Catheter-Directed Thrombolysis and Direct Oral Anticoagulant Class Switch to Prevent Pulmonary Embolism From Large Inferior Vena Cava Thrombus.

BACKGROUND Inferior vena cava (IVC) thrombosis is an uncommon complication of deep vein thrombosis and may be associated with a high risk of severe pulmonary embolism, particularly in the presence of free-floating thrombi. Although IVC filter placement may be considered to prevent embolization, prolonged filter dwell increases the risk of recurrent venous thrombosis and reduces retrieval success, highlighting the importance of early thrombus clearance. However, evidence regarding optimal interventional and anticoagulation strategies for IVC thrombosis remains limited. CASE REPORT A 46-year-old

Microscopic Polyangiitis With Pituitary Dysfunction and Spontaneous Renal Aneurysm Rupture.

Antineutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis (AAV) is a relatively uncommon autoimmune disease, predominantly causing kidney or lung injury. Pituitary dysfunction is exceedingly rare in microscopic polyangiitis (MPA). Rupture or hemorrhage of renal artery aneurysms is also scarcely reported in AAV, especially in MPA. In this study, we describe a 68-year-old woman who presented with shortness of breath at rest, polydipsia, and polyuria. In addition to an increased p-ANCA titer (1:10) and the presence of anti-myeloperoxidase antibodies, T1-weighted magnetic resonance imag

Beyond safety: adverse events and unanticipated advantages of SGLT2 inhibitors.

BACKGROUND: Sodium-glucose transporter 2 inhibitors (SGLT2i) are widely used for diabetes management and have demonstrated benefits in treating acute and chronic heart failure (HF) and chronic kidney disease (CKD). Their increasing application has revealed various side effects, although only a few are currently recognized as drug-related adverse events, based on ongoing observations. AIMS: This review synthesizes positive and negative side effects linked to SGLT2i and proposes management strategies. METHODS: A literature search of PubMed/EMBASE, Web of Science, and Google Scholar from the past

Cystic Fibrosis beyond Childhood: A Case Report of Recurrent Heat-Induced Electrolyte Disturbances Unmasking a Late Diagnosis.

INTRODUCTION: Cystic fibrosis (CF) is an autosomal recessive genetic disorder caused by pathogenic variants in the CFTR gene, leading to impaired chloride and bicarbonate transport across epithelial tissues. While typically diagnosed in infancy due to classic respiratory and gastrointestinal symptoms, atypical forms can present later in life with subtle or isolated manifestations. This case represents a rare monosymptomatic and atypical adult-onset presentation of CF, with electrolyte disturbances as the sole clinical manifestation. CASE PRESENTATION: We present the case of a 31-year-old man

An automated method of streamlining waiting list by clinical risk fast-tracking for patients awaiting TAVR: SWIFT TAVR algorithm.

INTRODUCTION: Transcatheter aortic valve replacement (TAVR) is increasingly in demand for treating severe aortic stenosis in a variety of surgical risk profiles. This means increasing wait times and elevated morbidity and mortality on the waitlist. To address this, we developed the SWIFT TAVR algorithm to prioritize patients based on clinical risk and reduce wait times. METHODS: The SWIFT algorithm, implemented in Microsoft Excel, calculates a clinical risk score from three parameters: left ventricular ejection fraction (LVEF), peak aortic valve gradient, and syncope. Scores categorize patien

Effects of Hook Maneuver on Oxygen Saturation Recovery After -40 m Apnea Dive-A Randomized Crossover Trial.

To reduce the risk of syncope, trained breath-hold divers (BHDs) use a specialized breathing technique after surfacing called "hook breathing" (HB). It consists of a full inspiration followed by a Valsalva-like maneuver and with subsequent exhalation performed against resistance to generate continuous positive airway pressure during exhalation. This study analyzed the influence of HB on oxygen saturation recovery after a -40 m depth apnea dive in trained BHDs. Thirteen BHDs performed two dives to -40 m at different days, one followed by HB after a dive and the other using usual breathing (UB).

