Harvesting large fascia lata sheaths: a rational approach.

This article describes an "S"-shaped incision for the open approach of harvesting wide sheets of fascia lata with reference to the important anatomical landmarks. Forty-three patients required dural replacement in cases of tumors, trauma, or cerebrospinal fluid leak involving the anterior skull base. The extended anterior suberanial approach to the skull base was used for all patients. Early functional status of the operated limb in seven of the patients treated first was assessed by physical examination and then by means of the computerized Kinetic Communicator (Kin-Com; Medex Diagnostics, Ca

Recovery of olfactory function after an anterior craniofacial approach.

We assessed recovery of olfactory function in five consecutive patients who underwent surgical resection of midline skull base tumors using an anterior craniofacial approach in which the cribriform plate was mobilized with the dura. Olfactory function was evaluated before and after surgery using an intravenous olfactory test with prosultiamine and also a standard olfactory acuity test. Before surgery, one patient showed anosmia attributable to obstruction of the nasal cavity and olfactory cleft, and four patients showed normal olfactory function. After surgery, one patient was irreversibly ano

[The retro-auricular, transmastoid, infralabyrinth approach. A simple route for excision of tumors of the jugular foramen].

BACKGROUND: The jugular foramen is a complex area of the skull base. Its contents, the anatomical relationships in the region, and its location at the skull base, are responsible for problematic surgical approaches. The classical infratemporal surgical routes remain complex and difficult to perform. The lateral approach through a mastoidectomy is almost always associated with an anterior transposition of the facial nerve although that transposition is usually unnecessary. METHODS: Progressive drilling along the sinusojugular axis, inferior to the labyrinth and medial to the third portion of t

Inferior mastoidectomy-hypotympanic approach for surgical removal of glomus jugulare tumors: an anatomical and radiologic study emphasizing distances between critical structures.

The technique of inferior mastoidectomy-hypotympanic dissection, with preservation of the middle and inner ear structures, has been established for radical surgical removal of glomus tumors involving the skull base. In order to study the gross anatomic differences and correlate with the computed tomography (CT) scan, 20 human temporal bones were dissected and accurate measurements between vital structures were made. This study revealed a wide variation in distances between the neurovascular structures, whereas the distances between inner ear structures were not statistically different. This or

Head trauma and exposure to prolactin-elevating drugs as risk factors for male breast cancer.

On the basis of information obtained from a population-based cancer registry in Sweden, male patients with breast cancer (n = 95) were found to have experienced significantly more brain concussions and skull fractures than male patients with lung cancer (n = 383) or malignant lymphoma (n = 69). Other risk factors significantly associated with breast cancer among men were drug treatment associated with prolactin elevations, radiation treatment, family history of breast cancer among first-degree relatives, a history of gynecomastia, gonadal injury, and treatment for inguinal hernias. The results

Primary dural lymphoma of the chronic lymphocytic lymphoma/small lymphocytic lymphoma-mimicking meningioma: illustrative case.

BACKGROUND: Lymphomas originating from the dura mater of the skull, known as primary dural lymphomas (PDLs), are rare. Among these, cases of chronic lymphocytic leukemia (CLL)/small lymphocytic lymphoma (SLL) are exceptionally uncommon, and no standardized treatment protocols have been established. The authors report a rare case of CLL/SLL manifesting as PDL mimicking a skull base meningioma and review the relevant literature. OBSERVATIONS: A 71-year-old male presented with an extra-axial tumor extending from the petroclival to middle cranial fossa, which demonstrated progressive enlargement

Case Report: Oral-Nasal Damage in a Person Using Fentanyl and Xylazine.

BACKGROUND: Xylazine, an alpha-2-adrenergic agonist, was added to illicitly manufactured fentanyl within the past decade. Intranasal drug use is common and associated with damage to nasal structures and the palate. There have been no case reports detailing intranasal damage secondary to fentanyl and xylazine use. CASE SUMMARY: We present a case of a 39-year-old woman with a long history of intranasal opioid use and, more recently, xylazine use who presented for treatment due to nasal septum perforation and oral-nasal fistulas. She started treatment for opioid use disorder and was referred to

Congenital Vault Depression in an Infant Requiring Manual Pelvis Disengagement During Delivery.

Congenital vault depression (CVD) is a rare neonatal cranial finding that typically results from chronic intrauterine pressure and remodeling rather than acute delivery trauma. To the best of our knowledge, this report is the first that describes CVD in a term infant delivered via Caesarean section requiring manual pelvic disengagement due to a significantly impacted fetal head. Upon delivery, a 4 x 3 cm depression was noted in the right temporoparietal bone; however, the infant remained neurologically intact, and imaging via X-ray and computed tomography confirmed a concave deformity without

Rare Complications of Chronic Rhinosinusitis Associated With Nasal Polyps and Osseous Metaplasia.

Chronic rhinosinusitis with nasal polyps (CRSwNP) is typically a benign condition; however, osseous metaplasia (OM), skull base erosion, and intracranial extension are rare complications, especially in young patients. Reporting such cases is important to increase awareness of unusual presentations of CRSwNP and guide management. We report the case of an 18-year-old male with a history of asthma and prior polypectomy who presented with worsening frontal headaches, subtle visual changes, and right eye proptosis. Imaging revealed extensive pansinusitis with complete frontal sinus opacification, b

A deep learning framework for bone fragment classification in owl pellets using YOLOv12.

