Skeletal Muscle Injury Biomarkers: Assay Qualification Efforts and Translation to the Clinic.

Skeletal muscle (SKM) injury or myopathy results in structural or functional defects in SKMs that can be caused by variety of factors such as (1) genetic, (2) drug-induced, (3) disease progression (cachexia), or (4) aging (sarcopenia). Creatine kinase (CK) and aspartate transaminase (AST) activity assays have been routinely used as SKM injury biomarkers, but they lack sensitivity and tissue specificity. In collaboration with the Predictive Safety Testing Consortium, we evaluated the diagnostic performance of a muscle injury biomarker panel (MIP) compared to CK and AST and their correlation wit

Branched-chain amino acids ameliorate heart failure with cardiac cachexia in rats.

AIMS: Heart failure (HF) is associated with changes in energy metabolism of the heart, as well as in extra-cardiac organs such as the skeletal muscles. Cardiac cachexia is a common complication and is associated with poor prognosis. Branched-chain amino acids (BCAAs) reportedly improve sarcopenia and cancer cachexia. We tested the hypothesis that BCAA ameliorates HF with cardiac cachexia. MAIN METHODS: We used Dahl salt-sensitive (DS) rats fed a high-salt diet as a model of HF. DS rats fed a low-salt diet were used as a control. BCAA were administered in drinking water from 11weeks of age, wh

μ-Crystallin controls muscle function through thyroid hormone action.

μ-Crystallin (Crym), a thyroid hormone-binding protein, is abnormally up-regulated in the muscles of patients with facioscapulohumeral muscular dystrophy, a dominantly inherited progressive myopathy. However, the physiologic function of Crym in skeletal muscle remains to be elucidated. In this study, Crym was preferentially expressed in skeletal muscle throughout the body. Crym-knockout mice exhibited a significant hypertrophy of fast-twitch glycolytic type IIb fibers, causing an increase in grip strength and high intensity running ability in Crym-null mice. Genetic inactivation of Crym or blo

Long-term bezafibrate treatment improves skin and spleen phenotypes of the mtDNA mutator mouse.

Pharmacological agents, such as bezafibrate, that activate peroxisome proliferator-activated receptors (PPARs) and PPAR γ coactivator-1α (PGC-1α) pathways have been shown to improve mitochondrial function and energy metabolism. The mitochondrial DNA (mtDNA) mutator mouse is a mouse model of aging that harbors a proofreading-deficient mtDNA polymerase γ. These mice develop many features of premature aging including hair loss, anemia, osteoporosis, sarcopenia and decreased lifespan. They also have increased mtDNA mutations and marked mitochondrial dysfunction. We found that mutator mice treated

Myofiber ellipticity as an explanation for transverse asymmetry of skeletal muscle diffusion MRI in vivo signal.

Due to its unique non-invasive microstructure probing capabilities, diffusion tensor imaging (DTI) constitutes a valuable tool in the study of fiber orientation in skeletal muscles. By implementing a DTI sequence with judiciously chosen directional encoding to quantify in vivo the microarchitectural properties in the calf muscles of three healthy volunteers at rest, we report that the secondary eigenvalue is significantly higher than the tertiary eigenvalue, a phenomenon corroborated by prior DTI findings. Toward a physics-based explanation of this phenomenon, we propose a composite medium mod

Results of a prospective pilot trial on mobility after whole body vibration in children and adolescents with osteogenesis imperfecta.

OBJECTIVE: To evaluate the effect of whole body vibration on the mobility of long-term immobilized children and adolescents with a severe form of osteogenesis imperfecta. Osteogenesis imperfecta is a hereditary primary bone disorder with a prevalence from 1 in 10000 to 1 in 20000 births. Most of these children are suffering from long-term immobilization after recurrent fractures. Due to the immobilization they are affected by loss of muscle (sarcopenia) and secondary loss of bone mass. SUBJECTS: Whole body vibration was applied to eight children and adolescents (osteogenesis imperfecta type 3

Slowing down aging from within: mechanistic aspects of anti-aging hormetic effects of mild heat stress on human cells.

