A Framework for Emergency Department-Integrated Hepatitis C Test-and-Treat in the United States.

Despite the availability of curative, direct-acting antiviral therapy, hepatitis C virus elimination remains incomplete. Losses across the care cascade continue to limit impact, from initial diagnosis to sustained virologic response. Fewer than 1/3 of individuals ultimately achieve cure. These gaps reflect a delivery system that does not align with the population most affected, many of whom have inconsistent engagement with outpatient care. The emergency department is critical, but underused point of contact, where the burden of undiagnosed and untreated infection is high. This manuscript pres

Uncovering core stemness biomarkers in HBV-HCC through integrated multi-omics.

Chronic hepatitis B virus (HBV) infection is a major etiology of hepatocellular carcinoma (HCC), with cancer stem cells (CSCs) playing a critical role in tumor progression and therapeutic resistance. However, the specific mechanisms linking HBV infection to stemness features in HBV-associated HCC (HBV-HCC) remain incompletely defined. To identify potential regulators, we conducted an integrated in silico multi-omics analysis using single-cell and bulk transcriptomic datasets. Comparative stemness scoring across major cell populations prioritized the endothelial compartment for further analysis

3D Radiomics Analysis Based on Liver CT Imaging for Predicting Hepatic Encephalopathy in HBV-ACLF Patients.

Hepatic encephalopathy (HE) is an independent risk factor for mortality in patients with hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF). We aim to assess the effectiveness of 3D deep learning based on CT imaging in predicting HE in HBV-ACLF patients. A retrospective study was conducted on 222 HBV-ACLF patients who underwent CT scans at two medical centers. The cohort from Huashan Hospital, consisting of 146 patients, was randomly divided into a training cohort (n = 102) and a validation cohort (n = 44). An external test cohort (n = 76) was sourced from Shanghai Public Heal

Reverse vaccinology and immunoinformatics identify multi-epitope vaccine candidates against molluscum contagiosum virus.

Molluscum contagiosum virus (MCV) is a human-specific poxvirus responsible for a common contagious skin infection, yet no licensed prophylactic vaccine is available. Therefore, this study aimed to identify potential vaccine candidates against MCV using integrated reverse vaccinology and immunoinformatics approach. Candidate proteins involved in immune evasion, viral entry, and virion assembly were assessed for physicochemical properties, transmembrane topology, antigenicity, allergenicity, and toxicity. Linear B-cell, cytotoxic T-lymphocyte (CTL), and helper T-lymphocyte (HTL) epitopes were pr

Spatiotemporal Transmission Characteristics of Global Monkeypox - Worldwide, January 2022-September 2025.

INTRODUCTION: Since 2022, the global monkeypox (mpox) epidemic has undergone clade replacement from clade IIb to clade Ib, accompanied by a geographic shift in the epidemic's epicenter. However, systematic analyses spanning the entire epidemic cycle from 2022 to 2025 remain limited. Integrating global epidemiological and genomic data is essential for determining spatiotemporal transmission dynamics. METHODS: Global mpox surveillance data, mpox virus whole-genome sequences, and demographic data were collected. Retrospective spatiotemporal scan statistics were used to identify significant trans

Finger-actuated microfluidic device for point-of-care circulating cell-free nucleic acid extraction.

Timely and accurate nucleic acid extraction is a critical bottleneck in point-of-care (POC) molecular diagnostics, particularly for low-abundance cell-free nucleic acids (DNA, RNA, miRNA) in plasma. We present a fully finger-actuated microfluidic device for power-free, pipette-free, nucleic acid extraction. The prototype device integrates finger-actuated valves, pump and blister reagent storage, enabling a complete, intuitive, "sample-to-eluate" workflow, including lysis, binding, washing and elution via manual actuation in approximately 35 minutes. The device operation was benchmarked against

[Staging-based integrative treatment strategies of traditional Chinese and Western Medicine for zoster-associated pain].

Zoster-associated pain (ZAP) is a neuropathic pain syndrome caused by the reactivation of the varicella-zoster virus. Based on disease course, it can be classified into three stages: acute herpetic neuralgia (AHN), subacute herpetic neuralgia (SHN), and postherpetic neuralgia (PHN). Among these, PHN is the most common and the most difficult complication to treat, characterized by severe pain and prolonged duration, often leading to emotional disorders such as anxiety and depression. Its incidence is significantly higher in elderly individuals and immunocompromised patients. Western medicine ma

Immunoreactivity of pan MHC-I epitopes on Crimean-Congo hemorrhagic fever virus glycoprotein C-terminal.

