Extensive longitudinal lupic myelitis: A case report with a literature review.

Systemic lupus erythematosus (SLE) is a chronic autoimmune inflammatory disease that may present with a wide range of clinical manifestations, including neurological involvement. Transverse myelitis (TM) is a well-known but rare manifestation of SLE, typically affecting the cervical, thoracic, or lumbar regions of the spinal cord. However, longitudinal myelitis (LM), which extends across more than 4 spinal segments, is exceedingly rare in the context of SLE. Here, we report a case of extensive longitudinal myelitis in a 47-year-old female with a history of SLE presenting with progressive lower

Giant cell tumor of the thoracic spine: An unusual cause for spinal cord compression.

Giant cell tumors (GCTs) of bone are uncommon neoplasms, typically located in the metaphysis of long bones, with rare occurrences in the spine, especially in the thoracic region. We report the case of a 34-year-old woman with a history of psoriasis and celiac disease, who presented with progressive inflammatory back pain and paraparesis. Imaging revealed an osteolytic mass at the T11 vertebra, causing dorsal spinal cord compression. Emergency surgery was performed, with histopathology confirming GCT. Despite initial recovery of motor function, surgical stabilization was later necessary to prev

Spinal Tuberculosis in a Young Male Patient With Empty Sella Syndrome and Panhypopituitarism.

Spinal tuberculosis (TB), or Pott's disease, is a rare but serious form of extrapulmonary TB that primarily affects the thoracic spine and can result in severe neurological complications. Patients with underlying endocrine disorders, such as panhypopituitarism, are at increased risk of developing infections due to immune suppression caused by hormonal deficiencies and long-term steroid replacement therapy. We report the case of a 24-year-old Malay male patient with a known history of empty sella syndrome and panhypopituitarism on hormone replacement therapy, who presented with progressive left

Basal ganglia-spinal cord pathway that commands locomotor gait asymmetries in mice.

The basal ganglia are essential for executing motor actions. How the basal ganglia engage spinal motor networks has remained elusive. Medullary Chx10 gigantocellular (Gi) neurons are required for turning gait programs, suggesting that turning gaits organized by the basal ganglia are executed via this descending pathway. Performing deep brainstem recordings of Chx10 Gi Ca2+ activity in adult mice, we show that striatal projection neurons initiate turning gaits via a dominant crossed pathway to Chx10 Gi neurons on the contralateral side. Using intersectional viral tracing and cell-type-specific

PCDHA9 as a candidate gene for amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis (ALS) is a devastating neurodegenerative disease. To identify additional genetic factors, we analyzed exome sequences in a large cohort of Chinese ALS patients and found a homozygous variant (p.L700P) in PCDHA9 in three unrelated patients. We generated Pcdhα9 mutant mice harboring either orthologous point mutation or deletion mutation. These mice develop progressive spinal motor loss, muscle atrophy, and structural/functional abnormalities of the neuromuscular junction, leading to paralysis and early lethality. TDP-43 pathology is detected in the spinal motor neur

Dysregulation of Aldh1a2 underlies motor neuron degeneration in spinal muscular atrophy.

Lower motor neuron degeneration is the pathological hallmark of spinal muscular atrophy (SMA), a hereditary motor neuron disease caused by loss of the SMN1 gene and the resulting deficiency of ubiquitously expressed SMN protein. The molecular mechanisms underlying motor neuron degeneration, however, remain elusive. To clarify the cell-autonomous defect in developmental processes, we here performed transcriptome analyses of isolated embryonic motor neurons of SMA model mice to explore mechanisms of dysregulation of cell-type-specific gene expression. Of 12 identified genes that were differentia

The role of neuronal plasticity in cervical spondylotic myelopathy surgery: functional assessment and prognostic implication.

Cervical spondylotic myelopathy (CSM) is a degenerative disease representing the most common spinal cord disorder in the adult population. It is characterized by chronic compression leading to neurological dysfunction due to static and dynamic injury of the spinal cord in cervical spine. These insidious damage mechanisms can result in the reorganization of cortical and subcortical areas. The cerebral cortex can reorganize due to spinal cord injury and may play a role in preserving neurological function. To date, the gold standard treatment of cervical myelopathy is surgery, comprising anterior

Temporary induction of hypoxic adaptations by preconditioning fails to enhance Schwann cell transplant survival after spinal cord injury.

