Early calcium replacement after minimal blood transfusion is associated with adverse outcomes.

PURPOSE: Calcium disturbances are common in patients with traumatic injuries, exacerbated by shock and blood product resuscitation. Recently, empiric calcium supplementation has been recommended with the first unit of blood transfusion, despite a lack of data supporting this practice. We hypothesize that there are risks associated with calcium administration in patients receiving small volume transfusion. METHODS: This is a retrospective review of patients admitted to our Level 1 trauma center from 2018 to 2024 who received 1-3 units of red blood cells (RBCs). Patients were stratified by calc

Reconstruction of Acquired Labial Adhesions in a Young Adult Female Patient Using Split-Thickness Skin Grafting: A Rare Case.

Labial adhesions are rarely encountered in women of reproductive age and are more commonly seen in prepubertal girls or postmenopausal women. They are typically associated with hypoestrogenism, chronic inflammation, trauma, or cicatricial dermatoses. Thus, acquired labial adhesions without identifiable predisposing factors in reproductive-age women are rare and can pose a reconstructive challenge. These adhesions may lead to urinary, gynecological, and psychological morbidity. In this report, we present the case of a 24-year-old woman with progressive narrowing of the vaginal introitus over 5-

Suction vs Water Seal for Initial Treatment of Traumatic Pneumothorax Trial (SEAL IT): A Pragmatic Cluster-Randomized Study.

BACKGROUND: Chest tubes for traumatic pneumothorax are commonly managed with initial suction before transition to water seal, despite limited evidence supporting this practice. We hypothesized that initial water seal would reduce chest tube duration without increasing complications. STUDY DESIGN: We conducted a single-center, pragmatic, month-based cluster-randomized study of adult trauma patients with pneumothorax requiring chest tube placement from October 2024 to May 2025. Patients with a hemothorax component >300 mL or chest tube placement before computed tomography were excluded. Patient

Impact of Artificial Intelligence Assistance on Diagnosing Traumatic Pneumothorax: A Comparison of Specialist and Non-Specialist Emergency Physicians.

INTRODUCTION: Supine chest radiography is routinely used in trauma care; however, its sensitivity is limited in pneumothorax detection. Although artificial intelligence (AI) has recently been introduced as a diagnostic support tool for chest radiograph interpretation, its use in trauma radiographs and its interaction with physician experience remain unclear. We aimed to evaluate the diagnostic performance of AI-assisted image interpretation in traumatic pneumothorax detection among emergency medicine specialists and non-specialists. METHODS: In this retrospective single-center study, 34 supin

Split hamate fractures as part of complex ulnar carpometacarpal injuries: radiographic and functional outcomes.

BACKGROUND: Split hamate fractures, which most often are a complicating part of fracture dislocations of the 5th or 5th and 4th carpometacarpal joints, are rare and challenging injuries that can severely affect hand function. The injury usually requires surgical treatment. However, both short- and long-term data on the effectiveness of these interventions are limited. The objective of this study is to advance understanding of this rare hand surgical condition by investigating its epidemiology, etiopathogenesis, subsequent complications, and functional outcomes. METHODS: Between 2010 and 2024,

A Five-Year Retrospective Study: Relationship Between Cervical Spine Whiplash Injuries and Demyelinating Diseases in the Pediatric Population.

OBJECTIVES: Whiplash injuries are among the most common cervical spine injuries in the pediatric population. Limited literature demonstrates a potential link between cervical spine trauma and demyelinating disease, such as multiple sclerosis. These associations have primarily been reported from case-controlled studies, while cohort studies have not supported these findings. In this study, we aimed to evaluate whether a relationship exists between cervical spine trauma and the short-term development of demyelinating disease in pediatric patients. We hypothesized that patients experiencing cervi

Direct Peritoneal Resuscitation in Critically Ill Patients with an Open Abdomen is Associated with Increased Risk of Intraperitoneal Fungal Infections.

Background: Damage control laparotomy (DCL) is a well-established tool to stabilize critically ill surgical patients. Direct peritoneal resuscitation (DPR), whereby the open abdomen is continuously irrigated with glucose-based hypertonic dialysate, is a valuable adjunct that improves abdominal closure rates and decreases wound complications. Infectious implications of its use remain underexplored. Objective: To assess the impact of DPR on the incidence intra-abdominal fungal infections (AFIs) in critically ill surgical patients. Methods: A retrospective chart review was performed of trauma and

Trauma Pneumonectomy: Has Survival Improved Over Two Decades?

IntroductionThe hypothesis of this study is that recent advances in mechanical cardiopulmonary support and operative management have improved survival in patients requiring a trauma pneumonectomy.MethodsRetrospective, single center study from January 2003 to December 2023 of all patients who underwent a pneumonectomy for trauma. Data collected included demographics, admission physiology, use of venovenous extracorporeal membrane oxygenation (VV-ECMO), and mortality, defined as early (within 24 hours of surgery) and late (>24 hours after surgery). Outcomes were compared between decades, the fir

Minimally invasive stabilization of the anterior pelvic ring in fragility fractures using a submuscularly implanted internal fixator - a retrospective case series of 34 geriatric patients.

PURPOSE: Various surgical techniques for osteosynthesis in fragility fractures of the pelvis (FFP) are described. Since 2012, a submuscularly placed internal fixator has been used to stabilize the anterior pelvic ring. Indications for this procedure are a dislocation in the anterior pelvic ring and/or severe pain with associated immobility. This technique potentially has several advantages compared to subcutaneous procedures, including improved patient comfort, less irritation, and enhanced biomechanical stability due to the deeper rod placement. METHODS: Digital files of patients aged ≥ 65 y

Evaluation of functional outcomes and return to work in thoracolumbar junction fractures (T11-L2) following posterior spinal fusion: A prospective observational study.

PURPOSE: Fractures at the thoracolumbar junction (T11-L2) represent a critical spinal trauma pattern due to biomechanical transition zones. Treatment decisions, especially between conservative and surgical approaches like posterior spinal fusion (PSF), remain debated. This study evaluates the functional, neurological, and occupational outcomes of patients treated with or without PSF. METHODS: A prospective cohort of 80 patients with thoracolumbar junction fractures was followed for 9 months at a tertiary trauma referral center. Fifty-five patients received PSF, and 25 underwent non-surgical m

Smaller total and subregional cerebellar volumes in posttraumatic stress disorder: a mega-analysis by the ENIGMA-PGC PTSD workgroup.

Although the cerebellum contributes to higher-order cognitive and emotional functions relevant to posttraumatic stress disorder (PTSD), prior research on cerebellar volume in PTSD is scant, particularly when considering subregions that differentially map on to motor, cognitive, and affective functions. In a sample of 4215 adults (PTSD n = 1642; Control n = 2573) across 40 sites from the ENIGMA-PGC PTSD working group, we employed a new state-of-the-art deep-learning based approach for automatic cerebellar parcellation to obtain volumetric estimates for the total cerebellum and 28 subregions. Late video-assisted thoracoscopic surgery versus thoracostomy tube reinsertion for retained hemothorax after penetrating trauma, a prospective randomized control study.

BACKGROUND: Early video-assisted thoracoscopic surgery (VATS) is the recommended treatment of choice for retained hemothorax (RH). A prospective single-center randomized control study was conducted to compare outcomes between VATS and thoracostomy tube (TT) reinsertion for patients with RH after penetrating trauma in a resource constrained unit. Our hypothesis was that patients with a RH receiving VATS instead of TT reinsertion would have a shorter hospital stay and lesser complications. MATERIALS AND METHODS: From January 2014 to November 2019, stable patients with thoracic penetrating traum
