Percutaneous catheter ablation of arrhythmias in children.

BACKGROUND: Supraventricular tachycardias (SVTs) are the most common arrhythmias in children, with Wolff-Parkinson-White (WPW) syndrome the most prevalent entity. Percutaneous catheter ablation is now the standard of care for SVT in pediatric patients, although specific techniques are required due to their age. OBJECTIVE: To evaluate the results of percutaneous catheter ablation of arrhythmias in children at our institution. METHODS: This was a retrospective study of the clinical records of patients aged 18 years who underwent electrophysiologic study and ablation between January 2002 and De

Beyond Fontan conversion: Surgical therapy of arrhythmias including patients with associated complex congenital heart disease.

BACKGROUND: Arrhythmia operations may be extended to patients with failed ablation procedures or associated structural defects requiring surgical intervention. The purpose of this study is to review our experience with arrhythmia operations in 29 patients who did not have Fontan conversions after the introduction of catheter ablation. METHODS: Between July 1992 and January 2002, 29 patients had operations for refractory atrial (n = 24) or ventricular (n = 5) arrhythmias. Mechanisms of arrhythmia included atrial reentry (n = 11), atrial fibrillation (n = 5), automatic atrial (n = 3), accessory

Older patients fare better with the Ross operation.

BACKGROUND: The Ross operation has an established position in young patients. We address the question of whether any age group profits most from the Ross operation, and we compare the results in various ages. METHODS: From February 1995 to August 2001 we performed 250 Ross operations. Group 1 consisted of 46 patients, ages 2 to 25 years (median age, 15 years). Group 2 consisted of 123 patients, ages 26 to 49 years (median age, 39 years). Group 3 consisted of 81 patients, ages 50 to 67 years (median age, 55 years). Echocardiography was performed perioperatively, at 2 to 6 months, and then year

[Holter monitoring in patients with focal cerebral ischaemic attacks (author's transl)].

One hundred patients with focal cerebral ischaemic attacks of suspected embolic origin were investigated by Holter monitoring to determine whether paroxysmal arrhythmia may have been responsible for the episodes. There were 57 men and 43 women aged from 16 to 79 years (mean 50 years). Ninety-seven had residual focal neurological deficits and 3 had transient ischaemic attacks. The neurological lesions were verified by cerebral angiography in 68. Twenty-one had arterial hypertension and 9 had old myocardial infarction or angina. Nine had a history of palpitations. None had cardiac valve disease.

What causes sudden death in patients with chronic heart failure and a reduced ejection fraction?

Sudden death characterizes the mode of demise in 30-50% of patients with chronic heart failure and a reduced ejection fraction. Occasionally, these events have an identifiable pathophysiological trigger, e.g. myocardial infarction, catecholamine surges, or electrolyte imbalances, but in most circumstances, there is no acute precipitating mechanism. Instead, adverse left ventricular remodelling and fibrosis creates an exceptionally fragile and highly vulnerable substrate, which can be characterized using the model developed in theoretical physics of 'self-organizing criticality'. This framework

Late gadolinium enhancement among survivors of sudden cardiac arrest.

OBJECTIVES: The aim of this study was to describe the role of contrast-enhanced cardiac magnetic resonance (CMR) in the workup of patients with aborted sudden cardiac arrest (SCA) and in the prediction of long-term outcomes. BACKGROUND: Myocardial fibrosis is a key substrate for SCA, and late gadolinium enhancement (LGE) on a CMR study is a robust technique for imaging of myocardial fibrosis. METHODS: We performed a retrospective review of all survivors of SCA who were referred for CMR studies and performed follow-up for the subsequent occurrence of an adverse event (death and appropriate de

Recurrent right ventricular cardiac myxoma in a patient with Carney complex: a case report.

INTRODUCTION: Carney complex is a multiple neoplasia syndrome involving cardiac, endocrine, neural and cutaneous tumors with a variety of pigmented skin lesions. It has an autosomal dominant mode of inheritance. Approximately 7% of cardiac myxomas are related to the Carney complex. Myxomas that occur as part of the Carney complex affect both sexes with equal frequency. Cardiac myxomas with Carney complex are reported mostly in the left side of the heart and are less common on the right side. As per our review, this is the first reported case of Carney complex with right ventricle cardiac myxom

[Antiarrhythmic agents in the prevention of sudden cardiac death].