Hypoxic preconditioning is protective in multiple models of injury and disease, but whether it is beneficial for cells transplanted into sites of spinal cord injury (SCI) is largely unexplored. In this study, we analyzed whether hypoxia-related preconditioning protected Schwann cells (SCs) transplanted into the contused thoracic rat spinal cord. Hypoxic preconditioning was induced in SCs prior to transplantation by exposure to either low oxygen (1% O2 ) or pharmacological agents (deferoxamine or adaptaquin). All preconditioning approaches induced hypoxic adaptations, including increased expres

New developments for spinal cord stimulation.

Spinal cord stimulation (SCS) is a well-established therapy for the treatment of chronic neuropathic pain. Newer SCS waveforms have improved patient outcomes, leading to its increased utilization among many pain conditions. More recently, SCS has been used to treat some symptoms in several movement disorders because of its good profile tolerability and capacity to stimulate local and distant areas of the central nervous system. After the original experimental findings in animal models of Parkinson's disease (PD) in the late 2000s, several studies have reported the beneficial clinical effects o

Hypoxic stress visualized in the cervical spinal cord of ALS patients.

Objective: Amyotrophic lateral sclerosis (ALS) is a progressive and fatal motor neuron disease. Hypoxic stress is suspected as the pathogenesis of ALS, however, no positron emission tomography (PET) study for hypoxic stress has been conducted in the spinal cord of ALS patients. Methods: In the present study, we examined cervical spinal hypoxic stress of nineALS patients with upper extremity (U/E) atrophy by18F-fluoromisonidazole (FMISO) PET.Results: On the ipsilateral side of C1 and C5 levels, 18F-FMISO uptake increased significantly compared with the contralateral side (*p < 0.05) and the cont

Degenerative Cervical Myelopathy: A Brief Review of Past Perspectives, Present Developments, and Future Directions.

Degenerative cervical myelopathy (DCM) is the most common cause of spinal cord injury in developed countries; its prevalence is increasing due to the ageing of the population. DCM causes neurological dysfunction and is a significant cause of disability in the elderly. It has important negative impacts on the quality of life of those affected, as well as on their caregivers. DCM is triggered by a variety of degenerative changes in the neck, which affect one or more anatomical structures, including intervertebral discs, vertebrae, and spinal canal ligaments. These changes can also lead to struct

Combined Spinal and TAP Blocks for Laparoscopic Cholecystectomy for a Patient with Crigler-Najjar Type 2: A Case Report.

Crigler-Najjar syndrome is a rare disease which is associated with congenital deficiency of uridine-diphosphate-gulukronyltransferase (UDP-glucuronosyltransferase, UGT) enzyme. In the surgery of these patients, it is necessary to use an anesthetic method that causes less damage to the liver. Spinal anesthesia is a good alternative to general anesthesia in these patients. Transversus abdominis plane block is a new method for ultrasound guided abdominal wall block. It is less invasive and relatively safer than conventional regional anesthetic techniques. Our case is a 30-year-old male patient wi

Challenges in the Diagnosis and Management of Spinal Tuberculosis: Case Series.

Though uncommon in developed countries, spinal tuberculosis must still be considered in patients with a suspicious clinical history, to avoid delays in treatment. This case series highlights the special considerations that need to be taken into account while tackling the diagnostic and therapeutic challenges associated with this disease. We present two interesting cases of spinal tuberculosis. The first case was a 26-year-old female who presented with chronic back pain and an initial misdiagnosis of ankylosing spondylitis. The second case was a 26-year-old male with new lower extremity weaknes

[Superficial Siderosis].

Superficial siderosis (SS) is a rare, neurodegenerative disease that results from toxic accumulation of hemosiderin on the surface of the brain and spinal cord. The Japanese guidelines for diagnosis and classification of SS have been classified into three categories, according to the distribution of iron deposition and the clinical symptoms, as follows: "classical" type, "localized" type and "atypical" type. In "classical" type SS, patients typically present with slowly progressive and irreversible cerebellar ataxia, sensorineural hearing loss, and/or myelopathy, due to involvement of the acou

[Treatment for Patients with Charcot-Marie-Tooth Disease: Orthopaedic Aspects].

The orthopedic manifestations in patients with Charcot-Marie-Tooth disease include deformity and dysfunction of the extremities and spine. Conservative treatment is the first choice. Orthosis and rehabilitation can improve function, and are important for the prevention of joint contractures. Foot problems are most commonly observed and require surgical treatment. Foot deformities include pes cavus, cavovarus, claw toes, or drop foot. Single or combined surgeries selected for soft tissues are plantar release, tendon transfer, or Achilles tendon lengthening, and those for bones are osteotomies a
