Targeted muscle reinnervation is associated with significant analgesic-sparing effect in Asian post-amputation patients: Early utilisation and clinical outcomes.

PurposeTo evaluate early clinical outcomes of targeted muscle reinnervation (TMR) for post-amputation pain in an East Asian population and to describe its utilisation in Hong Kong.MethodsAll patients who underwent major limb amputation with TMR at our institution from January 2017 to December 2024 were reviewed. Fourteen patients (16 TMR procedures) with a minimum of 6 months follow-up were case matched to 16 control patients who underwent major limb amputation without TMR. Primary outcomes were post-amputation pain and analgesic use at latest follow-up (≥6 months). Secondary outcomes included

Arterial switch with pulmonary artery banding: A staged repair strategy for transposition of great arteries with complex intracardiac anatomy.

OBJECTIVE: We present initial results of arterial switch operation (ASO) with pulmonary artery banding (PAB), followed by intracardiac repair for patients with transposition of great arteries (TGA) associated with unusually complex intracardiac anatomy. METHODS: Between 2000 and 2023, these patients (1.9% of 1744 TGA in our institution) were retrospectively reviewed. Staged repair was either strategy A (2000-2018; 1. PAB, 2. ASO + intracardiac repair) or strategy B (2014-2023; 1. ASO + PAB, 2. Intracardiac repair); aortic arch repair could be associated. We compared those strategies. RESULTS

Cleft-to-commissure conversion in atrioventricular septal defect using artificial chordae: Technical report and case illustration.

BACKGROUND: Primary repair of complete atrioventricular septal defect (cAVSD), typically performed at age 3 to 6 months, is associated with favorable early survival and long-term outcomes. However, a significant incidence of reoperation persists, predominantly for left atrioventricular valve (LAVV) regurgitation (LAVVR) and left ventricular outflow tract obstruction. Optimal management of the LAVV cleft remains controversial. Although routine cleft closure is frequently used to mitigate progressive LAVVR and subsequent reoperation, this approach can be complicated by LAVV stenosis, especially

Minimally invasive versus open surgery for the management of intradural extramedullary tumors: a systematic review and meta-analysis.

PURPOSE: Minimally invasive surgery (MIS) has been adopted for intradural extramedullary (IDEM) tumors resection to reduce access-related morbidity associated with open surgery (OS). Whether these tissue-sparing corridors preserve oncologic adequacy and intradural integrity while improving perioperative efficiency remains uncertain. This study compared MIS and OS for IDEM tumors across morbidity, resection quality, and recovery outcomes. METHODS: A PRISMA-guided systematic review and meta-analysis was conducted. PubMed, Embase, and CENTRAL were searched to 1st December 2025 for comparative st

Collared Cementless Stems Show Equivalent Rates of Periprosthetic Fractures Compared to Collarless Cemented Stems in an Exclusively Elderly Cohort of Patients.

BACKGROUND: The optimal femoral stem fixation for total hip arthroplasty for older patients remains debated due to the heightened risk of periprosthetic fractures (PPFx) with decreased bone density. Previous studies in elderly patients have shown cemented fixation leads to fewer revisions, reduced PPFx, and improved long-term implant survival. We hypothesized that modern collared cementless stems would reduce PPFx and overall complications compared to cemented and uncemented collarless stems. To our knowledge, this study represents the first single-center analysis of outcomes in older patients

ORIF versus revision arthroplasty for vancouver B2 periprosthetic femoral fractures after bipolar hemiarthroplasty: A multicenter retrospective study.

BACKGROUND: Periprosthetic femoral fractures (PFFs) following bipolar hemiarthroplasty (BHA) are a serious complication in frail elderly patients. Revision arthroplasty is generally recommended for Vancouver B2 fractures with stem loosening, but it is associated with high perioperative risks. Open reduction and internal fixation (ORIF) has emerged as a less invasive alternative, but its efficacy remains controversial. This multicenter study compared the clinical outcomes of ORIF and revision arthroplasty for Vancouver B2 PFFs after BHA. METHODS: We retrospectively reviewed data from the Traum

Outcomes of Metaphyseal Versus Diaphyseal Stems in Conversion Total Hip Arthroplasty After Intramedullary Nailing.

BACKGROUND: Diaphyseal-engaging stems are conventionally used in conversion total hip arthroplasty (cTHA) after prior intramedullary nailing (IMN) to bypass screw holes and previously instrumented meta-diaphyses. However, it remains unclear if cementless metaphyseal or standard cemented stems can also provide satisfactory fixation in this scenario. This study compared the early outcomes of cTHA using three femoral stem designs: metaphyseal cementless, diaphyseal cementless, and standard cemented. METHODS: A retrospective review of 80 patients who underwent cTHA after prior IMN at a single aca

Synchronous versus Staged Implantation of Inflatable Penile Prosthesis and Artificial Urinary Sphincter: A Nationwide Analysis of Infection and Revision Outcomes.

