Mechanism-matched Helicobacter pylori treatment in the potassium-competitive acid blocker era.

Helicobacter pylori infection remains common worldwide and causes chronic gastritis, peptic ulcer disease, gastric mucosa-associated lymphoid tissue lymphoma, and gastric cancer. Eradication therapy has become less reliable as resistance to clarithromycin, metronidazole, and levofloxacin has reduced the performance of older empiric regimens. Potassium-competitive acid blockers (P-CABs), molecular resistance testing, fecal susceptibility assays, and new antimicrobial strategies have changed treatment selection, but none removes the need to match regimen design to microbial biology and antibioti

Pediatric Eosinophil-Predominant Collagenous Gastritis: From Conventional Therapy to Dupilumab: A Case Report.

Background and Clinical Significance: Collagenous gastritis (CG) is a rare disorder characterized by subepithelial collagen deposition, typically presenting in children with iron deficiency anemia (IDA) and abdominal pain. Its pathogenesis remains unclear, and no standardized treatment exists. Case Presentation: A female patient developed IDA at 4.5 years of age, requiring repeated intravenous iron infusions, and recurrent epigastric pain. CG was diagnosed at 13.5 years, with marked gastric eosinophilia (peak 685 eosinophils/mm2) and subepithelial collagen thickening (103.6 μm). Extensive eval

Association between Gastric Acid Suppression Therapy and Scattered White Spots in the Duodenum: A Retrospective Observational Study.

Background and Study Aims Endoscopic duodenal findings remain undescribed after long-term use of antacids despite their known effects on duodenal physiology. We identified scattered white spots (SWSs) as an endoscopic finding of chronic acid suppression in the duodenum and aimed to confirm this association. Patients and Methods In this single-center, two-arm, retrospective study, we reviewed 200 patients who underwent outpatient esophagogastroduodenoscopy: 100 consecutive patients receiving continuous acid-suppression (proton pump inhibitors [PPI] or potassium-competitive acid blockers [P-CAB]

Fourteen-Day Amoxicillin- or Tetracycline-Containing Bismuth Quadruple Therapy versus 14-Day Metronidazole-Based Triple Therapy as the Treatment for Clarithromycin-Resistant Helicobacter pylori Infection: A Multicenter Randomized Controlled Trial.

BACKGROUND/AIMS: Combination therapy comprising a proton pump inhibitor (PPI), amoxicillin, and metronidazole (PAM) is used to treat clarithromycin-resistant Helicobacter pylori in the Republic of Korea, but eradication rates are decreasing due to an increasing incidence of clarithromycin resistance. We compared PAM, bismuth compounds plus PAM (PAM-B), and the combination of PPI, bismuth compounds, metronidazole, and tetracycline (PBMT) to determine whether PAM-B can achieve an eradication rate higher than that of PAM and comparable to that of PBMT. METHODS: This prospective multicenter study

New insights on microglial lysosomal acidification: A therapeutic target of neurodegenerative diseases.

BACKGROUND: Microglia, the resident immune cells of the central nervous system (CNS), maintain brain homeostasis and respond to pathological insults. Microglial dysfunction has been implicated in the pathogenesis of several neurodegenerative diseases, including Alzheimer's disease, Parkinson's disease, and multiple sclerosis. Impaired lysosomal function, particularly defective lysosomal acidification, leads to the accumulation of undegraded material, thereby promoting neuroinflammation and neuronal damage. PURPOSE: This review examines the mechanisms governing lysosomal acidification in micro

Intracellular interaction between FcRY receptor and IgY in ovarian vascular endothelial cells during avian maternal IgY transfer.

Maternal immunoglobulin Y (IgY) is transferred from the bloodstream to the yolk of maturing oocytes in avian species, which is crucial for the passive immunity of neonatal chicks. The receptor responsible for maternal blood IgY transfer to the yolk remains unidentified. Recently, we found that an IgY receptor, FcRY, is expressed in the vascular endothelial cells of ovarian follicles. However, there is no evidence of the direct binding of IgY to FcRY in vascular endothelial cells of avian ovarian follicles. To investigate how ovarian FcRY interacts with IgY, we performed a microscopic analysis

Risk factors for dysplastic lesions in the proximal stomach in patients with familial adenomatous polyposis.