Sudden cardiac death due to ventricular arrhythmias remains a significant problem. In most studies about 50% of all death related to coronary artery disease and heart failure are sudden and unexpected and are caused by acute fatal ventricular tachycardia and fibrillation. Most of the patients suffering sudden cardiac death have some kind of structural heart disease but 80% of SCD events are associated with coronary artery disease, 10-15% with dilated and hypertrophic cardiomyopathy, and only small fraction with the less common disorders as valvular heart disease, ventricular dysplasia and card

Characterization and natural history of patients with LMNA-related dilated cardiomyopathy in the phase 3 REALM-DCM trial.

AIMS: LMNA-related dilated cardiomyopathy (DCM) is a rare disease with an incompletely defined phenotype. The phase 3 REALM-DCM trial evaluated a potential disease-modifying therapy for LMNA-related DCM but was terminated due to futility without safety concern. This study utilized pooled data from REALM-DCM to descriptively characterize the phenotype and progression of LMNA-related DCM in a contemporary cohort of patients using common heart failure (HF) measures. METHODS: REALM-DCM enrolled patients with stable LMNA-related DCM, an implanted cardioverter defibrillator or cardiac resynchroniza

Multimodal imaging of a coronary artery fistula in a Welsh Springer spaniel.

A four-year-old intact male Welsh Springer spaniel was presented for a prebreeding evaluation. Thoracic auscultation revealed a grade 2/6 left basilar systolic murmur. Echocardiography demonstrated left atrial and left ventricular dilation with systolic dysfunction, including a left atrium-to-aortic root ratio of 2.24, normalized left ventricular internal dimension in diastole of 2.44, and fractional shortening of 19%. Color and spectral Doppler interrogation identified continuous flow with a peak velocity of approximately 1.5 m/s in the left pulmonary artery. Transesophageal echocardiography,

Atrial fibrillation cryoablation complicated by diffuse coronary artery vasospasm and cardiac arrest: a case report.

BACKGROUND: Pulmonary vein isolation (PVI) is recommended for rhythm control in selected patients with symptomatic atrial fibrillation (AF). Coronary artery spasm (CAS) is a rare but recognized complication of AF ablation and may result in life-threatening arrhythmias. CASE SUMMARY: A 47-year-old man underwent cryoballoon PVI for symptomatic paroxysmal AF. Transient inferior ST-segment elevation occurred following left-sided PVI and resolved spontaneously. Shortly after completion of the procedure, recurrent ST-segment elevation developed, progressed to malignant ventricular arrhythmia and ca

Bioinspired Cold-Laminated Ultrathin Hydrogels as a Broadly Adaptive Platform for Physiological Monitoring.

Soft bioelectronics provide a powerful platform for physiological monitoring, yet conventional hydrogel-based devices are constrained by inherent trade-offs between thickness, robustness, and multifunctionality, restricting their adaptability across diverse epidermal and implantable applications. Here, a novel cold-lamination strategy is developed to mechanically interlock a TPU nanomesh within a temperature-responsive hydrogel network, mimicking the structure of the extracellular matrix. This method enables the fabrication of devices with precise control of thickness (17-90 µm), tunable Young

Unmasking Brugada syndrome: a case report of diagnostic oversights.

BACKGROUND: Brugada syndrome (BrS) is an unusual cardiac channelopathy associated with an increased risk of ventricular fibrillation (VF) and sudden cardiac death (SCD), highlighting the critical importance of early diagnosis. CASE SUMMARY: A 22-year-old male patient presented with recurrent episodes of syncope, palpitations, and dyspnoea. Initial electrocardiograms (ECGs) showing a Brugada Type 1 pattern were misinterpreted as incomplete right bundle branch blocks. The patient was erroneously diagnosed with epilepsy due to misinterpretation of his syncope episodes as seizures. After further

Electrocardiographic abnormalities secondary to acute central nervous system injury.

