Excessive bleeding from genital ulcers of Behçet's disease.

Although genital ulcers are a common manifestation of Behçet's disease, bleeding from these lesions is unusual. In this report, a patient with excessive bleeding from genital ulcers is presented. A 22-year-old man with recurrent oral and genital ulcers, erythema nodosum, folliculitis, positive pathergic reaction, uveitis, superior vena cava syndrome, and generalized lymphadenopathy fulfilled the diagnostic criteria of Behçet's disease. Three years after presenting he was admitted to our clinic with excessive bleeding from large ulcers on the scrotum and upper thighs and with a leg ulcer. Bleed

Morphology of glaucoma drainage implantation.

Implantation of glaucoma drainage devices has become a standard procedure in various forms of intractable glaucoma. Indications include extensive conjunctival scanning at the upper limbal area, neovascular glaucoma (in addition to pars plana vitrectomy and endolasercoagulation), and secondary glaucoma due to uveitis or trauma. Implanting a glaucoma drainage device is an alternative to trabeculectomy with antimetabolites in young patients in whom delayed complications such as bleb-related endophthalmitis are more likely to occur. Intermediate-term success rates ranging from 70% to 90% are encou

Causes of blindness in Irbid, Jordan.

To determine the most common causes of blindness and the age of onset of these conditions among Jordanians of Irbid, a study of all registered members in two societies for the care of the blind was carried out. Of all registered members, 185 individuals were blind according to the World Health Organization definition and satisfied our inclusion criteria. It was found that genetically determined causes made up 41% of the total causes. In 57% of the subjects, blindness occurred in the first two decades of life. The specific leading causes of blindness, in the order of their frequencies, were tap

[Causes of intraocular lens removal].

The causes of 25 cases of intraocular lens (IOL) explanation were reviewed retrospectively. Of the IOLs removed, 4 were anterior chamber (AC) IOLs and 21 posterior chamber (PC) IOLs. The average interval between cataract extraction with lens implantation and the IOL explantation in AC IOLs was 23.5 months and in PC IOLs 7.1 months. The causes of AC IOL removal were pseudophakic bullous keratopathy in 1 and uveitis-glaucoma-hyphema syndrome in 3 cases. The causes of PC IOL explantation included dislocation or subluxation in 12, pseudophakic bullous keratopathy in 4, pseudophakic retinal detachm

Extraocular muscle involvement in sarcoidosis.

BACKGROUND: Sarcoidosis is a granulomatous inflammatory disease that may have a variety of ocular and orbital manifestations. The most common ocular manifestation is uveitis, and the most common orbital manifestation is dacryoadenitis. Extraocular muscle involvement in sarcoidosis has rarely been reported. The authors report a case of sarcoidosis involving the extraocular muscles of a 15-year-old boy with bilateral, painful, external ophthalmoplegia and enlargement of all extraocular muscles on computed tomography (CT) scan. RESULTS: Lateral rectus muscle biopsy and transbronchial lung biopsy

Encephalitis and chorioretinitis associated with neurotropic African horsesickness virus infection in laboratory workers. Part II. Ophthalmological findings.

Four laboratory workers developed uveitis-chorioretinitis, associated with encephalitis in 3 cases. The retinitis was characterised by haemorrhages and areas of retinal oedema, most marked over the posterior polar regions, and was associated with exudative retinal detachments. The lesions progressed over weeks and showed a severe retinal arterial vasculopathy with arteriolar narrowing, ghost vessel formation and the development of optic atrophy. The picture in 2 of the patients resembled that of the acute retinal necrosis syndrome (ARN). Antibodies to African horsesickness (AHS) virus were det

Diffuse type alteration of the ocular wall in different eye diseases.

The ocular wall dimensions (thickness and volume) were measured by means of ultrasonography in 61 eyes of 61 patients with different eye diseases and 31 eyes of 31 healthy subjects with or without refractive error. The results confirmed, that in spite of the great variance in the ocular wall thickness, depending on the axial eye length, the volume of the ocular coats is constant in healthy eyes (ca. 1.65 cm3). In patients with ocular hypotony (eyes after intraocular surgery or trauma, and eyes with uveitis or with phthisis bulbi) and in patients with exophthalmus, the ocular wall was thicker a

[Cerebral sinus vein thrombosis in Behçet's disease].

A 20-year-old Turkish patient presented with superior sagittal sinus thrombosis which led to the final diagnosis of Behçet's disease. The frequency of cerebral venous thrombosis is generally underestimated and may be the first clinical manifestation of Behçet's disease. Uveitis as a main diagnostic criterion of Behçet's disease may be absent, especially in young men. In cases of intracranial hypertension, cerebral venous thrombosis and Behçet's disease should be taken into consideration, especially in a high risk population from eastern Mediterranean countries and Japanese islands. Role of interleukin 6 in the inflammatory response after cataract surgery. An experimental and clinical study.

The mechanism of uveitis following cataract surgery, which is mediated in part by the cyclooxygenase pathway, is complex, the complexity of which is likely to be due to several factors. We investigated the possible local involvement of interleukin 6 (IL-6) in this response. Using a specific bioassay, we showed a dramatic increase of IL-6 levels (> 4 x 10(3) times) in all of the aqueous humors of 12 patients following cataract surgery. The IL-6 levels in serum samples were below detection limits, indicating a local production of this cytokine. In addition, the injection of highly purified recom

Kveim-Siltzbach test revisited.

