Adequate hepatopetal portal venous inflow is a prerequisite for graft viability and functional recovery after liver transplantation. In some recipients, large portosystemic collaterals persist despite transplantation, diverting portal flow and predisposing to portal steal and subsequent graft dysfunction. Spontaneous splenorenal shunts (SSS) represent a particularly challenging manifestation of this phenomenon. Several surgical strategies have been proposed to restore effective portal inflow, including splenectomy, shunt ligation, and left renal vein ligation (LRVL). Compared with alternative

Wiskott-aldrich syndrome (WAS) is a rare X-linked primary immunodeficiency characterized by thrombocytopenia, eczema, and recurrent infections. Live vaccines, such as Bacillus Calmette-Guérin (BCG), routinely administered in tuberculosis-endemic regions, may cause severe complications in undiagnosed patients. We report a 2-year-old boy with genetically confirmed WAS who developed a rare BCG-related brain abscess. The patient initially presented with neonatal thrombocytopenia and eczema, followed by recurrent respiratory and gastrointestinal infections. Whole exome sequencing identified a patho

Hemophagocytic lymphohistiocytosis (HLH), also known as hemophagocytic syndrome, is a life-threatening hyperinflammatory syndrome. Primary HLH is more common in children, while secondary HLH, triggered by malignancy, infection, or autoimmunity, predominates in adults. We report the case of a 56-year-old man referred to our institution with a 10-month history of fever (>38.3°C) meeting diagnostic criteria for fever of unknown origin (FUO). Clinical evaluation revealed splenomegaly and pancytopenia. Given the high clinical suspicion of HLH, an H-score of 219 points was calculated, and management

Colonic varices are a rare manifestation of portal hypertension and an uncommon cause of lower gastrointestinal bleeding, particularly when involving the proximal colon. We report a case of bleeding hepatic flexure varices in a 23-year-old male with extrahepatic portal vein obstruction (EHPVO) and prior splenectomy with lienorenal shunt. The patient presented with recurrent hematochezia and severe anemia. Upper gastrointestinal endoscopy was negative for a bleeding source, while colonoscopy revealed large tortuous varices at the hepatic flexure with stigmata of recent hemorrhage. Contrast-enha

Low-grade fibromyxoid sarcoma (LGFMS) is a rare soft-tissue sarcoma with deceptively bland histology. There is a recognized risk of late local recurrence and metastasis. Intra-abdominal presentations are uncommon and may remain clinically silent until the tumor is large, at which point complete resection can require multivisceral surgery. We report a 61-year-old woman diagnosed with intra-abdominal LGFMS in 2021. Histopathology showed classic fibromyxoid architecture with strong MUC4 expression, focal CD34 positivity, and a Ki-67 index of approximately 6%. After a margin-negative ileocecal res

Thyroid storm (TS) is a rare, life-threatening exacerbation of hyperthyroidism, where the thyroid gland produces too much of the thyroid hormones, often triggered by causes such as infection or surgery. Although rare, trauma can precipitate the transition from thyrotoxicosis to TS. Though typically seen in patients with known hyperthyroidism - especially Graves' disease - the onset of TS can be unpredictable and rapidly fatal if not promptly addressed. We present a 20-year-old male who developed TS following a road traffic accident. On arrival, the patient was in hypovolemic shock and subseque

Background and Clinical Significance: Pancreatic ductal adenocarcinoma (PDAC) is an aggressive malignancy with a poor prognosis, even in resectable disease. Molecular alterations in the four major driver genes (KRAS, SMAD4, CDKN2A, and TP53) have been associated with disease progression and patient outcomes. However, reliable prognostic biomarkers remain limited, particularly in resected PDAC, where recurrence rates remain high; Case presentation: We report the case of a 71-year-old woman incidentally diagnosed with PDAC carrying KRAS G12R with an ultra-low-variant allele frequency (VAF) SMAD4

Spontaneous splenic rupture (SSR) is a rare but potentially life-threatening condition, most commonly associated with underlying infectious, haematological, vascular, or neoplastic processes. Clinical presentation is often non-specific, which may lead to delayed diagnosis. Imaging, particularly contrast-enhanced computed tomography (CECT), plays a pivotal role in confirming splenic injury, identifying predisposing lesions, and guiding management. We present the case of a woman aged in her seventies with chronic atrial fibrillation on antiplatelet therapy who developed spontaneous splenic ruptu

A 55-year-old woman experienced 2 months of vaginal bleeding after intercourse. After gynecological examination, cervical biopsy, and imaging examinations including magnetic resonance imaging (MRI) and positron emission tomography/computed tomography (PET/CT), she was diagnosed with stage IIB cervical squamous cell carcinoma, which was associated with human papillomavirus (HPV) infection. The patient subsequently underwent robotic-assisted para-aortic lymphadenectomy, and the pathology results showed no evidence of cancer spread. She completed a treatment plan that included chemoradiotherapy,

INTRODUCTION: Immune thrombocytopenia (ITP) is an autoimmune disorder leading to low platelet counts and increased bleeding risk. The management of ITP, particularly in chronic or refractory cases, presents ongoing challenges, necessitating the development of novel therapeutic approaches that target its complex immunopathophysiology.
AREAS COVERED: This review evaluates the current ITP treatment landscape, including established first-line and second-line therapies, and provides an in-depth analysis of emerging drug classes. Key areas include neonatal Fc receptor inhibitors, Bruton's tyrosine

