Early Palliative Care in Advanced Hematologic Malignancies: A Systematic Review of Patient-Centered Outcomes.

Background: Patients with hematologic malignancy (HM) experience high symptom burden (SB) and diminished quality of life (QOL). While early palliative care (EPC) benefits solid tumors, its impact in HM remains uncertain. Objectives: To systematically review the effects of EPC on patient-centered outcomes in individuals with HM. Methods: MEDLINE, Web of Science, Cochrane Library, and Scopus were searched for English-language articles published between 2020 and 2024. Eligible studies included adults with advanced HM receiving EPC compared with usual care, reporting outcomes on SB, QOL, place of

The impact of myelosuppression on quality of life of patients treated with chemotherapy.

Side effects from chemotherapy-induced myelosuppression can negatively affect patients' quality of life (QoL). Neutropenia increases infection risk, and anemia frequently results in debilitating fatigue. Additionally, the bleeding risk associated with thrombocytopenia can lead to fear and anxiety. However, traditional interventions for myelosuppression fall short of the ideal. Granulocyte colony-stimulating factors reduce the risk of severe neutropenia but commonly lead to bone pain. Erythropoiesis-stimulating agents are not always effective and may cause thromboembolic events, while transfusi

Patient outcomes, patient experiences and process indicators associated with the routine use of patient-reported outcome measures (PROMs) in cancer care: a systematic review.

PurposeIn current cancer care, there is a growing debate about the value of using patient-reported outcome measures (PROMs) in daily clinical follow-up. A systematic review of the literature was conducted to assess the evidence of the effectiveness of the routine use of PROMs in daily cancer care in terms of patient outcomes, patient experiences and process indicators and to identify the effect of giving feedback about PROM findings to patients and/or health care professionals (HCPs).MethodsA systematic search was performed. Studies were eligible for inclusion when they (1) used a PROM as an i

Acceptability and usefulness of the EORTC 'Write In three Symptoms/Problems' (WISP): a brief open-ended instrument for symptom assessment in cancer patients.

BackgroundThe use of open-ended questions supplementing static questionnaires with closed questions may facilitate the recognition of symptoms and toxicities. The open-ended 'Write In three Symptoms/Problems (WISP)' instrument permits patients to report additional symptoms/problems not covered by selected EORTC questionnaires. We evaluated the acceptability and usefulness of WISP with cancer patients receiving active and palliative care/treatment in Austria, Chile, France, Jordan, the Netherlands, Norway, Spain and the United Kingdom.MethodsWe conducted a literature search on validated instrum

Assessing the impact of open-label designs in patient-reported outcomes: investigation in oncology clinical trials.

BackgroundKnowledge of treatment assignment may affect patient-reported outcomes (PROs), which is of concern in oncology, where open-label trials are common. This study measured the magnitude of open-label bias by comparing PROs for similar patient groups in oncology trials with different degrees of concealment.MethodsIndividual patient data from ipilimumab arms of 2 melanoma and docetaxel arms of 2 non-small cell lung cancer (NSCLC) trials were adjusted for differences using propensity score weighting. Patients were aware of treatment assignment in CA184-022 and CheckMate 057 (open-label) but

ACcurate COnsensus Reporting Document (ACCORD) explanation and elaboration: Guidance and examples to support reporting consensus methods.

BackgroundWhen research evidence is limited, inconsistent, or absent, healthcare decisions and policies need to be based on consensus amongst interested stakeholders. In these processes, the knowledge, experience, and expertise of health professionals, researchers, policymakers, and the public are systematically collected and synthesised to reach agreed clinical recommendations and/or priorities. However, despite the influence of consensus exercises, the methods used to achieve agreement are often poorly reported. The ACCORD (ACcurate COnsensus Reporting Document) guideline was developed to he

Virtual reality hypnosis during protective isolation: a parallel group randomized trial.

BACKGROUND: Patients in protective isolation often develop symptoms of anxiety. Virtual reality therapy and hypnosis could be of benefit to these patients, as both have been shown to relieve anxiety in a hospital setting. In this study, we investigated the use of virtual reality hypnosis (VRH), which induces a state of hypnosis. The primary aim was to determine whether VRH would reduce patients' anxiety during protective isolation. The secondary aim was to determine whether it would also relieve any pain. METHODS: This parallel group, open-label, single-center randomized trial included adult

Sex differences in cancer incidence and survival: a Danish nationwide population-based study assessing 35 cancer sites.

BACKGROUND: Sex differences in cancer incidence and survival have been documented, but underlying mechanisms remain unclear. We examined sex differences in incidence and survival for non-sex-specific cancers and the role of socioeconomic factors and comorbidity. METHODS: All individuals living in Denmark from 2004-2020 were included. Incidence rate ratios (IRRs) and excess mortality ratios (EMRs) for 35 cancers were estimated using Poisson regression adjusted for age and year. Modification of associations between sex and death by cohabitation, education and comorbidity was assessed. RESULTS:

Phenotypes and rates of cancer-relevant symptoms and tests in the year before cancer diagnosis in UK Biobank and CPRD Gold.

Early diagnosis of cancer relies on accurate assessment of cancer risk in patients presenting with symptoms, when screening is not appropriate. But recorded symptoms in cancer patients pre-diagnosis may vary between different sources of electronic health records (EHRs), either genuinely or due to differential completeness of symptom recording. To assess possible differences, we analysed primary care EHRs in the year pre-diagnosis of cancer in UK Biobank and Clinical Practice Research Datalink (CPRD) populations linked to cancer registry data. We developed harmonised phenotypes in Read v2 and C
