Nursemaid's Elbow With Transient Wrist Drop in a Toddler: An Atypical Presentation.

Radial head subluxation, commonly known as nursemaid's elbow, is a frequent upper extremity injury in young children and typically presents with refusal to use the affected arm without focal distal motor dysfunction. We report an atypical presentation in a previously healthy two-year-and-10-month-old girl who was brought to the emergency department after an unwitnessed event with no clearly identified mechanism of trauma. She held her right elbow flexed against her chest in a posture suggestive of radial head subluxation, and additionally refused to let go of her wrist and demonstrated a wrist

Differential Activation of the Multifidus and Erector Spinae During Asymmetric Spinal Stabilizing Exercise in Adolescent Idiopathic Scoliosis.

OBJECTIVE: To identify the asymmetric spinal stabilizing exercise (ASSE) postures that selectively activate the multifidus (Mu) relative to the erector spinae (ES) in patients with adolescent idiopathic scoliosis (AIS), thereby supporting the development of curve-specific exercise programs for three-dimensional spinal deformities. METHODS: Surface electromyography recordings were obtained bilaterally from the ES and Mu muscles during ASSE postures. Signals were normalized to the maximal voluntary isometric contraction. The asymmetry ratio, Mu/ES ratio, and additional asymmetric contraction of

Technical Nuances of Anterior Column Construction for the Treatment of Osteomyelitis-Induced Cervical Kyphoscoliotic Deformity: An Operative Video Illustration.

Pyogenic vertebral osteomyelitis is a bacterial infection of the vertebral body often caused by hematogenous spread from a distant site with 3%-11% of cases affecting the cervical spine.1,2 Patients at risk for osteomyelitis are intravenous drug users, patients with diabetes, patients in dialysis, and males older than 50 years of age. In severe cases where infection causes osseous destruction of the vertebral column lending to a loss of normal sagittal and coronal plane alignment, neurologic impairment, or spinal instability, surgical correction may be required.3 A 38-year-old woman with a med

Early-Onset Dropped Head Syndrome and Person-in-the-Barrel Syndrome After Radiation Therapy: Clinical, Electromyographic, and MRI Findings.

Dropped head syndrome (DHS) involves severe weakness of the neck extensor muscles causing the mandible to drop to the chest wall. Isolated neck extensor weakness is a rare complication of radiotherapy. This condition may result within a few weeks or months following radiotherapy (early-onset) or several years after radiotherapy (late-onset), with the latter more commonly encountered. Person-in-the-barrel syndrome is marked by bilateral brachial diplegia, intact cranial nerves, and preserved lower extremity strength. We describe the unique clinical profile of a patient with a six-week history o

Axial and appendicular body proportions for evaluation of limb and trunk asymmetry.

Background and purpose - When children with irregular body proportions or asymmetric limbs present, it may be unclear where the pathology is located. An improved understanding of the clinical ratio between upper extremity, lower extremity, and spine length may help elucidate whether there is disproportion between the trunk and limbs, and whether there is a reduction deficit of the shorter limb rather than hypertrophy of the longer limb. Patients and methods - We used the Brush Foundation study of child growth and development, which was a prospective, longitudinal study of healthy children betw

Association of Fluid Status and Body Composition with Physical Function in Patients with Chronic Kidney Disease.

BACKGROUND: Impairment of physical function and abnormal body composition are the major presentations in patients with chronic kidney disease (CKD). The aim of this study is to investigate the relationship between body composition and physical function in CKD patients. METHODS: This cross-sectional study enrolled 172 of CKD stages 1-5 from February 2013 to September 2013. Handgrip strength (upper extremity muscle endurance), 30-second chair-stand test (lower extremity muscle endurance) and 2-minute step test (cardiorespiratory endurance) were used as indices of physical function. Limb salvage surgery in spinal cord injury patients.

