Assessment of reach-to-grasp trajectories toward stationary objects.

BACKGROUND: Patients with pronounced spasticity reveal difficulties in hand opening during the approaching grasping phase. The general description and assessment procedures of reach-to-grasp movement for rehabilitation purposes is still not established. There is a necessity to develop a universal methodology to describe the approaching phase in grasping which would allow clinical evaluation of movement pathologies. METHODS: In the paper, the evaluation of approaching trajectories assessed during grasping by healthy subjects is described. The experiment, undertaken by 7 healthy volunteers, con

Persistence of hypsarrhythmia in children beyond the age of three years.

Occurrence of hypsarrhythmia after the age of 3 years is rare. The objective of this study is to describe a group of patients who have persistence of hypsarrhythmia after the age of 3 years. The authors retrospectively reviewed the EEGs of 24 patients with hypsarrhythmia. Electroencephalographies (EEGs) were scored using a hypsarrhythmia scale. The clinical data of 7 patients with EEG scores greater than 9 at ages ≥ 3 years were analyzed. The mean age was 5.7 years (range, 3-8.7 years). EEG background amplitudes ranged from 200 to 500 µV in 5 patients and it was greater than 500 µV in the othe

[Development of Lennox-Gastaut syndrome in adulthood].

INTRODUCTION: Lennox-Gastaut syndrome (LGS) is an epileptic encephalopathy that starts in childhood and has an unknown pathophysiological mechanism. Studies on the progress of LGS reveal a poor prognosis. PATIENTS AND METHODS: A retrospective study of 1629 patients with epilepsy was conducted. Patients included in the study were those with an age of onset during childhood, tonic seizures and atypical absences (who might also suffer from other types of seizures); electroencephalogram (EEG) showing generalised slow spike-wave and generalised fast activity; mental retardation and a follow-up tim

Internal anal sphincterolysis for chronic anal fissure: a prospective, clinical, and manometric study.

BACKGROUND: The author proposes a technique of finger fragmentation of internal anal sphincter fibers to relieve anal sphincter spasm in patients with chronic anal fissures. This prospective study evaluates the clinical and functional outcome in a group of patients with chronic anal fissures. MATERIALS AND METHODS: Forty-five patients suffering from chronic anal fissure underwent the procedure termed "sphincterolysis." Anal manometry was performed prior to and at 6 and 24 months after treatment. Anal incontinence was evaluated by means of a continence score. Patients were asked to rate the le

Risus sardonicus after thalamic haemorrhage.

We describe an uncommon movement disorder after stroke. A 70-year-old man was admitted for a right thalamic haemorrhage and 1 week later developed bilateral contractions of the face. Electromyographic study revealed a bilateral facial dystonia. The association of this bilateral facial dystonia and vertical gaze palsy produced the aspect of a "risus sardonicus."

Recognition and management of exercise-induced bronchospasm.

Exercise-induced bronchospasm is an obstruction of transient airflow that usually occurs five to 15 minutes after physical exertion. Although this condition is highly preventable, it is still underrecognized and affects aerobic fitness and quality of life. Diagnosis is based on the results of a detailed history, including assessment of asthma triggers, symptoms suggestive of exercise-induced bronchoconstriction, and a normal forced expiratory volume at one second at rest. A trial of therapy with an inhaled beta agonist may be instituted, with the subsequent addition of inhaled anti-inflammator

Outcomes of botulinum toxin treatment for patients with spasmodic dysphonia.

BACKGROUND: Spasmodic dysphonia (SD) is a focal dystonia of the larynx. Although individuals with SD have variable degrees of difficulty in everyday communication and speaking, many report significant impairments. The impact of SD on the quality of life of people with the disorder has not been well measured. OBJECTIVES: To assess the impact of SD using a voice-specific, validated outcomes instrument, the Voice Handicap Index (VHI), and to evaluate the effect of botulinum toxin treatment on quality of life. METHODS: The VHI measures 3 subscales (physical, functional, and emotional) of impact

Use of pocket-sized turbine spirometer in monitoring exercise-induced bronchospasm and bronchodilator responses in children.

For field studies of asthma, portable hand-held pulmonary function testing devices are required. Other than for peak flow measurements, little has been done to validate their use in children. Fifty children aged 5-15 years having asthma symptoms were examined using an exercise challenge (8 min free running outdoors) and a bronchodilation test (salbutamol inhalation at a dose of 0.15 mg/kg). Pulmonary function was measured with a turbine spirometer, with a Wright peak flow meter (WPEF) and with a flow-volume spirometer (FVS). A fall of 15% or more in peak expiratory flow associated with wheezin

A dangerous case of colic: anomalous left coronary artery presenting with paroxysms of irritability.

Anomalous left coronary artery originating from the pulmonary artery (ALCAPA) is a rare, but potentially lethal, cause for irritability in infancy. We present the case of a 12-week-old male infant who was managed as a colic patient for several weeks before the diagnosis of ALCAPA was established. A brief review of previous case reports demonstrated that paroxysms of irritability can be a presentation of this disorder. In the majority of cases the manifestations of ALCAPA can be differentiated from other diseases known to cause a infant to be colicky.