A 76-year-old man presented with a pancreatic cystic lesion on magnetic resonance imaging before surgery for bladder cancer. Endoscopic ultrasound(EUS)suggested a malignant tumor such as a neuroendocrine neoplasm, but EUS-fine-needle aspiration was not performed. In addition, a single tumor 10 mm in size was found in the liver(S5), and liver metastasis was suspected. We judged that surgery would be useful for treating the neuroendocrine neoplasm and liver metastasis, so we performed distal pancreatectomy, splenectomy, and partial hepatectomy. Histopathological examination revealed anaplastic c

Congenital thrombocytopenia/platelet disorders are heterogeneous disorders of platelet number and/or function. Pathogenic variants in the genes implicated in megakaryocyte differentiation and platelet formation cause thrombocytopenia in these patients. Recent advances have elucidated several causative genes for these disorders, but identifying the underlying causative genes remains challenging. Patients with these disorders often receive inappropriate treatments, including glucocorticoids and splenectomy, for chronic immune thrombocytopenia (ITP). In Japan, we have developed a diagnostic syste

Liver transplantation (LT) is the only life-saving option when acute-on-chronic liver failure (ACLF) does not improve with conservative therapy. Acute pancreatitis (AP) can cause chronic liver disease progression to ACLF. However, deceased donor LT for patients with AP has had mixed results, and no consensus has been established regarding the indication for LT. We report the first successful living donor LT (LDLT) for ACLF caused by severe AP. The 38-year-old patient with alcoholic liver disease was transferred to our institute with worsening refractory ascites. During the pretransplant workup

Splenic rupture secondary to blunt trauma is a common condition. Non-traumatic, also known as spontaneous or pathological splenic rupture is an uncommon, but potentially life-threatening condition. Spontaneous splenic rupture caused by a primary splenic tumor is rare. In this case study, we present a special, benign tumor causing splenic rupture. Our 78-year-old female patient was hospitalized due to left shoulder pain and chest discomfort. Her blood pressure was low, the laboratory tests showed anemia, and the chest CT scan involving also the upper abdomen raised the suspicion of a splenic ru

Pediatric cholelithiasis is being increasingly diagnosed owing to the widespread use of ultrasonography, raised pediatric obesity and use of long-time parenteral nutrition. Clinical presentation is variable and complicated onset could lead to severe consequences. The aim of this study is to present a 15-years-experience of a pediatric third-level-center in treating cholelithiasis. A retrospective study collecting children with gallstone operated on between 2006 and 2020 is presented. Demographic data, clinical presentation, sonographic findings, risk factors, surgery, complications, follow-up

This review considers experimental findings on splenic repair, obtained in two types of small animal (mouse, rat, and rabbit) models: splenic resections and autologous transplantations of splenic tissue. Resection experiments indicate that the spleen is able to regenerate, though not necessarily to the initial volume. The recovery lasts one month and preserves the architecture, albeit with an increase in the relative volume of lymphoid follicles. The renovated tissues, however, exhibit skewed functional profiles; notably, the decreased production of antibodies and the low cytotoxic activity of

Liver transplantation (LT) is a definitive treatment for the decompensated liver cirrhosis and fulminant liver failure. With limited availability of cadaveric liver allograft, ABO incompatible (ABOi) living donor liver transplantation (LDLT) plays an important part in further expansion of donor pool. Over the years, with the introduction of Rituximab and improving desensitisation protocol, outcomes of ABOi LDLT are on par with ABO compatible LT. However, ABOi LDLT protocol varies markedly from centre to centre. Intravenous Rituximab followed by plasmapheresis or immunoadsorption effectively re

INTRODUCTION: Isolated splenic metastasis emanating from colorectal cancer is an extremely rare finding, which usually indicates widely disseminated and multiple metastatic cancer. There have only been 39 cases of isolated splenic metastasis reported in the English literature to date.
PATIENT CONCERNS: An 84-year-old female patient presented to our department with dark-red bloody stool that had persisted for 1 month and with an increased serum carcinoembryonic antigen (CEA) level.
DIAGNOSES: A colonoscopy showed a rectal mass located 3 cm from the anal margin, which was 45 mm in diameter. Th

Germ cell tumors (GCTs) often occur in male testes and female ovaries. Extragonadal GCTs account for approximately 2% to 5% of all GCTs and mainly occur in the mediastinum, retroperitoneum, and pineal gland. In this study, we reported a rare case of gastric adenocarcinoma with GCT components. The patient's serum α-fetoprotein (AFP) level was higher than normal. Abdominal computed tomography (CT) showed a 10-cm × 10-cm tumor between the spleen and the bottom of the stomach. Gastric endoscopy indicated an ulcerative lesion extending from the bottom of the stomach to the antrum. Tissue biopsy ide

Vaccination is the most cost-effective means of preventing and even eliminating infectious diseases. However, adverse reactions after vaccination are inevitable. In addition to common vaccine-related adverse reactions, some rare but serious adverse reactions have been reported, including secondary immune thrombocytopenia (ITP). The measles-mumps-rubella (MMR) vaccine is currently the only vaccine for which a cause-effect relationship with immune thrombocytopenia has been demonstrated with an incidence of approximately 0.087-4 per 100,000 doses, and the complication is mostly observed in childr
