[Clinical description of mixed mania].

DSM-IV mixed states have become the mixed mania and mixed depression in the new DSM-5. One noticeable point is the introduction of nine cations, among which the "with mixed features" specification. These non exclusive specifications may contribute to a more precise identification of mixed clinical pictures, and therefore to offer a more efficient therapeutic answer. Different dimensional approaches are widely documented. They allow the isolation of a mixed factor which is clinically associated with two other specifications: anxious distress and psychotic features. These severity markers may en

[Medical management of patients with atypical degenerative parkinsonian syndromes].

Atypical degenerative parkinsonian syndromes (progressive supranuclar palsy, multiple system atrophy, corticobasal degeneration, Lewy body dementia) are an important differential diagnosis to idiopathic Parkinson's disease. However, because these disorders are characterized by the degeneration of multiple neuronal populations, treatment approaches are much less specific than in Parkinson's disease, where dopamimetic drugs represent the mainstay of therapy. Thus, and because the progression of these disorders is usually more aggressive than Parkinson's disease, many physicians face a form of th

Schizoaffective disorder: clinical aspects, differential diagnosis, and treatment.

Schizoaffective disorders are a clinical reality and their concept has a long history, but there are still problems regarding their diagnostic definition and nosologic categorization. The present definitions of the International Classification of Disease and the Diagnostic and Statistical Manual of Mental Disorders are not sufficient enough to define schizoaffective disorders, especially because of deficits on the longitudinal axis. Schizoaffective disorders occupy a position between schizophrenia and pure mood disorders, especially regarding prognosis and premorbid and sociodemographic variab

Movement disorder, memory, psychiatric symptoms and serum DHEA levels in schizophrenic and schizoaffective patients.

OBJECTIVE: Reports of low levels of dehydroepiandrosterone (DHEA) or its sulphate (DHEA-S) in some schizophrenic patients and in some persons with poorer motoric and cognitive functioning led us to examine clinical correlates of serum DHEA and DHEA-S levels in schizophrenic patients. METHOD: Ratings of abnormal movements, memory and psychiatric symptoms in 17 medicated chronic schizophrenic or schizoaffective inpatients at a state hospital were correlated with serum DHEA and DHEA-S levels, and their ratios with serum cortisol. RESULTS: Controlling for age, higher DHEA levels and/or higher DH

Contested boundaries of bipolar disorder and the limits of categorical diagnosis in psychiatry.

The authors' primary objective is to outline the phenomenology, importance, and available data on issues concerning the boundaries between bipolar disorder and diagnoses such as schizophrenia, unipolar depression, and personality disorders. In addition, by illuminating the many difficulties with the boundaries of one of psychiatry's more robust diagnoses, they hope to awaken in the reader a healthy skepticism about current psychiatric nosology. For a topic of this scope, a literature review must be selective. For each boundary area, a mixture of classic and recent papers covering a range of va

Self-medication among addicts with primary psychiatric disorders.

Thirty-two dually diagnosed inpatients described drug-induced changes in their psychiatric symptoms. All heroin addicts and most alcoholics reported drug-induced improvement of their primary psychiatric symptoms; all cocaine addicts reported symptom worsening. These findings suggest that chronic cocaine abuse may not necessarily be related to attempts at self-medication of psychiatric symptoms.

Clinical and demographic characteristics of chronic inpatients: implication for treatment and research.

Clinical and demographic data were collected on 203 state mental hospital patients whose length of stay was greater than one year. They comprised 45% of the average adult census and 37% were less than 35 years old. Males were younger and females more educated and likely to have been married at one time. The majority have diagnoses of schizophrenia or organic mental disorders; a few have personality disorders. Patients were treated with drugs but risk/benefit parameters remain to be defined. Thirty-two percent of patients have had a violent episode during the past six months. The prevalence of

[Suicidal behavior of the elderly].

Age extensively influences suicidal behaviour. Compared with younger persons, the elderly show higher suicide rates. Suicide attempts more often show characteristics of "unsuccessful suicides". Somewhat more frequent then in younger persons the suicide attempts are caused by depressive or schizophrenic psychosis. Nevertheless with the elderly the most frequent diagnoses are neurosis and depressive reaction. The elderly react depressive more often in relation to life events as conflicts with or loss of relatives or contracting a severe physical disease. Conclusions are drawn for a more adequate

Psychomotor disturbance in depression: defining the constructs.

Four hundred and thirteen depressed patients were rated on eighteen signs of psychomotor disturbance, and the data examined by factor analyses. A three-factor solution was favoured. In addition to 'retardation' and 'agitation' dimensions (whose derived factor scores suggested independence of those two dimensions), a third 'non-interactive' dimension was evident--with derived factor scores correlating significantly with both the retardation and agitation dimensions. Thus, a 'trunk and branch' analogy was suggested for construing psychomotor disturbance, with a truncal 'psychic' component arbori

Reducing Caregiver Burden in First-Episode Psychosis: Effects of a Psychoeducational Pathway Led by an Advanced Practice Nurse-A Multicentre Stepped-Wedge Randomized Trial.