The Kveim-Siltzbach (KS) skin test has been in use worldwide for fifty years. It is a safe, simple and specific out-patient technique to confirm the diagnosis of sarcoidosis and to provide evidence of activity of the disease. It is most helpful in delineating sarcoidosis as a cause of erythema nodosum, uveitis, liver granulomas, hypercalciuria and meningitis. It is the patient's preference when he is confronted with the choice between a skin test or alternatively bronchoscopy, lung biopsy or aspiration liver biopsy. It also creates considerable academic interest for it reflects granuloma forma

Sex ratios and hormones in HLA related rheumatic diseases.

The major diseases associated with HLA-B27 (Reiter's disease, ankylosing spondylitis, acute anterior uveitis, and psoriatic arthritis) all occur much more commonly in men. Published evidence indicates that the antigen HLA-B27 is associated with high testosterone concentrations in men. Moreover, the antigen HLA-B44 exerts a protective effect on one of these diseases (psoriatic arthritis), and there are external grounds for supposing that HLA-B44 indexes an antiandrogenic process. These data are interpreted as support for the hypothesis (first adumbrated nearly 20 years ago) that HLA antigens in

An experimental model of early central nervous system syphilis.

Although up to 40% of patients with early syphilis have evidence of central nervous system (CNS) invasion by Treponema pallidum, the pathogenesis of CNS syphilis is not understood. A rabbit model that mimics early CNS involvement in humans was developed and characterized. Mild cerebrospinal fluid pleocytosis was evident 2 weeks after intracisternal inoculation of T. pallidum and peaked at 9 weeks. The VDRL test in cerebrospinal fluid was reactive in 24% of animals, most commonly at 9 weeks after infection. T. pallidum could be isolated from the CNS of animals infected for 4, 6, or 9 weeks but

B27+ disease versus B27- disease.

When studying heterogeneity of B27-positive disease versus B27-negative disease, very different patterns of disease associations but also a lot of similarities may be observed. In general, B27-positive disease has an earlier onset, a more severe and prolonged clinical course and is more often complicated by acute anterior uveitis and peripheral arthritis, whereas B27-negative disease is accompanied by psoriasis, inflammatory bowel disease and erythema nodosum in a higher percentage of the cases. Despite these differences, B27-positive disease may be completely undistinguishable from B27-negati

Lens luxation in the dog and cat.

Dislocation, or luxation, of the lens arises as a result of rupture of the zonular fibers, which suspend the lens from the ciliary body. In the dog, lens luxation is most frequently encountered as a primary, heritable condition in which there appears to be an inherent weakness of the zonule. The condition is limited mainly to the Terrier breeds but has also been described in the Tibetan Terrier and Border Collie. The disease is essentially bilateral but seldom becomes apparent clinically before 3 or after 7 years of age. In most primary luxations, the lens passes into the anterior chamber and

[Neurological manifestations of Behçet's disease].

Characterised classically by the association of buccal and genital ulceration and uveitis with hypopyon, Behçet's disease has many other manifestations, amongst which the neurological ones (often referred to as Neuro-Behçet) are important in view of their frequency and their gravity. Anatomically, it produces a subacute haemorrhagic and necrotising meningo-encephalitis, which most typically effects the hypothalamus and brainstem. Clinically, there is extreme polymorphism, central manifestations being the most frequent: seizures, organic brain syndromes, disorders of consciousness, aphasia, hem

Immunoassay of serum muramidase (lysozyme) in ocular diseases.

Serum levels of muramidase activity were measured in 162 patients with different ocular diseases and 84 healthy subjects by electro-immuno-diffusion technique. We demonstrated for the first time that electro-immuno-diffusion technique could be successfully applied for the estimation of serum muramidase concentrations. Serum muramidase was found to be high in significant number of cases with granulomatous uveitis, tuberculous keratitis, central serous retinopathy and Eales' disease. Tuberculosis was presumed to be the cause in them by the process of exlusion. Patients with high serum muramidase

An immunohistochemical study using various antibodies directed against antigens of inflammatory cells, neurons, and Müller cells, interstitial collagens (types I and III), basement membrane collagens (types IV and V), and basement membrane glycoproteins (laminin and fibronectin) was performed to characterize the components of two cyclitic membranes (in an enucleated eye and a surgical specimen) from a 15-year-old patient with an eight-year history of chronic bilateral uveitis. When specimens were obtained, no inflammation was seen clinically. Correlative light and electron microscopic examinat

Pediatric aphakic glaucoma.

Three cases of pediatric open-angle aphakic glaucoma are presented. These patients had congenital cataracts extracted by the extracapsular techniques of phacofragmentation or irrigation and aspiration. Each required a secondary membranectomy and had development of glaucoma six to 25 months after cataract extraction. Surgical therapy was required in each case of glaucoma, and multiple procedures were necessary. Visual outcome in the involved eye was less than 20/400. The cause of the open-angle aphakic glaucoma is most likely secondary to damage to the trabecular meshwork initiated by uveitis a

Biometry and clinical characteristics of congenital cataracts and microphthalmia in the Miniature Schnauzer.

Forty-two Miniature Schnauzer pups and adults with congenital cataracts and microphthalmia were evaluated by serial ophthalmic examinations, slit lamp biomicroscopic photography, and A-scan ultrasonography. The cataracts were evident when the eyelids opened at 2 weeks, affecting predominantly the lens nucleus and posterior cortex. Lenticonus was evident in 19% of the cataractous lenses. Progression of the cataracts was variable and related to involvement of the equatorial and posterior cortices. Lens-induced uveitis developed in some adult dogs with advanced hypermature cataracts. The globe an
