Effect of robotic assistance on the "learning curve" for laparoscopic hand-assisted donor nephrectomy.

BACKGROUND: The number of living-related donor kidney transplantations have increased since the advent of minimally invasive surgery. Robotic technology has emerged as a promising alternative to laparoscopic techniques. The authors reviewed their institution experience with robotic hand-assisted donor nephrectomies (RHADNs). METHODS: Between August 2000 and April 2006, 273 robotically assisted left donor nephrectomies were performed using a hand-assisted technique. Prospectively collected information for 214 patients regarding complications, hospital stay, blood loss, warm ischemia time, oper

Robotic vs. laparoscopic redo ileocolonic resection for recurrent Crohn's disease.

Robotic surgery is increasingly utilized for primary ileocolic Crohn’s disease (CD), but its role in recurrent CD is less established. This study aimed to evaluate the feasibility and outcomes of robotic redo ileocolic resection (ICR). We retrospectively reviewed adult patients with recurrent CD who underwent either robotic (RR-ICR) or laparoscopic redo (LR-ICR) ICR at our institution between January 2020 and November 2024. Perioperative variables, conversion rate, short- and long-term postoperative outcomes were compared. Sixty-six patients were included: 24 (36.4%) underwent RR-ICR and 42 (6

Better outcomes of robotic liver resection in segment VII/VIII compared to open and laparoscopic approach.

BACKGROUND: With the introduction of minimally invasive surgery, laparoscopic and robotic approaches have become increasingly popular for liver resections. However, few studies have compared the perioperative outcomes of open (OLR), laparoscopic (LLR), and robotic liver resections (RLR) together at once for lesions located in the superior parts of the right anterior and posterior segments, S7 and S8. This study reports data for S7 and S8 up to 5 years post-operation. METHODS: Perioperative data of S7 and S8 resections performed at Prince of Wales Hospital from January 2014 to December 2024 wer

Robot-assisted versus laparoscopic total gastrectomy: a western high-volume tertiary referral center experience.

Robot-assisted gastrectomy has been increasingly adopted as a standard of care, but comparative evidence in Western cohorts remains limited, particularly for total gastrectomy, where esophagojejunostomy and D2 lymphadenectomy are technically demanding. The aim of this study was to compare short-term outcomes after robot-assisted and laparoscopic total gastrectomy at a European high-volume tertiary referral center. This retrospective single-center cohort study included all patients undergoing minimally invasive total gastrectomy (laparoscopic or robot-assisted) for gastric cancer or prophylaxis

Robot-assisted versus open radical cystectomy with extracorporeal urinary diversion for bladder cancer: a GRADE-assessed systematic review and meta-analysis of randomized controlled trials with subgroup analyses by orthotopic neobladder utilisation.

BACKGROUND: Robot-assisted radical cystectomy (RARC) has become a minimally invasive alternative to open radical cystectomy (ORC) for muscle-invasive bladder cancer, with potential advantages in perioperative outcomes and recovery times. However, their comparative effectiveness remains debatable, particularly when extracorporeal urinary diversion is employed in both approaches. This systematic review and meta-analysis integrated evidence from randomized controlled trial data comparing RARC with ORC using extracorporeal reconstruction, with novel subgroup analyses by neobladder utilization and

The management of pancreatic tail during robotic-assisted spleen-preserving left-sided pancreatectomy.

BACKGROUND: The anatomical relationship between the pancreatic tail and splenic vessels influences both pancreatic resection completeness and spleen preservation success in robotic spleen-preserving left-sided pancreatectomy (RSPLP). However, surgical strategies for managing the pancreatic tail during RSPLP remain unreported. METHODS: Clinical data from 46 consecutive patients undergoing robot-assisted left-sided pancreatectomy with intended spleen preservation were analyzed. Pancreatic tails were classified into four anatomical types (I-IV) based on their relationship with splenic vessels or

Central ligation or partial preservation of the right gastric artery does not seem to affect conduit or anastomotic perfusion during robot-assisted resection of gastroesophageal junction cancer: a randomized clinical trial.

The gastric conduit can be created with partial preservation or a central ligation of the right gastric artery. Central ligation may facilitate complete removal of lymph node (LN) station 3; however, whether this influences conduit and anastomotic perfusion is unknown. Hence this study investigated whether a central ligation of the right gastric artery would affect conduit or anastomotic perfusion compared with partial preservation (local standard) during robot-assisted resection of gastroesophageal junction cancer. Patients scheduled for robot-assisted resection of gastroesophageal junction c

Open vs. laparoscopic vs. robotic pouch excision: unveiling the best approach for optimal outcomes.

INTRODUCTION: Despite advantages for patients with ulcerative colitis, Crohn's disease, and familial adenomatous polyposis, restorative proctocolectomy with ileal pouch-anal anastomosis carries a risk of pouch failure, necessitating pouch excision. The traditional open approach is associated with potential complications. Robotic and laparoscopic techniques are emerging, but comparative outcome data are limited. METHODS: We conducted a retrospective study of consecutive adult patients undergoing robotic, laparoscopic, and open ileal pouch excision at Mayo Clinic, Rochester, MN, between January