Family caregivers play a pivotal role in the recovery of people experiencing first-episode psychosis (FEP). However, they may suffer from high levels of psychological distress and lack access to structured support. Although international recommendations highlight the need for psychoeducational interventions for caregivers, their implementation in France remains limited. This study aims to evaluate the impact of a structured, multicomponent support pathway, coordinated by advanced practice nurses (APNs), for family caregivers of people with FEP. This multicentre, stepped-wedge (cluster with seq

Automated Analysis of Smiling Mimicry Across the Psychosis-Risk Continuum.

AIM: Early identification and support for individuals with psychotic-like experiences (PLEs) are essential for timely intervention and improved outcomes. Emotional mimicry, which refers to the automatic imitation of another person's emotional expression, such as a smile, is a fundamental social process that supports smooth interpersonal interaction and is known to be impaired in schizophrenia. However, its role across the broader psychosis-risk continuum remains unclear. This study examined smiling mimicry (the emotional mimicry of smiling) in individuals with PLEs and explored its relationshi

Correlates of Engagement and Associations With Outcomes in a Cannabis Harm-Reduction Mobile App for Youth With First-Episode Psychosis: Exploratory Analysis of the CHAMPS Pilot Randomized Controlled Trial.

BACKGROUND: Continued cannabis use among young people with first-episode psychosis (FEP) has been linked to poorer clinical and functional outcomes (eg, increased symptom severity and higher relapse rates). Digital harm-reduction interventions may represent a promising, person-centered approach to reduce at-risk cannabis use behaviors in this population. However, evidence remains limited regarding which subgroups are more likely to engage with these interventions and whether specific levels of engagement are required to achieve more favorable cannabis-related outcomes. OBJECTIVE: This explora

Different profiles of multi-dimensional symptoms and psychological characteristics and their related suicide behavior among college students with psychotic-like experiences.

BACKGROUND: Participants with psychotic-like experiences (PLEs) have higher suicide behavior (e.g., suicide attempts, suicide deaths), but it remains unclear which features influence suicide behavior in this population. This study aims to identify different PLEs subgroups based on multiple clinical symptoms and psychological characteristics relevant to suicidality, and examine their associations with suicide behavior. METHODS: Among 6390 college students at baseline, 2913 screened positive for lifetime PLEs. Latent profile analysis was conducted to identify different PLEs profiles based on ba

Psychotic-like experiences and cognitive biases in adolescents: birthweight and sex moderating effects.

INTRODUCTION: Genetic and environmental factors are involved in the pathogenesis of psychosis. Obstetric complications such as low birth weight are among the more extensively studied environmental risk factors for schizophrenia and the psychosis spectrum. Cognitive biases, such as "jumping to conclusions" (JTC), are also linked to psychosis risk. The study aimed to examine the occurrence of cognitive biases and psychotic experiences in adolescents and explore whether birth weight is associated with these variables, moderated by sex. METHODS: A total of 240 adolescents (aged 14-15) from Sabade

First-episode psychosis among people who are incarcerated: What is hidden behind the "prison psychosis" concept?

"Prison psychosis" is a historical concept that is still used in clinical practice, but little explored in research. In this short paper, we propose to revisit this concept by studying first-episode psychosis (FEP) in the prison population. We analyzed a series of incarcerated people who were hospitalized for a FEP from November 2020 to May 2021 in a French facility. We dated the onset of the first psychotic symptoms (PSs) in relation to the date of incarceration and of hospitalization and we calculated the duration of untreated psychosis (DUP). While the characteristics of our sample were con

Engagement, Disengagement and Re-Engagement in Mental Health Services Among Young Patients With First-Episode Psychosis: A Scoping Review.

INTRODUCTION: Engagement in mental health services among young patients with first-episode psychosis (FEP) is crucial for preventing relapse. We conducted this scoping review to establish the proportion and determinants of engagement, disengagement and re-engagement in outpatient mental health care among young patients from 13 to 35 years with FEP. METHODS: We used the guidelines for scoping review by the Joanna Briggs Institute (JBI) and the Arksey and O'Malley framework. We searched for published and unpublished studies guided by the inclusion criteria: studies published in English from 199

The trajectory of sedative adverse events caused by antipsychotics: a meta-analysis of individual participant data from randomised, placebo-controlled, clinical trials in acute phase schizophrenia.

BACKGROUND: Sedative adverse events are common in patients with schizophrenia undergoing antipsychotic treatment, which affects treatment adherence and the patients' quality of life. Although tolerance to sedation is believed to develop, robust evidence documenting the timing of sedation onset and resolution remains elusive. To address this gap, we aimed to assess the dynamics of onset and resolution of sedation across various antipsychotics in patients with schizophrenia. METHODS: In this meta-analysis, we included placebo-controlled, randomised controlled trials (RCTs) of antipsychotic mono
