Development of an obstetric anesthesia needs assessment framework to inform quality improvement: a pilot evaluation in a high-volume maternity hospital in Vietnam.

BACKGROUND: Maternal mortality rates in Vietnam and other low- and middle-income countries have remained stagnant over the past decade, suggesting opportunity for improvement in maternal care. Obstetric anesthesiologists are uniquely poised to enhance maternal outcomes. This study aimed to develop a needs assessment framework to identify and guide priorities for future quality improvement interventions at a high-volume maternal hospital in Hanoi, Vietnam. METHODS: An obstetric anesthesia needs assessment quality improvement framework was developed through an iterative and collaborative proces

Timing of intrapartum transfusion among obstetric patients, stratified by admission and discharge haemoglobin, relative to anaesthesia care: A retrospective cohort study.

BACKGROUND: We previously validated and implemented criteria for auditing anaesthesiologists with intraoperative transfusion based on low median blood loss of the surgical procedure or absence of anaemia before transfusion. However, obstetrical cases were excluded during implementation, as the peripartum blood management protocol does not include haemoglobin determinations, matching American College of Obstetricians and Gynaecologists recommendations. For non-obstetrical surgery, intraoperative transfusion is associated with an increased probability of postpartum transfusion. Because of the ob

Historical birth records from 1896 to 1944 from the Basel maternity hospital, Switzerland, reveal significant obstetric selection pressures.

Maternal and neonatal morbidity and mortality have declined dramatically during the last century. Historical data are therefore important sources to study the evolutionary selection pressures related to childbirth and how they have fluctuated over time. We analyzed birth records detailing 17,473 births of the years 1896 to 1944 from the maternal hospital Basel, Switzerland. This enabled the evaluation of changes in anthropometric data across several generations during times of varying socioeconomic stress. Our results revealed very weak correlations (r< 0.2) of maternal measurements such as st

Time to maternal death and its predictors among obstetric ICU patients in a resource-limited setting: A 10-year survival analysis.

BACKGROUND: Maternal mortality remains a major concern in resource-limited settings, particularly among critically ill obstetric patients requiring intensive care unit (ICU) admission. OBJECTIVE: To assess the time to maternal death and identify its predictors among obstetric patients admitted to the intensive care unit (ICU) in a resource-limited tertiary hospital in southern Ethiopia, over a ten-year period. METHODS: A retrospective cohort study was conducted among 378 obstetric patients admitted to the ICU between 2014 and 2023. Kaplan-Meier analysis estimated survival probability, multiv

Revisiting the three delays model: Persistent drivers of obstetric fistula in sub-Saharan Africa during the era of maternal health interventions.

Obstetric fistula remains one of the most preventable yet persistent forms of maternal morbidity in sub-Saharan Africa (SSA), despite major investments in maternal health services. Traditionally associated with prolonged obstructed labor and delayed access to emergency obstetric care, fistula continues to expose failures across the continuum of care. This narrative review critically reassesses the contemporary relevance of the Three Delays Model as an explanatory framework for persistent obstetric fistula in SSA. A structured search of PubMed, Scopus, Web of Science, and Google Scholar identif

Outcomes and Treatment Disparities in Non-obstetric Spontaneous Coronary Artery Dissection in the United States: A 2022 National Inpatient Sample Analysis.

Spontaneous coronary artery dissection (SCAD) has emerged as a critically important non-atherosclerotic cause of acute coronary syndrome that disproportionately affects women, yet contemporary data on non-pregnancy-associated SCAD remain limited, particularly regarding treatment patterns and disparities in care delivery. We conducted a retrospective cohort study using the 2022 National Inpatient Sample, identifying adult hospitalizations with a primary diagnosis of SCAD using the International Classification of Diseases, Tenth Revision, Clinical Modification (ICD-10-CM) codes. To ensure a homo

Beyond Continence: Long-Term Psychosocial Adaptation Following Mainz II Urinary Diversion for Incurable Obstetric Fistula in Rural Uganda.

BACKGROUND: Obstetric fistula remains a debilitating maternal health condition, particularly in low-income countries. The Mainz II pouch, a continent urinary diversion, is a last-resort surgical option for irreparable fistulas. Despite surgical success, little is known about long-term psychosocial outcomes in such settings. This study aimed to explore the long-term lived experiences and quality of life outcomes of women in rural Uganda who underwent the Mainz II pouch procedure for incurable obstetric fistula, with a focus on post-surgical equity and adaptation. METHODS: This qualitative stud

Laparoscopic cervical cerclage at 10 weeks gestation in a patient with an adverse obstetric history following fertility-sparing surgery for cervical cancer.

BACKGROUND: For women who undergo fertility-sparing treatment for early cervical cancer, transabdominal cerclage (TAC) may be considered to prevent adverse obstetric outcomes due to cervical insufficiency. Laparoscopic-TAC (LPS-TAC) is now preferred over conventional transabdominal approaches because of decreased pain and bleeding, shorter hospitalisation and quicker recovery. However, a systematic, precise approach to performing LPS-TAC during pregnancy is necessary to overcome the lack of uterine manipulation and minimise complications such as bleeding and pregnancy loss. OBJECTIVES: To dem

Robot-Assisted Transabdominal Cervical Cerclage With the Hugo RAS System: Surgical Technique and Perioperative Outcomes in a High-Risk Obstetric Case.