Advances in the care and rehabilitation of patients with spinal cord injuries (SCI) have resulted in extended survival following injury. Increasingly, we are faced with difficult chronic lower extremity ischemic complications in SCI patients. Recognizing limitations associated with amputation in these nonambulatory patients, we report the preliminary results of a program of selective limb salvage via arterial reconstructive surgery. Retrospective chart review was performed on the records of the Veterans Affairs Palo Alto Health Care System SCI unit. Since 1989, 15 revascularization procedures

[Complex injury of the elbow joint].

High-velocity trauma now often results in complex injuries to the upper extremity, and especially the elbow joint. These can lead to both an enormous reduction in the range of motion of the shoulder, elbow and wrist joints, in severe cases with complete loss of upper extremity function. A complex injury is defined as a fracture and/or dislocation of the elbow in association with (1) a serial injury of the upper extremity, (2) a severe soft tissue trauma, or (3) concomitant injury to vessels or nerves. Serial fractures, in particular can lead to enormous problems with treatment and are often as

Transverse nail ridging (Beau's lines) and rough nails after bone marrow transplantation.

BACKGROUND: Tibial hemimelia is a rare malformation with a wide clinical spectrum of presentation. The severity of this condition can be typed using different classification systems. It can exist as an independent entity or can be associated with upper limb or visceral malformations. The aims of our study are therefore, a. to report the incidence of upper limb deformities in relation to the severity of tibial hemimelia classified by the Jones classification, b. incidence of tibial hemimelia as a part of a syndrome c. to report the overall incidence of the associated upper limb and visceral def

Respiratory arrest in patients undergoing arteriovenous graft placement with supraclavicular brachial plexus block: a case series.

Supraclavicular brachial plexus block is commonly used for upper extremity surgery. Respiratory arrest in three patients with end-stage renal disease after ultrasound-guided supraclavicular brachial plexus block for creation of an arteriovenous graft over a 6-month period is presented. Patients with renal failure may represent a group at particular risk for respiratory failure following supraclavicular brachial plexus block.

Congenital mirror hand deformity.

Ulnar dimelia or commonly called mirror hand is a rare congenital anomaly of upper extremity characterized by duplication of ulna, absence of the radius with symmetrical polydactyly. This anomaly may be associated with shoulder dislocation, fibular dimelia, idiopathic scoliosis, cirrhosis, pyloric hypertrophy or polycystic kidney. We report a new case of this rare congenital anomaly which was not associated with any other malformation.

Serratus anterior transposition muscle flaps for bronchial coverage: technique and functional outcomes.

Because of its consistent anatomy, long vascular pedicle, malleability, low complication rate, and low donor site morbidity, we prefer serratus anterior transposition muscle flaps for prophylactic coverage of irradiated bronchi and treatment of bronchopleural fistulas. Our surgical technique is described, and our outcomes are discussed. Serratus anterior transposition muscle flaps can be performed with minimal morbidity and minimal impairment of upper extremity function.

Angiosarcoma in postsurgical lymphedema. An unusual occurrence in a man.

A 63-year-old man underwent partial left axillary node dissection for metastatic melanoma of unknown primary. Lymphedema of his left upper extremity developed after surgery. Eight years later, extensive cutaneous angiosarcoma developed in the edematous limb. The occurrence of cutaneous angiosarcoma in postsurgical lymphedema is extremely rare in men; to our knowledge, our case is the fifth such case thus far reported.

The clinical efficacy and mechanism of gamma frequency electroacupuncture stimulation on the rehabilitation of upper limb motor function in stroke patients: study protocol of a randomized clinical trial.

BACKGROUND: The field of neuroscience has discovered that gamma oscillations (30-100 Hz) in the brain play a key role in neuroplasticity, information conduction and neuroprotective mechanisms. Electroacupuncture stimulation (ES) is a modern treatment method that combines the advantages of Chinese acupuncture with those of electrical stimulation, and is widely used in the field of stroke rehabilitation. At present, there is a lack of research on the clinical efficacy and mechanism of applying gamma frequency (40-Hz) ES, a new neuromodulation mode of integrated Chinese and Western medicine, to i