Most patients with acute central nervous system (CNS) lesions, including acute ischemic stroke, intracranial bleeding, traumatic brain or spinal cord injury, exhibit electrocardiographic (ECG) abnormalities. These are generally transient and considered secondary to the neurological condition. The most common neurogenic ECG alterations include ST-segment elevation or depression, T wave inversion, prominent U wave, prolonged QTc interval, sinus bradycardia and atrioventricular block. The primary mechanism involves autonomic nervous system alterations due to damage to the central autonomic networ

A combined valve-sparing aortic root replacement and primary tetralogy of Fallot repair in an unoperated adult: a case report.

BACKGROUND: Progressive aortic root dilation represents a life-threatening sequela of uncorrected tetralogy of Fallot (TOF), predisposing patients to aortic regurgitation, dissection, and rupture. Valve-sparing aortic root replacement (VSARR) offers an attractive alternative to mechanical valve replacement by eliminating prosthesis-related complications and lifelong anticoagulation. However, experience with VSARR in unoperated adult TOF patients remains exceptionally limited. CASE PRESENTATION: We report a 35-year-old male with unrepaired TOF who presented with progressive exertional dyspnea,

Adopting a Collaborative Strategy to Address the Complexities of Implanting a Subcutaneous Implantable Cardiac Defibrillator for Secondary Prevention in a Patient With Fabry Disease and Motor Neuron Disease.

A man in his mid-40s, diagnosed with motor neuron disease (MND) and Fabry disease (FD), was sent to cardiology after experiencing a cardiac arrest at home secondary to ventricular fibrillation (VF). Fabry disease is a rare X-linked inherited lysosomal storage disorder caused by deficient alpha-galactosidase A (AGAL-A) activity that leads to an accumulation of globotriaosylceramide (Gb3) in affected tissues, including the heart. Motor neuron disease is an uncommon condition that progressively damages parts of the nervous system. He experienced a positive neurological recovery, enabling the card

Proarrhythmogenic Echocardiographic Markers in Metabolic Syndrome: A Cross-Sectional Study.

In metabolic syndrome, cardiomyocyte changes induced by metabolic and proinflammatory factors impair repolarization and exacerbate the heterogeneity of the transmural dispersion of repolarization, and this is proarrhythmogenic. Limited data in the literature on the capabilities of speckle tracking echocardiography for assessing proarrhythmogenicity in metabolic syndrome exists. 71 patients with newly diagnosed metabolic syndrome, aged 35-55 years, were studied. Ischemic heart disease was excluded in all patients with stress test cycle ergometry, CT-angiography or selective coronary angiography

Post-Marketing Surveillance of the Safety of Landiolol in Patients With Recurrent Hemodynamically Unstable Ventricular Tachyarrhythmias.

BACKGROUND: Landiolol is a short-acting, intravenously administered β1-adrenergic receptor blocker that can rapidly control heart rate in refractory and urgent fatal arrhythmias (ventricular fibrillation [VF] and hemodynamically unstable ventricular tachyarrhythmia [VT]). This indication was approved in Japan in 2019 based on results of the J-Land II clinical trial. We conducted post-marketing surveillance (PMS) to assess the safety and effectiveness of landiolol in real-world settings in Japan. METHODS AND RESULTS: This PMS examined the safety, focusing on adverse drug reactions (ADRs) relat

B-Type Natriuretic Peptide as a Marker of Subclinical Heart Disease in a High-Burden Emergency Department Population With Sustained Asymptomatic Hypertension.

B-Type natriuretic peptide (BNP) levels below contemporary thresholds for diagnosing congestive heart failure are associated with subclinical heart disease (SHD) and adverse cardiovascular outcomes in community patients with uncontrolled asymptomatic hypertension. This study aimed to determine the accuracy of BNP for detecting SHD in emergency patients with sustained asymptomatic hypertension, where SHD is universally prevalent. Conducted at two urban academic emergency departments within a seven-hospital healthcare organization, this proof-of-concept study included adults with sustained asymp