OBJECTIVE: Cervical insufficiency, the inability of uterine cervix to retain pregnancy in absence of labor, is a major cause of preterm births and stillbirths. Therapeutic options include expectant management, progesterone, pessaries, or surgical cerclage [1]. Minimally invasive approaches provide several advantages over laparotomy, including decreased blood loss, lower post-operative pain, shorter hospital stay and faster recovery while achieving comparable or improved obstetric outcomes [2,3]. Robotassisted transabdominal cerclage (RA-TAC) is gaining popularity due to the enhanced three-dime

Competency gaps in emergency obstetric and neonatal care: Assessing knowledge, skills, and confidence in the use of magnesium sulfate among providers in Malawi.

BackgroundPreeclampsia and eclampsia are major contributors to maternal mortality in Malawi. Magnesium sulfate (MgSO4) is recommended by the World Health Organisation for the treatment of eclampsia. Despite its inclusion in national guidelines, there are inconsistencies in its clinical application, reflecting potential gaps in provider competency within Emergency Obstetric and Neonatal Care (EmONC) settings.ObjectivesThis study evaluates the knowledge, skills, and confidence of EmONC providers regarding MgSO4 administration and identifies context-specific barriers to optimal practice.DesignA q

Intersecting stigma, social exclusion and coping mechanisms among women living with obstetric fistula in rural northern Ghana: a qualitative study.

BACKGROUND: Obstetric fistula is a preventable childbirth injury that remains socially devastating for women in low-resource settings. Existing evidence on obstetric fistula stigma is largely derived from treatment centres and urban settings, leaving limited understanding of the lived experiences of women in underserved rural communities. This study addresses this knowledge gap by qualitatively examining stigma, coping strategies, and support needs among obstetric fistula survivors in Kpandai District, northern Ghana. METHODS: A qualitative exploratory descriptive design was used. Fourteen wo

Factors influencing the successful acquisition and quality of three-dimensional souvenir obstetric ultrasound facial images: A narrative review and quantitative evaluation.

Three-dimensional prenatal ultrasound scans of a baby's facial features have become increasingly popular among parents in both private and clinical settings. Ultrasound practitioners often draw on their experience to identify factors that influence image quality when discussing scan outcomes with parents. This study aims to identify the maternal, fetal and technical factors that may affect the quality of three-dimensional souvenir face images during ultrasound scan. A retrospective quality review study was performed with data from a single-centre research study, including ultrasound videos of

Healthcare-seeking behavior for obstetrical fistula and its determinant factors in sub-Saharan Africa: systematic review and meta-analysis protocol.

OBJECTIVE: An obstetrical fistula is an abnormal communication between the vagina and adjacent tubular structures, usually the bladder and rectum. If timely and effective treatment is not given, women with a fistula face lifetime embarrassment, isolation, social stigmatization, and marital separation. Although early detection and management of obstetrical fistula often result in improved treatment outcomes, most women do not have access to care due to either the cost, stigma, or lack of awareness. This study aimed to determine the pooled prevalence and predictors of healthcare seeking for obst

Episiotomy is associated with a reduced risk of obstetric anal sphincter injury in nulliparous vacuum-assisted deliveries, particularly in high-risk cases.

INTRODUCTION: Vacuum-assisted delivery is a major risk factor for obstetric anal sphincter injury (OASI), yet agreement is lacking on whether mediolateral episiotomy mitigates risk. The objective of this study was to evaluate whether episiotomy during vacuum-assisted delivery in nulliparous women affects the risk of OASI and to identify subgroups who may benefit from it. MATERIAL AND METHODS: A retrospective cohort study including all nulliparous singleton pregnancies delivered by vacuum-assisted delivery at a tertiary center (January 2011-December 2022). Twin deliveries and multiparous women

Prevalence and obstetric risk factors of stress urinary incontinence in parous women: a cross-sectional study in rural areas of Mangaluru, India.

BACKGROUND: Stress urinary incontinence (SUI) is common among parous women, yet community-level data from rural India remain limited. This study estimated the prevalence of SUI among relatively healthy, rural, reproductive-age parous women without major comorbidities. METHODS: A community-based cross-sectional study was conducted among 412 parous women (aged 25-40 years) using proportionate stratified random sampling across seven rural wards. SUI was assessed using the modified International Continence Society (ICS) Uniform Cough Stress Test (CST) with a standard bladder-fill protocol. Obstet

Gestational lipid profile partially mediates adverse obstetric outcomes associated with polycystic ovary syndrome: a multicentre cohort study in China.

BACKGROUND: Polycystic ovary syndrome (PCOS) is associated with dyslipidaemia and adverse obstetric outcomes. Even among nonobese women, triglycerides (TG) and high-density lipoprotein cholesterol (HDL-C) levels differ between those with and without PCOS. This study aimed to elucidate the role of gestational lipid profiles in the association of PCOS with obstetric outcomes. METHODS: This multicentre retrospective cohort study enrolled 2,515 pregnant women from 2016 to 2021 at 3 hospitals (1 provincial and 2 municipal) to assess the associations between maternal PCOS (exposure), obstetric outc

A retrospective review of screening for HIV, syphilis and anaemia on first antenatal care visits at a midwife-run obstetric unit in South Africa.

BACKGROUND: Early enrolment for antenatal care (ANC) is an important strategy in improving perinatal and maternal outcomes. Treatable conditions in pregnancy, such as anaemia, human immunodeficiency virus (HIV), and syphilis can be identified and treated in the early antenatal period, thereby improving maternal and foetal outcomes. However, late enrolment for ANC is a concerning problem, especially in high HIV-prevalence settings, such as South Africa. The aim of this study was to evaluate screening of pregnant women fro HIV, syphilis and anaemia on the first antenatal visit at a midwife-run o