BACKGROUND AND STUDY AIMS: Gastric cancer is a recently recognized challenge in management of familial adenomatous polyposis (FAP), mostly developing in the proximal stomach amid carpeting fundic gland polyposis. Dysplastic lesions in the proximal stomach might be the precursor lesions for gastric cancer in FAP. We aimed to describe incidence of dysplastic lesions in the proximal stomach and to identify risk factors for these dysplastic lesions. PATIENTS AND METHODS: Data were collected from FAP patients who had undergone esophagogastroduodenoscopy (EGD) between 2015 and 2023 at our academic

Regular arrangement of collecting venules as a predictor of Helicobacter pylori absence using white-light endoscopy: a multicenter prospective study outside Asia.

BACKGROUND AND AIMS: Endoscopic assessment of the regular arrangement of collecting venules (RAC) is a simple and reliable tool for predicting the absence of Helicobacter pylori (H. pylori) infection in the stomach, particularly in Asian populations. While initial studies in Western countries have yielded similar findings, RAC assessment has not yet been widely adopted in these settings. This study aims to evaluate the diagnostic accuracy of RAC in determining H. pylori status in a non-Asian population. METHODS: This prospective, multicenter study was conducted in 12 hospitals across non-Asia

Robust inhibitory glycinergic transmission and the effect of bafilomycin, folimycin and EIPA: lessons from the auditory brainstem.

Sustained synaptic transmission requires the continuous replenishment of released synaptic vesicles (SVs). This process is particularly critical in neuronal circuits that operate at high rates and with high temporal precision, such as those in the auditory brainstem. Here, we investigated the effect of SV (re-)filling on inhibitory synapses between the medial nucleus of the trapezoid body (MNTB) and the lateral superior olive (LSO). These synapses transmit information with high speed and fidelity, properties essential for auditory computations such as sound localization. We specifically examin

Population pharmacokinetics of edoxaban in geriatric patients with atrial fibrillation.

BACKGROUND: Atrial fibrillation affects nearly half of geriatric inpatients, with edoxaban frequently used for stroke prevention. However, geriatric patients are underrepresented in drug trials with edoxaban. The limited data and high pharmacokinetic (PK) variability result in using this high-risk medication without a full understanding of its exposure in this population. OBJECTIVES: To characterize the dose-exposure relationship and predict the sufficiency of current fixed-dose guideline for geriatric patients. METHODS: We conducted a single-center prospective PK study in geriatric inpatien

The circulatory arrest recovery ammonia problem (CARAP) hypothesis: A proton magnetic resonance spectroscopy (1H-MRS) study of brain metabolism during neonatal cardiopulmonary bypass surgery.

OBJECTIVE: Congenital heart disease affects 1% of US births, with many babies requiring major cardiothoracic surgery under cardiopulmonary bypass (CPB), exposing the more critical patients to neurodevelopmental impairment. Optimal surgical parameters to minimize neuronal injury are unknown. We used proton magnetic resonance spectroscopy (1H MRS) and blood ammonia assays in a neonatal pig model of CPB to compare 2 approaches, complete circulatory arrest (CA) versus antegrade cerebral perfusion. METHODS: Two-week old piglets (N = 17) were put on a CPB pump and placed in a 3-T magnetic resonance

Real-world management of achalasia and esophagogastric junction outlet obstruction in Italy: results from a national survey.

There is significant variability in clinical guidelines for achalasia, and precise indications for Esophagogastric Junction Outflow Obstruction (EGJOO) are lacking. The recommendations provided in the published literature could be difficult to translate into the clinical practice due to the discrepancy in the available resources. This survey aims to provide insight into the different diagnostic and therapeutic approaches adopted nationwide. An electronic 31-item questionnaire was sent among the members of the Italian Society for Endoscopic Surgery of Endoscopic Surgery and New Technologies (SI

Non-pharmacological approaches in gastroesophageal reflux disease: Evidence-based dietary and lifestyle interventions.

Gastroesophageal reflux disease (GERD) affects 18-28 % of Western populations, with 10-40 % experiencing refractory symptoms despite proton pump inhibitor therapy. We conducted a comprehensive narrative review of dietary interventions in GERD, analyzing studies that measured clinical efficacy through validated symptom scores (GERD-Q, RDQ) and objective reflux parameters (24-h pH-impedance monitoring, acid exposure time). We examined pathophysiological mechanisms, intervention efficacy, and clinical implementation strategies across diverse patient populations. Structured dietary interventions d