INTRODUCTION: Crimean-Congo hemorrhagic fever virus (CCHFV) is a tick-borne pathogen causing severe hemorrhagic disease with high mortality. The viral glycoprotein Gc mediates membrane fusion and represents a key target of CD8⁺ T-cell responses. However, systematic identification and comprehensive evaluation of pan-MHC-I-restricted Gc epitopes remain limited. METHODS: An integrated immunoinformatics workflow incorporating five prediction algorithms (IEDB, NetMHCpan4.1, SMMPMBEC, SYFPEITHI, and Rankpep) was applied to identify high-affinity 9-mer epitopes from the CCHFV Gc protein restricted b

Molecular and Serological Surveillance of Dengue Virus and Vector Dynamics in the Highlands of Southwestern Saudi Arabia.

Dengue fever is an emerging endemic disease in Saudi Arabia; however, important gaps remain regarding transmission dynamics, vector distribution, and serotype circulation in highland areas. This study primarily aimed to investigate dengue virus (DENV) infection and vector occurrence in the highland governorates of the Jazan region. A cross-sectional study enrolled 182 febrile patients tested using RT-qPCR, ELISA, RT-PCR-based serotyping, and rapid diagnostic tests. Concurrent entomological surveillance was conducted using CDC light traps, with morphological and molecular identification of Aede

Dengue Vaccines in a Changing Epidemiological Landscape: Current Evidence, Unresolved Challenges, and Public Health Considerations.

BACKGROUND: Dengue has expanded rapidly beyond traditional tropical and subtropical regions, driven by climate change, urbanization, population mobility, and the spread of competent Aedes vectors. Vaccination is an increasingly important component of dengue prevention, but development has been complicated by four viral serotypes, antibody-dependent enhancement, variable baseline serostatus, and the need for balanced and durable tetravalent immunity. METHODS: We conducted a narrative review of PubMed/MEDLINE, Google Scholar, ClinicalTrials.gov, and relevant public health and regulatory sources

Tick-borne pathogens in a nature reserve area in the Netherlands: serological and molecular assessments in large herbivores and Ixodes ricinus ticks.

Ticks are vectors of a wide range of pathogens, relevant to both veterinary and public health. Although tick surveillance is frequently conducted in the Netherlands, data on tick-borne pathogens in free-living herbivores remains limited, leaving the impact of these pathogens on animal health poorly understood. This study aimed to characterise tick species and determine seroprevalence and infection prevalence of selected tick-borne pathogens in large herbivores at the Oostvaardersplassen nature reserve and questing ticks collected in the area. Between September 2023 and December 2024, blood was

RVG-targeted extracellular vesicles loaded with echinatin attenuate dopaminergic neurodegeneration via the IGF-2/PI3K/Akt pathway in Parkinson's disease mice.

BACKGROUND: Parkinson's disease (PD) is a progressive neurodegenerative disorder characterized by the loss of dopaminergic (DA) neurons. The development of effective neuroprotective therapies is severely hampered by the blood-brain barrier (BBB), which restricts drug delivery to the central nervous system. This study aimed to develop a novel brain-targeted nanodelivery system by functionalizing extracellular vesicles (EVs) with a rabies virus glycoprotein (RVG)-derived peptide to deliver echinatin (Echi), and to systematically evaluate its therapeutic efficacy and underlying mechanisms in a mo

HIV-HPV interactions via extracellular vesicles among tobacco smokers and nonsmokers.

Human Immunodeficiency Virus (HIV) and Human Papillomavirus (HPV) co-infections are significantly prevalent, especially among African Americans (AA), a situation further compounded by the prevalence of tobacco smoking. Extracellular vesicles (EVs) are integral to the mechanisms of viral pathogenesis, as they are pivotal in the modulation of immune responses and the inflammatory process. This research study examines the varying concentrations of EVs, their associated biomarkers, and the cytokine/chemokine profiles present in plasma obtained from individuals infected with HIV and those coinfecte

Positive effects of probiotics in people living with HIV: a narrative review.

Despite effective antiretroviral therapy (ART), no clear guidelines exist on microbiome-targetednadjuncts for people living with HIV (PLWH). Human immunodeficiency virus (HIV-1) infection damages the gastrointestinal tract by altering the mucosal surface and prompting dysbiosis. Gut microbial imbalance, characterized by the predominance of opportunistic bacteria, leads to increased microbial translocation, which, in turn, results in chronic inflammation, digestive tract symptoms, and reduced quality of life. PLWH have substantial dysbiosis compared to healthy controls. In addition, gastrointes