Since aging is primarily the result of a failure of maintenance and repair mechanisms, various approaches are being developed in order to stimulate these pathways and modulate the process of aging. One such approach, termed hormesis, involves challenging cells and organisms by mild stress that often results in anti-aging and life prolonging effects. In a series of experimental studies, we have reported that repeated mild heat stress (RMHS) has anti-aging hormetic effects on growth and various cellular and biochemical characteristics of human skin fibroblasts undergoing aging in vitro. These be

Effect of an Integrated Exercise Training Program on Muscle Mass and Functional Capacity in Sarcopenic Patients With Decompensated Liver Cirrhosis.

BACKGROUND: Liver cirrhosis is an end-stage disease with extensive fibrosis that disrupts normal architecture and impairs hepatic function. Progression to decompensation triggers severe complications like ascites, jaundice, and hepatic encephalopathy. A major extrahepatic consequence is sarcopenia (muscle wasting), which affects over 65% of these patients. Driven by ammonia accumulation and chronic inflammation, this muscle loss drastically increases frailty and mortality risk. Combined aerobic and resistance exercise (CARE) represents a highly promising, underutilized rehabilitation strategy

Beyond body surface area: CT-derived body composition as a predictor of chemotherapy toxicity and survival in pancreatic ductal adenocarcinoma: a systematic review.

BACKGROUND: Pancreatic ductal adenocarcinoma (PDAC) is characterised by a high prevalence of severe muscle wasting (sarcopenia) and fatty muscle infiltration (myosteatosis), yet chemotherapy dosing still relies on body surface area (BSA), a metric that does not reflect individual patients' lean body mass (LBM) or muscle quality. A growing body of evidence from oncology meta-analyses demonstrates that low skeletal muscle mass (sarcopenia) independently predicts chemotherapy toxicity across multiple cancer types, and that myosteatosis is associated with significantly increased mortality risk. We

Conservative kidney management versus dialysis for stage 5 chronic kidney disease in older people.

RATIONALE: The prevalence of kidney failure in people aged 65 years and above is gradually increasing. However, there is insufficient evidence to determine which treatment is better for people with kidney failure, conservative kidney management (CKM) or dialysis. OBJECTIVES: We aimed to assess the effects of CKM and dialysis in older people with kidney failure. SEARCH METHODS: We searched the Cochrane Kidney and Transplant Register of Studies, MEDLINE, Embase, the WHO International Clinical Trials Registry Platform (ICTRP), ClinicalTrials.gov and regional databases (LILACS, KoreaMed, CADTH),

Mitochondria-targeted drug delivery systems for aging-related diseases: Mechanisms, strategies, and advances.

Mitochondrial dysfunction, characterized by increased oxidative stress, impaired energy metabolism, disrupted mitochondrial dynamics, abnormal quality control, and activation of apoptosis, represents a critical pathological basis for a wide range of aging-related diseases (ARDs), including neurodegenerative diseases, cardiovascular disorders, sarcopenia, and osteoarthritis. Mitochondria-targeted drug delivery systems increase the effective concentration of bioactive molecules within these dysfunctional organelles, offering novel strategies for the precise modulation of mitochondrial function.

Loss of Splicing Homeostasis as a Hallmark of Aging.

Alternative splicing is a fundamental mechanism that ensures accurate gene expression, supports cellular adaptability, and expands protein diversity beyond the limits of a fixed gene pool. With aging, splicing fidelity weakens, contributing to decline in RNA homeostasis and disrupting essential cellular functions, including mitochondrial oxidative phosphorylation, genome stability, and immune regulation, and in turn accelerating tissue and organ dysfunction. Evidence from senescent cells, aged tissues, and model organisms shows that altered levels of splicing factors and increased RNA polymera

Bridging the Developmental Gap: Pediatric Considerations for the ISHLT Consensus Statement on Frailty in Lung Transplantation.

