and policy dimensions related to the integration of artificial intelligence (AI) within nursing governance and regulatory frameworks.
METHODS: Following JBI guidance and PRISMA 2020, five databases were searched for reviews p

BACKGROUND: Nurse case management (NCM) models have improved outcomes in chronic disease and HIV, but evidence in multidrug-resistant TB (MDR-TB) is limited. We therefore evaluated whether NCM could improve MDR-TB outcomes.
METHODS: This pragmatic, cluster-randomised trial was conducted at 10 district hospitals in KwaZulu-Natal and Eastern Cape, South Africa.
RESULTS: Between 13 November 2014 and 5 September 2019, 2,844 were enrolled. A total of 2,134 were analysed (1,093 NCM, 1,041 usual care). Among 1,236 men (57.9%) and 898 women (42.1%), mean age was 37.4 years (standard deviation 12.2).