Predictors of Right Ventricular Pacing in Patients Undergoing Implantable Defibrillator Placement.

INTRODUCTION: Identifying patients who will develop a need for right ventricular (RV) pacing after defibrillator (ICD) placement would help inform appropriate device selection, specifically to identify patients who might be more suitable for dual chamber or biventricular transvenous defibrillators versus single-chamber or subcutaneous devices which do not provide pacing. We sought to determine predictors of RV pacing in patients who did not have a pacing indication at the time of initial ICD implant. METHODS: This observational study assessed single-chamber ICD recipients implanted from 2006

Pathogenic KCNH2 variant in monozygotic twins with speech delay and lower risk type 2 long QT syndrome.

Type 2 Long QT Syndrome (type 2 LQTS) is a cardiac channelopathy caused by pathogenic variants in the KCNH2 gene, often associated with delayed cardiac repolarization and increased risk of arrhythmias. While its impact is traditionally considered cardiac, emerging studies suggest a potential role of KCNH2 dysfunction in neurogical disorders. We describe monozygotic twin sisters carrying the pathogenic frameshift variant KCNH2 c.2959_2960delCT (p.Leu987Valfs*131; rs748706373), inherited from their asymptomatic father. Clinically, both twins presented with severe language delay, absence of point

Supravalvular aortic stenosis mimicking hypertrophic obstructive cardiomyopathy: a case report.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is a relatively common genetic disorder often associated with left ventricular outflow tract (LVOT) obstruction. However, the presence of certain structural abnormalities such as supravalvular aortic stenosis (SVAS) can contribute to elevated LVOT gradients and thereby mimic obstructive HCM. This case highlights the importance of multimodality imaging in accurately determining the aetiology of LVOT obstruction, which is critical to guiding appropriate management. CASE SUMMARY: A 27-year-old male with progressive dyspnoea, fatigue, and recent synco

The Uncommon Phenomenon of Short QT Syndrome: A Scoping Review of the Literature.

Background: Short QT syndrome (SQTS) is a rare inheritable channelopathy characterized by a shortened corrected QT interval on an electrocardiogram and a significant risk of atrial and ventricular arrhythmias, potentially leading to sudden cardiac death. Despite advancements in our understanding of SQTS, knowledge gaps persist due to its extreme rarity. This scoping review aims to summarize the available knowledge on its clinical presentations, genetic mutations, and management strategies, while identifying areas for further investigation. Methods: This scoping review was conducted across the

Coexistence of R-Wave Oversensing and Undersensing in an Implantable Loop Recorder: The Issue of Multiple Sensing in Implantable Loop Recorders.

BACKGROUND Implantable loop recorders (ILRs) are important tools in diagnosing unexplained syncope and palpitations through prolonged rhythm monitoring. However, ILRs can produce false-positive arrhythmia detections, due to signal oversensing and undersensing, which can complicate clinical interpretation. Oversensing typically involves P waves, T waves, or myopotentials; however, simultaneous oversensing and undersensing of multiple waveform components is rare. We report a case of R-wave oversensing of P waves and T waves, with undersensing of QRS complexes, resulting in erroneous atrial fibri

Successful implantation of a leadless pacemaker in a patient after tricuspid valve replacement following cardiac tumor removal: a case report and literature review.

INTRODUCTION: Intravenous-cardiac leiomyomatosis (ICL) is a rare condition involving the presence of leiomyomatous tumors within the cardiac venous system. When repairing the tricuspid valve (TV) is not feasible, tricuspid valve replacement (TVR) becomes necessary. However, TVR can lead to complications, including atrioventricular block (AVB), due to potential damage to the cardiac conduction system during surgery. This case report presents the successful implantation of a leadless pacemaker (LP) in a patient who developed advanced AVB after undergoing cardiac tumor removal and bioprosthetic T