The recent International Society for Heart and Lung Transplantation consensus statement on frailty provides a crucial framework for operationalizing objective frailty metrics and prehabilitation in adult lung transplant candidates. However, translating adult frailty constructs to pediatric candidates with end-stage lung disease presents unique physiological and developmental challenges, as measures of physical performance must account for ongoing linear growth, somatic maturation, and neurodevelopmental trajectories. These complexities are heightened in critically ill pediatric patients, parti

Myostatin inhibition with orally administered Lactobacillus casei expressing a modified human myostatin protein: functional benefits and translational potential in advanced Duchenne muscular dystrophy.

BACKGROUND: Duchenne muscular dystrophy (DMD) is a progressive neuromuscular disorder that requires novel therapeutic approaches beyond dystrophin restoration. Myostatin, a negative regulator of muscle growth, has emerged as a promising target to enhance muscle mass and function. METHODS: We evaluated the efficacy of an orally administered Lactobacillus casei strain expressing a modified human myostatin protein (BLS-M22), in 32-week-old mdx mice. Animals received BLS-M22 or control (L. casei-pgsA) for 8 weeks (control group = 8, treated group = 7) and 12 weeks (control group = 8, treated grou

Covert hepatic encephalopathy in cirrhosis: implications for early diagnosis and appropriate management.

Covert hepatic encephalopathy (CHE) is a frequent and clinically relevant complication of liver cirrhosis, affecting approximately 30-70% of patients. Despite the absence of overt neurological symptoms, CHE is associated with impaired quality of life and increased risks of falls, traffic accidents, hospitalization, progression to overt HE (OHE), and mortality. The pathophysiology of HE, including CHE and OHE, is multifactorial and involves complex interactions among hyperammonemia, systemic inflammation, oxidative stress, gut dysbiosis, bile acid dysregulation, and sarcopenia along the gut-liv

Macrophage and fibro-adipogenic progenitor communication in skeletal muscle regeneration: tissue homeostasis and pathogenic remodeling.

Skeletal muscle regeneration depends on coordinated interactions between macrophages, fibro-adipogenic progenitors, and muscle stem cells. Following injury, macrophages transition from proinflammatory to anti-inflammatory phenotypes, regulating debris clearance, cytokine secretion, and the activity of fibro-adipogenic progenitors and muscle stem cells. Fibro-adipogenic progenitors transiently support muscle stem cell-mediated regeneration but, if not cleared appropriately, differentiate into fibroblasts or adipocytes, contributing to fibrosis and fatty infiltration. Dysregulated macrophage-fib

Challenges and Methodologies to Assess Protein Requirement and Quality Across Different Life Stages in Dogs: A Review.

Determining protein requirements (PRs) for dogs remains a longstanding challenge. During growth, the rapid rate of protein deposition increases the demand for amino acids. In adult dogs, differences in overall diet digestibility and lower energy requirements of domestic dogs have led to discrepancies between the minimum crude protein (CP) value proposed by the National Research Council (NRC; 80 g of CP/kg of diet) and the 180 g of CP/kg of diet proposed by the European Pet Food Industry Federation (FEDIAF) and the Association of American Feed Control Officials (AAFCO), although most commercial

Rehabilitation for Cardiorenal Multimorbidity: Epidemiology, Functional Phenotypes, and Effects on Physical Function, Renal Trajectory, and Prognosis.

Background/Objectives: Cardiac and renal dysfunction frequently coexist and interact bidirectionally, constituting cardiorenal syndrome (CRS). In aging societies, this overlap is increasingly conceptualized within cardiovascular-kidney-metabolic (CKM) syndrome, in which metabolic risk factors, chronic kidney disease (CKD), and cardiovascular disease cluster and worsen prognosis. Patients with cardiorenal multimorbidity exhibit reduced exercise tolerance, physical activity, and skeletal muscle function, leading to frailty, disability, recurrent hospitalization, and reduced tolerance of disease-