INTRODUCTION: The timing of implantation of inflatable penile prostheses (IPP) and artificial urinary sphincters (AUS) in patients requiring both devices remains controversial, with limited data comparing outcomes between synchronous dual implantation versus staged procedures. OBJECTIVES: To assess the association between timing of IPP and AUS implantation and subsequent rates of device infection, revision, removal, or replacement using real-world administrative claims data. METHODS: We performed a retrospective review using the MerativeTM Marketscan® research databases from 2007 to 2023. Me

Randomized Trial of Burr-Hole Surgery With or Without Irrigation for Chronic Subdural Hematoma: Impact of Postoperative Pneumocephalus on Recurrence.

BACKGROUND AND OBJECTIVES: Although postoperative subdural drainage is established as an effective strategy to prevent recurrence after burr-hole surgery for chronic subdural hematoma, the necessity of intraoperative irrigation remains unclear. We conducted a randomized controlled trial to compare burr-hole surgery with and without intraoperative irrigation, with postoperative subdural drainage performed in both groups, and to explore factors associated with recurrence. METHODS: Between May 2020 and October 2023, 242 patients undergoing initial burr-hole surgery for chronic subdural hematoma

Conversion Total Hip Arthroplasty Following Prior Internal Fixation: A Comparison With Primary and Revision Total Hip Arthroplasty.

BACKGROUND: With rising life expectancy, the incidence of total hip arthroplasty (THA) following prior femoral and/or acetabular fracture fixation is increasing. Conversion THA (cTHA) following these procedures is technically complex and associated with unique challenges. However, direct comparisons between primary THA (pTHA), cTHA, and revision THA (rTHA) remain limited. METHODS: This was a retrospective cohort study comparing intraoperative and postoperative outcomes among patients undergoing cTHA from April 2012 to October 2022. This was defined as THA following previous fixation with hard

Associations between preoperative hypoalbuminemia and clinical outcomes following total hip or knee arthroplasty: a systematic review and meta-analysis.

BACKGROUND: Preoperative hypoalbuminemia is common in patients undergoing total hip or knee arthroplasty (THA/TKA). While it has been linked with poorer postoperative outcomes, there remains a paucity of systematic reviews or meta-analyses dedicated to conducting a comprehensive evaluation of this issue. METHODS: A comprehensive search was conducted across the PubMed, Embase, and Web of Science databases. Studies were selected and analyzed in adherence to the Meta-analysis of Observational Studies in Epidemiology (MOOSE) guidelines, and the quality of the evidence was evaluated using the Grad

Volar Locking Plate Versus External Fixation for Distal Radius Fractures: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

Distal radius fractures (DRFs) are among the most common fractures of the upper extremity, and management remains controversial. Both volar locking plate (VLP) and external fixation (EF) are widely used. We aim to compare functional, radiographic, and safety outcomes between VLP and EF in the treatment of DRFs to guide evidence-based management. We systematically searched PubMed, Scopus, Web of Science, and Embase through October 2025 for English-language RCTs comparing VLPs versus EFs in adults with DRFs. Quality assessment was performed using the Cochrane Risk of Bias tool (version 2). Meta-

Outcomes of Simultaneous Versus Staged Hardware Removal and Total Knee Arthroplasty.

BACKGROUND: This study compares clinical and functional outcomes between simultaneous hardware removal during total knee arthroplasty (TKA) and staged TKA after prior hardware removal. METHODS: We retrospectively reviewed 155 patients who had prior knee hardware and underwent elective primary TKA between 2012 and 2024 at an urban academic institution. Patients were categorized into "simultaneous" removal during TKA (n = 127) or "staged" TKA after removal (n = 28), and stratified by hardware type (minor/moderate/major). RESULTS: Simultaneous procedures involved significantly less "major hardw

Radiographic paraspinal muscle size in adult spinal deformity: a systematic review and meta-analysis.

PURPOSE: Reduced trunk muscle quantity or quality, commonly assessed using imaging-based metrics, has been associated with adverse outcomes in spine surgery. However, evidence specific to adult spinal deformity (ASD) surgery remains heterogeneous and methodologically limited. This systematic review aimed to synthesize existing muscle metrics and surgical outcomes in patients undergoing ASD surgery. METHODS: A systematic search of PubMed and EMBASE was performed to identify studies evaluating imaging-based muscle quantity or quality in adult patients (≥ 18 years) undergoing thoracolumbar corre

Stress shielding in radial head arthroplasty: a comparative analysis of partially vs. fully grit-blasted press-fit stems.

BACKGROUND: Stress shielding is a known cause of periprosthetic bone loss in joint arthroplasty, but its clinical implications in radial head arthroplasty (RHA) remain unclear. Design modifications, such as partially grit-blasted stems, aim to mitigate proximal radial bone resorption while maintaining stability. We hypothesized that partially grit-blasted stems will have less stress shielding than fully grit-blasted stems. METHODS: A retrospective review of 28 patients who underwent press-fit RHA with either partially grit-blasted (n = 21) or fully grit-blasted (n = 7) short-stem implants bet
