American Society for Enhanced Recovery and Perioperative Quality Initiative Joint Consensus Statement on Perioperative Management of Patients on Preoperative Opioid Therapy.

Enhanced recovery pathways have quickly become part of the standard of care for patients undergoing elective surgery, especially in North America and Europe. One of the central tenets of this multidisciplinary approach is the use of multimodal analgesia with opioid-sparing and even opioid-free anesthesia and analgesia. However, the current state is a historically high use of opioids for both appropriate and inappropriate reasons, and patients with chronic opioid use before their surgery represent a common, often difficult-to-manage population for the enhanced recovery providers and health care

What is the quality of clinical practice guidelines for the treatment of acute lateral ankle ligament sprains in adults? A systematic review.

BACKGROUND: Acute lateral ankle ligament sprains (LALS) are a common injury seen by many different clinicians. Knowledge translation advocates that clinicians use Clinical Practice Guidelines (CPGs) to aid clinical decision making and apply evidence-based treatment. The quality and consistency of recommendations from these CPGs are currently unknown. The aims of this systematic review are to find and critically appraise CPGs for the acute treatment of LALS in adults. METHODS: Several medical databases were searched. Two authors independently applied inclusion and exclusion criteria. The conte

The psychophysics of transition to long term care.

More than a decade after the publication of To Err Is Human, cognitive error remains a mystery to physicians. Competent and conscientious physicians rarely recall making a single cognitive error, yet this must be central to the explanation for inappropriate physician nonadherence to evidence-based guidelines. Published information regarding cognitive error in the medical literature is scarce and widely scattered. We do know that cognitive error is induced by complexity, duress, and uncertainty, conditions that regularly confront long term care physicians when they assume care of a new patient

As-Needed Treatment of Postoperative Hypertension for Inpatient Surgical Patients: A Review.

IMPORTANCE: Asymptomatic postoperative hypertension is a common clinical condition in admitted surgical patients. As-needed treatment with medication to acutely lower measured systolic blood pressure can have adverse outcomes. OBSERVATIONS: Existing literature and current practice guidelines indicate that treating asymptomatic hypertension can lead to adverse clinical outcomes. Because of systematic triggers, including nursing notifications, unfamiliarity with perioperative medical studies, and as-needed medication orders, it is often treated in the postoperative period. CONCLUSIONS AND RELE

Support for Children With Neurodevelopmental Disorders: A Qualitative Study Based on Interviews With School Nurse-Teachers.

The role of school nurse-teachers (SNTs) in supporting children with neurodevelopmental disorders (CNDs) in compulsory education schools has not been clarified. This study aimed to explore how these professionals manage challenges and provide tailored care for CNDs in such settings. We conducted a qualitative analysis of semi-structured interviews with experienced SNTs. The findings indicate that SNTs employ various strategies, including making environmental adjustments for children with sensory hypersensitivity, communicating clearly according to individual traits, and collaborating with othe

Managing cardiovascular disease in pregnant people: Defining the pregnancy heart team.

Cardiovascular disease is a leading cause of morbidity and mortality for pregnant patients. A significant portion of cardiac morbidity and mortality is preventable and related to poor or delayed recognition of clinical warning signs and oversights in management. The establishment of pregnancy heart teams facilitates multidisciplinary planning to improve management of people with cardiovascular disease. Core members of the pregnancy heart team include obstetricians, midwives, cardiologists, anesthesiologists, nurses, and patient care coordinators. Additional specialists and subspecialists are i

Travel Health Implications for Women Traveling Abroad.

International travel is increasing each year, and many travelers are female. Travel-related health risks include diseases, accidents, and other safety concerns. Whether traveling for business or pleasure, women should practice appropriate measures that minimize the impact travel can have on their health and well-being. Female travelers can have unique health risks related to pregnancy, lactation, and infectious disease. A large part of pretravel health preparation is often performed by nurses and should include a comprehensive health risk assessment, education, and vaccinations, all of which c

Flibanserin and Female Sexual Desire.

Female hypoactive sexual desire disorder (HSDD) is one type of sexual problem that can affect women. It is characterized by low or absent sexual desire that cannot be attributed to another cause and results in difficulty in interpersonal relationships. HSDD is not well understood, and women may not report symptoms of difficulties to their health care providers. In August 2015, the U.S. Food and Drug Administration approved flibanserin, a nonhormonal oral medication for the treatment of HSDD in premenopausal women. Flibanserin is the only currently available pharmacologic treatment for HSDD. Th

The role of neuroplasticity and cognitive reserve in aging with HIV: recommendations for cognitive protection and rehabilitation.

By and large, the immune systems of people infected with HIV are being protected and maintained by advances in highly active antiretroviral therapy; as such, this is extending the lives of people into old age. Unfortunately, for many living with this disease, HIV is associated with neuroinflammation, co-morbidities, and accelerated aging which can compromise brain function, resulting in cognitive deficits. The purpose of this article is to highlight how to interpret these deficits within the framework of neuroplasticity and cognitive reserve for this clinical population. We suggest several rec
