The effect of root orientation on inferior alveolar nerve injury after extraction of impacted mandibular third molars based on propensity score-matched analysis: a retrospective cohort study.

BACKGROUND: The injury of the inferior alveolar nerve (IAN) is one of the most serious complications of impacted mandibular third molars (IMTMs) extraction. The influence of the root orientation of IMTMs on IAN injury is still controversial. A deeper understanding of the risk factors of IAN injury conduces to better prevention of IAN injury. This study aims to explore whether root orientation is an independent risk factor of IAN injury during IMTMs extraction using the statistical strategy of propensity score matching (PSM). METHODS: This retrospective cohort study included 379 patients with

Self-assessed neurological disturbances after surgical removal of impacted lower third molar: a pragmatic prospective study on 423 surgical extractions in 247 consecutive patients.

OBJECTIVES: The aim of this study was to assess the combined role of current radiographic risk indicators and patient age in predicting lower lip sensitivity disturbances after surgical removal of impacted lower third molars. The question was which combinations indicate low or high risk. MATERIALS AND METHODS: A prospective study was implemented involving 247 consecutive outpatients who underwent 423 surgical extractions. The predictor variables were patient age and risk indicators observed on panoramic radiographs. The outcome variable was the incidence of self-assessed lip sensitivity alter

Borneol inhibits TRPA1, a proinflammatory and noxious pain-sensing cation channel.

Borneol, a natural product isolated from several species of Artemisia, Blumea and Kaempferia, has a widespread use in traditional medicine. TRP ion channels are a class of nonselective cation channel proteins involved in a variety of physiological and pathological processes in mammals. TRPA1, a member of TRP family of cation channels, is involved in plethora of processes including noxious-cold, noxious-pain sensations, inflammation and the detection of irritant chemicals. Borneol is chemically related to camphor (a known inhibitor of TRPA1 ion channels); therefore, it is beneficial to investig

Otorhinolaryngological patient injuries in Finland.

OBJECTIVES/HYPOTHESIS: Otorhinolaryngology (ORL) is considered a specialty associated with few serious patient injuries. Research data that support this belief are, however, scarce. We analyzed claims associated with ORL to determine the number of Finnish cases and the possible common denominators. STUDY DESIGN: Register study of ORL cases in the Patient Insurance Centre (PIC), the Regional State Administrative Agencies (RSAA), and the National Supervisory Authority for Welfare and Care (Valvira) during the years 2004 to 2008. METHODS: These three agencies are the main actors in the field of

Photophobia in essential blepharospasm--a positron emission tomographic study.

To localize regional alterations in cerebral glucose metabolism in essential blepharospasm (EB) patients with photophobia. We have studied 22 EB patients by performing positron emission tomography and [(18)F]-fluorodeoxyglucose analysis. The patients were classified into two subgroups, namely, EB with photophobia (P group) and EB without photophobia (NP group), and compared with a healthy control group (n = 44). There were no significant differences between the two patient groups with respect to the severity of motor symptoms or the duration for which the condition persisted. The FDG-PET image

Coronectomy of a third molar with cyst lining enucleation in the management of a dentigerous cyst.

UNLABELLED: A dentigerous cyst was treated with enucleation and coronectomy of the associated tooth to minimize the risk of inferior alveolar nerve damage, resulting from direct surgical trauma or indirectly from intra-operative or post-operative jaw fracture. Radiographic monitoring over 33 months confirmed migration of the roots through healed bone and progressive obliteration of the residual pulp canal chamber. These observations are not associated with failure of the technique but are, in fact, commonly observed in successful cases of coronectomy. A brief discussion of the risks versus ben

A case of masticatory disturbance incidental to trigeminal schwannoma: changes in occlusal force and masticatory sensation before and after radiosurgery.

We report here a case of masticatory disturbance evoked by trigeminal schwannoma, in which we have evaluated the changes in occlusal force and masticatory sensation before and after treatment for the tumour. The patient was a 43-year-old woman and her chief complaint was a loss of masticatory sensation on her left side. MR imaging revealed an enhanced tumour in the left cavernous sinus/Meckel's cave. The left masseter muscle function and occlusal force showed remarkable decreases before treatment; however, the sensory thresholds of her facial skin and dental pulp were not significantly differe

Are there sex-related differences in responses to repetitive olfactory/trigeminal stimuli?

Sex differences in olfactory sensitivity have been reported since the late 1800's with women typically outperforming men on tests of odor detection, discrimination or identification. It is not known whether women adapt differently than men to olfactory and trigeminal stimuli. Seventeen healthy volunteers participated (9 female, 8 male, mean age 22 years) in the study. As established by an odor identification test (UPSIT, score > or =38) all subjects had normal olfactory function. Event-related potentials (ERPs) were recorded in response to olfactory (25% v/v phenyl ethyl alcohol) and trigemina

Barcelona Smell Test - 24 (BAST-24): validation and smell characteristics in the healthy Spanish population.