Cough induced stress fracture and arthropathy of the ribs at extreme altitude.

Cough and chest wall pain at high altitude have only received passing mention in the medical literature. Increased minute ventilation of cold dry air at very high altitude is likely to cause airway irritation. This in turn may result in airway drying, mucus production, postnasal drip from vasomotor rhinitis, and bronchospasm acting individually or in combination to stimulate the vagal cough reflex. The cough is exacerbated further at extreme altitudes above 5500 m, and may result in intercostal muscle strain and single or multiple rib fractures. We present a case of multiple cough induced stre

Cervical carotid artery vasospasms causing cerebral ischemia: detection by immediate vascular ultrasonographic investigation.

BACKGROUND: The etiology of cerebral ischemic accidents in young adults often remains unclarified. CASE DESCRIPTION: A 32-year-old woman presented after multiple episodes of left monocular visual impairment and right-sided focal signs. MRI revealed a low-flow infarction on the left; color-coded duplex sonography (CCDS), however, showed normal vascular findings. During the inpatient rehabilitation, a renewed visual impairment occurred; an immediate CCDS examination now demonstrated a filiform stenosis of the left internal carotid artery (ICA) 4 cm above the origin and indirect signs of a sever

Olsalazine is contraindicated during pelvic radiation therapy: results of a double-blind, randomized clinical trial.

PURPOSE: A randomized clinical trial from Great Britain suggested a possible beneficial effect of acetylsalicylate in the prevention of radiation-induced bowel toxicity. Olsalazine is an orally administered drug designed to deliver 5-aminosalicylate to the large bowel with minimal systemic absorption. A randomized clinical trial was undertaken to assess the effectiveness of olsalazine in preventing acute diarrhea in patients receiving pelvic radiation therapy. METHODS AND MATERIALS: Patients receiving pelvic radiation therapy were randomized, in double-blind fashion, to olsalazine 250 mg, two

Medical treatment of West syndrome in Japan.

Although corticotropin (ACTH) is still the most effective drug for the treatment of West syndrome, a variety of other treatment modalities have been tried because of corticotropin's frequent and sometimes serious side effects. A recent survey on the treatment of this devastating disorder by Japanese child neurologists disclosed different therapeutic approaches from those taken by American or European child neurologists. Most Japanese child neurologists use vitamin B6 as the first agent and corticotropin as the third or second drug. Moreover, the dosage of corticotropin used by them is consider

Quantitative eye and head movement recordings of retinal disease mimicking spasmus nutans.

PURPOSE/METHODS: To investigate whether quantitative head and eye movement recordings can distinguish patients with spasmus nutans from patients with retinal diseases mimicking spasmus nutans. A patient with congenital stationary night blindness was followed up for seven years with electro-oculographic eye movement recordings. RESULTS/CONCLUSIONS: Rhythmic head movements and fine, intermittent, asymmetric, disconjugate, high-frequency, out-of-phase pendular nystagmus were recorded. Eye and head movement recordings of patients with congenital stationary night blindness can mimic spasmus nutans

Monozygotic twin patients with congenital ichthyosis, microcephalus, spastic quadriplegia, myoclonus, and EEG abnormalities.

A pair of monozygotic twins manifested widespread ichthyosis on the trunk and extremities, mainly on the extensor surface of the lower legs. Both patients had severe developmental retardation, microcephalus, spastic quadriplegia, myoclonus, and hearing impairment. Cranial computed tomography and magnetic resonance imaging revealed diffuse cortical atrophy. Electroencephalography documented diffuse slow wave dysrhythmia and marked epileptic discharges, namely bilateral occipital dominant, and low-voltage multifocal spikes which appeared asynchronously as a burst, mainly during sleep. [What is the place of conventional radiology in the exploration of esophageal dyskinesia and hiatal hernia?].

Esophageal dyskinesias are due to various aetiologies and sometimes present with marked clinical symptoms. The barium swallow remains the preferred first-line examination. Upper GI endoscopy and manometry are useful to confirm the functional nature and eliminate any concomitant or secondary mucosal lesions of these motor disorders. The clinical signs of hiatus hernia are essentially due to complications related to gastro-esophageal reflux. They can be asymptomatic or may present in the form of atypical, sometimes extra-gastrointestinal, symptoms. Although conventional radiology allows the diag

Ketamine infusion for postoperative analgesia in asthmatics: a comparison with intermittent meperidine.

Narcotics commonly used for postoperative analgesia may release histamine and cause bronchospasm in asthmatics. Ketamine, on the other hand, provides analgesia and has the additional advantage of preventing and relieving bronchospasm. We therefore delivered subanesthetic doses of ketamine in combination with midazolam (5.88-6.42 micrograms.kg-1.min-1 and 1.17-1.28 micrograms.kg-1.min-1, respectively), via an infusion for postoperative analgesia after elective abdominal hysterectomy in patients with asthma. Data were compared with those from a similar group of patients receiving conventional in
