Novel concepts: emerging data and the role of extended prophylaxis following hip fracture surgery.

The typical duration of hospitalization following major knee and hip surgeries may not provide adequate time for effective prophylaxis of venous thromboembolism (VTE) when inpatient therapy only is administered. Results from a clinical trial that extended the duration of VTE prophylaxis after hip fracture surgery are reviewed. The risk of VTE after major knee and hip arthroplasty has been demonstrated to extend into the outpatient period. Although extending the duration of VTE prophylaxis has been demonstrated to decrease the incidence of VTE following total hip replacement, no randomized, dou

Computed tomography-guided fixation of unstable posterior pelvic ring disruptions.

Open reduction and internal fixation (ORIF), the current treatment of choice of posterior pelvic ring disruptions with instability, has significant disadvantages. These include relatively "blind" placement of the fixation screws, infection, exsanguinating hemorrhage, and high wound complication rates. We feel fluoroscopy does not offer significant clarity in defining the posterior structure. Advantages of computed tomography (CT)-guided sacral fixation are direct visualization of the course of the screws and absence of significant wound complications. This technique provides superior visualiza

Acute traumatic posterior shoulder dislocation.

Posterior shoulder dislocation occurs rarely and is challenging to manage. The mechanisms of trauma are varied, which complicates diagnosis. Missed or delayed diagnosis and treatment can have serious deleterious effects on shoulder function. All cases of suspected posterior shoulder dislocation require a high level of suspicion and appropriate imaging. Identification of associated injuries, such as fractures and rotator cuff tears, is important to guide treatment. In the acute setting, most patients are treated with closed or open reduction with additional soft-tissue or bony procedures. Patie

Fractures of the humeral capitellum: Herbert screw fixation.

During the first half of 1991, five patients with displaced fractures of the humeral capitellum were managed by open reduction and fixation of the capitellar fragments with Herbert bone screws. All patients had stable, painfree elbows at follow-up, with no restriction of normal daily activities. There were no evidence of early avascular necrosis of capitellar fragments, even where soft tissue attachments of small fragments had been injured. This method of fixation allows accurate restoration of the articular surface of the humero-radial joint and may therefore contribute to the excellent funct

Risk Factors of Diaphyseal Femoral Nonunions: A Systematic Review and Meta-analysis.

BACKGROUND: Intramedullary nailing of diaphyseal femur fractures often results in reliable union, with union rates ranging from 96% to 100%. However, femoral nonunions (FNUs) can be a debilitating clinical condition for patients. The purpose of this review was to identify and characterize the predictors of nonunion after femoral midshaft fractures. METHODS: A systematic search was conducted using key terms "femoral shaft non-union," "femoral diaphyseal non-union," "femoral shaft fracture," "femoral diaphysis fracture," and "femoral midshaft fracture" published before June 2024. Proximal Femoral Fractures Following Ipsilateral Transfemoral Amputation in Working-Age Men: A Report of Two Cases.

Lower limb amputation is associated with progressive loss of bone mineral density (BMD) in the residual limb, particularly at the amputated-side proximal femur. However, proximal femoral fractures in working-age individuals with transfemoral amputation who are independently ambulatory with a prosthesis remain insufficiently described. We report the cases of two working-age men who sustained ipsilateral proximal femoral fractures after minor falls at home while not wearing their prostheses. Case 1 sustained a left femoral neck fracture classified as Garden grade I six years after amputation, wh

Locking compression plate fixation of Vancouver type-B1 periprosthetic femoral fractures.

BACKGROUND: Fractures occurring at or near the distal tip of a hip prosthesis with a stable femoral stem (Vancouver type-B fractures) are associated with many complications because of the inherently unstable fracture pattern. Locking compression plates use screws that lock into the plate allowing multiple points of unicortical fixation. Such unicortical fixation may lower the risk of damage to the cement mantle or a stable femoral stem during the treatment of a periprosthetic femoral fracture. The purpose of this study was to analyze clinically and radiographically a group of patients with a V

Incidence of Carpal Tunnel Syndrome in Distal Radius Fractures Treated by Various Modalities in a Tertiary Care Center: A Single Center Study.

Background Distal end radius fractures (DRF), which account for 17.5% of all fractures, are the most frequent fracture seen in emergency rooms. In patients with DRFs, delayed carpal tunnel syndrome (CTS) occurs in about 20% of cases. When patients are treated with DRFs using different modalities, CTS results in poor functional outcomes. Our study aims to identify the prevalence of CTS in DRF patients receiving treatment with various modalities. Materials and methods Two hundred twenty patients with a history of DRFs who were treated by a variety of modalities at R.L. Jalappa Hospital and Resea

Finite Element Analysis of 2- and 3-Point Internal Fixation Methods for the Treatment of Zygomaticomaxillary Complex Fracture.

Open reduction followed by internal fixation (ORIF) has been regarded as the most effective technique for surgical repair of zygomaticomaxillary complex (ZMC) fractures. However, the ideal internal fixation method to achieve stable reduction remains controversial. This research aims to assess and compare the stability of the 2- and 3-point screw-plate fixation methods using finite element method (FEM). Based on computed tomography (CT), the finite element models of 2-point (ZFS + IOR) and 3-point (ZFS + IOR + ZMB) fixation for isolated displaced ZMC fracture were reconstructed. The force of 12
