Evaluation of a marker-less, intra-operative, augmented reality guidance system for robot-assisted laparoscopic radical prostatectomy.

PURPOSE: Robot-assisted laparoscopic radical prostatectomy (RALRP) using the da Vinci surgical robot is a common treatment for organ-confined prostate cancer. Augmented reality (AR) can help during RALRP by showing the surgeon the location of anatomical structures and tumors from preoperative imaging. Previously, we proposed hand-eye and camera intrinsic matrix estimation procedures that can be carried out with conventional instruments within the patient during surgery, take < 3 min to perform, and fit seamlessly in the existing surgical workflow. In this paper, we describe and evaluate a comp

Feasibility of photoacoustic-guided teleoperated hysterectomies.

Hysterectomies (i.e., surgical removal of the uterus) are the prevailing solution to treat medical conditions such as uterine cancer, endometriosis, and uterine prolapse. One complication of hysterectomies is accidental injury to the ureters located within millimeters of the uterine arteries that are severed and cauterized to hinder blood flow and enable full uterus removal. This work explores the feasibility of using photoacoustic imaging to visualize the uterine arteries (and potentially the ureter) when this imaging method is uniquely combined with a da Vinci® surgical robot that enables te

A prospective evaluation of the risk factors for development of wound dehiscence and incisional hernia.

OBJECTIVE: Post-laparotomy wound dehiscence, evantration and evisceration are important complications leading to an increase in both morbidity and mortality. Incisional hernias are frequently observed following abdominal surgeries and their occurrence is related to various local and systemic factors. This study aims to analyze the factors affecting wound healing by investigating the parameters that may cause wound dehiscence, incisional hernia, sinus formation and chronic incisional pain. MATERIAL AND METHODS: The records of 265 patients who underwent major abdominal surgery were analyzed. Th

Screen-imaging guidance using a modified portable video macroscope for middle cerebral artery occlusion.

The use of operating microscopes is limited by the focal length. Surgeons using these instruments cannot simultaneously view and access the surgical field and must choose one or the other. The longer focal length (more than 1 000 mm) of an operating telescope permits a position away from the operating field, above the surgeon and out of the field of view. This gives the telescope an advantage over an operating microscope. We developed a telescopic system using screen-imaging guidance and a modified portable video macroscope constructed from a Computar MLH-10 × macro lens, a DFK-21AU04 USB CCD

Carbon fiber-reinforced PEEK implants in oncologic spine surgery: a multicenter experience on implications for postoperative patient management.

OBJECTIVE: Carbon fiber-reinforced polyetheretherketone (CFRP) implants have been used safely in treating spinal oncological disease and promise improved imaging follow-up and radiotherapy planning. However, data on the implant's routine use in the clinical setting with implications for postoperative management are scarce. The aim of this observational study was to provide real-world insight into the current use of CFRP instrumentations in spinal oncological disease and to define their actual relevance for postoperative imaging follow-up and adjuvant treatment planning. METHODS: Datasets of p

Maintenance of construct integrity with subaxial cervical pedicle screws when crossing the cervicothoracic junction in complex pediatric deformity: an international multicenter study.

OBJECTIVE: In cases of complex pediatric spinal deformity, posterior spinal instrumentation crossing the cervicothoracic junction (CTJ) may be required. This is most frequently encountered for revision surgery to address proximal junctional kyphosis (PJK). In the cervical spine, lateral mass screws are most commonly used, although they may result in biomechanically weak proximal constructs and predispose to instrumentation failure. The use of supplemental anterior constructs has recently been reported with promising results, but this necessitates an additional surgical procedure. Subaxial cerv

Precision dissection strategy for myocutaneous flap in frontotemporal cranioplasty based on tissue biophysical properties.

BACKGROUND: Safely and efficiently separating myocutaneous flaps that are densely adherent to the dura mater remains a critical challenge in cranioplasty. Traditional dissection methods and prophylactic implantation of anti-adhesion materials both have limitations. This study aims to propose and validate a "Tissue Biophysics-Based Myocutaneous Flap Dissection Strategy." By systematically analyzing the biophysical properties of different tissue interfaces and dynamically matching the optimal dissection instruments and techniques, we seek to optimize surgical outcomes. METHODS: We retrospective

Disinfection of 3D-printed surgical guides using virgin coconut oil (in vitro study).

BACKGROUND/OBJECTIVE: Disinfection of a 3D-printed surgical guide is of utmost importance as it comes into contact with hard and soft tissue during implant placement so it poses a potential risk of pathogenic transmission. Methods used for disinfection in the surgical field should be reliable, practical, and safe for the instruments and the patients. The objectives of this study were to compare the antimicrobial potential of 100% Virgin Coconut Oil, 2% Glutaraldehyde, and 70% Ethyl Alcohol used to decontaminate 3D-printed surgical guides. MATERIALS AND METHODS: Thirty identical surgical guide

[Patient surgical masks during regional anesthesia. Hygenic necessity or dispensable ritual?].

OBJECTIVE: The use of surgical face masks (SFM) is believed to minimize the transmission of oro- and nasopharyngeal bacteria to wounds and surgical instruments. However, there are disadvantages for patients undergoing regional anaesthesia and wearing masks: deficient assessment of lip cyanosis, anxiety, retention of CO2, costs. Up to now no studies have been published investigating whether or not SFMs, worn by patients during regional anaesthesia, will reduce bacterial convection. METHODS: We investigated 72 patients during aseptic operations: 24 individuals with regional anaesthesia and SFMs

A Prospective Clinical Study to Evaluate Safety and Performance of an Intra-Surgical Hydraulic Distractor During Spinal Surgeries.

IntroductionSpinal surgeries often require the manipulation of bone structures while at the same time protecting sensitive tissues. Existing surgical systems involve manual force, which has a high risk of damaging the vertebrae and instruments interfere with the visibility between the vertebrae, which complicates work. The OrtoWell® Distractor System, an intra-surgical hydraulic tool for vertebrae separation, offers incremental and stable force control and better visibility during distraction procedures in spinal surgeries. This study aims to fill the gap of still pending clinical trials using