Non-invasive monitoring of small mammal populations is critical for both biodiversity conservation and integrated pest management, particularly in agroecosystems. Barn owl (Tyto alba) pellet analysis has long served as a valuable tool for inferring prey abundance, yet conventional bone classification is labour-intensive and requires specialized expertise. Here, we introduce a deep learning framework that automates the detection and classification of rodent bone fragments from owl pellets using the YOLOv12 object detection architecture. A dataset comprising 978 annotated images, encompassing sk

Pixel-responsive optimization beamforming method for ultrasound transcranial imaging.

The propagation of acoustic waves through bone remains a longstanding challenge in transcranial ultrasound imaging. As a highly scattering medium, the skull causes significant distortions in the ultrasonic wavefield, introducing complex aberrations that hinder precise image reconstruction. Conventional delay-and-sum (DAS) algorithms, which process pixels independently, fail to account for inter-pixel relationships, limiting their ability to correct such distortions. To address this issue, we propose a Pixel-Responsive Optimization (PRO) Beamforming Method that leverages backscattered signals f

A case of hypoglossal nerve palsy with evolving cranial nerve involvement in renal cell carcinoma: a case report.

BACKGROUND: Renal cell carcinoma is a rare pediatric solid tumor that typically presents with hematuria, abdominal mass, or flank pain. It is uncommon for renal cell carcinoma to manifest with headache and isolated extra-urogenital symptoms. We present, to our knowledge, the first case of renal cell carcinoma with bony metastases, presenting initially as isolated cranial nerve twelve palsy. Although bony metastases can occur in renal cell carcinoma, skull-based metastases and cranial neuropathies are exceedingly rare, especially in the pediatric population. CASE PRESENTATION: We describe the

[Giant pericranial sinus with extensive occipital calvarium defect].

BACKGROUND: Giant non-traumatic and non-iatrogenic cranial vault defects are poorly studied due to their rarity. Therefore, diagnosis and analysis of their causes are difficult. In available literature, we found only 4 cases of giant pericranial sinus accompanied by extensive cranial vault defects. OBJECTIVE: To present a rare and difficult diagnostic case of cranial vault lesion with non-traumatic and non-iatrogenic extensive bone defect, as well as to propose treatment of this pathology. UNLABELLED: The patient presented with complaints of a «growing» parietal-occipital cranial vault defec

The Transcaruncular Corridor of the Medial Transorbital Approach to the Frontal Lobe: Technical Nuances and Applications.

BACKGROUND AND IMPORTANCE: Medial orbital access through a transcaruncular corridor has yet to be fully characterized as a potential approach to intradural lesions within the skull base. Transorbital approaches present unique potential in the management of complex neurological pathologies and require subspecialty collaboration across multiple disciplines. CLINICAL PRESENTATION: A 62-year-old man presented with progressive confusion and mild left-sided weakness. He was found to have a right frontal lobe mass with significant vasogenic edema. A comprehensive systemic workup was otherwise unrema

Review of olfactory cleft roof anatomy.

Progress in skull-base endoscopic surgery has improved our knowledge of olfactory cleft (OC) anatomy. This article presents a review of current knowledge concerning the OC roof: descriptive anatomy, radiological exploration, and endoscopic observation. The OC is a narrow area in the most superior part of the nasal cavity. Its roof is the thinnest structure separating the nasal cavities from the brain; it comprises three superimposed tissues: nasal mucosa, ethmoid cribriform plate (ECP), and dura mater. The ECP comprises the anterior ECP containing the ethmoidal slit (ES) medially and the cribr

Expanding the clinical spectrum of idiopathic intracranial hypertension.

PURPOSE OF REVIEW: Idiopathic intracranial hypertension (IIH) is a disorder of raised intracranial pressure (ICP). Although the majority of patients with IIH present classically with headache and papilledema, some patients may have unusual presentations or manifestations. Recent advancements in neuroimaging have facilitated the identification of other presentations associated with IIH. This review provides an overview of the expanding clinical spectrum of IIH. RECENT FINDINGS: Presentations of IIH that are considered unusual include highly asymmetric or unilateral papilledema, IIH without pap

The growth and expansion of meningeal lymphatic networks are affected in craniosynostosis.

Skull malformations are associated with vascular anomalies that can impair fluid balance in the central nervous system. We previously reported that humans with craniosynostosis and mutations in TWIST1 have dural venous sinus malformations. It is still unknown whether meningeal lymphatic networks, which are patterned alongside the venous sinuses, are also affected. We now show that the growth and expansion of meningeal lymphatics are perturbed in Twist1 craniosynostosis models. Changes to the local meningeal environment, including hypoplastic dura and venous malformations, affect the ability of

Type III cutaneous atypical meningioma of the scalp.

Meningiomas occur rarely in extracranial sites, including the skin, where they may pose a diagnostic challenge because of their histopathologic overlap with several other spindle-cell tumors. Cutaneous meningiomas are divided into type I (congenital), type II (ectopic), and type III (via direct extension) lesions. We present a rare case of atypical meningioma of the skin in a 71-year-old woman who presented with a painful and enlarging lesion on the left central frontal scalp. Biopsy showed bone and soft tissue with involvement of a spindle cell neoplasm, consisting of whorled nests with atypi