OBJECTIVES: Smell tests for clinical use have been developed in different countries, but no single test has gained general acceptance. The objectives of the study were to evaluate the smell outcomes in a Spanish population. METHODS: A prospective study on healthy volunteers (n = 120) without olfactory disturbances was performed. The volunteers were differentiated by gender, age, and smoking habit groups. We used a new olfactory test, the Barcelona Smell Test 24 (BAST-24) that consists of 24 odours scoring smell detection, identification, and forced choice. RESULTS: Volunteers showed the high

Refractory skin injury in complex knock-out mice expressing only the GM3 ganglioside.

We generated double knock-out mice lacking the GM2/GD2 and the GD3 synthase gene by mating single gene mutants, and we analyzed the abnormal phenotypes of the mutant mice expressing only the GM3 ganglioside. We observed a refractory skin lesion that appeared primarily on the face of the mutant mice at 25 weeks after birth or later. Frequent scratching of the wound sites was observed in mutant mice with the skin injury, suggesting that it is a triggering factor that exacerbates the injury. This was confirmed by isolating mice in special cages for metabolic study in which the skin injury develop

Mental neuropathy: report of five cases and review of the literature.

Mental nerve neuropathy, also referred to as numb chin syndrome, is a rare, seemingly harmless symptom. It is more often associated with cancer, either as first symptom or during the outcome, than with benign diseases. In this review, we will focus on the numb chin syndrome presenting as an isolated neurological symptom. We report five patients with mental nerve neuropathy associated with metastatic disease (small cell lung cancer, prostatic cancer and breast cancer). In one patient, numb chin syndrome preceded the discovery of the disease, while, in the four others, it occurred as a sign of r

[Valentin's nerve, its role in the innervation of the chin region. Preliminary report].

The mylo-hyoid nerve, a branch of the inferior alveolar nerve, gives rise to many motor and sensory branches. Motor branches innervate the anterior belly of the digastric muscle. Sensitive branches innervate the submandibular gland and also the skin just under the bony chin. Valentin's nerve, the terminal cutaneous branch of the myo-hyoid nerve (described by Valentin in 1843), can be damaged by horizontal mentoplasty (sliding-mentoplasty). Sensory disorders (hypoesthesia, anesthesia) can occur in case of Valentin's nerve injury without lesions to the mental nerve.

Isolated trigeminal sensory change due to pontine hemorrhage.

Isolated trigeminal sensory change has been rarely described as a manifestation of pontine hemorrhage. We report a 31-year-old man who developed sudden and long-lasting trigeminal sensory disturbances. Brain magnetic resonance imaging showed a hemorrhage in the right lateral pontine tegmentum corresponding to the trigeminal sensory nucleus, and the venous phase angiogram demonstrated abnormal vascular structures. Pontine hemorrhage secondary to venous malformation can be one of the causes of isolated trigeminal sensory change.

Effects of trauma to the mandibular nerve on human perioral directional sensitivity.

The capacity of 4 patients who had previously experienced trauma to their mandibular nerves to distinguish opposing directions of tactile motion over the distribution of the mental nerve was compared to that of 8 neurologically normal adults. Brushing stimuli were delivered to the perioral region and were precisely controlled for their velocity, the length of skin traversed, the width of skin contacted, and the orientation and direction of motion. A temporal, 2-alternative, forced choice method was used to obtain estimates of directional sensitivity, d'. It was discovered that impairment in cu

Cross-sectional tomography. A diagnostic technique for determining the buccolingual relationship of impacted mandibular third molars and the inferior alveolar neurovascular bundle.

Twenty-two patients with 31 impacted mandibular third molars were examined with a new, precise, cross-sectional tomographic technique to assess the radiographic size, shape, branching pattern, location, and degree of cortication of the mandibular canal, and the inclination of impacted mandibular third molars in the buccolingual plane. The mandibular canal, including bifid canals, was accurately identified in 30 cases (96.8%). The cross-sectional appearance of the canal was an uncorticated, or partially corticated, radiolucent oval that measured on average (+/- SD) 2.9 +/- 0.7 x 2.5 +/- 0.6 mm

[Nerve dysfunctions following mandibular surgery on an outpatient basis].

The extent and reversibility of any functional impairment of the inferior alveolar and lingual nerves are important criteria for establishing the indication for microsurgical reconstruction. Postoperative dysfunctions were observed by 31.4% of 384 patients operated in the posterior segment of the lower jaw. In more than half of these cases the dysfunctions disappeared after 8 weeks. After 6 months the remission tendency was very slight. After a mean 4 years following operation 6% of the patients reported subjective disorders. Impaired functions were demonstrated objectively in 11.4% of the cas

Management of intraoral injuries.

Primary closure of intraoral wounds can be done up to 24 hours after the injury. A mucosal seal decreases the possibility of infection and allows the wound to heal more rapidly and less painfully than a wound left open to heal by secondary intention. Defects of the lips must be closed with exact alignment of the vermilion-cutaneous margins. Nerve repairs must be done without tension at the repair site. Parotid duct repairs are mandatory to prevent cyst and fistula formation. Anterior permanent teeth may be replanted.
